4.Transfusion of Cryopreserved Platelets to Surgical Patients

Objective To estimate the efficacy and safety of cryopreserved platelets in surgical patients.Methods 238 patients in recent years with hemorrhage or high risk of hemorrhage induced by thrombocytopenia or functional platelet defects during or after surgery, who had been transfused with cryopreserved platelets, were included in our study. The following items were recorded: bleeding time before and two hours after transfusion of cryopreserved platelets; the persist time of blood oozing from wound after transfusion; platelet count before and one hour after transfusion; platelet count before and immediately after surgery; and the number of patients with adverse transfusion reactions. The hemostatic efficacy was judged by the bleeding time and platelet count change.Results Bleeding times before and two hours after cryopreserved platelets transfusion were (9?3)min,(4?2)min respectively; blood oozing from wound stopped 0.5~2 hours after transfusion; the platelet level was elevated by(5~42)?10 9/L one hour after transfusion; the postoperative platelet count was decreased by(130?69)?10 9/L compared to preoperative count; four patients had urticaria and five had febrile reaction. The hemostatic efficacy was 95%(226/238) Conclusion Cryopreserved platelets function immediately to bring about the cessation of blood oozing from the wound,and elevation of peripheral blood platelet count.
5.Flow cytometry detection of CD62p reexpression by preserved platelets and it's value in evaluation of the quality of preserved platelets

Objective 1) To establish and optimize a flow cytometry method for detecting CD62p reexpression by preserved platelets.2) Daily determination of the CD62p expression by platelets stored at 22℃ to assess its value in evaluation of preserved platelet quality.Methods 1) Statistic methods including factorial experiment was applied to optimize the major conditions for sample management, and the feasible negative and positive control for FCM analysis of CD62p expression were check out.2) Thirteen donors' platelets including ten PRP and three apheresis concentrations were stored in a flatbed shaker at 22?2℃ for 12 days. The CD62p expression were detected daily. The correlation between CD62p expression and in vivo residual life span of 22℃ stored platelet were analyzed.Results 1) Satisfactory conditions for analysis CD62p anew expression by FCM were obtained. 2) The CD62p anew expression by stored platelets was correlated to the in vivo residual life span of the platelets with the correlation coefficients between 0.91 and 0.99.Conclusion 1) This FCM method is simple, sensitive, stable, and easy to carry. 2) Platelet CD62p expression can be used to monitor the function of stored platelets and has the potential in the study of new platelet preservation technique.
6.The feasibility of platelet long cryopreserved at-80℃

8.A Study on Trehalose Loading Technique of Human Platelets before Freezing-dried

Objective To establish the technique of loading trehalose into human platelets including temperature, time and concentration of trehalose loading. Methods Intracellular trehalose concentration versus loading time curves at 37℃ and 16℃, mean plateletee volume(MPV) versus loading time curves, and loading concentration curves were investigated and compared. Then loading time and temperature for high loading efficiency of trehalose into human platelets were ascertained. Intracellular trehalose versus trehalose loading concentration curves were made, and loading efficiency was caculated. Results The trehalose loading efficiency of 4 hours at 37℃ was significantly higher than that at 16℃ (11.6% vs 5.6%). MPV at 16℃ increased 43.2% compared with at 37℃, but had no changes along with the change of loading time and concentration. When loading at 37℃, MPV change was positively related with loading time and concentration, and loading time and loading concentration had a cooperative effect. MPV had no changes in 1~4hours loading when trehalose loading concentration was under 50mM. MPV increased with the increase of loading time and concentration when trehalose loading concentration was above 50mM. When trehalose loading concentration was 45mM, loading efficiency at 37℃ reached 13.86% 4 hours aftr loading. Conclusion The basic parameter of trehalose loading technique of human platelets was loading temperature at 37℃, loading time for 4 hours, and loading concentration of 45mM.
9.EVALUATION OF PRESTORAGE LEUCODEPLETION IN MASSIVE BLOOD TRANSFUSION

The patients received massive blood transfusion in General Hospital of the Chinese Pepole's Liberation Army between 1999to 2001were reviewed. The following items of leucodepleted blood transfusion (LDBT)group and routing blood transfusion(RBT) group at pretransfusion, the first day, third day, and seventh day after blood transfusion were recorded and compared: glutamic pyruvic transaminase(GPT), glutamic oxaloacetic transaminase(GOT), lactate dehydrogenase(LDH), total bilirubin(TB), direct bilirubin(DB), alkalescence phosphorylase(ALP), total protein(TP), albumin(ALB), glucose (GLU), Ca 2+ , total CO 2, pH and febrile nonhemolytic transfusion reactions(FNHTR). The results showed that compared with RBT group, GPT in LDBT group on the third day after blood transfusion was significantly lower ( P 0 05). 10.MULTIPLE ORGANINJURY BY HIGH CONCENTRATED PLATELET-LEUCOCYTE AGGREGATIONS IN RED BLOOD CELL CONCENTRATION AND ITS COUNTERMEASURE

To study the formation and change of platelet leucocyte aggregation (PLA) in red blood cell concentrations(CRCs) stored at 4℃ and the preventive effectiveness of filtration, 8 units of CRCs were each divided into two equal parts in two integrative bags and two paired groups were formed: control group and filtration group. The following items of control group and filtration group were detected weekly. Platelet and leukocyte counts, concentration and percentage of platelet lympocyte (P lym), platelet monocyte (P mon), platelet neutrophil (P neu) aggregatoins, and concentration of total PLA. The results showed that rate and concentration of PLA in the control group were significanly higher than those in the circulating blood. The rates of P lym and P mon in CRCs stored at 4 ℃ were two to five times of those before storage, and the rate of P neu in CRCs stored at 4 ℃ was ten to twenty times of that before storage. PLA in the CRCs separated from plasma rose to a high level at the first day of preparation. The concentration of PLA in CRCs reached the highest level at one week after stored at 4 ℃ and then decreased, but a high concentration was retained all the while. P neu is the major part (about 80%~90%) of whole PLA in CRCs. PLA was not detectable by flow cytometry during the whole storage course. The results suggested that high concentrated PLA was detected in CRCs stored at 4 ℃, which could be prevented by leucocyte depleting filtration.
