1.Research in correlation of the occurrence of nosocomial infection and nursing management in operation room for general surgery patients

ObjectiveTo investigate the related factors of the occurrence of nosocomial infection and nursing management in operating room for general surgery patients,and provide a scientific basis to reduce the incidence of nosocomial infection of the operating room. Methods304 patients with nosocomial infection from August 2009 to August 2010 in our hospital for surgery were set as the control group.151 cases from September 2010 to October 2011 were named as the intervention group.All cases were analyzed retrospectively.The nursing factors which might affect nosocomial infection in operation room were analyzed by univariate and multivariate logistic regression analysis. ResultsBy univariate analysis,eight operation room nursing factors related to nosocomial infection were screened out:the type of incision,operation time,the following surgery or not,into ICU time,preoperative use of antibiotics,visitors of operations or not,invasive operation.5 highly correlated factors were screened out by multi-factor logistic regression analysis:admission to the ICU time,incision type,operation time,preoperative use of antibiotics,then following surgery.The infection rate and prolapse rate of the control group were significantly higher than those of the intervention group. ConclusionsIn nursing work of the operation room,we can take a series of nursing management interventions to reduce the incidence of nosocomial infection of general surgery.
2.Bushen Huoxue Huayu Recipe up-regulates expression of insulin receptor substrate-1 and phosphorylation of tyrosine in adipose tissue in androgen sterilized rats

OBJECTIVE: To investigate the expression of insulin receptor substrate-1 (IRS-1) and phosphorylation of tyrosine in androgen sterilized rats (ASRs) and the effects of Bushen Huoxue Huayu Recipe (BHHR), a compound Chinese herbal medicine for tonifying kidney and activating blood, on the ASRs, and to analyze the possible mechanism. METHODS: ASR models were established by means of testosterone injected subcutaneously at the age of 9 days. The model rats were randomly divided into BHHR group (n=15) and untreated group (n=15); the rats in normal group (n=15) were injected with oil. The rats in the BHHR group were treated with BHHR for 30 days, and the rats in model group and normal group were treated with distilled water for 30 days. Body weight and area under the curve (AUC) of insulin were measured after treatment. Expression of IRS-1 and phosphorylation of tyrosine in adipose tissue in ASRs were analysed by immunohistochemical method. RESULTS: The body weight and insulin AUC in the untreated group were increased and more than those in the BHHR group and the normal control group (P<0.05). Expression of IRS-1 and phosphorylation of tyrosine in normal group were significantly higher than those in the untreated group (P<0.05), and those in BHHR group were improved and significantly different from those in the untreated group (P<0.05). CONCLUSION: BHHR can increase the expression of IRS-1 and phosphorylation of tyrosine in adipose tissue in IR rats, which may be one of its mechanisms in improving insulin sensitivity of the target tissues.

Mesenchymal stem cells (MSCs) have good immune regulatory function,can inhibit many immune cell proliferation,direct effects on activation and proliferation of T cell,play a role in immune regulation by Treg cells or by the secretion of soluble factors regulating Thl/Th2 secretion and reaction equilibrium,inhibit the inflammation through the anti-inflammatory,regulation of cytokines expression at the same time,so reduce the expression levels of matrix metalloproteinase-2 and-9,which may promote lacrimal gland tissue damage,and thus play a role in immune regulation.MSCs can reduce the autoimmune dry eye clinical index,recovery secretion function of part lacrimal gland.This article reviews the research advances in inmmmne regulation of MSCs on autoimmune dry eye.
5.Effects of mesenchymal stem cells on pathological changes and cytokines expression in rabbit model of autoimmune dry eye

Objective To establish the model of autoimmune dry eye and analyze the level of cytokines and the pathological changes of autoimmune dry eye rabbits treated with human umbilical cord mesenchymal stem cells (MSCs).Methods Twentyfour female New Zealand rabbits were randomly divided into three groups:Normal control group,dry eye group and MSCs treatment group,8 cases in each group.After treated with MSCs for six weeks,the pathological changes were observed,and the expression of Th1 and Th17 cell differentiation related cytokines mRNA in lacrimal gland tissue was detected by real-time quantitative PCR.The percentage of regulatory T cells (CD4 +Foxp3 + cells) in the lacrimal gland and spleen tissues of the dry eye model group and MSCs treatment group was measured by flow cytometry.Results After treatment with MSCs for 6 weeks,HE staining from each rabbit lacrimal gland showed that,the normal control group showed no or only a few lymphocytes;The lacrimal gland cells were atrophied in the dry eye group,the distribution of lymphocyte scattered around the gland and small blood vessels;Compared with dry eye group,the lacrimal gland lymphocyte infiltration reduced in MSCs treatment group,and the cell morphology were better.Compared with dry eye group,lymphocyte infiltration and aggregation in the conjunctival conjunctiva in MSCs treatment group were significantly reduced,the epithelial structure was intact,and the degeneration and atrophy cells were rare.The expression of inflammatory factors including interferon-γ(IFN-γ),Interleukin-17 (IL-17) and transcription factor T-bet mRNA were decreased in MSCs treatment group,there were significant difference compared with dry eye group (all P < 0.05);Thl7 cell associated cytokines IL-17 expression levels were decreased,but no significant difference was found between two groups (P > 0.05);The expression of transcription factor RORrt mRNA in dry eye group was significantly decreased,the difference was statistically significant (P < 0.05).The expression of inflammatory factors TNF-α in the lacrimal gland tissue of MSCs treatment group was significantly lower than that in the dry eye model group,while the expression of the anti-inflammatory factor TGF-β was significantly higher than that in the dry eye model group,and the differences were statistically significant (all P<0.05).The regulatory T cells (CD4 + Foxp3 + cells) in the lacrimal gland of dry eye model group accounted for 10% of lymphocytes was,while the treatment group accounted for 27.8%,MSCs group was significantly higher than dry eye group,the difference was statistically significant (P < 0.05).Conclusion MSCs can reduce the histopathological changes of the immune dry eye,and they may have immunoregulatory effects on autoimmune dry eye.The mechanisms of immunomodulatory effects may be related to the balance of inflammatory cytokines and anti-inflammatory cytokines.

7.Effect of Dahuang Zhechong pills on arterial thrombosis in rabbits.

OBJECTIVE:
To investigate the effect of Dahuang Zhechong pills (DZ) on arterial thrombotic model in vivo.
METHODS:
Sixty-five rabbits were randomly divided into 7 groups: normal, model (collagen encapsulated thread-drawing),model+aspirin (ASA), model+clopidogrel (CP),model+ASA+CP, model+ low dosage DZ (DZL), and model+high dosage DZ (DZH). All rabbits except the normal group were fed with the drugs repectively for 8 days,and sacrificed at 2 hours after the last feeding, obtained aortae. The pathological changes in the aortae were observed under microscope,and the level of FDP, D-dimer and tissue factor (TF) were measured by enzyme-linked immunosorbent assay (ELISA).
RESULTS:
The vascular vessels were filled with thrombi in the model group and the elastic membranes of the vessel wall were seriously injured. The arterial thrombi were observed around the vascular wall in the DZL group, but some of the thrombi were dissolved. The number of thrombi was remarkably decreased in the DZH group, and most thrombi were dissolved and the vascular intimal membranes were intact. Compared with the model group, the dry and wet weight of the thrombi and the level of D-dimer, FDP, and TF in the plasma were significantly attenuated (P<0.01) in all the treatment groups. There were no significant difference between the DZL group and the ASA group in the dry weight, D-Dimer, and FDP (P>0.05). The pathological changes in the vascular vessel and the elevation of plasma parameters in the DZL group were similar to those in the ASA and CP groups (P>0.05). The dry and wet weight, D-dimer, FDP, and TF in the plasma in the DZH group were significantly lower than those in the DZL group (P<0.01 or P<0.05, separatively), and closed to those in the ASA+CP group.
CONCLUSION
Dahuang Zhechong pills are potential novel anti-thromobotic agent for arterial thrombosis.
