2.Early catch-up growth status and its influencing factors in small for gestational age preterm infants

Objective To study the early physical growth pattern,catch-up growth situation,and the influencing factors of early growth in small for gestational age (SGA) preterm infants.Method Our study was a single center,retrospective study.Criteria for infant inclusion were prematurity,SGA (birth weight less than the 10th percentile of related gender and gestational week,according to Fenton curve 2013),born between January 2012 to October 2015,admitted to our neonatal intensive care unit (NICU) within 24 h after birth,hospitalization more than 7 days,and discharged with complete oral feeding.Corrected age (CA) was used to evaluate growth.According to our follow up plan,anthropometric data (weight,length,head circumference) were collected at corrected full term (40 ± 4 weeks),CA (3 ± 1.5) months and CA (6 ± 1.5) months.Catch-up growth was defined as ΔZ greater than 0.67 compared with that at birth,successful catch-up was defined as anthropometric data higher than 10th percentile in target population.The characteristics and influencing factors were compared between infants with and without catch-up growth.Result Eighty-one SGA preterm infants were involved,45 boys and 36 girls.The average gestational age was (34.6 ± 1.7) weeks,birthweightwas(1617 ± 348) g,birthlengthwas(41.0 ±3.2)cm and head circumference was (29.7 ± 2.0) cm.At corrected gestational age (40 ± 4) weeks,CA (3 ± 1.5) months and CA (6 ± 1.5) months,follow-up rate was 86.4%,66.7% and 58.0%;catch-up growth in weight was 32.9%,55.6% and 66.0%;successful catch-up growth in weight was 52.9%,64.8% and 66.0%.At CA (40 ±4) weeks,there were more boys,sooner recover birth weight,and less patent ductus arteriosus (PDA) in catch-up infants (P < 0.05).At CA (3 ± 1.5) months,catch-up infants had large gestational age,and they were longer at discharge,shorter hospital stay,less PDA,and greater body weight at CA 40 weeks,the difference was statistically significant (P < 0.05).At CA (6 ± 1.5) months,there were difference in hospitalization days,percentile of body weight at CA 40 weeks and percentile of all three anthropometrics at CA (3 ± 1.5) months between catch-up and no catch-up growth infants (P < 0.05).Multiple factor analysis showed that percentile of weight at CA 3 months was the independent risk factor of catch-up growth in weight at CA 6 months (P =0.002,OR =1.221,95% CI 1.076 ~ 1.385).For every 5 percentile increase in body weight percentile at CA (3 ± 1.5) months of age,the likelihood of complete body weight catch-up growth at CA (6 ± 1.5) months increased 2.965 times (95% CI 1.480 ~ 5.942).Conclusion Both weight and length of SGA preterm infants showed a trend of rapid gain between corrected gestational age (40 ± 4) weeks to CA (3 ± 1.5) months.The factors that influencing the completion of catch-up growth are different at different age.The weight,length,and head circumference percentile at CA about 3 months are good predictors of growth pattern and situation at CA 6 months for the SGA preterm infants.
3.Investigation of phased health education needs and anxiety status of preterm fathers

Objective To explore the postnatal health education needs of preterm fathers so as to provide evidences for the formulation of health education on preterm parents.Methods A total of 60 fathers of preterm infants admitted in Peking University First Hospital NICU from January 2015 to June 2015 were selected and surveyed at different time points including admission,discharge and the first follow-up. Preterm parents care needs questionnaire and State Anxiety Inventory (S-AI) were used to evaluate phased health education needs and anxiety status of preterm fathers.Results At the admission time, fathers care more about the condition and prognosis of preterm infants,with the S-AI score of (49.57±10.88). At the discharge time,preterm fathers showed great needs of nursing skills and ability,with skill need score of (20.27±6.07) and ability need score of (11.96±3.37). After the discharge,preterm fathers focused on feeding skills and knowledge of observation and treatment for special situations.Conclusions Preterm fathers have phased needs and anxiety for infants from admission to follow-up stage. Nurses need to provide phased and individual health education and support to increase the nursing ability of preterm parents so as to improve the psychological states of preterm parents.
4.Efficacy and safety of arterial thrombolysis in patients with wake-up ischemic stroke of anterior circulation non-major arteries

Objective:To investigate the efficacy and safety of recombinant tissue plasminogen activator (rt-PA) in patients with wake-up ischemic stroke (WUIS) of anterior circulation non-major arteries.Methods:Sixty-seven patients with WUIS of anterior circulation non-major arteries (time from falling asleep/last use of bathroom at night to intravenous thrombolysis≤9 h and >4.5 h) admitted to Department of Neurology, Beijing Shijingshan Hospital and Department of Neurosurgery, Beijing Muyangliu Hospital from January 1 st, 2017 to December 31 st, 2021 were chosen; these 35 patients accepted routine antiplatelet therapy after intravenous thrombolytic therapy were chosen as control group, and the other 32 patients accepted rt-PA arterial thrombolysis after intravenous thrombolytic therapy were chosen as study group. Vascular recanalization after arterial thrombolysis was observed in study group (cerebral infarction thrombolysis grading 2 and 3: good vascular recanalization). Neurological deficit improvement during treatment, clinical prognoses (modified Rankin scale scores of 0-2: good prognosis) and hemorrhagic transformation incidence 3 months after treatment were compared between the 2 groups. Results:The vascular thrombolytic recanalization rate of study group was 81.25% (26/32). Patients in study group had significantly decreased National Institute of Health stroke scale scores compared with those in control group 1, 7, and 14 d after thrombolytic therapy ( P<0.05). The good prognosis rate of study group (62.50%, 20/32) was significantly higher than that in control group (37.14%, 13/35, P<0.05). No significant difference in hemorrhagic transformation rate was noted between the 2 groups (15.6% [5/32] vs. 5.71% [2/35], P>0.05). Conclusion:Patients with WUIS of anterior circulation non-major arteries (time from falling asleep/last use of bathroom at night to intravenous thrombolysis≤9 h and >4.5 h) benefit from arterial thrombolysis with rt-PA, and risk of secondary intracerebral hemorrhage is not obviously increased.
5.Effect of low-frequency pulsed electromagnetic field combined with whole-body vibration on bone density and mechanics performance of disuse osteoporosis rats

Objective To discuss the effect of low-frequency pulsed electromagnetic field (ELF-PEMFs) combined with-whole-body vibration(WBV) on the bone mineral density,bone microstructure and the biological mechanics performance inhindlimb unloading osteoporosis rats.Methods Forty female 4 months aged SD rats were randomly divided into five groups:blank group,control group,ELF-PEMFs group,WBV group,low-frequency pulsed electromagnetic field combined with wholebody-vibration(ELF-PEMFs + WBV) group.The blank group was the normal control which comprise the healthy rats.The controlgroup was the disused osteoporosis model.Except for the blank group,thehind limb unloading rats were kept for six weeks.No intervention were provided to the blank group and the control group,other three groups were treated with physical therapy.The bodyweight of rats in each group was record.Bone mineral density was measured by Dual energy X-ray absorptiometry.The right femurs was analyzed by Micro-CT and the left femur was tested through a biomechanical machine.Results After 8 weeks of continuous treatment,the ELF-PEMFs group,WBV group,ELF-PEMFs+WBV group were significantly increased in weight and bonedensity (PELF-PEMFs=0.015,PWBV=0.016,PELF-PEMFs+WBV=0.007),(tELF-PEMFs=5.956,PELF-PEMFs=0.000;tWBV=6.127,PWBV=0.000;tELF-PEMFs+WBV=4.639,PELF-PVMFs+WBV=0.000).The difference was statistically significant (F=0.091,P=0.018).The ELF-PEMFs +WBV group was significantly higher in bone formation index.The items of BV/TV,Tb.N,Tb.Th and SMI in Micro-CT analysis increased,while Tb.Sp and Conn.D index decreased significantly.Trabecular bone structure of all the three groups was improved,however the ELF PEMFs +WBV group was better than the other two groups (vs.ELF-PEMF group,PBV/TV=0.041,PTb.N=0.026,PTb.Th=0.009,PConn.D=0.030,PTb.Sp=0.045,PSMI=0.032;VS.WBV group,PBv/Tv=0.018,PTb.N=0.004,PTb.Th=0.022,PConn.D=0.042,PTb.Sp=0.039,PSMI=0.049).The maximum load,energy absorption and bone stiffness index in biomechanicalperformance test were improved.The combination treatment group were significantly improved (F=0.167,P=0.038).Conclusion Low-frequencypulsed electromagnetic field combined with whole body vibration therapy can improve bone mineral density,bone microstructureand the biological mechanics performance.They have synergistic effect and the combination therapy is better than single treatment.
