3.A study on the adverse effects of extracorporeal shockwave lithotripsy (ESWL) for renal stones

Objective To study the reason and the preventive measures that adverse effects of extracorporeal shockwave lithotripsy (ESWL) for renal stones during treatment.Methods From April 2015 to January 2016,165 patients who underwent ESWL treatment for renal stones were enrolled this retrospective study.All patients were in supine position.Direction of shockwave was by the path of preabdomen.The adverse effects during treatment were observed.The related factors were analyzed by Logistic regression analysis.Results There were 70 cases(42.4%) with pain,10 cases(6.1%) with cold sweat,8 cases (4.8%) with accelerated heart rate,5 cases (3.0%)with descent of heart rate,2 cases (1.2%) with nausea and vomiting.In the univariate Logistic regression analysis,four variables (BMI,history of ESWL treatment,stone location and energy) were found showing statistical significance.Multivariate Logistic regression analysis showed that the final significant related factors were BMI < 24.0 kg/m2 (OR =4.155,CI 1.373-12.571),history of ESWL treatment (OR =0.163,CI 0.058-0.454),stone in pelvis(OR =123.051,CI 17.638-858.439),stone in middle calyceal (OR =169.171,CI 29.677-964.343),energy 86.3-94.7J (OR =0.094,CI 0.025-0.362).Conclusions Pain was the most common adverse effect during the treatment of ESWL.Stone location (pelvis and middle calyceal) was an independent risk factor,BMI < 24.0 kg/m2 was one of the related risk factors.Both energy 86.3-94.7J and history of ESWL treatment were preventive factors.
4.The follow-up study of image guided 125I seed permanent brachytherapy for the treatment of unresectable pancreatic cancer

Objective To explore the clinical efficacy of image-guided 125I radioactive seed interstitial implantation therapy for unresectable pancreatic cancer.Methods 25 patients with unresectable pancreatic cancer evaluated by retrospective follow-up were enrolled in this study,13 patients received radioactive seeds implantation while 12 patients were given non-surgical treatment.We observe and compare the clinical benefits,objective curative effect,complications,adverse reaction,survival between the two groups of patients.Results Compared with the non-surgical treatment group,the clinical benefit rate in the radiotherapy seed implantation group was 92% (12/13) while that of the non-surgical group was 42% (5/12),the difference was of statistically significance.The numbers of cases evaluated as effective were 6 (46%) and 4 (33 %) respectively,the difference was not statistical significant (x2 =0.427,P > 0.05);The radioactive seed implantation group had no serious postoperative complications;3 cases who received subsequent chemotherapy in radioactive seed implantation group(23%,3/13) and 3 cases in non-surgical treatment group(25%,3/12)suffered from serious adverse reactions,the difference was of no statistical significance(x2 =0.013,P >0.05);Comparing the survival rate between the two groups,x2 =0.001,P =0.969,the difference was of no statistical significance.Conclusions The therapy of 125I radioactive seed implantation for unresectable pancreatic cancer significantly relieves cancer caused pain and improves quality of life.

By improving the computer routine inspection tour in practice, we can enhance the integrated benefit of the computer quarter inspection tour. As an effective approach of the network equipment preventive repair and mainte- nance, after the computer quarter inspection tour system was optimized and improved, it got the end-users and hospital or- ganization' high attention and full affirmation for the good integrated benefit.

7.Investigation on injury of liver and kidney among the workers exposed to terephthalic acid, ethylene glycol and(or) dowtherm A.

OBJECTIVETo study injury of liver and kidney among the workers exposed to terephthalic acid(TPA), ethylene glycol(EG) and(or) dowtherm A(DOW), and research for early biological monitoring indexes.
METHODSBy using the method of occupational epidemiology, an investigation of industrial hygiene in a chemical fibre corporation was carried out and the changes of the liver and kidney functions were analyzed among the workers who had been exposed to TPA, EG, DOW.
RESULTSThe values of serum gamma-glutamyl traspetidase(GGT) and total bile acid(TBA) in TPA + EG + DOW group men were (35.45 +/- 16.09) U/L, (10.29 +/- 6.76) mumol/L respectively and the values of serum alanine transaminase(ALT) and TBA in TPA + EG + DOW group women were(30.68 +/- 8.58) U/L, (9.53 +/- 6.63) mumol/L respectively, significantly higher than those in TPA, DOW and control groups(P < 0.05, P < 0.01). Compared with TPA, DOW and control groups, the values of urine N-acetyl-beta-D-glucosaminidase(NAG) and beta 2-2-microglobulim (beta 2-MG) in TPA + EG + DOW group of both men and women increased significantly(P < 0.05, P < 0.01), with(5.68 +/- 4.01) U/mmol Cr and (23.49 +/- 13.44) mg/mol Cr, and(6.68 +/- 4.68) U/mmol Cr and (22.80 +/- 13.00) mg/mol Cr, respectively. Analysis of regression indicated that both liver and renal injuries of the workers were evidently correlated with their exposure to TPA, EG and DOW after adjustment for the confounding factors such as sex, smoking, drinking, etc(P < 0.001).
CONCLUSIONBased on available knowledge, it is reasonable to assume that the joint actions should be considered on the injury of liver and kidney caused by TPA, EG and(or) DOW among the workers. Serum ALT, GGT, TBA, urine NAG and beta 2-MG should be suggested as biomarkers for liver and kidney damage.

8.Comparison of measurement results of citric acid, oxalic acid, and phosphoric acid in 24-hour urine preserved with thymol and concentrated hydrochloric acid

Objective:To compare the differences in the results of 24-hour urine citric acid, oxalic acid, and phosphoric acid detection between thymol and concentrated hydrochloric acid preserved urine.Methods:Urine samples were collected from 50 patients with upper urinary tract stones who were admitted to the Second Affiliated Hospital of Zhengzhou University from July 2023 to August 2023. There were 38 males and 12 females, with the age of (45.52 ± 13.68) years old. On the second day of admission, patients were required to empty their bladders in the morning and collect urine over a 24-hour period. The urine was evenly divided and stored in two sealed plastic containers. In one container, concentrated hydrochloric acid (15 ml, concentration of 6 mmol/L) was added as a preservative, forming the hydrochloric acid group. In the other container, 2 g of thymol was added as a preservative, forming the thymol group. The concentration of oxalic acid, citric acid, and phosphoric acid in urine were measured, and the measurement results were multiplied by the total amount of urine to obtain the final measurement results. The differences in the measurement results of citric acid, oxalic acid, and phosphoric acid between two groups were compared.Results:In the concentrated hydrochloric acid group, the median value of citric acid was 160.87(95.37, 245.61) mg/24h, the median value of oxalic acid was 33.09(26.32, 39.37) mg/24h, and the mean value of phosphoric acid was (731.06 ± 240.62) mg/24h. In the thymol group, the median value of citric acid was 340.40(166.56, 561.77) mg/24h, the median value of oxalic acid was 33.91(26.51, 44.56) mg/24h, and the mean value of phosphoric acid was (759.07 ± 273.23) mg/24h. Compared with the thymol group, the citric acid level in the concentrated hydrochloric acid group was significantly lower ( Z=4.31, P< 0.001), but there was no significant difference in oxalic acid ( Z=0.85, P=0.393) and phosphoric acid ( t=1.65, P=0.106) between the two groups. Conclusions:Compared with thymol, the results of 24 h urine citric acid with concentrated hydrochloric acid as preservative is lower, but there is no significant difference between oxalic acid and phosphoric acid.The use of thymol as a preservative may be more suitable than concentrated hydrochloric acid for the detection of citric acid, oxalic acid and phosphoric acid in 24-hour urine metabolism analysis.
