1.Application of remifentanil in the surgery for upper limb fracture in cldldren

Objective To study the application of remifentanil combined with brachial plexus block using in the operation of children with upper limb.Methods A total of 40 children,aged 4~10 years,undergoing arm-fracture operation,breathing oxygen with opening veil,were randomly divided into two groups.The children were administered basal anesthesia before brachial plexus block.During the operation,the study group(n=20)was infused remifentanil,the initial rate was 0.05μg·kg-1·min-1,and the control group was infused ketamine to reinforce the analgesia.Results All patients finished the surgery safely.In study group,children recovery time[(14.0±4.8)min]Was more quickly than that of control group[(30.4±10.3)min](t=6.5 88,P<0.01).Conclusion Remifentanil could efficiently assistant brachial plexus block in the operation of children with upper limb.
2.Influence of psychological intervention on life quality of patients with non-erosive reflux disease complicated with depression

Objective To study influence of psychological intervention on life quality of patients with non-erosive reflux disease(NERD) complicated with depression. Methods Patients with NERD and depression(95 cases) were divided randomly into the observation group(48 cases) and the control group(47 cases).The control group received oral administration of rabeprazole,10 mg,bid.The observation group was given psychological intervention based upon routine medication.Tbe reflux diagnostic questionnaire (RDQ), self-rating depression scale(SDS) and quality of life-index(QL-INDEX) scale were adopted for evaluation before and 8 weeks after treatment. Results The score of SDS was (41.58±6.85) in the observation group,which was lower than that of the control group,(50.42±9.26), after 8 weeks of treatment (P<0.01). The score of QL-INDEX was (8.56±1.38) in the observation group,which was higher than that of the con-trol group,(7.98±1.42), after 8 weeks of treatment (P<0.05). Conclusions Psychological intervention for patients with non-erosive reflux disease (NERD) complicated with depression can ameliorate their de-pression state and improve their life quality.
3.Effect of psychological behavior intervention on patients with functional constipation

Objective To investigate the effect of the psychological behavior intervention on the mental status and clinical symptom of patients with functional constipation. Methods All of 78 patients with functional constipation diagnosed by Rome m diagnostic code were randomly divided into study group (n =40) and control group (n = 38). The psychological behavior intervention was used in the study group, the caccagogue and symptomatic treatment was used in the control group. The self-rating anxiety scale(SAS),self-rating depressive scale (SDS) was analyzed and the improving of clinical symptom, adverse effect was compared before and after 4 weeks of treatment respectively. Results The scores of SAS and SDS in the study group significantly decreased after treatment [ (36.25±8.61 )scores vs (42.97±9.25 )scores, P<0.01 ;(38.81±6.53)scores vs (43.36±7.18 )scores ,P < 0.01].There was significant difference between the two groups after treatment (P <0.05).The effective rate of study group was higher than that of control group .(97.50% vs 84.21%,P <0.05). The rate of celiodynia, diarrhea and the recrudescence after the course of treatment in control group was 23.68%, 15.79% and 89.47% respectively, while this numerus in study group was 5.00%,2.50% and 5.00%. Conclusions Psychological behavior intervention plays an important role in the treatment of functional constipation. It can improve the mental status, clinical symptom, and decrease the adverse effect.
4.Endoscopic ultrasonography submucosal resection for treatment of gastric muscularis propria mesenchymal tumor

Objective To observe the efficacy and safety of endoscopic ultrasonography submucosal resection for treatment of gastric muscularis propria mesenchymal tumor.Methods Endoscopic ultrasonography preliminary diagnosis of 19 cases were diagnosed stromal tumors originating from gastric muscularis propria layer,the endoscopic ultrasonography diagnosis the source,echo,size,blood supply,etc.than treated by endoscopic submucosal excavation.To observe the therapeutic effect and complications.Results Eighteen patients were the full excavation,1 patient was failed.Intraoperative perforation was seen in 2 cases,the wound was healed by the titanium clamp.A small amount of bleeding happened in operation and bleeding was stopped by hot biopsy forceps or argon coagulation.No delayed bleeding happened.Conclusion Submucosal resection for gastric muscularis propria mesenchymal tumors is safe and effective,endoscopic ultrasonography has important value in the strict assessment.
5.The values of the new American Diabetes Association Diagnostic Criteria for screening of pre-diabetes and diabetes in patients undergoing elective coronary angiography

Objective Glycosylated hemoglobin A1c (HbA1c) has become one of key components in new American Diabetes Association (ADA) diagnostic criteria for pre-diabetes and diabetes,which is not included in the World Health Organization (WHO) 1999 criteria.Thus,we aimed to compare the two criteria in diagnosis of pre-diabetes and diabetes in patients with non-acute coronary syndrome (non-ACS) in Chinese.Methods Non-ACS patients who had undergone elective coronary angiography (CAG) in Peking Union Medical College Hospital without previously known diabetes were enrolled consecutively from October 2013 to April 2014.Fasting plasma glucose (FPG),2-hour plasma glucose after a 75 g oral glucose load (2hPG) and HbA1c levels before CAG were measured.Both WHO and new ADA criteria were applied for pre-diabetes and diabetes screening.Results One hundred and thirty-nine patients were included,aged (61.9 ± 10.1) years and 88(63.3%) were men among them.According to WHO 1999 criteria,34 patients (24.4%) were normal glucose regulation,57 (41.0%) pre-diabetes (including one impaired fasting glucose and 56 impaired glucose tolerance),and 48 (34.5%) newly diagnosed diabetes mellitus.ADA criteria identified 12 more patients with pre-diabetes and eight more with diabetes than WHO criteria.Compared with those diagnosed by both criteria,the proportion of women was higher in diabetic patients diagnosed with HbA1c alone.They also had lower hemoglobin [(124.88 ± 17.74) g/L vs (140.15 ± 15.17) g/L,P =0.013] and higher fibrinogen levels [(3.47 ± 0.41) g/L vs (3.02 ± 0.68) g/L,P =0.024] than those diagnosed by both criteria.Conclusion Compared with WHO 1999,new ADA criteria with HbA1c assessment are able to identify more previously unknown pre-diabetes and diabetes patients.Measurement of HbA1c might be needed to carry out routinely for screening patients with glucose metabolism abnormalities before elective CAG.
6.Expressions and clinical significances of microRNA-126 and vascular endothelial growth factor in gastric carcinoma

Objective To observe the expressions of microRNA-126 (miR-126) and vascular endothelial growth factor (VEGF) in gastric carcinoma tissues,and their associations with clinicopathologic features in gastric carcinoma.Methods Fifty cases of gastric carcinoma and matched tumor adjacent tissue specimens were collected from January 2012 to March 2014 in Shangqiu First People's Hospital of He' nan Province.Expression levels of miR-126 and VEGF were examined using reverse transcriptase-polymerase chain reaction.Their correlations and the relationships between them and the clinicopathologic features in gastric carcinoma were analyzed.Results The expression levels of miR-126 in the tumor tissues and in the adjacent tissues were 0.652 ± 0.102 and 0.379 ± 0.069,and the expression levels of VEGF in the two kinds of tissues were 1.523 ± 0.142 and 0.604 ± 0.152,with statistical differences (t =8.374,P =0.001;t =29.508,P =0.000).The expressions of miR-126 and VEGF were significantly positively correlated (r =0.735,P =0.001).The expressions of miR-126 and VEGF were associated with TNM stage (t =22.981,P =0.001;t =18.053,P =0.001),depth of tumor invasion (t =26.792,P =0.001;t =20.234,P =0.001) and tumor node metastasis (t=26.668,P=0.001;t=14.857,P=0.001).Conclusion miR-126 is high expressed in the gastric carcinoma tissue,which may up-regulate the expression of VEGF,and lead to the formation of new blood vessels,then to promote gastric carcinogenesis and the development process.
7.Substance P effects on aggregation and differentiation control of epidermal stem cells and skin wound repair

BACKGROUND:Epidermal stem cels are the source of cels for repairing the skin and its appendages. To study differentiation and migration of epidermal stem cels is crucial for perfect healing.
Objective: To investigate the effects of substance P in skin wound repair.
METHODS:Thirty Wistar rats were randomized into groups of substance P, capsaicin and control (n=10 per group). After the establishment of skin defect models, exogenous neuropeptide substance P, capsaicin, and normal saline were daily injected at the wound for continuous 5 days. Skin thickness, skin wound area, BrdU-positive cels were detected in the three groups at different time after injection.
Results and conclusions:No difference n the skin thickness was found in the three groups at 14 days after injection (P > 0.05), but with time, the epidermal thickness was reduced. At 42 days, the epidermal thickness was reduced remarkably in the substance P group and decreased certainly in the capsaicin group, but unchanged in the control group. The wound area of the substance P group was significantly lower than that in the control and capsaicin groups at 3, 5, 7 days after injection (P < 0.05), but there was no difference between the control and capsaicin group (P > 0.05). The BrdU positive rates in the three groups were significantly different at 3, 5, 7, 12 days after injection: the substance P group > the control group > the capsaicin group (P < 0.05). These results show that substance P can promote epidermal cel proliferation and accelerate wound healing.
8.Skin composite construction using adipose tissue-derived stem cells for wound healing

BACKGROUND:There is no clear understanding on the effects of subcutaneous fat and stem cels on wound healing. OBJECTIVE:To explore the therapeutic effects of skin composite prepared with adipose tissue-derived stem
cels on skin defects. METHODS: Epidermal cels, fibroblasts, adipose tissue-derived stem cels as seed cels and bovine colagen gel as a scaffold were used to build a complex with a variety of cels. A 6-mm diameter circular skin defect was made on the both sides of the rat back. The right side as experimental side was implanted with an 8-mm diameter multilayer skin composite, and the left side (control side) was only treated with a simple dressing. RESULTS AND CONCLUSION: For the constructed multi-layer skin composite, the epidermal layer was continuously merged into the multi-layer, the fibroblasts evenly distributed in the corium layer, and lipid droplets existed in the fat layer in which the cels distributed uniformly. Cel aggregation was obviously observed at the junction of different layers. In the experimental side, the rate of wound healing, granulation tissue thickness, the thickness of dermis and the capilary density were significantly higher than those in the control side. Taken together, we can construct multilayer skin composites with a variety of cels as seed cels, such as epidermal cels, fibroblasts and adipose tissue-derived stem cels, and bovine colagen gel as a scaffold, which promote wound healing and increase the thickness of dermis.

Objective To study the factors effecting on flow cytometric sperm chromatin structure assay.Methods The SCSA explores the metachromatic properties of acridine orange (AO) and flow cytometry to monitor the susceptibility of sperm chromatin DNA to acid induced denaturation in situ because of the low pH treatment. Results COMP?t, ?t, SD?t value were different upon on the storage methods (①sperm was preserved in 4℃, ②sperm was cryopreservated after dilution ③sperm was cryopreservated). 10.Gene analysis of new isolates HA of influenza A (H1N1) virus

Objective To obtain genetic characterization of the HA1 of new isolates of influenza A (H1N1) virus. Methods Virus was amplified in embryonated chicken eggs, the virion DNA was transcribed into cDNA by reverse transcriptase, cDNA amplified by PCR, the products of PCR were purified. Afterward, DNA sequence analysis was performed by the dideoxy-mediated chain termination method, using synthetic oligo nucleotide primers. Results The HA1 domain of new isolates of influenza A (H1N1) virus showed that their HA1 genes were 981 nucleotide in length coding for a HA1 protein with 327 amino acids, deletion of a glycosylation site and an amino acid. The homology of amino acid sequences of protein molecules on HA1 domains of new influenza A virus when compared with A/Guifang/10/94(H1N1) and A/Bayern /07/95 (H1N1) viruses,was 92.8% and 91.3% respectively; The homology of amino acid sequences of protein molecules on HA1 domains of A/Jingjun/11/98(H1N1) when compared with A/Jingjun/13/98(H1N1) or A/Jingjun/807/97(H1N1) viruses,was as high as 98%; The homology of amino acid sequences of protein molecules on HA1 domains of A/Guifang/10/94(H1N1) when compared with A/Bayern/07/95(H1N1) viruses was as high as 96%. Conclusions The HA1 gene of new H1N1 virus strains is different from those of A/Guifang/10/94 (H1N1) and A/Bayern/07/95(H1N1), they will probably be new mutation strains.
