1.A mini-plate fixation for the treatment of children's ulna and radius distal unstable fractures: A 18-case analysis

OBJECTIVE: To observe the clinical effect of mini-plate fixation on the treatment of children unstable fractures of the distal ulna and radius. METHODS: A total of 18 children with unstable fracture of the distal ulna and radius (15 boys and 3 girls, aging 4.1-13.3 years, with the mean age of 9.8 years) were selected from Department of Orthopaedics, Wuhan Children and Women's Hospital, Tongji Medical College, Huazhong University of Science and Technology from August 2006 to September 2009. The fracture occurred on right side (n=9) and left side (n=9). All children were treated with mini-plate fixation which was instead of the traditional pinning fixation. Before operation, stainless steel or titanium plates of adult digital bone, metacarpal bone, and metatarsal bone were chosen according to fracture types and sites. The distance from fracture line to epiphyseal plate was measured to get ready the T-, L-, clover or straight plates. Joint function, degree of forearm rotation loss, bone healing, and complications were monitored postoperatively and within following up. RESULTS: All cases were followed up in out-patient for an average 26.4 months. An average 10°limitation was measured on forearm pronation function of all children, but the function was recovered by doing exercises which did not influence daily activities. Supination, flexion and extension of wrist joint were generally normal, and the fracture was generally healed. Complications including re-displacement, refracture, malunion, delayed union, bone bridge formation, vascular and nerve injury were not found. CONCLUSION: Patients who were not satisfied with the reduction, re-displacement, or old distal radius fracture needed an open reduction. Micro-plate was fixed to avoid epiphyseal injury of distal forearm and reduce postoperative complications. An ideal mini-plate may replace the traditional pinning fixation.
2.Transfusion of leukocyte-depleted blood in pediatric open heart surgery

Objective To evaluate the efffects of leukocyte-depleted blood transfusion the protection of cardio pulmonary tissue and the reduction of postoperative bacterial infection.Methods Three hundred and forty-four pediatric patients were allocated to one of 3 groups based on transfusing different components of blood during open heart surgery:control group(n=109),routing blood transfusion group(RBT,n=113),leukocyte depleted blood transfusion group (LDBT,n=122). Blood samples were taken before operation,at 6h,and 24h after operation fir determination of creatininee kinase MB( CK MB),aspartate transaminase(AST),lactate dehydrogenase(LDH)and MDA. At the same time,we monitored changes of TPR and RI,and recorded changes of body temperature and days of utilizing antibiotics after operation.Results After operation serum concentrations of CK MB,AST, LDH and MDA significantly increased in all groups as compared to the values before operation,but the values of those in group LDBT were significantly loert than those in group RBT(P0.05).Conclusions Leukocyte depleted blood transfusion can effectively prevent the patients from cardion pulmonary ischemia-reperfusion injury and reduce postoperative bacterial infection in pediatric open heart surgery.
3.Experience of the bone traction treatment for displaced fracture of femur shaft in children

Objective To review the experience of the bone traction treatment for displaced fracture of femur shaft in children.Methods All of the cases were treated with femur supercondylar traction,and they were fixed in a stable position with hip joint in 90? flexion and 30?~45? abduction and with knee in 90? flexion.Result The effects of 86 cases were satisfactory.Conclution This method is simple and effective.It reduces complications of skin traction and operation to patients.
4.Study on Quality Standard of Zizhu Ointment

Objective To establish the quality standards of Zizhu ointment. Methods The TLC was applied to identify Radix arnebiae and borneol of Zizhu ointment, and the content of borneol was determined by gas chromatography. Results The TLC spots were clear, well-separated and easy to identify. The good linear range of borneol reference substance on calibration curve was 0.048 4-1.210 0 μg, and the recovery was 90%-110%, the relative standard deviation was less than 5%. Conclusion The method is simple and feasible, can be used as the quality control method of Zizhu ointment.
5.Efficacy of Huoxiangzhengqi Oral Liquid and Tablet of Radix et Caulis Acanthopanacis Senticosi on withdrawal symptoms from heroin

AIM:To observe the therapeutic efficacy of Huoxiangzhengqi Oral Liquid (Herba Agastaches seu Pogostemi, Radix Angelicae Dahuricae, Rhizoma Atractylodis, Rhizoma Pinelliae, Pericarpium Citri Reticulatae, Pericarpium Arecae) and Tablet of Radix et Caulis Acanthopanacis Senticosi on the withdrawal symptoms from heroin. METHODS: 143 cases were randomly divided into three groups, namely control group (n=44), treating group A (n=48) and B (n=51). They were detoxified by using Lofexidine Hydrochloride Tablet (LFX) for 12d, and then, the control group took an imitate preparation, the treating group A took Huoxiangzhengqi Oral Liquid and Tablet of Radix et Caulis Acanthopanacis Senticosi for 60d, But group B took Huoxiangzhengqi Oral Liquid and Tablet of Radix et Caulis Acanthopanacis Senticosi from the time when detoxification began. And then, 10d after the two treating groups stopped taking Huoxiangzhengqi Oral Liquid and Tablet of Radix et Caulis Acanthopanacis Senticosi, the protracted abstinent syndromes of the three group were observed and scored for 7d. One year later, the drug re abusing case of the three groups were investigated through urinoscopy. 6.Clinical and pathological characteristics of breast carcinosarcoma with osteosarcoma components

Objective To investigate the clinicopathologic and immunohistochemical characteristics of breast carcinosarcoma with osteosarcoma components and its differential diagnosis. Methods The pathologic features and clinical manifestations of the two patients were retrospectively reviewed. Immunohistochemistry was performed and the literature was reviewed. Results Histopathologically, the neoplasm consisted of invasive ductal carcinoma of no special type and poorly ( case 1 ) or well (case 2) differentiated osteosarcomatoid elements. The morphological transition from carcinoma to sarcomatoid elements was seen. Immunohistochemically,the carcinoma cells were positive for CK and EMA , the sarcomatoid elements were stained positive for vimentin, and a few cells of two elements were positive for S-100 protein. Ki-67 and VEGF were over expressed in both elements of Case 1.In case2 Ki-67 expression was low in both elements and VEGF over expression was only seen in sarcomatoid elements. Some of the carcinoma cells were positive for ER. Conclusions Carcinosarcoma of the breast with osteosarcomatoid elements is a rare type of mixed epithelial/mesenchymal metaplastic breast carcinoma. The types and proportion of carcinoma and sarcomatoid elements determine the diagnosis.
7.Variation of brain derived neurotrophic factor and its mRNA expression in infancy rats after experimental bacterial meningitis

AIM: To investigate the expression and effect of brain derived neurotrophic factor (BDNF) mRNA and its protein in infancy rats after exposure to bacterial meningitis. METHODS: Three week old rats were used to construct the models of bacterial meningitis (n=30) and mormal(n=18). At 24 h, 48 h, 5 d after inoculating, the expression of BDNF mRNA and its protein were detected by in situ hybridization and immunohistochemical staining methods, respectively. RESULTS: The increase in BDNF mRNA expression was detected by in situ hybridization at 24 h in experiment( 0.13320? 0.02750) compared to control( 0.06269? 0.01147)(P0.05). Meanwhile, in the brain from the experiment rats, strong positive hybridization and immunoreactivity were observed in the infiltrated inflammatory cells in leptomeninges, subarachnoid cavity, ventricles and brain parenchyma. CONCLUSIONS: These results support the hypothesis that BDNF might play a neuroprotective role in brain damage process in bacterial meningitis attacked rats. BDNF might take part in imune response. These results also support the hypothesis that BDNF has some relationships with other inflammatory mediators during acute inflammatory response of bacterial meningitis.
8.Optimal preparation procedure and quality evaluation of the superparamagnetic iron oxide coated by dextran

Objective To determine the optimal procedure for preparation of the SPIO nanoparticles modified by dextran polylysine,and to evaluate the quality of this product.Methods The optimal preparation procedure of four factors (solution pH value,dextran concentration,reaction temperature and stirring rate) affecting SPIO hydrodynamic size were obtained through orthogonal experiments (3 levels).SPIO nanoparticles were synthesized under an optimum procedure.The shape and hydrodynamic size were detected by transmission electron microscope (TEM) and Malvern Zetasizer respectively.The content of Fe was detected by atomic absorption spectrophotometer,while chemical structures of SPIO nanoparticles were characterized and analyzed by infrared spectroscopy (IR) method.X-ray powder diffraction method was used to identify the ingredients,and the magnetic parameters were measured by vibrating sample magnetometer.Furthermore,experiments with ovarian cancer cells were performed to primarily validate the magnetic property of SPIO nanoparticles.Results The results of the orthogonal experiments showed that the optimum preparation procedure was as follows:dextran concentration of 10 mg/ml,pH 10,reaction temperature of 80 ℃ and stirring rate of 600 r/min.The TEM results showed the SPIO nanoparticles were in spherical shape,homogeneously distributed and uniform in size,and the mean diameter was 7.0 nm.The content of Fe was (12.36±0.08) g/L.The IR results clearly showed that the Fe3O4 was coated by dextran successfully.The Xray powder diffraction method showed that the sample contained Fe3O4 and the magnetism parameters indicated that the sample had superparamagnetism.The experiments with ovarian cancer cells demonstrated that SPIO nanoparticles could enter into the cells and then the cells had certain magnetic properties.Conclusions The SPIO nanoparticles synthesized under the optimal procedure are stable,small in size,with good dispersion and are feasible to enter into cells for rendering certain magnetic properties.This study has provided a good foundation and potential for further research.
9.MEASUREMENTS OF SERUM FOLATE AND VITAMIN B_12 IN HEALTHY SUBJECTS

Serum folate and vitamin B12 were measured in 171 healthy children under 14 years of age, 30 normal adults and 30 samples of normal cord blood. All the 171 children had been supplied with adequate folic acid and vitamin B12 before measurements. The results -were: 1. The lower limits of normal serum folate values were 6.1 nmol/L under 4 years, 8.4 nmol/L at age of 4-14 years and 5.0 nmol/L in adults. 2. The lower limits of normal serum vitamin B12 values were 459 pmol/L under 1 year and 107 pmol/L at age of 1 year to adults.
10.Diagnostic and therapeutic status of the elderly outpatients with benign prostatic hyperplasia in Guangzhou

Objective To evaluate the diagnostic and therapeutic status of lower urinary tract symptoms (LUTS) in the elderly outpatients with benign prostatic hyperplasia (BPH) in Guangzhou.Methods The investigation was conducted in outpatient departments for elderly patients in 3 hospitals of Guangzhou. The eligible male patients with LUTS/BPH were chosen from patients who came to the clinic in the process of investigation. Results The 29.7% (1824/6140) of them had history of LUTS/BPH previously or presently. In 134 patients received medical questionnaire, the proportion of LUTS/BPH patients with mild, middle and severe degree based on international prostate symptom score (IPSS) was 24.5%, 72.5% and 3.0% respectively. The abnormal percentage of PSA was 37. 3 %. The most common examinations that patients received were directeral rectun examination (96.8%), PSA (88. 7%) and transabdominal ultrasonography (84.8%). The 44.7% of cases were given the treatment of 5α-reductase inhibitor, and 24.7% of cases received a combination therapy of 5α-reductase inhibitor plus α receptor blocker. The 16.7% of cases were given a combination treatment of 5α-reductase inhibitor plus herbs. The proportion of therapy with α receptor blocker and herbs was similar (6.8% and 7.1% respectively). Conclusions LUTS/BPH is one of the most common diseases in elderly male patients. The examinations conducted by doctors are unreasonable.The doctors are easy to ignore inquirement of patient's history and IPSS. The drugs conducted by doctors are reasonable basically.
