1.Effect of chewing gum on gastrointestinal function after laparoscopic abdominal surgery

Objective To observe the effect of chewing gum on EGG,blood motilin and anus exhausting after laparoscopic abdominal surgery. Methods 200 patients after laparoscopic cholecystectomy were randomly arranged into chewing gum group (Group G): to start chewing gum for 30 min at breakfast time on first day after operation, and also at lunch and dinner time, and stopped until the first anus exhausting, and the control group(Group C): no chewing gum. The time between endpoint of operation to first anus exhausting was recorded; blood motilin and electrogastrography (EGG)on the morning of 1,2,3 days postoperation after chewing gum was detected. Results The time between endpoint of operation to first anus exhausting was not significantly different between two groups, compared with the control group, blood motilin significantly improved on the first day and the second day,but there was no difference between two groups on 3rd day; EGG was no different in all three days between two groups. Conclusions Chewing gum doesn't improve bowel function after laparoscopic abdominal surgery.
2.The clinical effect of pulmonary infection control window for sequence ventilation on treatment of severe pneumonia with respiratory failure

Objective To explore the clinical effect of pulmonary infection control window for sequence ven-tilation on treatment of severe pneumonia with respiratory failure,and to observe and analyze the prognosis.Methods 102 severe pneumonia patients complicated with respiratory failure were selected.They were divided into the observation group and the control group by the random number table method.The patients of the observation group were treated with pulmonary infection control window for sequence ventilation treatment,the control group were treated with conven-tional sequence ventilation treatment.The change of blood oxygen partial pressure(PaO2 ),carbon dioxide partial pres-sure(PaCO2 )and blood oxygen saturation (SaO2 )before and after ventilation,and the treatment time and symptoms improve time and the complications such as ventilator associated pneumonia(VAP)of the two groups were observed. Results PaO2 and SaO2 of the observation group after the ventilation were (75.60 ±2.89)mmHg and (0.78 ± 0.09),which were higher than control group after the ventilation (70.48 ±3.16)mmHg and (0.71 ±0.11),and data of two groups after ventilation were higher than those before the treatment,differences were statistically significant (t =11.30,10.92,13.26,14.08,all P <0.05).PaCO2 of the observation group after the ventilation was 49.21 ±3.22 mmHg, which was lower than that of control group after the ventilation of 54.35 ±3.82 mmHg,and data of two groups after treatment was lower than before treatment,the differences were statistically significant(t =12.44,11.85,14.16, 13.42,all P <0.05).The total mechanical ventilation time of the observation group,respiratory difficulty relief time, lung then disappear time and hospital stay time were (5.24 ±1.83)d,(38.13 ±2.91 )h,(6.72 ±0.40)d and (15.33 ±3.18)d,which were lower than those of control group,the differences were statistically significant (t =9.87,10.21,10.46,11.08,all P <0.05).The general adverse reaction and the death rate of the observation group was 9.80% (5 /51),which was lower than 19.61% of the control group(χ2 =9.85,P <0.05).Conclusion There was effect of pulmonary infection control window for sequence ventilation treatment of severe pneumonia with respirato-ry failure,and it could improve effectively lung function and the prognosis and reduce the incidence of complications.
3.Effect of Q Switch Nd∶YAG solid laser in treatment of naevus fusco-caerulens zygomaticus

Objective To observe the effect of laser of Q Switch Nd:YAG 1 064nm wave length laser in the treatment of naevus fusco caerulens zygomaticus. Methods Since October 2000, forty nine cases of naevus fusco caerulens zygomaticus had been treated once every three months. Results Thirty two cases had been treated three times, and seventeen cases, four times. The total effective rate was 100 %. The pigmentation of all the cases were darker after the first treatment, later as the treatment progressed, the pigment became lighter. No pigment loss was seen. Conclusion Q switch Nd:YAG 1 064 nm wavelength laser has a good effect in the treatment of naevus fusco caerulens zygomaticus. It is safe and reliable.
4.New primary mutation of mtDNA in Leber′s hereditary optic neuropathy

Objective To analyze the new primary mutation in Chinese people with Leber′s hereditary optic neuropathy (LHON). Methods Genomic DNA was collected from 260 suspected LHON patients and 100 normal healthy persons. The mitochondria DNA mutation at nucleotide position (NP) 15257 and the hot spot (14452-14601 bp) of ND6 gene which include the mutations at NP (14482, 14498, 14568, 14596, 14495, and 14459) were screened by using polymerase chain reaction (PCR), heteroduplex-single strand conformation polymorphism (HA-SSCP) and restriction fragment length polymorphism (RFLP) analysis and sequencing. Primary mutation spectrum of Chinese race was analyzed. Results Eight kinds of polymorphism of mitochondria DNA were found in 260 suspected LHON patients and 100 normal healthy persons, including NP 14488C, 14518G, and 14617G which hadn't been reported (http://www.mitomap.org/). No mutation at NP 15257, 14482, 14498, 14568, 14596, 14495, and 14459 was found. Conclusion The NP 15257A may not be the primary mutation in Chinese. Because of the race difference, 14452-14601 bp in ND6 gene may not be the hot spot in Chinese patients with LHON, and other hot spots may exist.
5.Impact of health literacy on education retention of stroke patients

Objective To describe the health literacy level and to explore the effect of health literacy level on education retention of patients with stroke.Methods Totally 100 stroke survivors were selected as the subject by convenience sampling.They were evaluated by Short Test of Functional Health Literacy in Adults on admission,and they accepted 60-90 minutes education intervention during hospitalization.Evaluation retention were measured before discharge.Results The score of the overall health literacy was (15.60±1.36) points,among of them,low level was 37 cases,general level was 24 cases,high level was 39 cases.Age,education level and Mini-Mental Status Exam score had a significant effect on the health literacy of patients with stroke (x2/F=14.248,12.082,11.329,P< 0.05).After education intervention,the score of education retention was (6.69±2.00)points,and the score of education retention in low,general,high level health literacy were (5.08±2.06),(7.46±1.61),(7.51± 1.76) points,and there were significant differences(F=14.223,P < 0.05).Heahh literacy had a effect on education(P < 0.01).Conclusions The level of health literacy of stroke patients has the effect of improving the education retention.Improving and maintaining the health literacy of stroke patients should be the development direction of the prevention and treatment of stroke diseases.
6.Effect of platelet lysate on the biological characteristics of human mesenchymal stem cells

BACKGROUND:Platelet lysate has been known as a kind of lysate of autologous or alogeneic platelet-rich products. It not only removes the residual cel structure, reduces immunogenicity, but also retains many growth factors. Platelet lysate has been suggested as a substitute for fetal bovine serum to expand mesenchymal stem cels in vitro. OBJECTIVE:To observe the effects of platelet lysate on biological characteristics of human bone marrow mesenchymal stem cels and adipose mesenchymal stem cels, and provide some experimental data for clinical cel therapy and regenerative medicine. METHODS:Platelet lysate was prepared by repeated freezing and thawing from fresh blood. Healthy adult bone marrow and adipose tissue were colected. Human bone marrow mesenchymal stem cels and adipose mesenchymal stem cels were obtained by density gradient centrifugation and type I colagenase digestion. We tested the morphology, cel phenotype, differentiation characteristics, proliferation capacity, colony forming ability and the level of cytokine secretion of bone marrow mesenchymal stem cels and adipose mesenchymal stem cels after cultured with platelet lysis. RESULTS AND CONCLUSION:Bone marrow mesenchymal stem cels and adipose mesenchymal stem cels were successfuly cultured in vitro using platelet lysate. There were no significant differences in morphology, cel phenotype, colony forming ability and the level of cytokine secretion, and chondrogenic, osteogenic and adipogenic capacities between bone marrow mesenchymal stem cels and adipose mesenchymal stem cels. Adipose mesenchymal stem cels had a high cumulative population doublings than bone marrow mesenchymal stem cels (P < 0.05). These findings suggest adipose mesenchymal stem cels had a stronger proliferative ability, and are more suitable for large-scale expansionin vitro cultivation system of platelet lysate compared with bone marrow mesenchymal stem cels.
7.Effect of stage health education on living-relative donor kidney transplantation recipients

Objective To evaluate the effect of stage health education on living-relative donor kidney transplantationrecipients.Methods Forty-eightliving-relativedonorkidneytransplantation recipients were randomly assigned to receive stage ( study group,n =24 ) or conventional health education (control group,n =24 ).ResultsIn comparison with the control group,the study group showed better awareness of kidney transplantation and improved satisfaction with nursing ( x2 =8.83,x2 =10.32,P <0.05).Conclusions Stage health education improves the awareness of renal transplantation and promotes mental and physical recovery,thus resulting in better quality of life of living-relative donor kidney transplantation recipients.
8.In vitro expansion of human mesenchymal stem cells in animal serum-free media

BACKGROUND:Mesenchymal stem cells have been an important source of seed cells for tissue engineering, regenerative medicine and immunosuppressive therapy areas. The large expansion is the key to the clinical application of mesenchymal stem cells. OBJECTIVE:To summarize the research progress in the in vitro expansion of human mesenchymal stem cells in animal serum-free media. METHODS:A computer-based online retrieval of CNKI and PubMed databases was performed to search papers published between 2004 and 2014 using the key words of“mesenchymal (stromal) stem cells, animal serum-free media, humanized media, human serum, umbilical cord blood serum, platelet rich plasma, platelet lysate, defined medium”in English and Chinese, respectively. Final y, 41 articles were retained. RESULTS AND CONCLUSION:Human serum, umbilical cord blood serum, platelet rich plasma, platelet lysate and defined medium can replace fetal bovine serum for large-scale expansion of mesenchymal stem cells in vitro. These media or additives have their advantages and disadvantages;therefore, it is urgent to guarantee the functional effectiveness and transplantation safety before large-scale, systematic clinical application of mesenchymal stem cells.
9.Large-scale expansion of human umbilical cord mesenchymal stem cells in human platelet lysate as a substitute of fetal bovine serum

BACKGROUND:Classic media and fetal bovine serum are commonly used in the culture of umbilical cord mesenchymal stem cells, but the potential risk of serum culture limits its clinical application. OBJECTIVE:To use human platelet lysate alternative to fetal bovine serum for large-scale production of mesenchymal stem cells for therapeutic applications. METHODS:Human platelet lysate was prepared by repeated freezing and thawing, centrifugation, filtration, and concentration. Human umbilical cord mesenchymal stem cells were cultured in Iscove’s Modified Dulbecco’s Medium containing 5%concentrated platelet lysate (experimental group) or 10%fetal bovine serum (control group). After separation and enzymatic digestion, human umbilical cord mesenchymal stem cells were subcultured at a concentration of 3 000/cm2 up to the fifth generation. Then, cellmorphology and diameter, immune phenotype, osteogenic and adipogenic differentiation, and cloning efficiency were detected and compared between two groups. RESULTS AND CONCLUSION:Human umbilical cord mesenchymal stem cells cultivated in human platelet lysate-supplemented media showed a smal er size and more elongated morphology than those in fetal bovine serum-supplemented media. Colony forming unit-fibroblast analyses further showed no significant differences in colony efficiency. Human umbilical cord mesenchymal stem cells cultivated in human platelet lysate-supplemented media showed an increase of proliferation capacity;whereas, similar immunophenotypes remained in the two groups. In vitro assays revealed intact differentiation potential. Moreover, human umbilical cord mesenchymal stem cells cultivated in human platelet lysate-supplemented media showed a significantly higher capacity to differentiate towards osteocytes, indicating human platelet lysate is an alternative to fetal bovine serum for low-density production of mesenchymal stem cells for therapeutic applications.
10.Analysis of ambroxol hydrochloride on inflammatory factor level changes of TNF-alpha and hs-CRP in severe pneumonia and clinical therapeutic effect

Objective To investigate the therapeutic effect of ambroxol hydrochloride on severe pneumonia, and the influence on the inflammatory cytokines of TNF-alpha and hs-CRP etc.MethodsThe clinical data of 70 patients with severe pneumonia patients treated in our hospital from December 2014 to August 2016 were retrospectively analyzed.The patients were divided into control group(n=30) and observation group(n=40) according to the treatment methods, the control group was given routine treatment, the observation group was given ambroxol hydrochloride on the basis of conventional treatment.The therapeutic effect of the two groups was observed, the differences of serum levels of inflammatory cytokines, pulmonary function, cough and expectoration symptom scores of the two groups were compared before and after treatment.ResultsThe effective rate of the observation group was 97.50%, which was significantly higher than that of the control group(80.00%);There was no significant difference in serum inflammatory cytokine levels between the two groups before treatment, after treatment,the levels of IL-6, IL-8, hs-CRP and TNF-alpha the observation group were lower than those in the control group;There was no significant difference in the indexes of lung function between the two groups before treatment.After treatment, the levels of FVC, FEV1 and FEV1/FVC in the observation group were higher than those in the control group;There was no significant difference in symptom score between the two groups before treatment.After treatment, cough, sputum volume, cough ease and wheezing scores in the observation group were lower than in the control group.ConclusionAmbroxol hydrochloride has a good therapeutic effect on severe pneumonia,which can significantly reduce the levels of inflammatory cytokines such as TNF-α and hs-CRP, and improve the related symptoms, and has good application value.
