1.Study on the differences of Ketamine and Fentanyl in pediatric regional anesthesia

Objective To explore the difference and clinical effect in pediatric regional anesthesia with ketamine and fentanyl.MethodsThe clinical data of 60 cases of children with surgical treatment from June 2014 to December 2015 in Longde People's Hospital were retrospectively analyzed and randomly divided into two groups,with 30 cases in each group.The control group was given simple ketamine anesthesia;The intervention group was given ketamine combined with fentanyl anesthesia, of children with breathing,After anesthesia,the heart rate, respiratory,arterial pressure and other indicators and the anesthetic effect of the two groups were comprehensively evaluated.ResultsIn the intervention group after anesthesia, heart rate, respiratory rate, mean arterial pressure and pulse oxygen saturation was significantly better than the control group, there was statistical significance (P<0.05);and wake-up time in the intervention group, the dosage of ketamine and ketamine bolus times was significantly lower than the control group,there was statistical significance (P<0.05).ConclusionThe regional anesthesia operation were given ketamine and fentanyl assisted anesthesia, can stabilize children's heart rate, respiratory rate, shorten postoperative awake time, reduce anesthetic dosage, higher safety, anesthetic effect, can be widely applied in clinical.
2.Clinical study on the effect of transvaginal total hysterectomy under combined spinal epidural anesthesia

Objective To analyze the effect of transvaginal total hysterectomy under combined spinal epidural anesthesia. Methods From December 2015 to December 2016, in Longde people's hospital 80 cases underwent transvaginal total hysterectomywere randomly divided into two groups, he observation group and the control group. The control group was treated with continuous epidural anesthesia, the observation group were given combined spinal epidural anesthesia. The anesthesia effect and VAS scores were observed and compared. Results The excellent rate of anesthesia in the observation group was 95%, which was significantly higher than 75% in the control group(P<0.05). 1h after anesthesia, there was no statistically significant difference in the cough and static state VAS scores between the 2 groups; 6 and 12 hours after anesthesia, the cough and static state VAS scores was significantly better than those in the control group(P<0.05). Conclusion Compared with epidural anesthesia, combined spinal epidural anesthesia for vaginal total hysterectomy has a better anesthetic and analgesic effect, anesthesia safety is high, it is worthy of clinical application.

4.Malnutrition related factors in patients with gastric cancer

Malnutrition is one of the most important factors for the prognosis of patients with gastric cancer,but the mechanism of malnutrition is unclear at present. These patients are at risk of nutritional deple-tion from anorexia,dysphagia,and may be associated with the presence of two or more of the following cyto-kines with metabolic activity such as TNF-α,IL-1,IL-6,interferon,leptin,nuclear factor-κB and so on, which are secreted by the host-tumor interaction. The cytokines constitute a sophisticated regulatory network. Therefore,the study of the relationship between malnutrition and related factors in patients with gastric cancer may improve the nutritional status and prognosis of these patients.
5.Effectiveness of Microresection Combined with Radiofrequency Ablation under Laryngoscope and Rigid Telescope for Laryngeal Precancerosis

Objective To investigate the effectiveness of microresection combined with radiofrequency ablation under laryngoscope and rigid telescope for laryngeal precancerosis. Methods Fifty-one patients with laryngeal precancerosis, which was diagnosed pathologically, were treated with microresection combined with radiofrequency ablation under laryngoscope and rigid telescope. The patients were followed up for over 6 months. Results During a mean of 26-month follow-up (6 months to 4.5 years), 7 of the patients had recurrence (vocal cord leukoplakia in 3, atypical hyperplasia in 2, and laryngeal papilloma in 2); 4 had carcinomatous change (vocal cord leukoplakia in 1, atypical hyperplasia in 2, and laryngeal papilloma in 1);and 2 patients developed laryngeal web after surgical treatment for bilateral vocal cord leukoplakia. Objective To detect the serum estrogen level in women with involution al depression (ID) and to study the effect of estrogen on the occurrence of ID. Methods Serum estrogen levels at 7:00 am were detec ted by r adioimmunoassay in 40 normal menopausal women (Group A) and 49 cases of ID (Grou p B), and compared with those of 36 cases previously detected. Results Estro gen level was much lower in Group B than Group A (P 0.05 ). Co nclusion The occurrence of ID in menopausal women may be related to t he decrease of estrogen level, but the specific correlation of the above two has not been proved yet.
7.Esthetical effects of crown lengthening surgery on teeth with subgingival defect

Objective To observe the results of crown lengthening surgery on the teeth with subgingival defect.Methods Crown lengthening surgery was performed on 54 terior teeth,which were divided into two groups based on probing depth of pre-operation:group A (<3 mm) and group B (3-4 mm).All teeth were restored 8 weeks after operation and followed-up to 1 year.The parameters of plaque index (PLI),suleus bleeding index (SBI),maximal defect probing depth (PD) and mobility degree (MD) at different times were measured,respectively.Results PLI,BI and PD were significantly improved during the follow-up period (P<0.01).The success rates of both groups were 96.8% and 69.6%, respectively (P<0.01).No significant difference about MD in the group A one year after restoration (P>0.05),but a significant improvement in the group B (P<0.01).Conclusion Crown lengthening surgery is a good method on the teeth with subgingival defect,but one must select the right indication.

9.Effect of different withdrawal time of dexmedetomidine on the quality of general anesthesia recovery

Objective To explore effect of different withdrawal time of dexmedetomidine (DEX) on the quality of general anesthesia recovery.Methods Eighty patients of ASA Ⅰ or Ⅱ undergoing lymph surgery were randomly assigned to four groups (n =20).Groups D1,D2 and D3 received DEX 0.5 μg/kg as bolus before induction,continued with 0.5 μg/(kg · h) by infusion until one hour,30 min before the end of operation and the end of operation,respectively.Group C received equal volume of normal saline.Mean artery pressure (MAP),heart rate (HR) and pulse oxygen saturation (SpO2) were observed during and after operation.Spontaneous respiration recovery time,eyes open time,extubation time,orientation recovery time,observer's assessment of alertness/sedation score (OAA/S),restlessness score (RS),and visual analogue scales (VAS) were observed after operation.Results For MAP and HR in Groups D2 and D3,there were no statistically significant difference during extubation compared to those at preoperation,but at the same time point,they were lower than those in group C (P < 0.05).For groups C and D1,MAP and HR at extubation and 5 min after extubation were higher than those at preoperation (P <0.05).Spontaneous respiration recovery time,eyes open time,extubation time,orientation recovery time in group D3 were significantly longer than those in Group C (P < 0.05),while there were no statistically significantly difference between groups D1,D2,and group C.OAA/S in group D3 was significantly lower than that in group C (P < 0.05) at extubation.Incidence of restlessness,VAS,and cases given analgesic 2 hours after operation in groups D2 and D3 were significantly lower than those in groups C and D1.Compared to those in group C,dosages of propofol and remifentanil in groups D2 and D3 were significantly lower.Conclusions Dexmedetomidine administered of 0.5 μg/kg before induction,continued with infusion of 0.5 μg/(kg · h) until 30 min before the end of operation,may improve emergence,without influencing the awakening time of patients,and prolong the duration time of analgesia which comfort the patients.
10.The Clinical Diagnosis and Treatment of 143 Cases of Hodgkin's Lymphoma

Objective To investigate the clinical characteristics of hodgkin's lymphoma (HL) patients . Method The clinical data of HL patients in our hospital from January 1, 2003 to December 31, 2012 were analyzed retrospectively. The contents of investigation included gender, age, pathological type, first symptom, the incidence locations, clinical stages, extranodal involvement locations, ECOG and treatments.Results A total of 143 cases of HL were included, the M/F ratio was 2.5:1 with a median age of 37 years (11~73years) . The most frequent subtypes of HL were mixed cellular (34.3%) and nodular sclerosis (28.0%) . The nodal lymphomas accounted for 97.2%, while the extranodal lymphomas occupied 2.1%. The nodal lymphomas with extranodal invasion accounted for 29.5%, and the common locations were spleen (14.4%), lung (5.8%) and bone (5.0%) . The most clinical stage of all patients was stage Ⅰ/Ⅱ (60.8%), The differences were no significant in gender, pathological type, first symptom, the incidence locations and the effective rate of the first-line treatment (P>0.05), while were statistically significant in B symptoms, ECOG and treatments betweenⅠ/Ⅱgroups andⅢ/Ⅳgroups (P<0.05) . Conclusions In this group,the incidence of HL patients were mainly found in middle-aged people, males were lager than females.The pathological type, first symptom and the incidence locations had no obvious effect on clinical stage. The clinical stage had no obvious influence on the effective rate of the first-line treatment.
