1.Treatment for pancreatic pseudocysts, report of 53 cases

Objective To evaluate the treatment and clinical outcome of pancreatic pseudocysts.Methods Clinical data of 53 pancreatic pseudocyst patients treated from Jan 2008 to Oct 2014 was retrospectively analyzed.Results 9 cases underwent CT-guided percutaneous catheter drainage,among them 3 cases underwent reoperation.8 cases underwent endoscopic ultrasonography-guided transmural drainage,including transgastric approach in 7,transduodenal approach in 1,Procedures in all these 8 cases were successful and pseudocyst disappeared completely without complication.Surgical procedures were performed in 36 cases,3 by external drainage,1 of the 3 underwent reoperation;11 cases underwent cystgastrostomy,with 2 suffering from postopertative gastrointestinal bleeding;3 cases underwent cystduodenostomy;19 cases underwent Roux-en-Y cystojejunostomy,with 1 suffering from postopertative gastrointestinal bleeding.Follow-up ranged from 4 months to 6 years,there was no cyst recurrence.Conclusion The treatment of pancreatic pseudocysts is diversified and should be individualized.Internal drainage is the mainstay of treatment,preferably through endoscopic ultrasonography-guided transmural drainage,leaving difficult cases to open cystojejunostomy.
2.Comparison of tear secretion and tear film function between patients with type 2 diabetes mellitus and normal people

BACKGROUND: There are various reports on studies of tear secretion and tear film function in patients with diabetes mellitus over the past. In recent years, tear proteins have drawn more and more attentions on evaluation of tear film function.OBJECTIVE: To observe the contents of main tear proteins and basal tear secretion of patients with type 2 diabetes mellitus and normal persons so as to probe into tear secretion and tear film function of patients with diabetes mellitus.DESIGN: Case-control observation was designed.SETTING: Department of Ophthalmology, Southwest Hospital, Third Military Medical University of Chinese PLA.PARTICIPANTS: Totally 50 cases (100 eyes) of type 2 diabetes mellitus were employed, which were diagnosed in Department of Ophthalmology and Department of Endocrinology, Southwest Hospital of Third Military Medical University of Chinese PLA from December 2001 to December 2002. They were free from ocular surgical and laser treatment, local medication in recent period and contact lens. Of those, there were 25 cases (50 eyes) in proliferating diabetic retinopathy group and 25 cases (50 eyes) in nonproliferating diabetic retinopathy group. In addition, 25 cases (50 eyes) of normal persons with matched age and sex were taken as the control group.There was no significant difference in age and sex among 3 groups (x2=0.024,0.321 ;P > 0.05). All of the participants were in the know before the experiment.METHODS: [1] Tear collection: 10 cases (20 eyes) were randomized from two diabetic groups and the control successively. Capillary pipette method was used to collect non-irritative tear 10 μL from lower lacrimal punctum that was preserved in refrigerator at -20 ℃ (< 1 month). [2] Determination of total tear protein amount: Lorry method was used to determine the concentration of total tear protein, in which, calf serum albumin was taken as the criteria. [3] Determination of contents of main tear proteins: SDS-PAGE (sod.dodecyl sulfate-polyacrylamide gel electrophoresis) was used and Coomassie brilliant blue staining and Bio-Rad imaging analyzing system were applied for the analysis of isolated protein strips in quality and quantity. [4] Determination of rupture time of tear film: glass rod was used to get 20 g/L fluorescein sodium and drop in conjunctival sac. The examined person was required to blink gently for several times and open the eyes naturally and stare forward. That the first ruptured "black hole" was discovered on the complete tear film was taken as the rupture time of tear film. [5] Experiment of basal tear secretion: No.41 Whatmann filtering paper was used, folded in 5 mm, and placed at 1/3 of conjunctiva in the lower eyelid. Five minutes later, the paper was removed and length of it after wetting was measured. [6]Experiment with rose Bengal staining: glass rod was used to get 10 g/L rose Bengal and drop in conjunctival sac. After eyes blinking for several minutes, the observation was performed with green light filter under slit lamp (evaluation criteria: corneal conjunctiva of palpebral fissure stained+, stained to the inferior bulbar conjunctiva++,stained to the superior bulbar conjunctiva). Dry eye disease was diagnosed indirectly with red-stained epithelial cells and mucin.MAIN OUTCOME MEASURES: [1] Concentration of total tear protein. [2]Concentrations of various main tear proteins. [3] Rupture time of tear film. [4]Value of basal tear secretion. [5] Positive rate of rose Bengal staining.RESULTS: Totally 50 cases (100 eyes) in diabetes groups and 25 cases(50 eyes) in the control group all entered result analysis. [1] Concentration of total tear protein: there was no significant difference among 3 groups (P> 0.05). [2] Concentrations of various main tear proteins: the results of lysozyme, lactoferritin and tear specific prealbumin in proliferating diabetic retinopathy group were lower remarkably compared with the control[ (0.94±0.21)vs ( 1.33±0.31 )g/L , ( 1.10±0.24)vs ( 1.67±0.43 )g/L, (0.98±0.22) vs (1.49±0.32)g/L, P < 0.01]. Compared with non-proliferating diabetic retinopathy group, the results of lactoferritin and tear specific prealbumin were lower (P < 0.05). There was no significant difference in human serum albumin among 3 groups (P > 0.05). [3] Rupture time of tear film:compared with non-proliferating diabetic retinopathy group and the control, the rupture time of tear film in proliferating diabetic retinopathy group was reduced significantly [(7.68±2.21)s vs (9.92±2.37)s and(10.80±2.23)s,(P < 0.01 )]. [4] Value of basal tear secretion: the value in proliferating diabetic retinopathy group was less significantly than that in non-proliferating diabetic retinopathy group and the control [ (8.00±2.10)vs( 11.02± 1.97 )mm and ( 12.17±2.08 )mm, P < 0.05]. [5] Positive rate of rose Bengal staining: the positive rate in proliferating.diabetic retinopathy group was higher significantly than non-proliferating diabetic retinopathy group and the control (48% vs 24% and 14%, P < 0.05, P < 0.01).CONCLUSION: It is suggested in the results of this paper that abnormal tear secretion and tear film function are apt to present in patients with type 2 diabetes mellitus and the declined tear film function is more remarkably in the patients with proliferating diabetic retinopathy specially. SDS-PAGE benefits the discovery of changes in tear proteins in diabetic patients.
3.Comparison of effect of paste epoxy resins and biological ceramic paste on interleukin-1, interleukin-6 and interleukin-10, and hypersensitive c-reactive protein

Objective To explore effect of the paste epoxy resins and biological ceramic paste on interleukin 1 ( IL-1 ) , interleukin 6 ( IL-6 ) , interleukin 10 (IL-10), hypersensitive c-reactive protein (hs-CRP) .Methods From April 2013 to June 2015 I branch 120 cases were randomly selected as the research object.Random grouping method divided into observation group and control group, 60 cases of control group and 60 cases of observation group.In observation group was given with epoxy resin paste for root canal filling, control group was given with biological ceramic paste for root canal filling, compared two groups of IL -1, IL-6 and IL-10, hs-CRP.ResuIts after treatment in two groups IL-1,IL-6, hs-CRP content decreased significantly, and the observation group had a significantly higher degree of decline in the control group.Two groups of IL-10 level significantly increased, higher degree was significantly higher than the control group and observation group, which were statistically significant differences ( all P<0.05 ) . Observation group of patients’ pain time<1 d accounted for 35.00% higher than that of control group was significantly higher than 11.67%.And>3 d accounted for 20.00%was lower than the control group 41.67% significantly, which were statistically significant differences (all P<0.05).Observation group’ s curative effect for instituting accounted for 98.33%, were significantly higher than that of control group 88.33%, which were statistically significant differences ( P<0.05 ) .Observation group of PD, GI, GCF level was significantly higher than the control group, which were statistically significant differences (all P<0.05).ConcIusion Epoxy resin paste such as root canal filling material, can significantly improve patients with IL-10, IL-6, hs-CRP levels, relieve patients’ pain, clinical effect is remarkable.
4.Orbital implantation of hydroxyapatite: An analysis of 23 cases

This study was designed following eyeball extirpation to evaluate the therapeutic effects of hydroxyapatite and to explore the causes and preventing from exposure during orbital implantation. A total of 23 cases with autologou sclera and muscular suture selected from Department of Ophthalmology, Affiliated Hospital of Luzhou Medical College between January 2005 and December 2007 were retrospectively summarized and analyzed. Seventeen cases were implanted initially and the other 6 cases received secondary implantation. For the implantation with scleral-wrap, a 360? peritomy, limbic sclerotomy and full keratectomy were performed with corneoscleral scissors. Uveal tissue was completely removed. Posterior sclera was radially incised, and the hydroxyapatite orbital implant was implanted. Anterior sclera was sutured completely. Other implantations without using sclera-wrap were performed with the hydroxyapatite orbital implant in muscle cone and four rectal muscles were sutured to the hole of the hydroxyapatite orbital implant. Mattress suture of fascia tissue with double staggered position was performed in all the implantations. Conjunctiva was sutured discontinuously. This implant provided satisfactory functional motility and stability. Two cases developed implant exposure, suggesting that hydroxyapatite was the implant of choice for cosmesis following surgical removal of the eyeball, or enucleation. The complication rate is low and implant exposure, if it occurs, can be easily managed.

6.Optimization of immunodominant protein combinations for serological screening for Chlamydia trachomatis infection

Objective To optimize immunodominant protein combinations for serological screening for Cblamydia trachomatis (Ct) infection.Methods Both serum and genital swab samples were collected from 50 patients with Ct infection confirmed by colloidal gold immunochromatographic assay (GICA),and 30 GICA-negative clients without Ct infection at a sexually transmitted disease (STD) clinic in Tianjin Medical University General Hospital.The 30 serum samples from GICA-negative clients were also negative for microimmunofluorescence (MIF) assay.Eight Ct immunodominant proteins,including Pgp3,CPAF,CT143,CT101,CT694,CT875,CT813 and IncA,were selected as antigens to detect corresponding antibodies in the serum samples by enzyme-linked immunosorbent assay (ELISA) with the Ct proteins Hsp60 and major outer membrane protein (MOMP) as references.The results of ELISA were compared with those of the traditional gold standard method MIF assay to determine the immunodominant protein combination with the highest sensitivity and specificity.Results Of the 50 serum samples from patients with Ct infection,44 were positive and 6 negative by MIF.The results of ELISA with the combination of immunodominant proteins Pgp3,CT694 and CT875 as antigens were 97.73% (43/44) consistent to those of MIF assay.Of the 30 serum samples from GICA-negative clients,all were negative by MIF.Meanwhile,no antibody was detected against any of the immunodominant proteins Pgp3,CT694 and CT875 in any of the serum samples from GICA-negative clients.Conclusions The ELISA with the combination of immunodominant proteins Pgp3,CT694 and CT875 as antigens has good sensitivity and specificity for serological screening for Ct infection,and is simple to operate and easy to popularize.
7.Clinical effects of intermittent oro-esophageal tube feeding combined with Xuanqiaoliyan decotion in stroke patients with dysphagia

Objective:To investigate the clinical effects of intermittent oro-esophageal tube feeding (IOE) combined with Xuanqiaoliyan decotion in stroke patients with dysphagia.Methods:A prospective study was conducted. Stroke patients with dysphagia admitted to Yidu Central Hospital Affiliated Hospital of Weifang Medical University from January 2018 to December 2019 were enrolled. According to the simple random sampling method, the patients were divided into control group and observation group, with 50 cases in each group. The control group was given routine swallowing function training, including low-frequency pulse electrical stimulation, swallowing function training and acupuncture treatment. The observation group was given IOE and Xuanqiaoliyan decoction (prescription composition: Rhizoma acori tatarinowii 15 g, Radix polygalae 10 g, Rhizoma gastrodiae 15 g, Arisaema cum bile 6 g, Rhizoma typhonii 6 g, Scorpio 6 g, Bombyx batryticatus 6 g, Perilla frutescens 10 g, Rhizoma pinelliae 10 g, Pericarpium citri reticulatae 10 g, Rhizoma zingiberis recens 3 tablets, decoction 200 mL, twice in the morning and evening by oral or nasal feeding) on the basis of the control group. Both groups were treated for 14 days. The standard swallowing function assessment (SSA) and water swallow test were used to evaluate the swallowing function before and after treatment. The time required for the improvement of swallowing function, total hospitalization time and the therapeutic effects were observed and the safety assessment was conducted. Results:There were no significant differences in the gender, age, course of disease, and location and frequency of stroke between the two groups. After treatment, both the SSA scores in the two groups were decreased, and the grading of water swallow test was improved. The SSA scores in the observation group were significantly lower than that in the control group (19.8±1.8 vs. 23.2±3.2, P < 0.05), the recovery degree of water swallow test was higher than that in the control group [complete recovery (cases): 18 vs. 13, basic recovery (cases): 23 vs. 18, effective (cases): 9 vs. 19, χ 2 = -2.107, P = 0.008]. The total effective rate of swallowing function in the observation group was higher than that in the control group (94.0% vs. 80.0%, Z = 4.684, P = 0.012), the time for improvement (days: 12.8±2.6 vs. 16.9±4.3, t = 11.628, P = 0.008) and total hospitalization time (days: 20.8±4.2 vs. 33.5±5.6, t = 10.924, P = 0.015) were shorter than those in the control group. In the observation group, there was 1 case of throat discomfort during the operation of IOE, and the symptoms disappeared after the operation; there was 1 case of mild elevation of alanine aminotransferase (ALT) and blood urea nitrogen (BUN) respectively, which returned to normal after the treatment. No adverse symptoms and damage to the liver and kidney were observed in the control group. Conclusion:IOE combined with Xuanqiaoliyan decotion could significantly improve the swallowing function of stroke patients with dysphagia, shorten the hospitalization time, and improve the curative effects and lifequality.
8.Mini-hook locking plate fixation for avulsion intra-articular fractures of the metacarpophalangeal joint

Objective:To investigate the clinical effects of mini-hook locking plate fixation for the treatment of avulsion intra-articular fractures of the metacarpophalangeal joint.Methods:A retrospective case series study was conducted to analyze the clinical data of 13 patients with avulsion intra-articular fractures of the metacarpophalangeal joint admitted to Beijing Luhe Hospital of Capital Medical University from June 2016 to December 2017. There were nine males and four females, aged 22-61 years [(46.0±1.3)years]. All patients had closed fracture. According to Pechlaner classification, there were 5 patients with Grade A1 and 8 patients with Grade B1. All fractures were treated with open reduction and mini-hook locking plate internal fixation. Operation time, intraoperative blood loss, internal fixation position and bone healing time were recorded. The target joint range of motion (ROM) and grip strength were compared with the contralateral side two months after operation. Hand function was evaluated by total active motion (ATM). Visual analogue scale (VAS) was evaluated before operation two months after operation. Intraoperative and postoperative complications were recorded.Results:All the patients were followed up for 9-19 months [(11.3±1.1)months]. The operation lasted for 20-30 minutes [(25.5±1.3)minutes], with blood loss of 5-10 ml [(5.5±1.3)ml]. After operation, anatomical reduction was achieved in all patients and position of implant placement was suitable. Bone healing time was 4-8 weeks [(4.7±0.3)weeks] for all fractures. There were no significant differences in the target joint ROM compared with the contralateral side two months after operation ( P>0.05). ATM was excellent in 11 patients and good in 2, with excellent rate of 100%. VAS was significantly decreased from preoperative (5.9±1.1)points to (0.5±0.1)points two months after operation ( P<0.05). No internal fixation failure or wound infection occurred after operation. Conclusion:For avulsion intra-articular fractures of the metacarpophalangeal joint, mini-hook locking plate internal fixation has advantages of shortened operation time, rigid internal fixation after anatomical reduction, shortened bone healing time, fast recovery of hand function, obvious pain relief and few complications, which provides a treatment alternative for clinics.
9.Effect of peroneal tendon sheath integrity on posterior foot movement during calcaneal fracture

Objective In the patients with calcaneal fractures,the surgical method with intact peroneal tendon sheath was used to treat them,and compared with the traditional surgical method with dynamic retraction of the peroneal tendon,to analyze the influence of the surgical method on the posterior foot movement.Methods A retrospective analysis was made on 60 patients with calcaneal fracture admitted to the Department of Orthopaedics of our hospital from December 2016 to December 2020.The patients were divided into two groups according to the surgical method.The patients in the distraction group received the surgical treatment of traditional dynamic distraction of peroneal tendon(28 cases),and the patients in the preservation group received the surgical treatment of preserving the integrity of peroneal tendon sheath(32 cases).The outcome measures included perioperative indicators,Maryland score of foot function 12 months after surgery,AOFAS score of posterior foot and imaging indicators(Boler Angle and Gissane Angle)12 months after surgery.Results The surgical time(89.34±12.21 minutes)and hospitalization time(5.26±1.47 days)of the retention group were significantly better than those of the traction group[(124.22±11.56)min,(11.59±2.43)d](P<0.05).At the last follow-up,the AOFAS score(70.56±3.62)and Maryland's excellent and good rate(87.5%)of patients in the retention group were better than those in the traction group(81.37±3.58,71.4%)(P<0.05).The difference in imaging indicators between the two groups was relatively small(P>0.05).Conclusion The surgical method of keeping the peroneal tendon sheath intact in the treatment of patients with calcaneal fracture has definite clinical effects,and can also effectively reduce the adverse effects of surgical trauma on posterior foot activity.
10.Prognostic value of lymphocyte subsets in gastric cancer with different tumor infiltration patterns

Objective:To probe into the potential prognostic value of lymphocyte subsets in gastric cancer.Methods:This study included patients who underwent radical gastrectomy for gastric cancer from Aug 2014 to Dec 2016. The immunological differences was analyzed in different infiltration patterns. The overall survival of patients was analyzed by Kaplan-Meier method and Log-rank test. COX regression was performed to assess independent prognostic factors of the patients, and finally constructed nomogram.Results:The median number of peripheral CD4 and CD19 cells in infiltration pattern c was 750 (94-2 504) cells/μl and 186 (17-820) cells/μl; the median number of peripheral CD4 and CD19 cells in infiltration pattern a was 802 (203-2 071) cells/μl and 213 (5-948) cells/μl, the number of peripheral CD4,CD19 cells in infiltration pattern c was lower than that in infiltration pattern a, with statistically significant differences (CD4: Z=-3.061, P=0.002; CD19: Z=-2.016 , P=0.044). CD19 lymphocytes ( P=0.023) were associated with infiltration pattern a, CD8 lymphocytes ( P=0.027) were associated with infiltration pattern b, and CD4 lymphocytes ( P=0.026) were independent risk factors associated with the prognosis of infiltration pattern c. A nomogram can be constructed to evaluate the prognosis of patients. Conclusion:There are differences in the number of peripheral lymphocyte subsets in patients with different INF types. A nomogram can be constructed from lymphocyte subsets and clinicopathological features to assess patient prognosis.
