1.Virtual Screening ofα-glycosidase Inhibitors in Cortex mori and Folium mori

OBJECTIVE:To virtually screen potential α-glycosidase inhibitor ingredients from C. mori and F. mori,and to pro-vide reference for finding out new typeα-glycosidase inhibitor ingredient. METHODS:Surflex-Dock module of Sybyl-x 2.0 molecu-lar simulation software was used to perform the docking of small molecule compound,which was from the ingredients of C. mori and F. mori as ligand stated in literatures,with α-glycosidase. Total score of affinity scoring function was equal to 7 as the thresh-old value,to judge potential α-glycosidase inhibitor ingredient in C. mori and F. mori. RESULTS:After 70 small molecule com-pounds docked with α-glycosidase, 10 compounds showed binding activity (Total score≥7.00). Among them, moracin M-3′-O-β-D-glucopyranoside,5,7,2′-trihydroxyflavanone-4′-O-β-D-glucoside,mulberroside A,resveratrol-4,3′-di-O-β-D-gluco-pyranoside and 1,4-dideoxy-1,4-imino-(2-O-β-D-glucopyranosyl)-D-arabinitol had higher binding activity with α-glycosidase(Total score>8.00). CONCLUSIONS:Multi-constituents of C. mori and F. Mori show potential α-glycosidase inhibitory activity. The method is a kind of highly targeted,rapid and efficient approach to discover α-glycosidase inhibitor from traditional Chinese medi-cine.
2.Clinical investigation of mild cognitive impairment in patients with Parkinson's disease

Objective To describe the prevalence and neuropsychological character of mild cognitive impairment (MCI) associated with Parkinson' s disease(PD-MCI). Methods One hundred and three PD patients and a control group of 32 healthy old subjects were chosen. Psychometric assessment included the Mini Mental State Examination, the Dementia Rating Scale and a series of neuropsychol ogicaltests. The Hamilton Rating Scale of Depression was used to assess depression in PD patients. Results (1)Twenty-one (20.4%) PD patients was diagnosed with dementia, 45 (43.7%) had a MCI and only 37(35.9%) had no cognitive impairment; (2) Subjects with PD-MCI were older, had a later onset of the PD,and displayed more severe motor symptoms compared with those without cognitive impairment; (3) The prevalence and neuropsychological profile of PD-MCI were thought to correlate with the dominating side and subtype of Parkinsonian symptoms, for patients with left-sided dominant symptoms had a significantly higher chance of suffering MCI than those with right-sided dominant symptoms, the ratio being 74.2% vs 42.2%,χ<'2 =7. 589,P <0.05; The tremor-dominant group took less time than the mixed group for Stroop word test measurement ((80.8±39.9) s vs (94.4±30.0) s,t=3.332,P<0.01). Conclusion Identification of MCI is of important clinical significance, which helps to treat patients differently and thus predict the prognosis.
3.Effects of overexpression of TRPC6 on angiotensin Ⅱ-induced apoptosis of mouse podocytes

Objective To study the effect of overexpression of TRPC6 on Ang Ⅱ-induced apoptosis of mouse podocytes in vitro and to explore the possible mechanisms. Methods Mouse TRPC6 cDNA eukaryotie expression vector pEGFP-NI-mTRPC6 was transfected to conditionally immortalized routine podocyte cell line by liposome. The fluorescent microscopy was used to examine the expression of EGFP after 24 hours. The change of TRPC6 protein expression was observed by Western-blot. Podocytes were treated by different concentrations of Ang Ⅱ. The podocyte intracellular calcium concentration was measured with laser-scanning con_focal microscope. The expression of Bax and Bcl-2 mRNA was assessed by RT-PCR and the expression of Bax and Bcl-2 protein was measured by Western-blot. The apoptotic ratio of podocytes was monitored by flow cytometry and Hoechst staining. Results About 35% of the cells expressed EGFP. An up-regulation of protein expression of TRPC6 was detected in podocytes when transfected with pEGFP-N1-mTRPC6 (P<0.01). The overexpression of TRPC6 promoted the Ang Ⅱ-induced influx of extracellular calcium and elevated the expression of Bax but decreased the expression of Bcl-2 (P<0.01, P<0.05). The apoptotic ratio of podocyte was (2.50±0.72)% when treated by low-dose Ang Ⅱ (10-10 mol/L), and it was increased to (4.33±0.45)% when transfected with pEGFP-N1-mTRPC6 (P <0.05 ). Transfection with pEGFP-NI-mTRPC6 increased apoptosis rate from (15.46± 1.40)% to (18.33±0.87)%(P<0.01) by high-dose Ang Ⅱ (10-6 mol/L). Conclusion TRPC6 plays an important role in the Ang Ⅱ-induced apoptosis of podocytes by promoting the influx of extraeellular calcium, which leads to the apoptosis cascade initiation.
4.Application of spiral computed tomography in clinical dental implantation

Objective To evaluate the value of spiral CT and panoramic radiographs in preoperative assessment the jaw condition of patients for dental implantation.Methods 80 patients required dental implant restoration,these patients were Manned by CT and panoramic radiographs in preoperative,then selected suitable patients for dental implant restoration,reconstructed the jaw based on spiral CT data and guided the dental implant design and implanration.Results The spiral CT examination could be more accurate selection criteria for the patients needed dental implant restoration and more accurately showed the situation of edentulous ridge bone.Conclusion The reconstructed of three-dimensional imaging of spiral CT could precisely show the bone situation in the jaw needed dental implant and the important anatomic structures,could effectively assist denture design and the direction and depth of dental implants.was more superiority than panoramic radiographs.
5.CT manifestations of non-epithelial malignant tumors of bladder

Objective To investigate the clinical and CT features of non-epithelial malignant tumors of bladder to improve the understanding.Methods Clinical and CT data of 1 1 patients with non-epithelial malignant tumors of bladder were analyzed retrospectively. All patients underwent plain and contrast enhanced CT scan,and confirmed by surgery and pathology.Results Of 1 1 patients,there were 4 rhabdomyosarcomas,3 cancerous sarcomas,2 leiomyosarcomas and 2 small cell carcinomas.The CT performance were mostly cauliflower like or papillary,with wide basis and thickening of adjacent bladder wall.Diffuse thickening of the bladder wall was detected in all rhabdomyosarcoma lesions.The lesions were homogeneous or heterogeneous soft tissue-like density.CT showed calcification in 1 cancerous sarcoma,necrosis in 1 leiomyosarcoma and 1 rhabdomyosarcoma.Contrast enhanced scan showed mild to moderate or significant enhancement.Conclusion Besides the age of rhabdomyosarcoma,other non-epithelial malignant tumors of bladder have no special clinical and CT characteristics.CT examination can provide the information about location,size,surrounding tissue change,which is helpful to make the treatment plan.
6.Clinical characteristics and etiological changes of bronchiolitis before and after COVID-19 pandemic

Objective:To compare the clinical characteristics and etiology changes of patients with bronchiolitis before the pandemic of coronavirus disease 2019(COVID-19)with those after the pandemic, and to provide a basis for the clinical diagnosis, treatment and prevention of bronchiolitis.Methods:Retrospective analysis were made on the clinical characteristics and etiological changes of patients who were hospitalized with bronchiolitis in the Department of Pulmonology, Children′s Hospital of Soochow University before COVID-19 pandemic(from February 1, 2019 to January 31, 2020, called as Group 2019-2020)and after COVID-19 pandemic(from February 1, 2020 to January 31, 2021, called as Group 2020-2021). Medical records were reviewed to compare general conditions, clinical manifestations, and laboratory tests.Nasopharyngeal secretion examination results were collected to compare the differences in pathogenic composition.Results:A total of 285 patients were enrolled in the Group 2019-2020, while 190 patients in the Group 2020-2021.There were no significant differences in gender, age, symptom duration prior to admission and length of stay between the two groups( P>0.05). The proportion of moderate/severe cases in the Group 2020-2021 was lower than that in the Group 2019-2020[10.53%(20/190)vs 21.75%(62/285)]and the difference was statistically significant( χ2=10.062, P<0.05). The proportion of stuffy nose rhinorrhea in the Group 2020-2021 was higher than that in the Group 2019-2020, while the proportion of gastrointestinal symptoms(vomiting and diarrhea)in the Group 2020-2021 was lower than that in the Group 2019-2020 [57.37%(109/190)vs 47.37%(135/285)and 15.79%(30/190)vs 24.56 %(70/285)]and the differences were statistically significant( χ2 were 4.563 and 5.278 respectively, all P<0.05). There were no significant differences in the proportions of fever, dyspnea, shortness of breath and cyanosis between the two groups(all P>0.05). The creatine kinase isoenzyme(CK-MB)in the Group 2020-2021 was lower than that in the Group 2019-2020[4.15(2.90~5.60)vs 6.70(4.20~22.10)]and the difference was statistically significant( Z=-8.757, P<0.05). There were no statistically significant differences in white blood cell count(WBC), percentage of neutrophil(N%), blood platelet count(PLT), percentage of eosinophil(EOS%), C-reactive protein(CRP), alanine aminotransferase(ALT)and lactate dehydrogenase(LDH)between the two groups(all P>0.05). The total pathogen detection rate, positive rate of respiratory syncytial virus(RSV), positive rate of mycoplasma pneumoniae(MP)and mixed infection rate in the Group 2020-2021 were lower than those in the Group 2019-2020[65.26%(124/190)vs 75.09%(214/285), 14.21%(27/190)vs 30.18%(86/285), 6.32%(12/190)vs 15.09%(43/285), 16.84%(32/190)vs 25.61%(73/285)], with statistically significant differences( χ2 were 5.361, 16.026, 8.568 and 5.094 respectively, all P<0.05). The positive rate of rhinovirus in the Group 2020-2021 was higher than that in the Group 2019-2020[13.16%(25/190)vs 4.91%(14/285)]and the difference was statistically significant( χ2=10.285, P<0.05). There were no significant differences in the positive rates of human metapneumovirus, Boca virus and parainfluenza virus 3 between the two groups(all P>0.05). Conclusion:The clinical characteristics and etiology of patients with bronchiolitis have changed after the COVID-19 pandemic.The quarantine and protection measures reduce the transmission of associated pathogens and the severity of the disease.
7.Let-7i regulates glioma U251 stem cell differentiation through LIN28 protein

Objective To investigate the effect of MicroRNA (miRNA) let-7i on differentiation ofglioma U251 stem cells and the potential mechanisms.Methods Glioma U251 cells were cultured in vitro and CD133 positive cells were sorted by magnetic cell separation.Composite let-7i mimics and let-7i controls were transfected into U251 cells; the let-7i expression was detected by real time-PCR; the expressions ofCD133,nestin and LIN28 were detected by Western blotting; the differentiation status was assessed via glial fibrillary acidic protein (GFAP) immunocytochemistry.Composite LIN28 siRNA and siRNA control were transfected into U251 cells; the expressions of CD133 and nestin were detected by Western blotting.Targetscan software analysis and luciferase reporter system were employed to verified the possibility of LIN28 being the target gene of let-7i.Results let-7i in the U251 cells were over-expressed in group of let-7i mimics,enjoying 17.9 fold of group of let-7i control; the expressions of CD 133,nestin and LIN28 were 13.9%,43.7% and 53.6%,separately,of group of let-7i control; GFAP positive labeling index in group oflet-7i mimics (83.0±1.93)% was significantly higher than that of group oflet-7i control (39.7±6.73)% (P<0.05).The expressions of CD133 and nestin in group of LIN28 siRNA decreased to 23.7% and 37.9% of those in groups of siRNA control.LIN28 3'UTR existed let-7i paired binding sites and LIN28 could be the target gene of let-7i.Conclusion over-expressing let-7i could significantly down-regulate the expressions of CD133 and nestin,and enhance the glioma stem cell differentiation through the potential mechanism of down-regulating the expression of LIN28.
8.Experimental study on the prevention of bone loss by deferoxamine in type 1 diabetic mice with osteoporosis

Objective To observe the effect of a hypoxia mimicking agent deferoxamine (DFO) on the mineral density,volume,architecture,strength,and metabolism of the bones in type 1 diabetic mice withosteoporosis.Methods Type 1 diabetic mice model was established by intraperitoneal injections of streptozotocin.The mice were divided into control (normal mice),diabetes mellitus,and DFO groups.Micro-CT was used to analyze the bone mineral density,volume,architecture,and strength of the trabecule in the distal part of femurs.Three point bending test was carried out to evaluate the bone strength.Hematoxylin and eosin (HE) staining was performed to observe the alteration in the number of osteoblasts.Real-time PCR was used to detect the mRNA expressions of Runt-related gene 2 (Runx-2),osteoclacin,and tartrate resistant acid phosphatase (TRAP) in tibias.Western blot was used to detect the protein expressions of Hypoxia-inducible factor-1α(HIF-1α) and vascular endothelial growth factor (VEGF) in tibias.Results There was a decrease in mineral density,volume,strength of bones as well as deteriorated trabecular microarchitecture in diabetic mice as compared to control mice,which were partially improved by DFO treatment.Moreover,DFO treatment increased the number of osteoblasts and mRNA expression levels of Runx-2,osteoclacin,TRAP,as well as protein expression levels of HIF-1 α and VEGF(P<0.05).Conclusion Bone loss could be partially prevented by DFO treatment in type 1 diabetic osteoporosis mice,which might be ascribed to increased bone formation via stimulating hypoxia inducible factor singnaling pathway.
9.Evaluation of nursing effect of enhanced recovery after surgery model in elderly patients with hip fracture

Objectives:To investigate the nursing effect of perioperative enhanced recovery after surgery(ERAS)management model in elderly patients with hip fracture.Methods:A retrospective case-control study was conducted to analyze the clinical data of 94 elderly patients with hip fracture admitted to Second Affiliated Hospital of Soochow University from July 2017 to June 2018,including 24 males and 70 females aged 76-98 years[(83.7±5.4)years]. There were 54 patients with intertrochanteric fracture and 40 patients with femoral neck fracture. Surgical method was proximal femoral nail antirotation(PFNA)fixation or artificial hip arthroplasty. A total of 44 patients were managed by ERAS(enhanced recovery group)and 50 patients by traditional nursing(traditional rehabilitation group). The off-bed time was compared between the two groups from aspects of different fracture sites,bone mineral density and causes of injury. The visual analogue scale(VAS)was tested preoperatively and at postoperative days 1,3 and 7. The complications were recorded at postoperative 1 month. The activity of daily living(ADL)score was assessed preoperatively and at postoperative 1 week,1 month and 3 months. The Harris hip score was assessed at postoperative 1 week,1 month,3 months and at the last follow-up. The length of hospital stay and death at postoperative 3 months and at the last follow-up were recorded.Results:All patients were followed up for 12-24 months[(17.7±6.2)months]. In enhanced recovery group,the off-bed time of patients with different fracture sites(femoral neck,femoral intertrochanter),bone mineral density(>-2.5 SD,≤-2.5 SD)and causes of injury(falls,traffic accidents and others)were markedly shortened as compared with traditional rehabilitation group( P<0.01). There was no significant difference in VAS between the two groups before operation( P>0.05). However,the VAS in enhanced recovery group was(3.4±0.9)points,(2.7±0.5)points,(1.7±0.6)points at postoperative days 1,3 and 7,significantly lower than that in traditional rehabilitation group[(4.3±1.1)points,(3.5±0.5)points,(2.7±0.9)points]( P<0.01). One month after operation,the incidence of pulmonary infection was 0% in enhanced recovery group and 28%(14/50)in traditional rehabilitation group( P<0.01). While there showed no significant differences in incidences of cardiovascular system,nervous system,urinary tract infection,lower limb deep vein thrombosis,impaired liver and kidney function,anemia,electrolyte disorder or hypoalbuminemia between the two groups( P>0.05). There was no significant difference in preoperative ADL score between the two groups( P>0.05). The ADL score in enhanced recovery group was(37.0±6.6)points,(70.1±8.4)points,(86.2±9.3)points at postoperative 1 week,1 month and 3 months,significantly higher than that in traditional rehabilitation group[(26.5±10.4)points,(50.1±11.4)points,(70.7±9.0)points]( P<0.01). The Harris hip score in enhanced recovery group was(80.9±8.6)points at postoperative 1 month,significantly higher than that in traditional rehabilitation group[(71.1±9.2)points]( P<0.01). There were no significant differences in Harris hip score between the two groups at postoperative 1 week,3 months and at the last follow-up( P>0.05). The length of hospital stay was(4.7±2.4)days in enhanced recovery group,significantly shorter than(7.8±3.9)days in traditional rehabilitation group( P<0.01). The enhanced recovery group showed no death within 3 months after operation and 2 deaths[5%(2/44)]at the last follow-up,while the traditional rehabilitation group revealed 4 deaths[8%(4/50)]within 3 months after operation. There was no significant difference in the mortality between the two groups until the last follow-up( P>0.05). Conclusion:For elderly patients with hip fracture,perioperative ERAS management model can effectively shorten periods in bed,relieve postoperative pain,reduce incidence of postoperative pulmonary infection,accelerate recovery of hip function,improve quality of life,shorten length of hospitalization and promote early recovery.
