1.Enhancing the level of patient care by stepping up clinical consultation control
Chinese Journal of Hospital Administration 2001;0(08):-
Stepping up clinical consultation control is an effective way to improve the level of patient care and medical quality. A retrospective analysis of relevant clinical data led to the discovery that among the leading causes of clinical misdiagnosis and mistreatment were limitation in the scope of knowledge on the part of specialized physicians, inadequate collection of medical record data, over-reliance on accessory examinations, and one-sided clinical thinking. It is pointed out that stepping up clinical consultation control is of great importance as it can help solve clinical difficulties and raise the rate of accuracy in diagnosis; promote professional exchange and raise the rate of success in emergency treatment; cultivate and enhance young doctors expertise in diagnosis and treatment; protect the interests of patients and reduce medical disputes. The paper also puts forward measures for improving and implementing the system of clinical consultation: revising consultation criteria, standardizing consultation procedure, strengthening consultation supervision, and setting up a system of consultation result follow-up and retrospective case discussion.

Objective To evaluate MRI in judging the therapeutic effect of argon-helium cryoablation therapy for prostate cancer. Methods The clinical data and imaging materials of 16 patients with prostate cancer , who had received ultrasound-guided argon-helium knife cryogenic treatment at authors ’ hospital during the period from March 2012 to Oct. 2014, were retrospectively and comprehensively analyzed. The preoperative and postoperative laboratory test results were compared with MRI findings, and the ablation effect was assessed, focusing on the surgical residue, metastasis, etc. Results One months after the surgery, MRI demonstrated that satisfactory ablation extent was obtained in all patients, the ablated tumor tissue was characterized by long-T2 signal and no obvious recurrence could be found on DWI and DCE-MRI. A slight decrease of PSA level was observed. Six months after the surgery, MRI revealed that the prostate size was significantly reduced, PSA level was markedly decreased and no obvious evidence of recurrence was observed. No severe postoperative complications, such as urethral necrosis, urethral stricture or urethro-rectal fistula, occurred. During the 6-month following-up time, one patient died from other causes. Conclusion MRI has excellent clinical application value in estimating the ablation extent and in judging postoperative recurrence or metastasis of prostate cancer after argon-helium cryoablation treatment.

AIM: To evaluate the role of human angiotensin Ⅱ(AngⅡ) type Ⅰ receptor (AT1R) antisense cDNA (ahAT1) on migration of cultured artery smooth muscle cells (VSMCs). METHODS: Two recombinant adenoviral vectors, Ad/CMV. ahAT1 containing full length antisense cDNA targeting to human AT1R mRNA, and Ad/CMV.LacZ containing LacZ called report gene, were constructed by orientation clone technology and homologous recombination, and then were used to transfect VSMCs in vitro. AT1R expression detected by RT-PCR and immunohistochemistry, and migration of VSMCs measured by Boyden's Chamer methods, were compared between transfected and non- transfected VSMCs. RESULTS: Forty-eight hours after Ad/CMV. ahAT1 transfection, the level of AT1R mRNA decreased markedly (50% of control group), and AT1R protein expression was significantly less (P

Objective:To construct the continuous nursing health empowerment program for chronic heart failure (CHF) patients based on health empowerment theory so as to improve the participation and self-management of CHF patients in continuous nursing. Methods:From January to June 2019, we initially established the continuous nursing health empowerment program for CHF patients based on literature search combined with qualitative interview results. From July to August 2019, we selected 15 experts in cardiovascular medical nursing, geriatric nursing, community nursing and nursing management for two rounds of Delphi expert consultations by purposive sampling to construct the continuous nursing health empowerment program for CHF patients. Results:The recovery rate of two rounds of expert consultation questionnaires was 100% (15/15) , and the expert authority coefficient was 0.90. The results of the first round showed that the modification rate of experts was 66.67% (10/15) , adding 22 items, modifying 10 items, merging 13 items and deleting 7 items. The results of the second round showed that the modification rate of experts was 20.00% (3/15) , adding 1 item and modifying 2 items. After two rounds of Delphi expert consultations, we determined 5 first-level items, 18 second-level items and 75 third-level items. Conclusions:Construction of continuous nursing health empowerment program for CHF patients based on Delphi expert consultation help improve the participation and self-management of patients with CHF and improve the effectiveness of continuous nursing.
