1.Comparative analysis of immunological detection results in patients with POEMS syndrome

Objective To investigate the immunological feature, and to provide evidence for di-agnosis and treatment of POEMS syndrome. Methods The results detected with serum protein elec-trophoresis (SPE), immunofixation (IFE) and immunoglobulin quantitative assay were retrospectively compared and analyzed in 27 patients with POEMS syndrome. Results For 27 cases of POEMS syn-drome, the detection rate of M protein was significantly higher detected with IFE than that with SPE. Among M protein, IgA was much more frequent than IgG (16 cases vs 5 cases), and all were typeλ. Besides, there was statistical difference in SPE results between 16 cases of POEMS syndrome with M protein positive (detected with SPE) and healthy controls. Conclusion Negative M protein result can't exclude the diagnosis of POEMS syndrome. Further evidence should be searched for, and the clinical data should be emphasized
2.Diagnostic value of combined detection of serum NGAL,CysC,RBP and MMP-2 in diabetic nephropathy

Objective To investigate the value of the combined detection of NGAL ,CysC ,RBP and MMP‐2 in the diagnosis of type 2 diabetic nephropathy (T2DN) .Methods A total of 149 cases of type 2 diabetes mellitus(T2DM ) were divided into three groups according to the urine protein detection results :normal proteinuria group (NA group) ,micro proteinuria group (MA group) and clinical nephropathy group (CN group) .Contemporaneous 50 individuals undergoing healthy physical examination were selected as the control group (NC group) .The NGAL ,CysC ,RBP and MMP‐2 levels were analyzed and compared among 4 groups .Results The serum NGAL ,CysC and RBP levels in the NA group ,MA group and CN group were significantly higher than in the NC group (P<0 .05 or P<0 .01) ,while the MMP‐2 level was significantly lower than that in the NC group (P<0 .01) .The positive rate of combined detection of NGAL ,CysC ,RBP and MMP‐2 in the NA group ,MA group and CN group were 66 .0% ,85 .7% and 92% respectively ,which were higher than those of single detection (P<0 .05) .NGAL was positively correlated with CysC and RBP (P= 0 .05) and negatively correlated with MMP‐2 (P<0 .05) .Conclusion The serum levels of NGAL ,CysC ,RBP and MMP‐2 in the DN patients are significantly increased ,while the MMP‐2 level is significantlt decreased .Their combined detection can be used as an early monitoring and evaluation index of DN .
3.Study of the correlation between different upper limb abduction angles and the occurrence of malposition of PICC into internal jugular vein

Objective To investigate the effects of different upper limb abduction angles on the occurrence of malposition of PICC into internal jugular vein. Methods Totally 210 cases of patients treated with PICCs for unilateral breast cancer of our hospital from August,2015 to January,2017 were randomly assigned to three groups with 70 cases in each group. The abduction angle of the upper limb for placement was set at 45° ,90° and 160° , respectively. We chose the basilic vein of the uninfected arm using modified Seldinger technique under the guidance of ultrasound for PICC,and malposition was confirmed by detecting the tip of PICC in internal jugular vein. Results The incidence rate of internal jugular venous dislocation in 45° group was 7.14%(5 cases) and 8.57% in 90° group (6 cases),and no internal jugular venous dislocation in 160° group. There was no statistically significant difference among 45°,90° and 160° groups(χ2=5.95,P>0.05). Significant differences of group comparisons for 45° v.s. 160° and 160° v.s. 90° were found (P<0.05). There was no statistically significant difference between 45° and 90° groups (P>0.05). Conclusion Abduction angles of 45° ,90° and 160° can all be used for PICC placement. The abduction angle can be selected according to specific situation of patients instead of being limited to the standard abduction angle of 90°.
4.Establishment of a rabbit model of heart failure by aortic regurgitation

Objective To establish a New Zealand rabbit model of heart failure by aortic regurgitation.Methods Adapting catheterization-induced aortic regurgitation to establish a volume overloat rabbit model of heart failure.The SBP, LVSP, LVDP, LV+dp/dt and LV ±dp/dt were observed before and after modeling.The successful criteria of heart failure:the LV ±dp/dtmax was decreased more than 40%and the LVDP increased more than 40%, or the LV ±dp/dtmax fell down to less than 40%and the DBP should be decrease more than 40%.Evaluating the model by observing the coat color, mental status, physical activity, calculating the feed consumption index, weight gain index, heart rate, respiration frequency and other indicators.The activity of serum SOD and MDA concentration were assayed to determine the antioxidant capacity of the model animals.Enzyme-linked immunosorbent assay kit was used to detect the serum cAMP and cGMP con-centration.Gene chip technology was used to analyze the difference of gene expression.Results After modeling, the he-modynamic index of SBP, DBP and LVSP were significantly decreased, LVDP was significantly decreased, LVDP was sig-nificantly increased and the LV+dp/dt and LV ±dp/dt were significantly decreased.Compared with the normal control group, the model animals showed coat withered, less movement, less eating, unresponsiveness, listlessness, and reduced grab resistance after modeling.The respiratory rate of the model group was significantly increased, and this trend was in-creased over time.The serum SOD activity was lower, MDA concentration was higher, cAMP concentration was lower, and cGMP concentration was higher in the model group.665 differentially expressed genes were detected.Compared with the human gene sequences, 16 characteristic genes were obtained.In these 16 genes, which were closely related to heart func-tion, were mainly related to ion channels, muscle contraction, and signal transduction function.Conclusions This repor-ted method to establish rabbit model of heart failure by using aortic regurgitation is reliable.The aortic regurgitation increa-ses cardiac preload, than leads to an increase of the left ventricular end-diastolic volume, and finally results in left ventric-ular hypertrophy and heart failure.The results of myocardial tissue gene chip test show that there are some changes in gene expression of the model rabbits.
5.Clinical Significance of Combined Detection of Serum Specific Autoanti-bodies in Systemic Lupus Erythematosus

Objective To explore the clinical significance of combined detection of anti-nucleosome(AnuA),anti-SM,anti-double-stranded DNA(ds-DNA),anti-ribosomal P protein(ARPA)antibodies in systemic lupus erythematosus(SLE). Methods 186 SLE patients and 89 rheumatic diseases patients were studied. Latter included. Results Tour kinds of masculine gender rates(86.5%)of antibody consociation ex-aminations were high to examine in the any and single item antibody in the SLE(62.7%, 50.0%, 28.4%, 15.7%)and were 9.0%, 11.2%,5.6%, 2.2% respectively in control groups. They were markedly higher than those of control groups(P< 0.05). The sero-positive rate of ds-DNA an-tibody was markedly higher than anti-SM antibody and ARPA in SLE patients with negative AnuA(P< 0.05). The sero-positive rate of AnuA was markedly higher than other two antibody in SLE patients with negative ds-DNA antibody ,anti-SM, antibody and ARPA respectively(P< 0.05). Conclusion Combination of AnuA,anti-SM,ds-DNA and ARPA antibodies can significantly improve the precision in diagnosis of SLE.
