1.The synergistic effect and its mechanism of PLLA and PCCL on antisenility

Aim To study the synergistic effect and its mechanism of polysaccharide from Ligustrum Lucidum Ait(PLLA)and polysaccharide from Cuscuta Chinensis Lam (PCCL)on antisenility. Methods To observe the effect on immune organs and antioxidation systems of mice with senile model caused by D-galatose (D-gal) and meanwhile administered with PLLA∶PCCL 50 ∶50 mg or 50 mg ∶200 mg or 200 mg ∶50 mg or 200 mg ∶200 mg?kg -1?d -1 designed by orthogonal method. Results Thymus index and spleen index differently rose,MDA differently dropped,activities of SOD and GSH-PX differently ascended in liver and kidneys,LF differently descended in brain of mouse with senile model administered with 50 mg∶50 mg or 50 mg ∶200 mg or 200 mg ∶50 mg or 200 mg ∶200 mg?kg -1?d -1 PLLA ∶PCCL.The statistical analysis indicated that the combination of PLLA with PCCL yielded of notable or extremely notable reciprocal effect on the experimental index of antisenility. Conclusion The combination of PLLA with PCCL can yield notable synergistic effect on antisenility,the optimal combination of both drugs is PLLA ∶PCCL 50 mg ∶200 mg, its mechanism is probably connected with immunity inhacement,eliminati on of oxyen free radicals and active oxyen and antilipoperoxidation.
2.Effects of a polysaccharide from CCL on inhibiting oxygen free radical threshold of senile mice model

Objective To study the effect of a polysaccharide from cuscuta chinensis lam (PCCL) on antisenility and its mechanism. Methods Sixty Kunming mice were randomized into 6 groups. The three PCCL groups were administrated with PCCL of 100, 200, 400 mg?kg -1?d -1 orally, the positive control group with vitamin E of 200 mg?kg -1?d -1, the model group and control group with the same volume of control solution only. At the same time, the model group, the positive group and the three PCCL groups were subcutaneously injected of 5% D-gal at the dose of 0.5 ml at the nape, and the control with the same volume of saline solution. Seven weeks later, the MDA, SOD activity, GSH-PX activity in the liver and kidney of mice and lipofuscin (LF) in mouse brain were detected with the methods of TBA, Nitrate, DTNB and Sohal, respectively. The data were analyzed with SPSS software and the data between groups were analyzed with one-factor variance analysis. Results Thymus index and spleen index dropped, LF rose in brain, malondialdehyde (MDA) content rose and the activities of superoxide dismutase (SOD) and glutathione peroxidase (GSH-PX) dropped in liver and kidney in senile mouse model. PCCL administration of 100, 200, 400 mg?kg -1?d -1 made thymus index and spleen index rising, LF dropping in brain, MDA content dropping,SOD and GSH-PX LF rising in liver and kidney of senile mouse model. Conclusion PCCL may postpone senility, which mechanism probably connected with rising immunity, eliminating oxygen free radicals and antilipoperoxidation.
3.Detection of pulsation in unruptured cerebral aneurysms by electrocardiographic-gated 4-dimensional computed tomographic angiography and follow-up evaluation

Objective To evaluate the electrocardiographic(ECG)-gated 4-dimensional computed tomographic angiography (4D-CTA) in the determination of pulsation of unruptured cerebral aneurysms (URCAs).Methods This study included 48 patients with 62 URCAs.Examinations of ECG gated 4D-CTA of dual-source CT were performed.Twenty sets of image data with the time intervals of 5% in a cardiac cycle were obtained after postprocessing on the workstation.The convex was defined as the point of pulsation if small bubble or small pointed convex could be found in continuous three or more images at the same location.The 62 URCAs were divided into two groups based on whether having a point of pulsation.All the URCAs were scanned at the follow-up by 3D-CTA or ECG gated 4D-CTA more than 120 days later.The diameters of aneurysms and aspect ratio between the two groups were compared with independent t test,while neck of aneurysms and follow-up time were compared with two-independent samples Mann-Whitney U test.The other variables including history of hypertension,type 2 diabetes,smoking et al between the two groups were analyzed by x2 test.The sensitivity and specificity of aspect ratio for diagnosis of pulsation were analyzed by receiver operating characteristic (ROC) curve.Results Pulsation was observed in 28 of the 62 URCAs.Aspect ratios in whose pulsation was or was not detected were 1.5±0.3 and 1.1±0.3,respectivelyThe difference between the two groups was statistically significant (t=-2.274,P<0.001).The sensitivity and specificity of the aspect ratio were 75.0% and 70.6%.After follow-up more than 3 months,of the 28 URCAs in which pulsation was observed,11 showed a change in shape,while in 34 URCAs without visible pulsation,5 showed a change in shape.The aneurysms with detectable pulsation were more likely to show a change in shape (x2=4.891,P=0.027),with an odds ratio of 3.753.Conclusions URCAs with a large aspect ratio are easily to show the pulsation by ECG gated 4D-CTA.Besides,the aneurysms with visible pulsation are more likely to show a change in shape after follow-up,which suggests a higher risk of rupture.
4.The coverage of contracture of the first web space by flaps

Objective To introduce our experiences of reconstruct the first web space using flaps.Methods One hundred and twelve patients sustained contracture of the first, summarized the indications of different flaps in coverage of the first webspace and their prognosis of these flaps were retrospectively reviewed.Results The local flaps, such as double Z-plasty or Z-plasty, were suitable to cover minor contracture of the first webspace; the dorsal flap arising from the index finger was suitable for the coverage of the medium contracture, and the posterior interosseous artery flap or the reverse antebrachial island flap based on the dorsal branch of the anterior interosseous artery suitable for the severe cases. Conclusion The usage of flaps is important to resume the cosmetic shape and function of the first webspace after release of the contracture of the first webspace.
5.Analysis of the treatment results of scaphoid and lunate bone fracture with (or) dislocation

Objective To analysis the clinical diagnosis and treatment of the scaphoid and lunate bone fracture with (or) wrist dislocation.Methods A retrospective analysis of the clinical data included 77 cases,which got admitted according to the inclusion and exclusion criteria and followed up completely from 2005 to 2013.In this group,there were 64 males and 13 females whose age arranged from 15 to 73 with a mean age of 31.9.There were 41 cases of simple fracture,32 cases of bone fracture with dislocation and 4 cases of simple dislocation.Treatment of cases was divided into the conservative treatment group (9 cases) and the surgery group (68 cases).The surgery group was further divided into four subgroups according to surgical method (i.e.compression screw fixation,Kirschner wire fixation,proximal row of carpal resection,and lunate excision head shaped bone transposition).Clinical outcome was evaluated based on modified Mayo wrist score and X-ray.Results All of 77 patients were followed up completely.Follow-up time arranged from 6 months to 8 years,with an average follow-up time of 3 years and 5 months.9 cases were treated conservatively.In the surgical treatment group,there were 36 cases of compression screw fixation,34 cases of Kirschner wire fixation,1 case of proximal row of carpal resection and 1 case of the lunate bone resection heads transposition.The average healing time of compression screw group was 6.1 weeks and the Kirschner group was 7 weeks.According to the modified Mayo wrist score,excellent:51 cases (66.2%),good:11 cases (14.3%),fair:9 cases,and poor:6 cases.The rate of excellent and good was 80.5%.The cure rate of compression screw treatment was 97%,and 85% of Kirschner.The difference of cure rate was statistically significant.After surgery,there were four cases that got scaphoid malunion.The degree of deformity was not serious with only one case happened in the dominant hand,and the wrist pain got better after symptomatic treatment.1 case of scaphoid fracture with dislocation had the problem of wrist instability.18 weeks later,with ligament tightening surgery,postoperative Mayo score went back to excellent.Conclusion Lateral X-ray film of wrist is a preferred method for the examination of scaphoid and lunate fractures with (or) dislocation.Since it's easy to misdiagnose,films of special position are needed to be added.Surgery is preferred for treatment,and the selecting of surgical methods are based on the situation of fracture and dislocation.The advantages of screw fixation are obvious.
6.Clinical application of the extended reverse anterolateral leg flap

Objective To introduce the clinical application of the extended anterolateral leg flap,which is based on the superficial peroneal artery and lateral supramalleolar artery. Methods Through anatomic study on the blood supplies of the skin over the anterolateral portion of the leg.The combined superficial peroneal artery and lateral supramalleolar artery could extend the size of the traditional anterolateral leg flap.In addition,the point of pivot of the extended anterolateral leg flap could locate at the level of the lateral malleolus,which decreases the distance between the flap and recipient site.Clinically,we have used the extended anterolateral leg flaps in 60 patients to cover skin defect over the feet.The flap ranged from 16 cm×10 cm(pedicle length 8.0 cm)to 26 cm×7 cm(pedicle length 6.0 cm). Results In the series, the flap survived in 53 patients(88.3%)unevently,and partial necrosis occurred in 7 patients(11.7%).Conclusion Based on the combined superficial peroneal artery and lateral supramalleolar artery,the size of the traditional anterolateral leg flap could be inceased,which enlarges its value on the coverage of skin defects over the feet.
7.Changes of hemoglobin levels after chemotherapy in patients with ovarian cancer and influence of anemia on prognosis

Objective To investigate the changes of hemoglobin levels in patients with ovarian cancer after chemotherapy and the influence of anemia on the prognosis of patients.Methods A total of 73 patients with ovarian cancer were selected.SPSS 25.0 statistical software was used to analyze the changes of hemoglobin levels after chemotherapy.According to the hemoglobin content before chemotherapy,the patients were divided into anemia group (Hb > 110 g/L) and non-anemia group (Hb < 110 g/L).Log Rand was used to compare the difference of survival time between the two groups.COX model was used to analyze the related factors affecting the prognosis of ovarian malignant tumors.Results The hemoglobin content before and after six chemotherapy cycles were (108.24 ± 18.09) g/L and (100.17 ±21.06) g/L respectively,with significant difference (P =0.021).The incidence of anemia and the degree of anemia gradually increased.The rate of anemia after six cycles of chemotherapy was higher than before chemotherapy,with significant difference (P =0.027).Survival analysis showed that progression free survival (PFS) was significantly lower in the anemic group than in the non-anemic group (P =0.01).COX regression analysis showed that anemia,postoperative residual lesion size,and surgical clinical staging were independent factors affecting the prognosis of patients with ovarian malignant tumors.Conclusions Chemotherapy reduces the hemoglobin content in patients with ovarian malignant tumors and aggravates anemia.Anemia affects the survival rate of patients with ovarian malignant tumors and is an independent factor affecting the prognosis of patients.
8.Clinical analysis of 14 cases of niacin deficiency

The clinical data of 14 patients with niacin deficiency diagnosed and treated in Department of Dermatology, Affiliated Hospital of Jining Medical College from 2012 to 2021 were retrospectively analyzed. There were 11 males and 3 females aged 26-65 years. The etiological factors were alcoholism in 8 cases, gastrointestinal disease in 3 cases, medication history in 1 case, and unknown etiology in 2 cases.Patients had typical skin lesions, 1 case also had both digestive system and nervous system symptoms, and 3 cases had combined digestive system symptoms and 2 cases had neurological symptoms. All patients were systematically treated with oral nicotinamide and vitamin B complex, and also with topical drugs; and they all improved after 14-52 days of treatment. During regular follow-up, 2 cases of alcoholics and 1 case with diarrhea had recurrence. It is suggested that the typical clinical triad of niacin deficiency is uncommon, and the diagnosis is based on the medical history, clinical manifestations and relevant laboratory test, and the treatment with nicotinamide and vitamin B complex is usually effective; alcoholism is the main cause in male patients and is prone to recurrence.
9.Clinical characteristics of connective tissue disease-related interstitial lung disease: an analysis of 186 cases

Objective To analyze the clinical characteristics of connective tissue disease related interstitial lung disease (CTD-ILD).Methods The demographic data,clinical manifestations,blood gas analysis,lung function,chest high resolution CT (HRCT),histopathology of 186 patients with CTD-ILD admitted in Jinhua Central Hospital from January 2013 to December 2017 were retrospectively analyzed.Results In 186 patients with CTD-ILD,70 were males and 116 were females,with a mean age of 56.22± 12.27.Among them,61 cases were polymyositis/dermatomyositis (DM/PM)-ILD,44 cases were rheumatoid arthritis (RA)-ILD,26 cases were interstitial pneumonia with autoimmune features (IPAF),16 cases were systemic sclerosis (SSc)-ILD,14 cases were Sjogren syndrome (SS)-ILD,11 cases were overlap syndrome (OS)-ILD,7 cases were systemic lupus erythematosus (SLE)-ILD,5 cases were undifferentiated connective tissue disease (UCTD)-ILD,and 2 cases were mixed connective tissue disease (MCTD)-ILD.The incidence of respiratory symptoms was higher than that of systemic symptoms,and the incidence of systemic symptoms was higher than that of skin symptoms.Cough was the most common symptom of respiratory system,and arthralgia was the most common symptom of systemic symptoms,accounting for 54.9%(101/186) and 48.9% (91/186)respectively.Blood gas analysis showed that hypoxemia accounted for 56.5%(83/147) and type Ⅰ respiratory failure accounted for 26.5%(39/147).The proportion of restrictive ventilation dysfunction in pulmonary function tests was 70.9%(105/148) and the diffuse dysfunction was 81.1%(120/148).The manifestations on HRCT were reticular linear shadow (44.1%),ground-glass opacity (54.3%),consolidation (23.1%),nodules shadow (20.9%) and cystic low density shadow(9.1%).The histopathological diagnosis mainly was non-specific interstitial pneumonia (NSIP,43.0%) and usual interstitial pneumonia (UIP,40.0%).Conclusion The arterial blood gas analysis,lung function and chest HRCT screening,and lung histopathological examination are important for early identification and diagnosis of CTD-ILD.
10.Application of diffusion weighted imaging in the diagnosis and treatment of nasopharyngeal carcinoma

The judgment of early curative effect is important to the treatment of nasopharyngeal carcinoma and the prognosis of the patients.As a rapid imaging technique,magnetic resonance diffusion weighted imaging (DWI) is used to quantitatively analyze the changes of water molecules in the tissue by means of apparent diffusion coefficient (ADC),so as to diagnose the small lesions of nasopharyngeal carcinoma accurately,evaluate the early curative effect and observe the prognosis.With the continuous improvement and progress of MRI and data analysis technology,DWI in the diagnosis and treatment of nasopharyngeal carcinoma shows a broader application prospect.
