1.Association of UGT1A1*6 Polymorphisms with Irinotecan-induced Toxicities:A Meta-analysis

Objective To conduct a meta?analysis of literatures to explore the relationship of UGT1A1*6 gene polymorphism and irinotecan toxici?ty,so as to guide clinical treatment. Methods Papers were searched by PubMed database and manual search. The inclusion and exclusion criteria of studies were formulated and the methodologies quality was assessed,data were extracted and the statistical analysis was made using STATA12.0 software. Results A total of 12 articles were included according to the inclusion and exclusion criteria. Patients with mutated UGT1A1*6 showed an increased risk for neutropenia compared to wild UGT1A1*6(OR=2.37,95%CI 1.58?3.55,P=0.001). Both homozygous and heterozygous muta?tion showed an increased risk for neutropenia compared to wild type and the homozygous mutation(OR=5.09,95%CI 2.74?9.45,P<0.001) showed an even higher risk for neutropenia compared to the heterozygous mutation(OR=2.07,95%CI 1.37?3.13,P=0.001). For severe diarrhea, mutated UGT1A1*6 showed an increased risk compared to wild type(OR=1.48,95%CI 0.86?2.55,P=0.153),though without statistical signifi?cance. The homozygous mutation performed a significantly increased risk(OR=3.51,95%CI 1.33?9.25,P=0.011)and the heterozygous mutation also showed increased risk,however,the difference between them was not statistically significant. Conclusion UGT1A1*6polymorphisms can pre?dict irinotecan toxicity,especially for incidence of neutropenia.
3.Analysis of brain CT in 1542 patients with epilepsy

Objective To evaluate the diagnostic value of CT in epileptic pathogeny. Methods Clinical data and CT findings of 1542 patients with epilepsy were studied retrospectively. Result The positive CT finding was 42.35% . The epileptogenic focus which were found by CT, included in turn: brain atrophy, and excephaloma and cerebral congenital or development disorder, and cerebrovascular disease, and encephalomalacia, and unidentified hypodense or hyperdense lesions, calcification, and infection, and postoperative changes, and trauma and the sequelaes of hypoxic- ischemic encephalopathy .Conclusion CT is helpful to determine the diagnosis, types and therapy of epilepsy.
4.Infiltration of dendritic cells and lymphocytes in hepatocellular carcinoma tissue.

OBJECTIVETo explore the clinical significances of dendritic cells and lymphocytes infiltration in hepatocellular carcinoma (HCC) tissue.
METHODSClinicopathological data were collected from 44 patients with HCC who had under/gone curative tumor resection in our hospital. Immunohistochemical staining was used to detect the infiltration of dendritic cells in the tumor tissue, and lymphocytes infiltration was assessed simultaneously. The correlation between the infiltration of dendritic cells and lymphocytes and postoperative tumor recurrence and survival rate was analyzed.
RESULTSTumor recurrence was markedly late in patients with dendritic cells count >/= 20 and positive lymphocytes infiltration (group A, n = 17) as compared with those who did not meet both criteria simultaneously (group B, n = 27), with a median interval of 21.6 months for group A and 4.1 months for group B (U value = 105.5, P = 0.009). The 1-, 3-, 4-year survival rates were significantly greater in group A than in group B; they were 83.5% vs. 42.2%, 61.8% vs. 28.4% and 48.7% vs. 23.0%, respectively (Log rank = 7.68, P < 0.01).
CONCLUSIONThe infiltration of dendritic cells and lymphocytes in HCC tissue, as an independent prognostic factor, was closely related to postoperative prognosis.

5.Emphasize and standardize the implement of the multidisciplinary team model in the diagnosis and treatment of acute mesenteric ischemic disease

Acute mesenteric ischemic disease(AMI) is a critical surgical emergency. Due to its insidious and atypical initial symptoms, a delayed diagnosis significantly increases the mortality rate of AMI patients. The diagnosis and treatment process of AMI often involve close collaboration and communication among multiple disciplines and departments.Our team deeply recognizes the importance of a multidisciplinary team(MDT) model in the standardized management of the AMI disease. It can significantly improve the efficiency of disease diagnosis, delay disease progression, provide timely treatment, and improve the prognosis of patients, thereby reducing the overall mortality rate. Therefore, combining our center′s experience in treating AMI patients with the implementation of the MDT model, we elaborated on the key steps and quality control measures for the standardized application of the MDT model in AMI disease, with the aim of providing a theoretical foundation and experiential reference for colleagues and professionals.
6.Mechanism Study on Chaihuang Qingyi Huoxue Granules Reduce Inflammatory Damage in Severe Acute Pancreatitis Rats by Suppressing NLRP3 Inflammasome Activation

Objective To investigate the protective effect and mechanism of Chaihuang Qingyi Huoxue Granules on pancreatic tissue of rats with severe acute pancreatitis,and to observe its regulation on NLRP3 inflammasome activation.Methods Sixty-four SD rats were randomly divided into sham-surgery(SO)group,severe acute pancreatitis model(SAP)group,Chaihuang Qingyi Huoxue Granules(CH)group,and MCC950(NLRP3 inhibitor)group.Each group was further divided into 12-hour and 24-hour subgroups,with rats in each group.The SAP group,CH group,and MCC950 group were retrogradely injected with 3.5%sodium taurocholate(2 mL·kg-1)into the pancreatic ducts to establish SAP model.The MCC950 group was immediately intraperitoneally injected with MCC950(1 mg·mL-1)after model preparation.After awakening from anesthesia,the CH group was administrated by gavage with Chaihuang Qingyi Huoxue Granules solution(0.35 g·mL-1)once every 6 hours.Ascites,abdominal aortic blood,and pancreatic tissue were collected at 12 hours and 24 hours after SAP model construction.The serum amylase and lipase activities were detected using an automated biochemical analyzer.HE staining was used to observe pancreatic injury.Serum levels of IL-18 and IL-1β were detected by ELISA.The expressions of gene and proteins related to the activation of NLRP3 inflammasome were analyzed by IHC,qRT-PCR and Western Blot.Results Compared with the SAP group,the pathological damage of pancreatic tissues in the CH and MCC950 groups was significantly reduced,and the pathological score was significantly reduced(P<0.05).The levels of serum lipase,amylase,IL-18,and IL-1β were also significantly decreased(P<0.05).After treatment with Chaihuang Qingyi Huoxue Granules or intraperitoneal injection of NLRP3 inhibitor,the positive expressions of NLRP3,ASC and Caspase-1 in pancreatic tissues,as well as the mRNA levels of NLRP3,ASC and Caspase-1,the protein levels of NLRP3,ASC,Pro-Caspase-1 and Caspase-1 were significantly reduced compared to the SAP group(P<0.05).Conclusion Chaihuang Qingyi Huoxue Granules can inhibit the activation of NLRP3 inflammasome,reduce the mRNA and protein expressions of NLRP3,ASC and Caspase-1 in pancreatic tissues,and suppress the release of the downstream inflammatory factors IL-18 and IL-1β and alleviate pancreatitis damage in SAP model rats.
7.A study on correlation of hepatic metastasis of colorectal cancer to vascular endothelial growth factor, angiopoietin-2, and fibronectin.

OBJECTIVETo study the correlation of hepatic metastasis of colorectal cancer to vascular endothelial growth factor (VEGF), angiopoietin- 2 and fibronectin (FN).
METHODSThe transcription and expression of VEGF, angiopoietin- 2 and FN were detected by semiquantitative reverse transcription- polymerase chain reaction (SQRT- PCR) and immunohistochemical staining in the specimens from sixty patients with colorectal cancer.
RESULTSThe mRNA transcription and expression levels of VEGF, angiopoietin- 2 and FN in colorectal cancer tissues were obviously higher than those in paratumor normal tissues P< 0.05. The transcription and expression levels of VEGF were correlated with tumor invasion P< 0.05,Dukes' stage P< 0.05,and lymph node and/or hepatic metastasis P< 0.05. Angiopoietin expression and transcription levels were correlated with tumor differentiation. The expression of FN in extra cellular matrix (ECM) was significantly higher (P< 0.05),whereas ECM in basement membrane was significantly lower in cancer tissues than that in paratumor normal tissues (P< 0.05),which both were correlated with tumor invasion P< 0.05,Dukes' stage P< 0.05,and lymph node metastasis P< 0.05. Absence of FN protein in basement membrane was also correlated with hepatic metastasis.
CONCLUSIONColorectal cancer cells can secrete VEGF and contribute to metastasis and proliferation of tumor by stimulating the growth of tumor vessel. Both of VEGF and angiopoietin- 2 contribute to angiogenesis and the decrease of FN in basement membrane of cancer tissue is an important primary factor of hepatic metastasis.

Objective To clarify the mechanisms that the response of fibroblast-like synovial (FLS) cellsto methotrexate (MTX) in rheumatoid arthritis (RA) and to provide theory basis for the drug treatment of RA.Methods Synovial fibroblasts were isolated from synovial tissue specimens obtained from patients with RA andexposed to MTX.Cell viability was measured using a MTT assay and cell apoptosis was valued by flow cytometry.Western blotting analysis of LC3 and immunocytochemistry were used to analyze the induction of autophagy in RA-FLS after treating with MTX.Transfection of siRNA was used to interfere the expression of Beclin1 to down-regulate the autophagy,cell apoptosis was valued by flow cytometry and western blot analysis was used to test the PARPp85 with or without the presence of MTX.Statistical product and service solutions (SPSS) 18.0 statistical software was used for statistical analysis of all experimental data.Independent sample t test was used according to data distribution status,homogeneity of variance,and normal distribution.GraphPad Prism 5.0 was used to draw statistical graphs.Results MTX induced apoptosis was increased in RA-FLS.MTX stimulated the autophagy response in RA-FLS by inducing autophagosome formation.In RA-FLS,transfection with Beclin1 siRNA inhibited autophagy and increased the susceptibility to MTX,which induced cell death.Conclusion Autophagy of RA-FLS contributes to the resistance to apoptosis induced by methotrexate.

0.05 ). After once injection both observers considered the number of clearly recognized endocardial border segments increased significantly. The number evaluated by observers A increased from 2.68 ? 0.95 to 5.99 ? 0.10 while from 2.82 ? 1.03 to 5.99 ? 0.11 by observers B( P 0.05 ). The average contrast enhancement rate of LV endocardial border was 99.7 %. Perfluoropropane-albumin microsphere injection had no significant effection on vital signs such as blood prssure, heart rate and respiration. Electrocardiogram didn′t change markedly and the variance of the laboratory findings like blood and urine routine examination, hepatic and renal function was in normal range. Only one case( 0.33 %) had slight side-effects who suffered from mild nausea and diarrhea, which suggested the clinical safety of this contrast agent. Conclusions Perfluoropropane-albumin microsphere injection could enhance the resolution of LV endocardial borders and make the judgement of regional myocardial movement easier. It has little side-effects and will be appropriate for clinical use.
