1.The clinical efficacy of sacral hiatus epidural blocking combined with three-dimensional rapid traction for the treatment of lumbar disc herniation

Objective To observe the clinical efficacy of three-dimensional rapid traction combined with sacral hiatus epidural blocking for the treatment of lumbar disc herniation(LDH). Methods A totsl of 450 patients with LDH were divided into three groups.Group A received conventional slow traction,group B received three-dimensional rapid traction,and group C received sacral hiatus injections for epidural blocking combined with three-dimensional rapid traction.The Japanese Orthopadedic Association(JOA)scale for lumbar disease and a visual analogue scale(VAS)were used to evaluate the treatment effect and the patients'subsequent quality of life.All of the patients were followed up for 1 year and evaluated using standard clinical evaluations. Results The average VAS scores and JOA scores of the three groups were not significantly different before treatment,and all had significantly improved after treatment.The overall effectiveness rates were 63.33% for group A,90.67%for group B and 94% for group C,and these differences were all statistically significant.After one year the recurrence rates in the A,B and C groups were 20.7%,14.0%and 8.7% respectively,and these differences were again statistically significant.Conclusion Sacral hiatus epidural blocking combined with three-dimensional rapid traction showed good therapeutic effect and safety for LDH patients and was superior to simple rapid traction or slow traction.
2.Discussion of the establishment of the mechanism for innovation in hospitals

With the gradual deepening of the reform in the medical and health system, reform and innovation inside hospitals have come to carry increasingly great weight. In light of their hospital's practical experience and proceeding from factors that affect innovation, the process of innovation and the links of innovation, the authors put forward ways for hospital innovation and the principles that ought to be adhered to. They argue that in striving for innovation, hospitals should first of all give priority to the building of the contingents of department heads and young and middle aged key professional and technological members, set forth an advanced operating strategy and goal, highlight innovation through science and technology, and create a sound environment for innovation. On the basis of establishing an effective mechanism for innovation, hospitals ought to stick to the principles of humanistic management, close attention to the actual conditions and objective evaluation.
3. Effects of surfactants on water solubility and anti-hepatic fibrosis of curcuminoids

Objective To increase the water solubility of curcumin constituents by three kinds of food grade surfactants, and to study the effect of surfactants on the quality standard and stability and anti-hepatic fibrosis in vivo of curcumin solution. Methods The high-speed dispersion method was used to prepare the solution of curcuminoids-surfactants. The qualitative analysis of curcuminoids was confirmed by LC-MS/MS. HPLC-DAD fingerprinting was established and quantitative analysis was conducted. And the effects on anti-liver fibrosis of different concentrations curcumin ratio solution were evaluated by rat model. Results The maximum water solubility of curcumin constituents was 1.387 mg/mL. LC-MS/MS analysis showed that the structure of curcuminoids were stable; The results of fingerprinting analysis demonstrated that the similarity of 18 batches of samples were higher than 99%. The contents of curcumin, demethoxycurcumin and bisdemethoxycurcumin in different batches of samples were stable with the RSD of 1.60%, 2.24%, and 3.74%. The rat model of hepatic fibrosis showed that the contents of AST and ALT in free curcumin suspension group were (64.1 ± 20.3) and (45.1 ± 13.9) U/L, respectively, while that solution of curcuminoids-surfactants group was (19.4 ± 8.7) and (11.8 ± 4.9) U/L. Conclusion The solubility of curcuminoids was increased 517 times compared with the free state (2.677 μg/mL) after solubilization with surfactants, and repeated tests indicated that the structure and contents of curcuminoids were stable; the solubilized curcuminoids exhibited better anti-liver fibrosis effects than the free.
4.PROTECTIVE MEASURES TO PREVENT HEALTH CARE WORKERS FROM CONTRACTING SEVERE ACUTE RESPIRATORY SYNDROME(SARS)

Our objective is to prevent doctors and nurses who worked on SARS ward from contracting this highly communicable disease through observing strict isolation strategies. The following measures have been taken in our hospitals. The wards for SARS patients were remodeled according to the isolation requirements for infectious diseases. They were separated into three quarters, namely clean area, semi-contamination area and contamination area. Two buffer zones were set up to separate the three quarters. Four specific passages were separately allotted to health care staffs, patients, soiled materials disposal and handling and dispatch of specimens, to avoid cross infection by contact. According to the criteria for isolation of serial A respiratory infectious diseases, every health care worker was strictly requested to wear and take off isolation garments properly when they entered and left the ward. Most importantly, specific garbs for different areas were essential for efficient isolation. Respiratory tract, skin and mucous surfaces should be effectively protected on the ward. In addition, doctors and nurses should wear special masks or helmets and waterproof suit during the performance of intubation, CPR or operation. Direction of air flow in SARS ward was diverted away from the clean area by using ventilator in order to prevent air-borne contamination. The air outlet of SARS ward was efficiently filtered to cut down pollution of the environment. Living quarters of medical staff working on SARS ward were isolated with a safe distance from the ward. Their health condition was closely monitored daily. After working in SARS ward for four weeks, they should enjoy a special vacation to assure their good health condition. In the past spring, 882 SARS patients were admitted to both the 309 hospital and Xiao Tanghan hospital. 1 632 health care workers worked for those SARS paitents by turns. To our satisfaction, none of our staffs was infected by SARS. Our experiences indicated that health care workers on the SARS ward could be effectively protected from contracting SARS infection by taking efficient and strictly observed protective measures.
5.Early Analgesic Effect of Continuous Femoral Nerve Block for Total Knee Arthroplasty

Objective To compare the early analgesic effects of continuous femoral nerve block analgesia (CFNB) and patient controlled intravenous analgesia (PCIA) and their effects on functional recovery after total knee arthroplasty.Methods Retrospective analysis was conducted on 60 ASA Ⅰ and ASA Ⅱ patients with unilateral total knee arthroplasty in our hospital from January 2014 to January 2015.The patients were divided into two groups,CFBN group (treatment group) and PCIA group (control group),with 30 patients in each group.VAS scores of postoperative ipsilateral knee resting and movement pain,range of postoperative knee joint motion (ROM),muscle strength of femoral quadriceps,incidence rates of adverse reaction within 7 days after operation,HSS score 3 months before and after the operation,and discharge time were observed.Results Postoperative VIS score in CFNB group was lower than that in PCIA group.ROM and muscle strength of femoral quadriceps in CFBN group were better than those in PCIA group on day 1,2,3,5,and 7 after the operation.Postoperative adverse reaction rate within 7 days in PCIA group was significantly higher than that in CFNB group.HSS score three months after the operation in CFNB group was higher than that in PCIA group.Discharge time of CFNB group was significantly earlier than that of PCIA group (P<0.05).Conclusion Compared with PCIA analgesia,CFNB analgesia shows better analgesic effect.It can alleviate early knee joint pain after TKA,increase joint motion,significantly shorten the hospitalization time,promote joint exercises which are effective in functional rehabilitation,and decrease the incidence of adverse reactions like nausea and vomiting.
6.Expression of Apaf-1 and Caspase-9 in prostate cancer and its clinical significance

Purpose To investigate the expression of Apaf-1 and Caspase-9 in prostate cancer ( PCa) and benign prostatic hyperplasia ( BPH) , and to analyze their correlation with clinicopathological parameters of PCa. Methods Immunohistochemistry was used to de-tect Apaf-1 and Caspase-9 protein in 45 cases of PCa and 60 cases of BPH. Results The positive rate of Apaf-1 and Caspase-9 in PCa tissues was significantly lower than that in BPH (P<0. 05). The expression of Apaf-1 and Caspase-9 was not correlated with the age of patients and distant metastasis (P>0. 05), but it was correlated with the pathological grade and clinical stage of PCa (P<0. 05). Conclusion Apaf-1 and Caspase-9 are lowly expressed in PCa. There is positive correlation between the expression of Apaf-1 and Caspase-9 (rs =0. 645, P<0. 01). Apaf-1 and Caspase-9 might be correlated with the carcinogenesis and development of PCa.
7.Interventional therapy of deep venous thrombosis of lower limb after burn: initial experience in 13 cases

Objective To discuss the curative effect and specificity of interventional therapy for deep venous thrombosis (DVT) of lower limb in burned patients.Methods The clinical data of 13 patients with lower limb DVT after burn,including 7 males and 6 females with a median age of 46.1 years (37-67 years),were retrospectively analyzed.The causes of burn included flame burn (n=9),electric injury (n=2) and hydrothermal bum (n=2).The burned area was 1%-88% of the total body surface,with a mean of (37.08± 30.60) %.Lower limb DVT complicated by lower limb bum was observed in 11 patients,among them bum of both lower limbs was seen in 8 patients.Lower limb DVT associated with inhalation injury was found in 5 patients.Clinically,lower limb DVT was usually detected in 13-72 days after burn,with a mean of (38.69± 16.83) days.Interventional treatment was carried out in all 13 patients,and the curative effect was assessed.Results Technical success of interventional treatment was obtained in all 13 patients.Both inferior vena cava filter placement via right internal jugular vein approach (n=3) or via unaffected-side femoral vein approach (n=10) and anticoagulant therapy were conducted.Catheter-directed thrombolysis was employed in 7 patients,intravenous thrombolysis was adopted in 4 patients,and no thrombolysis therapy was used in 2 patients.No pulmonary embolism occurred.The curative effect rate of interventional treatment was 84.6%(11/ 13).Conclusion For the treatment of lower limb DVT after burn,interventional therapy is safe and reliable,but the selection of puncture site and the use or not use of indwelling catheter for thrombolysis should be carefully taken into consideration.
8.Type 2 diabetes in MKR transgenic mice cansed by skeletal muscle-specific double deficiency of IGF-1 and insulin receptors

Objective To examine the circulating glucose and body weight in the transgenic MKR mouse model who expressed dominant-negative IGF-1 receptor and insulin receptor in skeletal muscle leading to systemic insulin resistance and diabetes. Methods MKR mice were genotyped by PCR analysis of tail DNA.And in these mice we examined the circulating glucose and body weight once a week from 1 to 16 weeks of age, and the circulating insulin and glucose tolerance at age of two-month-old by using C57 mice as controls. Results The descendents of MKR mice kept hereditary feature. And these mice had hyperglycaemia from 3 weeks of age,and an increasing body weight slowly(P<0.01).Twenty-fold significant hyperinsulinemia was observed in MKR mice,and they were glucose intolerant in 2-month-old male and female (P<0.01).Conclusions The MKR mouse is an excellent model of type 2 diabetes
9.Transfection and expressiOn of green fluorescent protein gene in prostate carcinoma PC-3M cell line

Objective:To reseach the expression of green fluorescent protein(GFP)gene in prostate car-cinoma PC-3M cell line and the efffect of cell cycle.Methods:GFP gene recombination DNA was construct-ed and PC-3M cells were transfected with lipofectin regant.The expression of GFP was observed under in-vert fluorescence microscope.Meanwhile,the cell cycle of positive cell colon was assayed by flOw cytome-ter.Resuits:After transfection,about 10%~15%cells expressed green fluorescent protein and there wasno significant change compared the cell cycle of positive cell colon with parents cells.Conclusion:GFP sup-plys a new way in reseaching metabolic phenomenon of live cells.
10.Expression of dystrophin associated glycoprotein complex in Duchenne muscular dystrophy at different age groups

Objective To analyze the expression of dystrophin associated glycoprotein complex (DGC) in Duchnne muscular dystrophy (DMD) in different age groups. Methods The confirmed 24 DMD patients were divided into 3 groups according to their chronological age (children whose age between 1 to 3 were assigned to early childhood group; children whose age between 3 to 6 were assigned to preschool group; children whose age between 6 to 13 were assigned to school-age group). Several proteins from sarcolemmal muscle membrane were analyzed by immunohistocbemistry. Results The primary loss of dystrophin may lead to a secondary or completely deficiency of α-SG, β-SG, γ-SG,δ-SG, β-DG and dysferlin from sarcolemmal muscle membrane in different age groups. The expression of nNOS is completely deficient in the 3 groups, while the expression of caveolin-3 increased. Conclusions The expression of dystrophin related DGC proteins from DMD patients in different age groups showed diversed degrees of deficiency or complete deficiency. The deficiency of these proteins may occur during the embryonic period, while the increased expression of eaveolin-3 may have a relation with the regeneration of skeletal muscle fibers.
