1.Application of multifunctional mask care in patients with acute exacerbation of chronic obstructive pulmonary disease complicated with respiratory failure

Objective To study the effect of multi-functional mask care in patients with acute exacerbation of chronic obstructive pulmonary disease complicated with respiratory failure.Methods A total of 140 patients with acute exacerbation of chronic obstructive pulmonary disease (COPD) who underwent treatment in our hospital were divided into control group and study group according to random number table method,with 70 cases in each group.The patients in the control group were given double-hole nasal oxygen therapy and routine nursing,and the patients in the study group were given multifunctional mask care and targeted care.Symptoms of initial remission time,total examination time,length of stay and the blood gas,respiratory function changes before and after nursing were compared.Results The initial remission time,total examination time and hospital stay were lower in the study group than the control group (P < 0.05).The levels of p (02) in the two groups were higher than nursing before,and the study group was lower than the control group (P < 0.05);The levels of p(CO2) in the two groups were lower than nursing before,and the study group was lower than the control group,and levels of FEV1 and FEV1/FVC were higher than nursing before,and the study group was higher than the control group (P < 0.05).Conclusion Multifunctional mask care can effectively shorten the recovery time of patients with acute exacerbation of chronic obstructive pulmonary disease complicated with respiratory failure,speed up the relief of clinical symptoms,regulate blood gas indicators,improve lung function in patients,so it is worthy of clinical promotion.

Venous thromboembolism(VTE)is a prevalent complication in orthopedics.In recent years,machine learning has been widely applied in orthopedics.The essence of machine learning lies in utilizing algorithms to analyze vast amounts of data and construct risk prediction models that can accurately forecast unknown clinical outcomes.By integrating high-risk factors,machine learning aids medical professionals in precisely identifying and screening individuals with a high risk of VTE and offering them timely individualized interventions.This article reviews the concept and classification of machine learning,the advantages of machine learning in enhancing model prediction ability,and the current application status of machine learning in constructing risk prediction models for patients with VTE.
4.Establishment of standardized intervention system for chronic wounds and its' clinical practice

Objective:To establish a standardized intervention system for chronic wounds and observe its clinical practice application effects.Methods:From January to December 2019, convenience sampling was used to select 150 patients with chronic wounds admitted to Room 1 of the Wound Treatment Center of the No.988 Hospital of the Chinese People's Liberation Army as the control group, and 145 patients with chronic wounds admitted to Room 2 of the Wound Treatment Center as the observation group. Control group conducted conventional wound intervention, and observation group implemented standardized wound intervention. The wound and pain of the two groups of patients were compared.Results:After the intervention, the wound temperature of observation group was higher than that of control group, and the pH value of exudate, pain score, and Bates-Jensen score were lower than those of control group, and the differences were statistically significant ( P<0.05) . The wound healing time of observation group was shorter than that of control group, and the hospitalization cost was less than that of control group, and the differences were statistically significant ( P<0.05) . Conclusions:The standardized intervention system for chronic wounds can effectively improve the treatment effect of chronic wounds, shorten wound healing time, reduce wound treatment costs and the burden on patients, medical care systems and society, and can provide scientific guidance and basis for the implementation, further research and development of clinical wound care.
5.Construction of the quality evaluation scale of specification of management for humanistic caring in outpatients and its reliability and validity testing

Objective:To construct the quality evaluation scale of specification of management for humanistic caring in outpatients and test its reliability and validity.Methods:Referring to the group standards in Specification of Management for Humanistic Caring in Outpatients released by the China Association for Life Care,as well as relevant guidelines and literature,a pool of items for the quality evaluation scale of specification of management for humanistic caring in outpatients was formed.After expert consultation and expert argumentation,a quality evaluation scale of specification of management for humanistic caring in outpatients was constructed.From January to February 2024,243 hospital managers from 5 hospitals in Zhengzhou were selected as survey subjects to conduct item analysis,and reliability and validity testing on the scale.Results:Two rounds of expert inquiry and two rounds of expert argumentation were conducted,with questionnaire response rates of 92.00%and 100.00%,respectively,and expert authority coefficients of 0.952.In the second round of the expert inquiry scale,the mean importance score of the first-level indicators was 4.80 to 5.00,the full score ratio was 88.00%to 100.00%,the coefficient of variation was 0.04 to 0.17,and Kendall's coefficient of concordance was 0.857(P<0.001);the mean importance score of the second-level indicators was 4.60 to 5.00,the full score ratio was 80.00%to 100.00%,the coefficient of variation was 0.00 to 0.21,and Kendall's coefficient of concordance was 0.775(P<0.001);the mean importance score of the third-level indicators was 4.60 to 5.00,the full score ratio was 76.00%to 100.00%,the coefficient of variation was 0.00 to 0.21,and Kendall's coefficient of concordance was 0.830(P<0.001).Finally,a quality evaluation scale of specification of management for humanistic caring in outpatients was formed,including 5 first-level indicators,25 second-level indicators,and 58 third-level indicators.Exploratory factor analysis produced 5 common factors with a cumulative variance contribution rate of 74.628%.The Pearson correlation coefficients between the five-factor scores ranged from 0.648 to 0.798,and the correlation coefficients between the factor scores and the total score of the scale ranged from 0.784 to 0.938.The scale-level content validity index(S-CVI)of the scale was 0.945,the item-content validity index(I-CVI)was 0.725 to 1.000,the Cronbach's alpha coefficient of the total scale was 0.973,and the retest reliability coefficient was 0.934.Conclusion:The constructed quality evaluation scale of specification of management for humanistic caring in outpatients has good scientific validity and reliability,and can be used as an evaluation tool for specification of management for humanistic caring in outpatients.
