1.Effect of chewing gum on gastrointestinal function after laparoscopic abdominal surgery

Objective To observe the effect of chewing gum on EGG,blood motilin and anus exhausting after laparoscopic abdominal surgery. Methods 200 patients after laparoscopic cholecystectomy were randomly arranged into chewing gum group (Group G): to start chewing gum for 30 min at breakfast time on first day after operation, and also at lunch and dinner time, and stopped until the first anus exhausting, and the control group(Group C): no chewing gum. The time between endpoint of operation to first anus exhausting was recorded; blood motilin and electrogastrography (EGG)on the morning of 1,2,3 days postoperation after chewing gum was detected. Results The time between endpoint of operation to first anus exhausting was not significantly different between two groups, compared with the control group, blood motilin significantly improved on the first day and the second day,but there was no difference between two groups on 3rd day; EGG was no different in all three days between two groups. Conclusions Chewing gum doesn't improve bowel function after laparoscopic abdominal surgery.
2.Observation of layering fibula feet cushion in prevention of deep venous thrombosis after total hip replacement

Objective To evaluate the curative effect of layering fibula feet cushion to prevent deep venous thrombosis(DVT) after total hip replacement(THA).Methods 100 patients who underwent THA were divided into two groups at random.The research group was applied layering fibula feet cushion while the control group was applied general feet cushion.VAS,the occurrence rate of DVT,disappearance time of feet swelling and Harris hip function score were used to evaluate the curative effect.Results In according to these four observation indexes,the score of the research group was better than the control group.The effective rate of the research group was 96%,while the control group was 82%.There was a significant difference between the two groups.Conclusions The layering fibula feet cushion can effectively prevent deep venous thrombosis after total hip replacement.It can be generalized and applied in clinical nursing.
3.Allogeneic hematopoietic stem cell transplantation using reduced-intensity Bu/Cy conditioning for patients with hematologic malignancies

Objective To explore the efficacy of allogeneic hematopoietic stem cell transplantation using reduced-intensity Bu/Cy conditioning for patients with hematologic malignancies.Methods Five patients with hematologic malignancies were treated by allogeneic hematopoietic stem cell transplantation using reduced-intensity Bu/Cy conditioning,which consisted of busufan3~4 mg/(kg?d) for 3 days,cyclophosphamide50 mg/(kg?d) for 2 days, Ara-C 2 g/(m~2?d) for 1 or 2 days,and CsA 3 mg/(kg?d) and mycophenolate mofetil 1 g/d 7 days before the transplantation.Results Five patients established successful engraftment and no severe complications occured.After a follow-up of median 10.5(3-22)months,five patients still survived without diseases.Conclusion Reduced-intensity Bu/Cy conditioning may reduce transplantation-related toxicities.Allogeneic hematopoietic stem cell transplantation using reduced-intensity conditioning is a safe and effective option for the patients with hematologic malignancies.
4.Expression level of Interferon regulatory factor 5 in systemic lupus erythematosus

Objective To compare the expression level of interferon regulator factor 5 (IRF5) of the health controls and systemic lupus erythematosus (SLE) patients and analyze the relationship between IRF5 and SLE disease activity. Methods Peripheral blood monoeytes (PBMCs) from SLE patients and healthy donors were separated with Ficoll density gradient eentrifugation and total cellular RNA was isolated with Trizol from the PBMCs, the mRNA was reverse transcripted to cDNA. Real-time PCR was applied to determine the expression level of IRFS. The expression level of IRF5 between the two groups were compared. The correlations of expression level of IRF5 with SLE disease activity and other laboratory or clinical parameters of SLE patients were analyzed. Results The level of IRF5 was (2.1±2.2) in SLE patients and (1.5±1.2) in healthy controls, the difference was not statistically significant (P=0.161). And the levels of IRF5 in SLE patients were significantly correlated with their SLEDAI (r=0.616,P<0.01), but not correlated with other parameters such as bemoglobulin complements, immunoglobulin etc. Anti-dsDNA antibody positive patients had significantly higher expression of IRF5 compared to the anti-dsDNA-antibedy-negative patients. The IRF5 mRNA levels of SLE patients with fever or neuropsyehiatric symptoms were significantly higher than those of patients free of neuropsychiatrie involvement. Conclusion High expression level of 1RF5 may contribute to the pathogenesis of SLE in disease activity and antibody production.
5.Immunological characteristics of chronic graft versus host diseases murine model with lupus nephritis

Objective:To establish the murine systemic lupus erythematosus (SLE) model of chronic graft versus host diseases(cGVHD). To analyze the pathological changes and serological and immunological features in the animals. Methods: Female (C57BL/10?DBA/2)F1 hybrids aged 6-8 weeks were randomly divided into model group and healthy controls (HC). Lymphocytes from female DBA/2 were injected intravenously to the model group on days 0, 3 and 8,while PBS were injected to the HC under the same condition as a control group. Bradford was applied to monitor the development of albuminuria quantitively. Sera were tested by enzyme linked immunosorbent assay (ELISA) and indirect immunofluorescence (IIF) for the presence of autoantibodies. To compare the differences of CD4+CD25+ Treg cells between the two groups by flow cytometry (FCM) and the differences in the expression of Foxp3 by real time polymerase chain reaction(RT-PCR). The kidneys of model mice were removed in the 12th week and were made frozen sections for direct immunofluorescence(DIF)and paraffin imbedding for PASM staining. Results: The titers of proteinuria in model group in the 6th week, 8th week, 10th week, and 12th week were significantly higher than those of the HC groups(P=0.004, 0.005,respectively). National traditional Chinese medicine (TCM) master professor Ban Xiuwen had rich experiences on treatment of gynecological diseases with unique insights on diagnosis and treatment for disorders of menstruation, leukorrhea, pregnancy and delivery. FemaleYin-Wei, which was a gynecological disorder, was recorded in the book ofHuang-Di Nei-Jing. However, due to different reasons, TCM study on femaleYin-Wei was far less than the study on male impotence. ProfessorBan Xiuwen had specific considerations for this disease on the book of Treatment of Miscellaneous Gynecological Diseases. In his understanding, femaleYin-Wei was due to four aspects, which were the innate deficiency, yang deficiency and coldness of the uterus; seven emotion damage, liver-qi soothing disorder; spleen-stomach deficiency,qi-blood deficiency; and phlegm-dampness stasis,qi stagnation. Through the analysis and study on case records of femaleYin-Wei treatment by national TCM master professorBan Xiuwen, this article briefly elaborated professorBan Xiuwen’s understandings on femaleYin-Wei treatment from etiology, pathogenesis, treatment according to syndrome differentiation, and clinical experiences. Two femaleYin-Wei cases treated by professorBan Xiuwen were given as examples in order to analyze the etiology, pathogenesis, treatment according to syndrome differentiation and treatment experiences in details. The therapeutic effects of both cases were obvious, which were also important study documents.
7.CPU86017 and its enantiomers inhibit abnormal gene expression of calcineurin and NFκB in rat cardiomyopathy induced by L-thyroxin

AIM: To investigate the CPU86017 and its enantiomers inhibit abnormal gene expression of calcineurin and NFκB in rat cardiomyopathy induced by L-thyroxin and compare the effect of CPU86017 (racemate) with its 4 enantiomers: (7S, 13R), (7S, 13S), (7R,13S), and (7R,13R)-CPU86017 in this model. METHODS: The animals were randomly divided into 7 groups. The rat hypertrophied model was produced by treatment with L-thyroxin 0.2 mg·kg-1·d-1, sc for 10 d and treated with CPU86017 or its enantiomers 4 mg·kg-1·d-1, sc from d 6 to d 10. The changes in left ventricular (LV) weight index, redox system, and the NO and iNOS activity in the myocardium were investigated. The expression of mRNA of calcineurin、NF-κB in the left ventricle was measured. RESULTS: There were significant cardiac hypertrophy and oxidative stress in rats treated by L-thyroxin. The expression of calcineurin, NFκB mRNA were upregulated (P<0.05, compared with that of control). After treatment with CPU86017 (racemate and enantiomers), LV remodeling and the redox system were improved. CPU86017 and (7S,13R)-CPU86017 showed a better improvement on LV remodeling and the redox than the other isomers and restored the normal expression of calcineurin, NF-κB (P<0.05, P<0.01), respectively. CONCLUSION: It suggested that an up-regulation of calcineurin and NFκB possibly related to the altered intracellular calcium handling system plays a role in the progression of L-thyroxin induced cardiomyopathy and CPU-86017 and its 7S,13R-CPU86017 enantiomer effectively inhibit the abnormal expression of calcineurin and NFκB genes, the NOS enzyme and oxidant stress in the cardiomyopathy.
8.Effects of different positive end-expiratory pressures on efficacy of volume therapy guided by global end diastolic volume index and central venous pressure in patients with septic shock

Objective To compare the effect of different positive end-expiratory pressures (PEEPs) on the efficacy of volume therapy guided by global end-diastolic volume index (GEDVI) and central venous pressure (CVP) in patients with septic shock.Methods Twenty-five patients with septic shock complicated with respiratory failure,of both sexes,aged 18-64 yr,were enrolled in the study.Their APACHE [[scores were 13-31.The patients were endotracheally intubated and underwent volume-controlled ventilation,PEEP was 5-15 cmH2O,and pulse oxygen saturation was maintained > 90 %.The patients were divided into low PEEP (5-10 cmH2 O) group and high PEEP (11-15 cmH2 O) group depending on the different PEEP levels.6 % hydroxyethyl starch (200/0.5)6 ml/kg was infused over 30 min for volume therapy.Right internal jugular vein or subclavian vein was cannulated for CVP monitoring,and GEDVI was continuously monitored by pulse indicator continuous output monitoring (PiCCO) technology.CVP and GEDVI were recorded before and after volume therapy and the changing rate was calculated.Results Compared with CVP and GEDVI before volume therapy,CVP and GEDVI were significantly increased after volume therapy in low PEEP group (P < 0.05),and GEDVI was increased after volume therapy (P < 0.05) and no significant change was found in CVP after volume therapy in high PEEP group (P > 0.05).Compared with low PEEP group,the changing rate of CVP was significantly decreased (P < 0.05),and no significant change was found in the changing rate of GEDVI in high PEEP group (P > 0.05).Conclusion High PEEP can decrease the efficacy of volume therapy guided by CVP,while exerts no effect on the efficacy of volume therapy guided by GEDVI in patients with septic shock.
9.Instructions of Neural Response Telemetry for Prediction of the Rehabilitation Effects and Mapping in Children Cochlear Implant

Objective To study the stability of NRT threshold and the correlation between NRT threshold and the value of behavior T ,C in order to instruct mapping in infant cochlear implant ,and to study whether the elic‐itation of NRT waveform can be applied to determine the effects of rehabilitation .Methods There were 159 cases with complete information in data in this study .150 out of the 159 cases coincided with the study requirements of the changes and correlation .We had collected the NRT thresholds of the electrodes of No .1 ,6 ,11 ,16 ,22 in switch-on ,one month after switched -on ,half a year and one year after switched -on .The analysis of variance method was adopted to compare the threshold changes of each electrode in different stages .We had collected the values of behavior T ,C in one year after switched -on .Pearson regression analysis was used to compare the correlation be‐tween NRT threshold in switch -on and this value of behavior T ,C .We applied MAIS assessment scale to make a questionnaire for all 159 patients during the follow -up at one year after switched -on .We had gathered the scores in three aspects ,the average score in questionnaire and the average hearing threshold of every child .All children were divided into four groups according to whether the elicitation of NRT waveform during - operation ,post-op‐eration .Results Regarding the NRT threshold ,there was no significant difference in the pairwise comparison of four time points(P>0 .05) .There was positive correlation between NRT threshold and behavior T ,C(P<0 .01) , but the correlation was wake .The group with no waveform on the during -operation and post -operation had sig‐nificant differences with the other groups in all scores and average hearing threshold (p<0 .01) .And there was no statistical significance among the other three groups (P> 0 .05) .Conclusion The NRT threshold remains steady from switched -on .A positive correlation exists between NRT threshold and the actual T ,C value .The NRT threshold can be used to estimate the T ,C value .But the accuracy of the measured value is less than the behavior method .We still need to use behavior method to measure as soon as possible .The NRT waveform appearance on the post-operation is more significant than that on the during -operation to prognosis of rehabilitation .
10.The summary of experience and effect observation of cochlear implants in adults with different causes.

OBJECTIVE:
This article summarizes the matters warranting consideration in adult cochlear implants before and after operation with different deafness causes and investigates the impact of etiological variables that affect hearing and speech ability rehabilitation after cochlear implantation.
METHOD:
We retrospectively reviewed the preoperative data of 30 adults who have received cochlear implantation, switch on, and periodical post operative mapping. 'Nijmegen Cochlear Implant Questionnaire' was used for every patient during follow-up and test their hearing thresholds at one year after switched-on. The average score of every patient in the six aspects of questionnaire, along with the average hearing threshold, has been gathered.
RESULT:
The average hearing threshold of every patient is lower than 35 dBHL. The average score of 28 patients is more than or equal to 75 in the basic sound perception, advanced sound perception as well as self-esteem. The average score of 26 patients is more than or equal to 75 in capacity for action. Besides, as for sociability and speech ability, the average score of 22 patients is more than or equal to 75.
CONCLUSION
Auditory perception can be greatly improved regardless of deafness causes, on the premise that appropriate candidate is selected. The primary disease should be brought under control before and after the operation. The influence of various causes to the language ability rehabilitation largely depends on the development of the language center at deafness onset. Self-esteem and social identity can be significantly enhanced after cochlear implantation in adult patients, particularly those with acquired deafness.
