1.Relationship between brucellosis arthritis of the hip and femoral head necrosis

Objective To investigate the relationship between brucellosis arthritis of the hip and femoral head necrosis.Methods In August 2007 to August 2013,16 diagnosed and hospitalized patients with brucellosis complicated with femoral head necrosis in Heilongjiang Land Reclamation Bureau General Hospital were chosen as the objects of observation,and the patients' hospitalization information was analyzed retrospectively.Patients' medical records were investigated and analyzed.Observations included general condition of the patient,clinical symptoms and signs,imaging,pathology,treatment and curative effect.Results All the 16 patients had definite clinical manifestations of hip arthritis and femoral head necrosis including hip pain,limited mobility,positive 4 test,limp or bedridden,and sick limb muscle atrophy.Imaging examination:X-ray computer tomography (CT) showed cystic change and deformation collapse of femoral head.Magnetic resonance imaging(MRI) showed low signal on weighted T1 images,high signal on weighted T2 and soft tissue granulation hyperplasia could be seen in the hip joint.Pathological examination:most femoral bone tissue necrosis,dissolved,bone cells disappeared,a few bone cells present,and some fibrous tissue hyperplasia.Patients were treated with antibiotics according to the treatment principles of brucellosis,and the effect was obvious.Conclusions There is a causal relationship between brucellosis arthritis of the hip and femoral head necrosis.Therefore,positive and effective treatment of brucellosis arthritis of the hip in the early stages is crucial to prevent the occurrence of femoral head necrosis.
2.Data of the quantitative orofacial somatosensory functions of healthy subjects and its influence factors analysis

Objective:To establish the preliminary somatosensory data stratified by gender, age group, and sites in the trigeminal region through standardized quantitative sensory testing on healthy individuals, and to evaluate the effects of gender, age, and sites on somatosensory functions. Methods: The stan-dardised QST battery developed by the German Research Network on Neuropathic Pain consists totally of 13 different parameters. A total of 70 healthy individuals participated. The subjects were stratified into two groups according to age: younger group ( 16 female, 16 male, age 24 -40 years old ) and elder group (20 female, 18 male, age 41-69 years old) . The test was performed bilaterally over the infraor-bital, mental, and hand regions. Results:The preliminary orofacial somatosensory data stratified by gen-der, age group, and sites were presented. Female were more sensitive than male for most of the parame-ters ( P<0 . 05 ) . Age had a significant effect on most of the parameters ( P<0 . 05 ) , the younger group was more sensitive compared with the elder group (P<0. 01) for heat pain threshold (HPT): younger group (38. 07 ± 2. 94) ℃, elder group (39. 85 ± 3. 52) ℃;warmth detection threshold (WDT):youn-ger group (1.40 ±0.74) ℃, elder group (1.89 ±1.14) ℃; mechanical detection threshold (MDT):younger group (0. 73 ± 1. 66) mN, elder group (1. 41 ± 2. 82) mN; pressure pain threshold ( PPT):younger group ( 171. 71 ± 92. 51 ) kPa, elder group ( 196. 36 ± 73. 73 ) kPa; cold pain threshold (CPT):younger group (25. 90 ± 5. 38) ℃, elder group (21. 64 ± 6. 78) ℃; cold detection threshold (CDT):younger group ( -0. 97 ± 0. 55) ℃, elder group ( -1. 36 ± 0. 90) ℃, and wind-up ratio (WUR):younger group (3. 33 ± 2. 20), elder group (2. 67 ± 1. 68). The inverse results were demon-strated for mechanical pain threshold ( MPT ): younger group ( 111. 50 ± 88. 93 ) mN, elder group (104. 49 ± 94. 94) mN;mechanical pain sensitivity (MPS):younger group (6. 96 ± 5. 61), elder group (8.93 ±6.53), and vibration detection threshold (VDT): younger group (7.44 ±0.52) scale, elder group (7.55 ±0.48) scale (P<0.05). Somatosensory function was site dependent (P<0.001), the two trigeminal sites ( infraorbital and mental) were more sensitive than the hand for CDT, HPT, WDT, thermal sensory limen (TSL), MDT, MPT, MPS, and PPT (P <0. 001), but the inverse result was observed for VDT ( P <0 . 001 ) . Conclusion: The preliminary orofacial somatosensory data of Han Ethnicity stratified by gender, age group, and sites were established. The study evaluated the effects of gender , age and sites on orofacial somatosensory functions by employment standardized quantitative senso-ry testing.
3.The reference value of the static posturography parameters in normal subjects

Objective To study the range of the reference value of static posturography parameters in normal subjects. Methods Six hundred and eleven normal subjects were recruited. Static standing balance test of these subjects was performed by use of a computerized stabilometer with their eyes opened and closed,respectively,when their feet standing close and apart. Ten parameters were obtained. 95% confidence ranges were calculated using normal distribution methods,which were grouped by sex and age. Results Statistically significant differences of the 95% confidence ranges of equilibrium parameters were found among different sex and age groups,feet positions and eyes conditions,respectively. The value of the parameters of the male was higher than that of the female;the value of the equilibrium parameters was the lowest in the age group of 15~19 years,and the highest in 66~79 age group;and there were no significant differences among 20~35,36~50 and 51~65 age groups. The value of various parameters in standing with feet close was higher than that with feet apart. It was higher with eyes closed than that with eyes opened. Conclusion Sex,age,feet position and visual input affect balance function.
4.Establishment of a real-time fluorescent quantitative PCR for the rapid detection of viral reservoir in the peripheral blood of SIV-infected rhesus monkeys

Objective To establish a TaqMan probe-based real-time fluorescent quantitative PCR ( real-time PCR) for the quantitative and rapid detection of viral reservoir in peripheral blood mononuclear cells (PBMCs) isolated from rhesus monkeys with simian immunodeficiency virus (SIV) infection and to evaluate its preliminary application. Methods A pair of primers and one TaqMan probe were designed ac-cording to the conserved sequence of SIVmac239 strain for real-time PCR amplification. A length of 2 090 bp of nucleotide fragment was digested from the plasmid p239SpSp5 containing 5′-end long segments of SIV-mac239 strain by restriction enzymes EcoRⅠand SpeⅠ. The standards used for quantitative detection of SIV DNA in peripheral blood samples were prepared by a 10-fold serial dilution and used for graphing the stand-ard curve. The numbers of SIV DNA ( copies per 106 PBMCs) in rhesus monkeys during acute and chronic phases of SIVmac239 infection were determined and the virological characteristics of SIV DNA at different phages of infection were analyzed. Results A linear positive correlation between cycle threshold ( Ct) val-ues and concentrations (10 copies/μl to 109 copies/μl) of the standards was found. High levels of SIV DNA were monitored in SIV-infected monkeys 14 to 22 days after acute infection. The levels of SIV DNA in the acute phase of infection were about 1 to 2 logs higher than those in the chronic phase of infection. The num-bers of SIV DNA ( copies per 106 PBMCs) were 1 log lower than the SIV viral load in peripheral blood of the same monkey. The ratios of SIV DNA load to SIV RNA load ( DNA/RNA) in chronic phase of infection were higher than those in the acute phase. Conclusion The established TaqMan probe-based real-time fluorescent quantitative PCR was a highly sensitive and specific assay for the detection of SIV DNA with an advantage of wide linear range. It could be used for the quantitative evaluation of latent reservoirs of SIV.
5. Prognostic risk factors of patients with refractory acute left heart failure treated by continuous renal replacement therapy

Objective:
To find out the prognostic influencing factors of patients undergoing continuous renal replacement therapy (CRRT) for refractory acute left heart failure.
Methods:
Through the medical system and hemodialysis system in Foshan First People's Hospital, all patients who received CRRT for refractory acute left ventricular heart failure from January 1, 2012 to January 1, 2019 were searched. All patients were divided into two groups by the final outcome: survival group and death group. Age, sex, initial mean arterial pressure (MAP), primary heart disease, use of vasoactive drugs, urine output before treatment, hemoglobin, serum creatinine, serum albumin, C-reactive protein(CRP), brain natriuretic peptide (BNP), cardiac ejection fraction (EF) and CRRT treatment time were analyzed to find out the prognostic influencing factors.
Results:
A total of 130 cases were collected, including 96 cases in the survival group and 34 cases in the death group, with a total mortality rate of 26.15%. Compared to that in the death group, there were higher proportion of males (71.88% vs 50.00%,
6.Vitamin B12 status of vegetarians in Shanghai

Objective To investigate the vitamin B 12 status of vegetarians in Shanghai.Methods A total of 282 adult vegetarians and 282 omnivores matching by gender and age were recruited in Shanghai.Their dietary intakes were collected.The serum concentrations of vitamin B12,folate and homocysteine were tested.The red blood cell,hematocrit value,mean corpuscular volume and mean erythrocyte width were also examined.Results The daily average intake of dietary vitamin B12 was (0.46± 1.01) μg/d in vegetarians and only (0.1±0.46) μg/d in vegans,which was lower than that of omnivores [(3.91±6.92) μg/d,F=50.57,P<0.01].137 omnivores and 274 vegetarians had less dietary vitamin B12 level than recommended nutrient intake (RNI) and the difference was statistically significant (x2 =114.77,P< 0.01).54.26% of vegetarians,68.92% vegans,49.04% ovo-lacto vegetarians and 15.60% omnivores had hyperhomocysteinemia and the differences between vegetarians and omnivores were statistically significant (all P<0.01).After adjusting the confounding factors the hematocrit value was higher in vegetarians,vegans and ovo-lacto vegetarians than in omnivores (27.42%± 18.32%,28.73%± 18.19%,26.95%± 18.38% vs.8.96%± 16.59%,P<0.01).Vegans had lower red blood cell counts and higher hematocrit value and mean corpuscular volume than omnivores.Conclusion Vitamin B12 deficiency combined with an elevated level of homocysteine and red blood cell volume growth are common but serious issue in vegetarians,especially in vegans.
7.Effect of CXC chemokine receptor 7 on cell cycle of neuronal cells in ischemic stroke

Objective:To investigate the function and mechanism of CXC chemokine receptor 7 (CXCR7) in neuronal cells of ischemic stroke.Methods:The expression of CXCR7 in human neuroblastoma SH-SY5Y cells was interfered by small interfering RNA (si-RNA) technique. Oxygen-glucose deprivation/reoxygenation (OGD/R) injury model was constructed in SH-SY5Y cells. CXCR7 protein expression and cell cycle were detected by flow cytometry (FCM). The protein expression of CXCR7 and Akt signaling pathway was detected by Western blotting.Results:After 6 hours of OGD/R, the expression of CXCR7 was significantly decreased compared with OGD/R 0 hour (CXCR7/GAPDH: 0.483±0.098 vs. 1.000±0.000 by Western blotting and 0.686±0.0524 vs. 1.000±0.000 by FCM, both P < 0.01), cell cycle arrest in G0/G1 phase (1.190±0.040 vs. 1.000±0.000, P < 0.01). After CXCR7 si-RNA interference with SH-SY5Y cells, OGD/R was constructed again for 6 hours. Compared with negative control group (si-NC group) under the same environment, the expression of CXCR7 and phosphorylated Akt (p-Akt) was significantly decreased (CXCR7/GAPDH: 0.471±0.051 vs. 1.000±0.000, p-Akt/GAPDH: 0.616±0.027 vs. 1.000±0.000, both P < 0.001) and cell cycle arrest in G0/G1 phase (1.105±0.033 vs. 1.000±0.000, P < 0.05). Conclusion:The CXCR7 could regulate the cycle of neuronal cells in ischemic stroke through Akt signaling pathway, which has a protective effect on neuronal cells.

9.A case of X-linked hyper IgM syndrome with a novel CD 40LG mutation

A retrospective analysis was performed on the clinical data of a child with X-linked hyper IgM syndrome (XHIGM) with cholangiectasis as a major manifestation in Children′s Hospital of Fudan University in March 2017.The patient was a 4-year-old boy who was admitted to the hospital due to repeated diarrhea for half a year and yellow skin for 5 days.No abnormalities were found in his fetal period and birth history; The patient had 2 severe pneumonias and suppurative infection of the left axillary lymph node in infancy.Physical examination revealed delayed physical development, severe malnutrition, moderately stained yellow, lymphadenopathy and hepatomegaly.Laboratory examinations showed elevated leukocyte, eosinophils and C-reactive protein, low hemoglobin and albumin, high gamma-glutamyl transpeptidase (GGT), low IgG and normal IgM.Imaging examination revealed diffuse expansion of intrahepatic and extrahepatic bile ducts.Hepatic pathology showed hyperplasia in the bile canaliculus and some fibrous tissues around the large bile ducts.High-throughput sequencing identified a pathogenic mutation in the XHIGM gene CD 40LG (exon5 c. 506A>G, p.Y169C), with his mother as a carrier.After admission, the patient was given anti-infection, diet adjustment, albumin, intravenous immunoglobulin and ursodeoxycholic acid.The patient was discharged after the improvement in his condition.This case suggested that in addition to the common infection characteristics, XHIGM can also be manifested as diffuse intrahepatic, extrahepatic cholangiectasis and significantly elevated eosinophil.c.506A>G mutation in CD 40LG was the pathogenic mutation of this disease.
10.Effects of curcumin on endoplasmic reticulum stress PERK-CHOP signaling pathway in pancreas of type 2 diabetic rats

Objective:To investigate the hypoglycemic effect of curcumin on type 2 diabetic rats and its effect on the signaling pathway of endoplasmic reticulum stress RNA-dependent protein kinase like-CCAAT/enhancer binding protein homologous protein (PERK-CHOP).Methods:A total of 45 male SD rats were randomly divided into normal control group, model group and curcumin treatment group, with 15 rats in each group. The rats were given high-fat as well as high-sugar diet and intraperitoneally injected with streptozotocin (35 mg/kg, twice, once/day) to establish type 2 diabetes rat model. After the successful establishment of the model, the rats in the curcumin treatment group were given intragastrically with 200 mg/kg curcumin for 30 days. Serum adiponectin level was detected by ELISA, glucose was detected by glucose oxidase, Western blot was used to detect the expressions of PERK, p-PERK, CHOP and ATF4 protein levels in the pancreatic tissues of rats in each group, and mRNA expressions of PERK and CHOP in the pancreatic tissues of rat in each group were detected by real-time PCR.Results:Compared to the model group, the level of blood glucose (14.86 ± 2.77 mmol/L vs. 30.04 ± 3.25 mmol/L) in the curcumin treatment group significantly decreased, and the serum adiponectin level (94.12 ± 16.34 mg/L vs. 53.91 ± 9.08 mg/L) significantly increased ( P<0.05). The expressions of PERK mRNA (0.85 ± 0.07 vs. 1.64 ± 0.28), CHOP mRNA (0.53 ± 0.04 vs. 1.18 ± 0.33), PERK (1.12 ± 0.31 vs. 2.24 ± 0.43), P-PERK (1.23 ± 0.19 vs. 3.89 ± 0.32), CHOP (0.65 ± 0.07 vs. 0.92 ± 0.11) and ATF4 (0.73 ± 0.26 vs. 1.15 ± 0.31) in the curcumin treatment group significantly decreased ( P<0.05). Conclusions:Curcumin has hypoglycemic effect on type 2 diabetes rats, which can enhance the sensitivity of target tissues to insulin by increasing adiponectin levels. Curcumin can down-regulate the expression of PERK/P-PERK/CHOP/ATF4 protein levels in endoplasmic reticulum stress signal pathway, and reduce stress degree.
