1.Fibrin gel therapy for sacral nerve root sheath cysts by lamina vertebra puncture under CT-guide

Objective To study the effect of fibrin gel delivered with CT g ui ded trans-laminar injection on sacral nerve root sheath cysts. Methods From Apri l 1998 to February 2001, thirty-eight cases of sacral nerve root sheath cysts we re treated by fibrin gel injection. The location of cysts consisted of 12 L5 S1, 15 S1,2, 11 S2,3. All the fibrin gel was delivered by CT guided trans-laminar i njection. Results Thirty-eight cases were followed up for 3 to 37 months (averag e 9 months). Most of the patients obtained lower back and sacral pain and sacral nerve symptom alleviation. No infection, nerve injury, recurrence and cerebrosp inal fluid(CSF) leaking occurred. According to the critia of the clinic symptom releasing and the radiographic signs, there were 17 cases (44.7%) rated as excel lent, 18 (47.4%) good and 3 (7.9%) fair, the excellent and good rate was 92.1%. Conclusion CT-guided trans-laminar fibrin gel injection is an effective economic al and mini-invasive way to treat sacral nerve root sheath cysts.
2.Gambogic acid induces the apoptosis and autophagic cell death in human hepatoma cells

Objective To study the effect of gambogic acid on apoptosis and autophagy in human hepatoma cells HepG2, and to detect its possible mechanism. Methods After exposure of HepG2 cells to gambogic acid at different concentration for 24 h, cell proliferation rates was determined by MTT assay, apoptosis rate was detected by the flow cytometry (FCM), formation of autophagic vacuoles was observed by the monodansyl cadaverine (MDC) fluorescence staining, expression level of apoptosis-related proteins Bax, bcl-2 and autophagy related protein Beclin 1 was detected by Western blot. Results HepG2 cell growth was inhibited by the gambogic acid dose dependence. After exposure to gambogic acid at 0, 2.0, 4.0 and 8.0 μmol/L for 24 h, cell apoptosis rate was significantly increased to 5.31 %, 29.18 %, 31.50 % and 46.09 %(P <0.05), MDC average fluorescence intensity was also significantly increased to 6.3 ±1.1, 82.6 ±4.5, 132.9±15.7 and 157.7±9.0 (P<0.01). Western blot showed that gambogic acid could promote the expression of apoptosis protein Bax (0.17 ±0.02, 0.75 ±0.06, 0.78 ±0.05, 0.89 ±0.10, P <0.05), and decrease the expression of anti-apoptosis protein bcl-2 (1.18 ±0.04, 0.90 ±0.06, 0.64 ±0.08, 0.57 ±0.05, P <0.05), meanwhile, it could also increase the expression of autophagy related protein Beclin (0.67±0.03, 0.92±0.04, 0.95±0.07, 1.04±0.06, P<0.05). Conclusion Gambogic acid can inhibit the growth of human hepatoma HepG2 cells by inducing apoptosis and autophagic cell death.
3.HPLC fingerprint of Platycodon grandiflorum in different picking times

AIM:To establish HPLC fingerprints of Platycodon grandiflorum in different picking times in Henan province.METHODS:HPLC chromatography condition:Hypersil C18 column(250 mm?4.6 mm,5 ?m);the mobile phase was acetonotrile with 0.05% phosphoric acid and the gradient elution mode was applied in chromatographic separation;The flow rate was 0.5 mL/min;The temperature of column was 30 ℃;the detective wavelength was set at 210 nm;RESULTS:The overall similarity in samples collected in autumn was higher than that in spring,and autumn was the appropriate for harvest.CONCLUSION:This method is simple and accurate with a good reproducibility.It provids a reliable scientific basis for the quality control of Platycodon grandiflorum.
4.NBI system versus conventional endoscopy in diagnosis of colorectal adenomas: a systematic review

Objective To evaluate the validity of narrow-band imaging (NBI) system in detection of colorectal adenoma, as compared to that of endoscopy of the colon and rectum, in a systematic review. Methods Relevant literatures were retrieved from Medline (January 1966 to October 2008), OVID (January 1996 to October 2008), EMBASE (January 1980 to October 2008), Coehrane Library (Issue 3, 2008) and Chinese Biological Medicine Disk (CBM disk, January 1997 to October 2008). Quality of the literatures retrieved was assessed based on the Cochrane Reviewers' Handbook and Jadad's score. RevMan version 4. 2 software was used for meta-analysis. Results Seven randomized clinical trials (2838 patients) were included in the study. Compared with white-light colonoscopy, no significant difference was observed in terms of adenoma detection rate (OR 1.18, 95% CI 1.00-1.39, P=0.06) by NBI system, which could significantly improve total number of detection for fiat lesions of the colon and rectum (pooled WMD 0.14, 95% CI 0.02-0.26, P=0.02), but with a longer withdrawn time (pooled WMD 1.05, 95% CI 0.08-1.22, P<0.01). Conclusions Detection rate for flat lesions of the colon and rectum, not for adenoma, can be improved by NBI system and meanwhile its withdrawn time is prolonged, indicating that routine use of NBI system for detecting colorectal adenomas may be recommended only with its further refined technique.
5.Homocysteine and fibrinolysin in elderly patients with coronary heart disease

BACKGROUND: Homocysteine(HCY) is emerging as an independent risk factor for atherosclerosis. Its damage to the structure and function of endothelial cell(EC) is seemingly an important mechanism that leads to atherosclerosis.OBJECTIVE: To investigate the effect of HCY on fibrinolysis in elderly patients with coronary heart disease(CHD).DESIGN: A case-controlled study based on CHD patients and normal people as control group.SETTING: Department of general internal medicine and department of cardiology in a university hospital.PARTICIPANTS: The study was completed in the Department of Gerontology Internal Medicine and Department of Cardiology, Beijing Chaoyang Hospital, Capital University of Medical Science. Altogether 177 inpatients and outpatients from this hospital during December 2001 to August 2003 were selected and divided into three groups according to the results of coronary angiography(CAG): CHD group( n = 91 ) with 50 males and 41 females with the mean age of(66 ± 6) years, negative CAG group( n = 86) with 43 males and 43 females with the mean age of(60 ± 6) years, and normal control group( n = 85) with 43 healthy males and 42 healthy females with the mean age of(55± 5) years.METHODS: Peripheral blood samples were collected. ELISA double antibody method was applied to test tissue-type plasminogen activator(t-PA), plasminogen activator inhibitor-1 (PAI-1) and yon Willebrand factor(vWF). HCY was assayed with EIA method and the ratio of PAI-1 to t-PA was calculated.MAIN OUTCOME MEASURES: HCY level, t-PA, PAI-1 and vWF activities, and the ratio of PAI-1 to t-PA.RESULTS: The levels of HCY, PAI-1, PAI-1/t-PA and vWF in CHD group were significantly higher than those in negative CAG group and normal control group( P < 0.01 ); however, the level of t-PA was significantly lower than that in control group( P < 0. 01) . HCY was positively correlated with PAI-1,PAI-1 / t-PA and vWF, while it was negatively correlated with t-PA.CONCLUSION: The increase of serum HCY is accompanied with fibrinolytic dysfunction. HCY is positively correlated with PAI-1 and vWF but negatively correlated with t-PA. Therefore, HCY is a predictor for early coronary lesions,and can provide related laboratory data for the primary prevention and early treatment of CHD.

7.Small interfering RNA in silencing Bcl-2 expression and enhancing radiosensitivity of esophageal cancer cells

ObjectiveTo explore the effects of small interfering RNA (siRNA) specific to Bcl-2gene on radiosensitivity of esophageal cancer cells. Methods Bcl-2 gene siRNA ( Bcl-2 siRNA ) was induced into esophageal cancer EC9706 cells by lipofectamine.Bcl-2 protein expression and apoptosis of EC9706 cells were detected by flowcytometer. Clone forming assay was used to determine the inhibitory effects of X-ray radiation combined with Bcl-2 siRNA interference. ResultsWhen Bcl-2 siRNA had been induced into EC9706 cells, Bcl-2 protein expression in EC9706 cells was inhibited, and cell apoptosis was increased. Bcl-2 protein expression rates of EC9706 cells induced with Bcl-2 siRNA1, A2, A3 (25.13% ±2. 04% ,38.87% ± 3.34% , 30.55% ± 2. 73% ) were lower than the control group ( 84.28% ± 1. 47% )(t =4. 01,3.04,3.64, P < 0. 05 ). After interference, the apoptosis rate of EC9706 cells ( 33.86% ±1.04% ) was higher than the control group and siRNA negative group (5.51% ±0. 14% and 5.59% ±0. 46% ) (t =6. 55,6. 54,P <0. 01 ). Bcl-2 gene siRNA interference enhanced X-ray inducing apoptosis of EC9706 cells (56.76% ± 1.24% ), which was higher than the radiation alone group ( 24.51% ± 0. 48% )(t =3.59,P < 0. 05 ). The D0, Dq, and SF2 of combined treatment group were much lower than those of irradiation alone group . The sensitization enhancing ratio was 1.32 ( ratio of D0 values ) . Conclusions Bcl-2 gene siRNA could enhance the radiosensitivity of esophageal cancer EC9706 cells and may has a good future in clinical practice.
8.Advantages of foley catheter for nephrostomy tube after minimally invasive percutaneous nephrolithotomy

Objective:To assess the advantages and nursing experience of foley catheter for nephrostomy tube after minimally invasive percutaneous nephrolithotomy (mPCNL).Methods:From October 2015 to April 2016,the clinical data of 137 patients,who were diagnosed with upper urinary tract calculi and need to perform mPCNL,were collected and randomized into 2 groups:a foley catheter group (research group,n=69) and a normal nephrostomy tube group (control group,n=68).The patients in research group used foley catheter for nephrostomy tube,while those in the control group used normal nephrostomy tube.Bleeding volume,the days of bleeding,pipes shedding and pain degree were compared,and the experience of the nursing process was summarized.Results:The bleeding volume,the days of bleeding in the research group were significantly lower than those in the control group (both P<0.01).The pipes shedding rate in the research group were lower than that in the control group (P<0.05).There was no significant difference in postoperative pain scores between the 2 groups (P>0.05).There was no other complications and discomfortable symptoms in the experimental group.Conclusion:The use of foley catheter for nephrostomy tube after mPCNL is safe,and it can decrease the bleeding volume and pipes shedding rate.It doesn't increase the patient's postoperative pain and can reduce the difficulty and risk for postoperative nursing.
9.Expression of matrix Gla protein and bone morphogenetic protein 2 in renal papillary tissues in patients with calcium oxalate kidney stones

Objective:To compare expression levels of matrix Gla protein (MGP) and bone morphogenetic protein 2 (BMP-2) in Randall's plaque of renal papillary tissues in patients with calcium oxalate kidney stones and the underlying mechanism for stone formation.Methods:A total of 30 samples of Randall's plaque in renal papillary tissues from patients with calcium oxalate kidney stones were collected from the Department of Urology of Xiangya Hospital of Central South University from April,2015 to December,2015 and served as an experimental group.Ten samples of renal papillary tissues in patients undergone renal tumor nephrectomy were collected from the same hospital and served as a control group.The expressions of MGP and BMP-2 mRNA and protein were detected by quantitative real-time PCR and Western blot.Meanwhile,immunohistochemical technique was used to observe the expressions of MGP and BMP-2 in different parts of renal papillary tissues in the 2 groups.Results:1) The mRNA expression levels of MGP in the experimental group and the control group were 0.760±0.804 and 1.365±0.348,respectively,with significant difference between them (P<0.05).Them RNA levels of BMP-2 in the experimental group and the control group were 2.500±0.725 and 1.485±0.870,respectively,with significant difference between them (P<0.05).The expression levels of MGP protein in the experimental group and the control group were 0.130±0.424 and 0.202±0.704,respectively,with no significant difference between them (P>0.05).The expression levels of BMP-2 protein in the experimental group and the control group were 0.885±0.220 and 0.682±0.272,respectively,with significant difference between them (P<0.05).T-he immunohistochemistry showed that the protein expression of MGP in the experimental group was lower than that in the control group,while the protein expression of BMP-2 in the experimental group was higher than that in the control group (both P<0.05).Conclusion:The BMP-2 expression is increased while MGP expression is decreased in renal papillary tissues in patients with calcium oxalate kidney stones,and the formation of calcium oxalate kidney stone might be a kind ofosteogenetic reaction or ectopic calcification.
10.HPLC fingerprints of Platycodon grandiflorum from different habitats and the determination of platycodin-D

AIM: To establish HPLC-fingerprints and quantitatively determine platycodin-D from Platycodon grandiflorum.METHODS: HPLC analysis was carried out on Hypersil C18 column(250 mm ? 4.6 mm,5 ?m),with a mobile phase of acetonitrile-0.05 mol/L phosphoric acid system,gradient elution,with a flow at 0.5 mL/ min,an ultraviolet detection wavelength was at 210 nm for fingerprint and at 206 nm for platycodin-D,column tem-perature at 30 ℃.RESULTS: Twelve common peaks were identified in chromatograms with reference to platycod-in-D peak from the 18 batches of the samples.CONCLUSION: The method of the HPLC-fingerprint and quantita-tive analysis is rapid,simple and accurate with a good reproducibility and can be used for the quality control of Platycodon grandiflorum.
