Objective To establish a real-time PCR assay to detect bacterial contamination in platelet concentrates(PCs).Methods PCs were spiked with serial dilutions of Staphylococcus aureaus and Eschetichia coli, and the bacterial genomic DNA was isolated by modified Chelex-100 method and then quantitatively detected by Tagman real-time PCR.Filtration was performed to avoid contamination from other bacterial DNA in the reagents of extraction kit as well as the PCR mixture. Results The real-time PCR targeting conservative 16S rRNA gene showed high sensitivity and specificity in detecting bacterial contamination in PCs,and no cross-reaction with human genomic DNA and viruses was found. The Ct value of the lowest bacterial dose groups after filtration in both S.aureaus and E.coli were statistically different from the negative controls, with a minimal detection quality of 0.3CFUs/PCR for S.aureaus, and 0.1CFUs/PCR for E.coli.,respectively.Conclusion The modified Chelex-100 method to extract bacterial genomic DNA and the subsequent real-time PCR are convenient, sensitive and specific, and may be applied to the large scale clinical detection of bacterial contamination in PCs.

A sandwich format immunochromatographic assay for detecting foot-and-mouth disease virus (FMDV) serotypes was developed. In this rapid test, affinity purified polyclonal antibodies from Guinea pigs which were immunized with sucking-mouse adapted FMD virus (A/AV88(L) strain) were conjugated to colloidal gold beads and used as the capture antibody, and affinity purified polyclonal antibodies from rabbits which were immunized with cell-culture adapted FMD virus (A/CHA/09 strain) were used as detector antibody. On the nitrocellulose membrane of the immunochromatographic strip, the capture antibody was laid on a sample pad, the detector antibody was printed at the test line(T) and goat anti-guinea pigs IgG antibodies were immobilized to the control line(C). The lower detection limit of the test for a FMDV 146S antigen is 11.7ng/ml as determined in serial tests after the strip device was assembled and the assay condition optimization. No cross reactions were found with FMDV serotype C, Swine vesicular disease (SVD), Vesicular stomatiti svirus (VSV) and vesicular exanthema of swine virus (VES) viral antigens with this rapid test. Clinically, the diagnostic sensitivity of this test for FMDV serotypes A was 88.7% which is as same as an indirect-sandwich ELISA. The specificity of this strip test was 98.2% and is comparable to the 98.7% obtained with indirect-sandwich ELISA. This rapid strip test is simple, easy and fast for clinical testing on field sites;no special instruments and skills are required, and the result can be obtained within 15 min. To our knowledge, this is the first rapid immunochromatogarpic assay for serotype A of FMDV.

Objective To find noninvasive estimation of partial pressure of carbon dioxide in artery (PaCO2) by measuring the end-tidal CO2 partial pressure (PETCO2) in elderly patients with respiratory failure with chronic obstructive pulmonary disease (COPD). Methods All the 30 acutely exacerbated COPD subjects received routine clinical treatment including bronchodilators, mucolytics, glucocorticosteroid, antibiotics and oxygen therapy for 5-7 days, and part of them received noninvasive positive-pressure ventilation (NIPPV) treatment concurrently. They were both tested by eupnea method and prolonged expiratory method before and after treatment. Results Before treatment, PET CO2 (Q) (end-tidal CO2 pressure with eupnea, (50.72±8.93) mm Hg wassignificantly lower than PaCO2 (F=38.73, P<0.01 ). Yet, PETCO2(P) (end-tidal CO2 pressure with prolong expiration) was (70.35±8.91) mm Hg and PaCO2 was (71.25±9.08) mm Hg. There was no significant difference between PETCO2 (P) and PaCO2 (P>0.05). The similar results were found after treatment. By linear regression analysis, PetCO2(P) was remarkably positive correlated with PaCO2 before and after treatment (r=0.96 and 0.97, respectively, P<0.01). According to TABG,PCO2(C) which was calculated by the average expiratory time of fitting curve was (71.78±9.04)mm Hg. And there was no significant difference between PCO2 (C) and PaCO2 (P>0.05);Thesimilar results were founcl after treatment. By linear regression analysis, PCO2 (C) was remarkably positively correlated with PaCO2 (r=0.97 and 0.98, respectively, P<0.01) before and after treatment. Conclusions In COPD patients with type Ⅱ respiratory failure, conventional PETCO2 (Q) is significantly lower than PaCO2. Yet, PETCO2 (P) could exactly estimate PaCO2 and is appropriate to its dynamic monitoring. And PCO2 (C) which is calculated by mode Boltzmann on eupnea curve also obtain similar results.

Chinese Journal of Primary Medicine and Pharmacy 2020;27(22):2763-2767
Objective:To investigate the application effect of dexmedetomidine combined with propofol and remifentanil in fiberoptic bronchoscopy in elderly patients with chronic obstructive pulmonary disease(COPD).Methods:From October 2018 to November 2019, 112 elderly patients with COPD who were examined by fiberoptic bronchoscopy in the Affiliated Hospital of Medical College of Ningbo University were divided into control group and observation group, with 56 cases in each group.Propofol and remifentanil were used for anesthesia induction in both two groups.Dexmedetomidine (0.5μg/kg) was injected intravenously into observation group, and later 0.5μg·kg -1·h -1 continuous infusion, the control group was given the same dose of 0.9% sodium chloride injection. The OAA/S scores, propofol dosage, remifentanil dosage, wake-up time, heart rate, oxygen saturation, mean arterial pressure, pulmonary function changes and adverse reactions were compared between the two groups. Results:At the fiberbronchoscope entered immediately and 10 min after examination, the OAA/S scores in the observation group were (1.03±0.35)points, (4.42±0.28)points, respectively, which in the control group were (1.22±0.22)points, (3.01±0.22)points, respectively, the differences between the two groups were statistically significant( t=3.439, 29.631, all P<0.05). The dosages of propofol and remifentanil in the observation group were (3.00±0.28)μg/kg and (1.65±0.29)μg/kg, respectively, which were less than those in the control group [(3.70±0.39)μg/kg, (2.42±0.45)μg/kg], the recovery time was (8.40±1.58)min, which was shorter than that in the control group [(13.31±2.02)min], the differences were statistically significant( t=10.911, 10.763, 14.327, all P<0.05). The heart rate, oxygen saturation, mean arterial pressure, pulmonary function(FEV 1, MVV) in the observation group were significantly better than those in the control group( t=9.757, 11.981, 11.486, 11.913, 40.189, all P<0.05). The incidence of adverse reactions in the observation group was 7.14%(4/56), which was lower than 19.64%(11/56) in the control group(χ 2=6.737, P<0.05). Conclusion:Dexmedetomidine(0.5μg/kg, followed by 0.5μg·kg -1·h -1 continuous infusion) and target controlled infusion of propofol and remifentanil are safe for elderly patients with COPD, and the use of anesthetic drugs is small, which can shorten the recovery time after operation, and will not have a great impact on respiratory circulation.
