1.Clinical study on combination of procedure for prolapse and hemorrhoids and excision-closed hemorrhoidectomy for severe hemorrhoids

Objective To demonstrate the feasibility, effectiveness and reliability of combination of procedure for prolapse and hemorrhoids (PPH) and excision-closed hemorrhoidectomy (ECH) for severe hemorrhoids. Methods Sixty patients with Ⅲ- Ⅳ degree hemorrhoids which left remnant prolapsed hemorrhoids after stapled hemorrhoidopexy, were randomly divided into group A (PPH with additional ECH group,30 cases.) and group B [PPH with additional Milligan-Morgan hemorrhoidectomy (MMH) group, 30 cases]. Ten indexes of the results in two groups were compared. ResultsThe hemorrhoidectomy lasted (35.6 ± 5.3) min in group A and (47.3 ± 10.2) min in group B. The indexes related to 24 h visual analogue scale (VAS) after operation were (2.9 ± 1.2) scores in group A and (5.7 ± 1.9) scores in group B. Wound healing time were (6.3 ± 1.4) d in group A and (28.5 ± 4.8) d in group B. Many indexes showed the results in group A was better than those in group B ( P < 0.05 ). The six-month follow-up visits after operation showed no complications, such as recurrence in either group. ConclusionsCombination treatment for Ⅲ -Ⅳ degree hemorrhoids with PPH with additional ECH is a easy,safe,reliable,minimal-aching and quick-recovering method of hemorrhoidectomy with favorable results than PPH with additional MMH.
2.Determination of Calycosin-7-?-O-D-glucopyranpside in Danggui Buxue Dropping Pills by HPLC

Objective To determine the content of calycosin-7-?-O-D-glucopyranpside in Danggui Buxie Dropping Pills.Methods HPLC method was used. The chromatographic column was YMC-C18(150?4.6 mm, 5 ?m), the mobile phase was methanol and water(40 ∶60), the detection wavelenth was 235 nm, column temperature at 20 ℃and flow rate was 1.0mL/min.Results Calycosin-7-?-O-D-glucopyranpside was separated well with other components and was in good linearity in the range of 0.02~0.80 ?g. The average recovery was 97.62 %and RSD=1.06 %.Conclusion The method is rapid and accurate and can be used for quality control of Danggui Buxie Dropping Pills.
4.Dynamic Analysis of the Use of Antihypertensive drugs in a Teaching Hospital During Recent 5 Years

To find out the patterns and trend of clinical use of antihypertensive agents to provide a guidance for rational use of drugs, the antihypertensive drugs used in our hospital during 1995-1999 period was investigated in respect to the sum of consumption cost, drugs used and DDDs. Our results showed that the DDDs and the consumption cost for antihypertensive drugs have increased gradually for recent 5 years. The order of DDDs of drugs was as follows: calcium antagonists, adrenergic blocking drugs, ACE inhibitors, the drugs affecting sympathetic transmitters and peripheral vasodilators. The order of sum of money was as follows: ACE inhibitors, calcium antagonists and calcium antagonists respectively. It is concluded that a diverse array of anthhypertensive agents were available and widely used clinically and attention should be paid to the rational use of the drugs.
5.Determination of Puerarin in Heat clearing Toxin resolving Decoction by RP HPLC

Objective: To establish a RP HPLC for determination of puerarin in Heat clearing Toxin resolving Decoction. Methods: A Hypersil C 18 column with acetonitrile 2% glacial acetic acid (8.5:91.5) as a mobile phase was used. The flow rate was 0.8ml?min 1 and the detective wavelength was 250nm. Results: The calibration curve was linear at a range of 0.424 4.240?g for the puerarin (r=0.9998). The average recovery was 101.3% and the relative standard deviation (RSD) was 1.4%. Conclusion: The methld is simple, accurate with a good reproducibility and can be used as a quantitative analysis method for this preparation.
6.Effect of Ginseng and Angelica Sinensis Decoction(GASD) on Learning and Memory of Dementia Rat with Hippocampal Lesions Induced by Quinolinic Acid

By one trial passive avoidance respense-step-down task and water maze spatial localizat ion task, the effect of Ginseng and Angelica Sinensis Decotion (GASD)on pathological mod els of the anmesia rat with hippocampal lesions induced by quiuolinic acid was studied. Re sults suggest that GASD can improve learning aud memory deficiency in rats with bilateral hippocampal lesions after administration of quinolinic acid. The major mechanism of GASD may be related to the regulation of the glutamatergic fuuction and prevention of the neuro toxicity of quinolinic acid

OBJECTIVE:To explore the ward round model of clinical pharmacists.METHODS:The ward round model of clinical pharmacists was designed and explored based on clinical practice.RESULTS & CONCLUSION:Clinical pharmacists can engage in ward rounds either in cooperation with physicians or alone in special department,or by targeting at those patients using certain drugs.Each model has its distinctive characteristics and clinical pharmacists can choose suitable model based on their own level as well as the conditions in software,hardware and environment so as to facilitate rational administration.
8.Influence of Budesonide-Solution Inhaled Therapy on Airway Resistance in Asthmatic Bronchitis Children

Objective To examine the changes of the airway resistance(AR) in asthmatic bronchitis children before and after budeso-nide-solution inhaled therapy.Methods Fifty-six cases of asthmatic bronchitis children were randomly divided into A(regular treatment+budesonide-solution inhaled) and B groups(regular treatment),and 30 normal children at the same age as control group.The AR was eva-luated by Microloop lung function meter with MicoroRint sensor.The changes of AR were compared within above 3 groups.Results AR in asthmatic bronchitis children increased significantly compared with normal children.After 2 weeks treatment,AR decreased significantly in both A and B groups compared with that of 2 groups before therapy,AR in A groups declined to normal control level,but still kept higher level in B groups.Conclusions AR in asthmatic bronchitis children increase significantly.The effect of Budesonide-solution in asthmatic bronchitis children is partly via reducing AR to improve ventilated condition.
9.The application of nutrition support in critical ill patients

Objective To evaluate the efficiency of nutrition support in critical ill patients. Methods A retrospective analysis was made to the efficiency of the nutrition support applied to 233 critical ill patients PN+EEN→EN+per os→per os was employed as the model of nutrition support, and a nasal feeding pump was used for enteral nutrition. Results During the period of nutrition support, in the 233 patients, 26 dead, the mortality was 11.16%. Early enteral nutrition significantly decrease the mortality in critical ill patients. There existed significant differences in complications in three kinds of enteral nutrition may methods. Conclusion It is feasible to give the critical ill patients, it might help to reduced the mortality in ICU patients.
10.Glucocorticoid amplifies IL-2 induced the proportion of T regulatory cell to T effector cell and suppress allogeneic antigen reaction

Objective To established the method of increasihg the proportion of T regulatory cells(Tr) to T effector cells(Te), which could suppresses allogeneic ahtigen reaction, by in vivo of glucocorti-coid (dexamethasone, DXM) combined with IL-2. Methods After combined treatment to male C57BL/6N mice (donor) with DXM(5 mg. kg-1· d-1 ) and IL-2 (300 000 IU · mouse-1·d-1) for 3 d, spleen mono-nuclear cells were made and were carried out by flow cytometry analysis. Using the spleen cells of BALB/c mice as aliogeneic antigen to stimulate the spleen cells of male C57BL/6N mice for 7 d after combined treat-ment of glucoeorticoid and IL-2, the reaction of cell proliferation was detected. Results After the treatrment of DXM and IL-2, CD4+ CD25+ Foxp3+ Tr cells in the spleen of C57BL/6N mice increased abviously. The ratio of CD25+ Foxp3+ Tr to CD4+ T cells was 24.22%±7.60% in the group of DXM combined with IL-2, while the control group was 4.02% ±0. 84% ( P =0. 01 ). Compared with the control group (0. 14±0.01 ), the ratio of Tr to Te increased obviously in the group of DXM combined with IL-2 (0.43±0. 15 ) ( P = 0.01 ). The group of DXM combined with IL-2 also expressed more glucocorticoid-induced tumor necrosis factor receptor(GITR) and alloreaction was suppressed/n vitro obviously ( P < 0. 05 ). Conclusion DXM amplifies IL-2 induced the proportion of Tr to Te, and suppresses the cell proliferation stimulated by alloge-neic antigen.
