1.Evaluation of the application effect of foreseeable nursing and evidence-based nursing in anesthesia patients in the operation room
Chinese Journal of Practical Nursing 2012;28(25):46-48
Objective To discuss an effective nursing mode for operation room anesthesia. Methods 122 cases of patients with local anesthesia:in our hospital from January to December 2011 were divided into the nursing group and the control group with 61 cases in each group. They were given foreseeable nursing and evidence-based nursing respectively. The quality of nursing care, postoperative cognitive function and life quality were evaluated by self-made patient satisfaction questionnaire. Results The satisfactory degree of patients in the nursing group was 91.80%,compared with the control group of 88.52%,the result showed no significant difference. Each factor of life quality demonstrated no evident difference between two groups before treatment, but after treatment, the scores of emotional function and cognitive function were higher, while the score of somatic function was lower in the nursing group compared with those in the control group. Conclusions If the two methods of nursing can be combined in anesthetized patients in operation room,it can be of great importance to decrease the patients fear and improve the quality of life.

This study is to investigate effects of genistein on rat femoral bone metabolic in vitro. Rat femoral tissues was isolated and randomly divided into two groups including control group and genistein (1 x 10(-5) mol x(-1)) group. Determinations of alkaline phosphatase (ALP) activity, calcium content and osteoprotegerin (OPG), type I-collagen (Collagen-I), RANKL, Runx-2 and bone morphogenetic protein (BMP-2) mRNA expression were done by real-time PCR. The results showed that 1 x 10(-5) mol x L(-1) genistein could increase the activity of ALP and contents of Ca, regulate bone metabolism activity of OPG, RANKL, BMP-2, Collagen-I and Runx-2 mRNA expression level. Genistein can significantly modulate bone metabolism related gene expression level of rat femoral tissue in vitro, and can increase calcium content and the activity of ALP.

Data of 643 patients who underwent endoscopic polypectomy with high-frequency electrotome in 6 hospitals of Sichuan Province between June 2020 and September 2020 were summarized. The rate of complete polypectomy and the incidence of delayed post-polypectomy bleeding (DPPB) and perforation were analyzed. DPPB occurred in 18 cases (2.80%) and postoperative perforation occurred in 1 case (0.16%). All of the 1 828 polyps were completely resected (100.0%). Univariate analysis showed that polyps′ diameter≥10 mm, long peduncle or laterally spreading tumor (LST), adenomatous polyp, endoscopic mucosal resection, mixed cutting mode 1 of electrocoagulation were significantly correlated with DPPB( P<0.05). Multivariate Logistic regression analysis revealed that polyp diameter≥10 mm ( P=0.001, OR=3.575, 95% CI: 1.175-9.955), morphology of long peduncle or LST ( P=0.004, OR=2.981, 95% CI: 1.233-14.858) were independent risk factors for DPPB. Endoscopic colorectal polypectomy with high-frequency electrotome is effective and safe. Polyps′ diameter≥10 mm, polyps with long pedicle or LST are the risk factors for DPPB.

Objective To observe the effects of radial artery pressure combined with somatosensory evoked potential monitoring on decreasing the incidence of brachial plexus paralysis in ankylosing spondylitis (AS)patients with kyphosis of thoracic and lumbar vertebra who were corrected surgically by pedicle subtraction osteotomy (PSO).Methods A total of 36 AS patients with kyphosis of thoracic and lumbar vertebra undergoing the surgery of PSO were selected from January 201 4 to December 201 4.During surgery, all patients were applied with artery pressure combined with somatosensory evoked potential monitoring for protecting brachial plexus. Results Positive monitoring indicators occurred in 2 patients during surgery. And then, the monitoring indicators returned to normal by adjusting body position on time;they were not with the symptom of brachial plexus paralysis after surgery. However, positive monitoring indicators occurred in 1 patient and didn′t return to normal by repeatedly adjusting body position. Thus, the patient was found with the symptom of mild brachial plexus paralysis after surgery and returned to normal at 1 week after treatment. There were no neural complications caused by operation during surgery. Conclusions Radial artery pressure combined with somatosensory evoked potential monitoring can indicate the change of compression degree of the brachial plexus in AS patients with kyphosis of thoracic and lumbar vertebra undergoing the surgery of PSO,and it can effectively decrease the incidence of brachial plexus paralysis by appropriate treatment.

Objective:To investigate the role of CD40/CD40L Pathway in the formation of silicosis fibrosis. Methods:Totally 64 inpatients were recruited and assigned to the silicosis group and the control group, 23 in each group. The alveolar lavage fluid was collected from all patients and isolated. The expression of CD40L protein was detected by Flow Cytometry. The level of IL-8、The IL-6、INF-γ and MCP-1 was detected by ELISA. Two groups of BALF were co-cultured with HFL-1 cells, the expression of Collagen I and α-SMA was detected by Immunohistochemistry. Results:Compared with the control group, CD40L was highly expressed on T lymphocyte cells in silicosis group ( P<0.05) , and the contents of IL-8、The IL-6、INF-γand MCP-1 in Silicosis group were significantly higher than those in control group ( P<0.05) . After co-culture of BALF and HFL-1 cells, the expression levels of Collagen I and α-SMA in Silicosis group were significantly higher than those in control group ( P<0.05) . Conclusion:CD40-CD40L cross-linking system can promote the activation of T cells, release inflammatory factors, promote the synthesis of collagen I and α-SMA by fibroblasts, make the lung fibrous tissue proliferate, and lead to the formation of silicosis fibrosis.
