1.Recent progress in direct-acting antiviral agents for chronic hepatitis C

The current standard treatment for hepatitis C virus (HCV)infection is a combination of pegylated interferon (PEG-IFN)and ribavirin.However,it remains incurable in some patients.In recent years,direct acting antiviral agents (DAAs)that target specific HCV enzymes in HCV life cycle have been developed rapidly.A combination of PEG-IFN,ribavirin,and DAAs can increase the sustained viro-logical response rate in patients with chronic hepatitis C (CHC).For patients who cannot use or tolerate interferon therapy,a combination of various DAAs has good efficacy.Therefore,DAAs bring new hope for the treatment of CHC.
2.Resolution of Chromatographic Peaks byRadial Basis Function Neural Network Based onPlate Model Based on a Developed Sorting Genetic Algorithm

Radial Basis Function Neural Network Based on Plate Model (P-RBFNN) is constructed for resolution of chromatographic peaks of unknown components number. Then a two-phase sorting genetic algorithm (TP-SGA)-training structure and evolving is intruduced to train the network so that it has the ability of re-constructed structure. TP-SGA has robustness and random globe optimization. The alternate use of gradient descent and TP-SGA makes the network have the ability to learn structure, therefore makes itself adaptable to resolution of the chromatographic peaks of unknown components number. The method proposed here needs no artificial interference, not only has it robustness and globalism. With its characteristics related above and its ability of decomposing and analysing, this method has obvious advantages comparing with others.
4.Pregnancy outcomes of 103 women with pulmonary arterial hypertension

Objective To evaluate the pregnancy outcome of women with pulmonary arterial hypertension (PAH).Methods The medical records of 103 pregnant women with PAH admitted to Beijing Anzhen Hospital from January 2007 to March 2011 were studied retrospectively.Results (1) Degree of PAH and cardiac function.Among the 103 PAH,92 cases were patients with congenital heart disease and 13 cases were with rheumatic heart disease.They were divided by color doppler ultrasound into mild PAH group (34 cases),moderate PAH group (22 cases) and severe PAH group (47 cases).Per heart function classification,21 cases (20.4%,21/103) were class Ⅰ,44 cases (42.7%,44/103) were class Ⅱ,27 cases (26.2%,27/103) were class Ⅲ and 11 cases (10.7%,11/103) were class Ⅳ.More patients were class Ⅲ and Ⅳcardiac function in the severe PAH group than in the mild and moderate PAH groups,with statistically significant difference (P < 0.05).(2) Delivery mode.There were 44 term delivery and 23 preterm birth in the 103 PAH patients.Sixty-three cases (94%,63/67) received cesarean section and 4 cases had vaginal delivery.There were 36 iatrogenic abortion (35.0%,36/103).The iatrogenic abortion rate in the severe PAH group was significantly higher than those in the mild and moderate PAH groups (P < 0.05).(3) Perinatal outcomes.The full-term delivery rates in the mild and moderate PAH groups [80% (20/25) and 14/17] were significantly higher than the severe PAH group (40%,10/25),respectively (P < 0.05).And the premature birth rate of the severe PAH group(60%,15/25) was significantly higher than the mild and moderate PAH group [20% (5/25)and 3/17,P < 0.05].There were 4 neonatal asphyxia.The birth weight of mild,moderate and severe PAH groups were (3071 ± 443),(2910 ± 619) and (2381 ±589) g,respectively.The birth weight in the severe PAH group was significantly lower than the mild and moderate PAH groups (P < 0.05).(4) Mortality.Nine cases naternal death happened,with a mortality of 8.7% (9/103).All of them were severe pulmonary hypertension patients.Conclusions PAH is a serious complication during pregnancy.Women with mild PAH and normal cardiac function could continue pregnancy to full-term.Women with moderate PAH might continue pregnancy to 32 weeks under intensive care,cesarean delivery is preferable.Pregnant women with severe PAH should terminate pregnancy because of the high mortality.
5.Retrospective analysis of clinical profiles of Brucellosis

To analyze the clinical characteristics,complications and management of Brucellosis.As a retrospective review,23 patients with Brucellosis admitted into Peking University First Hospital over a 10-year period were reviewed.Most of them were middle-aged males.Common presenting clinical manifestations included high fever,sweating,arthralgia and headache.Splenomegaly was more common than hepatomegaly.There were liver injury (n =15),transient proteinuria (n =6),secondary hemophagocytic syndrome (n =2) and renal failure with infective endocarditis,pneumonia with infective endocarditis,renal abscess (n =1).Most of them were treated effectively with combination therapy.The clinical features of Brucellosis are both complicated and atypical.
6.Morphology of human skin fibroblasts after heat injury in vitro

AIM: Previous studies paid more attention to the effect of fibroblast immigration in normal surrounding tissue or blood on wound healing. This study explored the morphological character of fibroblasts after heat injury to support the new operation of autogenous epidermis overlapping denatured dermis. METHODS: The experiment was performed at the State Key Laboratory of Trauma, Burn and Combined Injury in the Third Military Medical University of Chinese PLA from April 2006 to April 2007. The dermal fibroblast from the infant prepuce disposed after surgery was cultured in vitro, and divided into 2 groups. In normal group, the dermal fibroblasts were put in water bath at 37 ℃ for 30 seconds; the cells in experimental group were stimulated at different temperatures (50, 51, 52, and 53 ℃) for different time (30, 60, 90, and 180 seconds). Cell survival was detected by MTT; morphology of injured cells were observed under inverted microscope and transmission electron microscope at 3 hours, 1, 2, 3, 4, 5, and 7 days; cell cycle was detected by flow cytometry. 7.Establishment of finite element model of lumbar motion segments and its biomechanical significance

BACKGROUND: The commonly used research methods of lumbar biomechanics are animal model,physical model and corpse model respectively. However, each model has its own disadvantages.OBJECTIVE: To establish the finite element model of lumbarmotionsegments in order to provide theoretical basis for biomechanical research of lumbar.DESIGN: Single sample study based on healthy volunteer.SETTING: Orthopaedic department of a university hospital.PARTICIPANTS: The experiment was completed in the Orthopaedic Department of Xiangya Hospital of Central South University from December 2003to August 2004. A healthy male volunteer was chosen as a mock object.METHODS:CT scanning was conducted to the spine(T12 - S1) of a healthy volunteer by 2 mm layer and got 264 images. The 3D model of normal Chinese male L4 5 motion segments was built by combininglumbar images scanned by CT and anatomic data through 3DSMAX software. Then it was transferred into finite elementmodel by finite element analyzing software SAP2000.MAIN OUTCOME MEASURES: ① Vertebral body model andintervertebraldisc model built by 3DSMAX software; ② Finite element model of motion segment made by SAP2000 software.RESULTS:The finite element model of lumbar L4-5 segments had been built. It had totally 2 120 nodes that include 1 728 Solid units, 592 Area units and 50 Link units.CONCLUSION: The finite element model of lumbar motion segments can be established through CT scanning, digital image processing and computer aid design to further study the biomechanics of spine.
8.68Ga-PSMA-11 PET/MR multiparameter imaging in the diagnosis of naive prostate cancer

Objective:To explore the diagnostic value of 68Ga-prostate specific membrane antigen (PSMA)-11 PET/MR multiparameter multimodal functional imaging in the diagnosis of naive prostate cancer (PCa), and to analyze its efficacy in the early stages of PCa. Methods:From July to September 2019, 45 suspected or pathologically confirmed PCa patients (average age: 69 years) who met the inclusion criteria were collected to perform 68Ga-PSMA-11 PET/MR examination in Nanjing First Hospital. After the scanning was completed, the method of region of interest (ROI) was used to semi-quantitatively calculate the tumor radioactive uptake in the fusion image, including the maximum standardized uptake value (SUV max), tumor metabolic volume (MTV) and mean standardized uptake value (SUV mean), and PSMA expression load was calculated(SUV mean×MTV). Apparent diffusion coefficient (ADC) values of ROI were measured in ADC images ( b=1 500 s/mm 2). The efficacy of 68Ga-PSMA-11 PET/MR in the preoperative diagnosis of PCa and the effect on clinical staging were evaluated with the pathological results. The correlation between prostate specific antigen (PSA) and radiation uptake in PCa tissues, PSMA expression load and ADC values was analyzed by Pearson correlation. Independent-sample t test was used to analyze the data. Results:Pathologically, 38 of the 45 patients were with PCa and 7 patients had benign prostatic hypertrophy (BPH), and metastasis occurred in 12 of the 38 PCa patients. 68Ga-PSMA-11 PET/MR detected 39 cases of PCa with one of which was false-positive. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of 68Ga-PSMA-11 PET/MR were 100%(38/38), 6/7, 97.4%(38/39), 6/6 and 97.8%(44/45), respectively. The tumor tissues of PCa often showed focal radioactive uptake, and the T 2 weighted imaging (WI) showed focal low signal and limited dispersion. BPH showed slightly uneven uptake, and T 2WI and diffusion weighted imaging (DWI) showed uneven diffuse signals. SUV max of PCa was significantly higher than that of BPH (24.66±19.21 vs 4.97±2.13; t=5.208, P<0.001). ADC values of PCa were significantly lower than that of BPH ((0.91±0.37)×10 -3vs (1.08±0.24)×10 -3 mm/s 2; t=2.816, P<0.05). SUV max and the expression loads of PSMA in PCa were positively correlated with PSA ( r values: 0.42 and 0.71, both P<0.05). ADC values of tumor tissues in PCa were negatively correlated with PSA ( r=-0.37, P=0.013). Conclusion:68Ga-PSMA-11 PET/MR has great merits in the early diagnosis and staging of PCa.
9.Monitoring depth of anesthesia and effect analysis in primary visual cortex of rats based on complexity of local field potential.

In the present study carried out in our laboratory, we recorded local field potential (LFP) signals in primary visual cortex (V1 area) of rats during the anesthesia process in the electrophysiological experiments of invasive microelectrode array implant, and obtained time evolutions of complexity measure Lempel-ziv complexity (LZC) by nonlinear dynamic analysis method. Combined with judgment criterion of tail flick latency to thermal stimulus and heart rate, the visual stimulation experiments are carried out to verify the reliability of anesthetized states by complexity analysis. The experimental results demonstrated that the time varying complexity measures LZC of LFP signals of different channels were similar to each other in the anesthesia process. In the same anesthesia state, the difference of complexity measure LZC between neuronal responses before and after visual stimulation was not significant. However, the complexity LZC in different anesthesia depths had statistical significances. Furthermore, complexity threshold value represented the depth of anesthesia was determined using optimization method. The reliability and accuracy of monitoring the depth of anesthesia using complexity measure LZC of LFP were all high. It provided an effective method of realtime monitoring depth of anesthesia for craniotomy patients in clinical operation.

10.Comparative analysis of various clinical laboratory diagnosis methods of pulmonary tuberculosis

Objective To discuss the value of combination of various clinical laboratory methods in the diagnosis of Mycobacteri-um tuberculosis(MTB) .Methods T-SPOT .TB ,sputum smear ,sputum culture ,and MTB-DNA detection were used to detect MTB .The sensitivity ,specificity ,rate of missed diagnosis ,coincidence rate and correct diagnosis index for tuberculosis diagnosis of the four methods and their pairwise combinations were analyzed and compared .Results Comparing with other methods ,T-SPOT . TB combined with MTB-DNA detection had the highest sensitivity ,coincidence rate and correct diagnosis index ,and the lowest rate of missed diagnosis ,with significant difference(P<0 .05) .Conclusion T-SPOT .TB combined with MTB-DNA detection could be used as an adjuvant method for the early diagnosis of tuberculosis .
