Objective To study the clinical effect and prognosis of lyophilized recombinant human brain natriuretic peptide at different times for acute left heart failure.Methods A total of 58 consecutive patients diagnosed as acute left heart failure from February 2013 to February 2014 were randomly divided into early group (30 patients) and late group (28 patients).The patients in early group received lyophilized recombinant human brain natriuretic peptide and other common treatment immediately after arriving at hospital,and the patients in late group received lyophilized recombinant human brain natriuretic peptide until symptoms were hard to control.Results After treatment,the NYHA class and the levels of brain natriuretic peptide (BNP) and left ventricular ejection fraction (LVEF) in early group and late group were significant differences:(1.4 ± 0.5) grade vs.(2.6 ± 0.7) grade,(934.2 ± 52.4) ng/L vs.(1 632.5 ± 147.6) ng/L,(49.6 ± 5.4)％ vs.(31.2 ± 6.7)％,P ＜ 0.01 or ＜ 0.05.The average stay and expenses in early group were significandy less than those in control group:(11.2 ± 3.4) d vs.(19.7 ± 4.2) d,(15.8 ± 4.6) thousand Yuan vs.(27.4 ± 6.3) thousand Yuan,P ＜ 0.05.The total effective rate and survival rate in early group were significantly higher than those in late group:66.7％(20/30) vs.39.3％(11/28),P ＜ 0.05.Conclusion Earlier application of lyophilized recombinant human brain natriuretic can significantly improve the symptoms in acute left heart failure patients.

Objective To study the mechanism of anti-inflammatory effects of licorice flavonoids(LF) isolated from the roots of Glycyrrhiza uralensis(licorice) on lipopolysaccharide(LPS)-induced lung inflammation in mice.Methods Mice were intratracheally instillated with either LPS(4 mg/kg) or saline.At 6 or 24 h after LPS intratracheal instillation,pathological examination and bronchoalveolar lavage were performed and the lung wet/dry weight ratio as an index of acute lung injury was assessed.Then,the numbers of total leukocytes,neutrophils and the activity of superoxide dismutase(SOD) and TNF-? protein in bronchoalveolar lavage fluid(BALF) were measured.LPS-induced myeloporoxidase(MPO) activity and expressions of TNF-? and IL-1? at the mRNA levels and TNF-? at the protein level in lung tissues were determined by RT-PCR and ELISA.Results LPS caused a severe lung inflammation,as indicated by the pathological findings and the lung wet/dry weight ratio.However,oral LF could attenuate these LPS-induced abnormalities.LF could decrease the numbers of total leukocyte cells and neutrophils,and increase the levels of SOD and TNF? BALF.In addition,LF significantly suppressed the MPO activity,TNF-? and IL-1? mRNA expression levels,and TNF-? protein level in the lung tissues.Conclusion Inhibition of inflammatory cytokines expressions and regulation of oxidation/antioxidation of LF may be its anti-inflammatory mechanism in LPS-induced lung inflammation in mice.

Objective To investigate the incidence,pathogens and clinical features of nosocomial infections in patients with ac-tive rheumatoid arthritis (RA).Methods A total of 2 452 active RA patients were reviewed and analyzed retrospectively in Shanghai hospitals between January 2009 and February 2011.The demographic and clinical characteristics were described.Re-sults A total of 721 episodes of infection were found in 503 patients (20.51%).About 40.97% of the infections were of respir-atory tract,22.35% of urinary tract,and 8.58% of skin and soft tissues.Major pathogens were bacteria (623,86.40%),in-cluding E.coli (226,31.35%),P .aeruginosa (40,5.55%),A.baumannii (48,5.42%),Enterococcus (144,16.25%), methicillin-resistant Staphylococcus aureus (15, 2.08%), methicillin-resistant coagulase-negative Staphylococcus (7, 0.97%).The other pathogens included fungi (74,10.26%),herpes zoster (16,2.21%),and Mycobacterium tuberculosis (8,1.10%).Conclusions Nosocomial infection is mainly seen in respiratory and urinary tracts,usually associated with E.co-li,P .aeruginosa,A.baumanii,methicillin-resistant Staphylococcus and Enterococcus .

High-throughput screening is a regular approach available for identitying new lead compounds for the growing validated drug targets in drug screening. However, it has also introduced a large number of peculiar molecules which interfere drug screening. Pan assay interference compounds (PAINS) interfere with the progress of drug screening in various ways, such as interfering with a biochemical assay, modifying the protein, aggregate-based inhibitors and so on. So it is of vital significance to remove them. This paper has consulted the concept, category of PAINS and reviewed the way of PAINS interfering and the countermeasures to cope with them to direct the approach of high through screening and improve the hits percent.

In October 2003, a new bacterial disease with symptoms similar to those caused by Xanthomonas axonopodis pv. poinsettiicola was observed on poinsettia leaves at a flower nursery in Zhejiang Province of China. Three Xanthomonas strains were isolated from infected plants and classified as X. axonopodis. They were differentiated from the pathotype strain LMG849 of X. axonopodis pv. poinsettiicola causing bacterial leaf spot of poinsettia by comparison of pathogenicity, substrate utilization and BOX-PCR genomic fingerprints.

Objective To study dynamic change for the prevalence of the hyperlipidemia and the plasma lipids level of staff and workers in group in China Ocean Petroleum so as to provide the evidence for the prevention of the patients suffered from hyperlipidemia. Method Four thousand eight hundred and nin-ty-nine workers fmished blood fat test and blood fat data separately for two times in 1998 and 2004. Results The TC average value of males rose to (5.68 ± 0.99) mmol/L from (5.46 ± 1.08) mmol/L and the TC av-erage value of females rose to (5.79 ± 0.99) mmol/L from (5.42 ± 1.04) mmol/L,both were statistical sig-nificant difference(P < 0.01 ).The TG average value of males rose to ( 1.65 ± 1.31 ) mmol/L from ( 1.48 ± 1.29) mmol/L and the TG average value of females rose to (1.46 ± 1.09) mmol/L from (1.28 ± 1.17) mmol/L,both were statistical significant difference (P < 0.01 ). The gross hyperlipidemia,HTC,HTG rate rose to 58.9%,45.1%,32.4% from 46.2%,32.1%,25.7% for the males and those rose to 58.0%,49.6%,26.2% from 42.0%,31.2%,19.3% for the females ,all were statistical significant difference (P < 0.01 ).After 6 years,the males control rate of the HTC and HTG were 27.5% and 29.9%,the females control rate of the HTC and HTG were 25.1% and 34.0%. Conclusions The staff and workers suffered from high hyperlipidemia and lipid level is higher in man group in China Ocean Petroleum and also faster. Treatment control rate of hyperlipidemia was very lower .Therefore,the prevention measure must be taken as fast as possible to reduce lipid level and hyperlipidemia patient's rate actively and improve control rate.

Objective To observe the characteristics of macular optical coherence tomography (OCT) changes before and after silicone oil removal in patients who had undergone pars plana vitrectomy with silicone oil tamponade for macula-off rhegmatogenous retinal detachment (RRD).Methods Thirty-nine eyes that underwent silicone oil removal were enrolled in this retrospective study.The patients included 24 males and 15 females,with an average age of (53.05±4.03) years,the duration of silicone oil tamponade ranged from 3 to 7 months.Best-corrected visual acuity,intraocular pressure,slit lamp microscope and prelens,indirect ophthalmoscopy and fourier domain OCT were measured for all patients before and at months 1,3 and 6 after silicone oil removal.The macular microstructure were observed before and after silicone oil removal.Results Submacular fluid was detected in 6 eyes (15.38％),at the last time of follow-up,submacular fluid resolved completely in 2 eyes with disrupted ellipsoid zone,and resolved partly in 2 eyes.Disrupted ellipsoid zone were observed before silicone oil removal in 16 eyes (41.02％),6 eyes showed simultaneous disrupted ellipsoid zone and disrupted external limiting membrane,and there were 2 eyes that external limiting membrane was not identified,at the last time of follow-up,disrupted ellipsoid zone restored in 2 eyes and the extent of disrupted ellipsoid zone became reduced in 4 eyes.Cystoids macular edema were found in 2 eyes (5.12％),it resolved completely in 1 eye and resolved partly in 1 eye at the last time of follow-up.Macular epiretinal membrane was detected in 10 eyes (25.64％),and macular epiretinal membrane was found before silicone oil removal in 5 eyes,at the last time of follow-up,the membrane became thickened in 2 eye;5 eyes developed macular epiretinal membrane after silicone oil removal,at the last time of follow-up,the membrane became thickened in 1 eye.Secondary macular hole were noted in 2 eyes.Microcystic macular changes were observed in 9 eyes (23.07％),it was observed in 7 eyes before silicone oil removal,and was observed in 2 eyes after silicone oil removal,at the last time of follow-up,the cysts reduced in 1 eye.Conclusion Submacular fluid,disrupted ellipsoid zone and microcystic macular are the main macular ultrastructural changes that developed in patients with RRD before and after silicone oil removal.

Objective To explore the influence of Shenqifuzheng injection on efficacy of chemotherapy and immune function in patients with gastric cancer after surgery.Methods 60 postoperative patients with gastric cancer were selected as the research subjects,and were randomly divided into the control group and observation group by the digital table,30 cases in each group.The control group was given FOLFOX regimen chemotherapy.The observation group received FOLFOX regimen combined with the treatment of Shenqifuzheng injection.The recurrence and metasta-sis,survival,immune function and adverse reactions of the patients were observed and the patients were observed before and after chemotherapy.Results The chemotherapy cycle of the observation group was (3.95 ±0.40),which was significantly higher than (3.40 ±0.30)of the control group,the difference was statistically significant (t =6.025,P <0.01).The progression free survival in the observation group was (9 ±2.4)months,which was significant-ly longer than (6.6 ±3.2)months in the control group,the difference was statistically significant (t =3.286,P <0.01).The reduction rate in the observation group was 6.67%,which was lower than 26.67% in the control group, the difference was statistically significant (χ2 =4.320,P <0.05).The incidence rate of nausea and vomiting in the observation group was 6.67%,which was lower than 26.67% in the control group,the difference was statistically sig-nificant (χ2 =4.320,P <0.05).KPS[(83.51 ±15.37)points]and QLQ -C30[(82.01 ±9.45)points]of the observation group were significantly higher than those in the control group [KPS (74.62 ±13.22)points and QLQ -C30 (74.62 ±13.22)points],the differences were statistically significant (t =2.298,2.491,all P <0.05).After chemotherapy,gastric cancer patients ratio of CD +3 ,CD +4 ,CD +4 /CD +8 in the observation group were higher than those of the control group and the ratio of CD +4 /CD +25 in the observation group was lower than control group,the differences were statistically significant (all P <0.05).Conclusion Shenqifuzheng injection combined with FOLFOX chemo-therapy for gastric cancer patients has low adverse reaction rate,and can effectively improve the immune function.

OBJECTIVE:To establish an HPLC method for the determination of the contents of Decussatin,Bellidifodin,Methylswertianin and Swerchirin in Swertia macrosperma C.B.Clark.METHODS:The determination was performed on VP-ODS C18(150 mm? 4.60 mm,5 ? m),with the mobile phase consisting of methanol-water-phosphoric acid(70∶ 30∶ 0.4),the detection wavelength set at 254 nm,the flow rate at 1.0 mL? min-1 and the column temperature at 20℃.RESULTS:The calibration curves of Decussating,Bellidifodin,Methylswertianin and Swerchirin were in good linearity in the range of 0.172～ 0.86 ? g,0.224～ 1.344 ? g,0.144～ 0.72 ? g and 0.154～ 0.77? g,respectively.The average recoveries were 98.18%(RSD=1.19%),99.32%(1.52%),98.44%(1.23%)and 97.68%(1.57%),respectively.CONCLUSION:The method is rapid and precise,with accurate and reliable results.

Objective To evaluate the therapeutic effect of etanercept in patients with Behcet's disease (BD). Methods Twenty six patients with active BD were eligible for anti-TNF-α treatment, which failed previously to commonly used drugs. All the patients were stopped previous therapy and treated with etanercept 25 mg twice a week. The clinical responses and laboratory parameters were evaluated at the baseline, week 2, 4, 8, 12 and 24, and week 12 of etanercept withdrawal. Results Etanercept was effective in several clinical lesions and lab. parameters at all time points. Especially at week 2, including oral ulcers, genital ulcers, erythema nodosum, pseudofolliculitis and swollen joints (P<0.01). Two of four cases of uveitis were contro-lled with addition of topical corticosteroids; The ileocecal junction ulcer of 1 patient healed after 2 months etanercept therapy; four cases of epididymitis reached complete remission at week 4. ESR(P=0.012) and CRP (P=0.013) were also rapidly decreased at week 2. Most patients were relapsed at week 12 of etanercept withdrawal compared with baseline except for swollen joints (0.65±1.09 vs 0.31±0.60, P<0.01) after etanercept withdrawal, in which the typical rebound was CRP from (481±312) mg/L to (549±267) mg/L (P=0.013).Conclusion Etanereept has rapid efficacy onset for most of the mucocutaneous disorders and possibly effective for arthritis, ophthalmitis, epididymitis and colonic ulcer of BD.
