Objective:To investigate the prognostic factors of patients with esophageal squamous cell carcinoma with pulmonary metastasis. Methods:Clinical characteristics of 135 esophageal squamous cell carcinoma patients presenting with pulmonary metastasis after treatment in Zhejiang Cancer Hospital from 2008 to 2018 were retrospectively analyzed. Thesurvival rate was calculated by Kaplan-Meier method. Univariate analysis was performed by log-rank test. Multivariate prognostic analysis was conducted by Cox models. Results:The median follow-up time of 135 patients with esophageal squamous cell carcinoma was 94.2 months (19.5-258.9 months), and 109 patients died (80.7%). The 1-and 2-year overall survival rates were 47.4% and 25.1%, with the median survival time was 11.1 months (7.3-14.9 months). Univariate prognostic analysis showed that age, number of lung metastases, treatment of lung metastases, lymph node metastasis, distant organ metastasis, and the interval between the first treatment and lung metastasis were the prognostic factors of esophageal squamous cell carcinoma with lung metastasis (all P<0.05). Multivariate analysis demonstrated that age and number of lung metastases were the independent prognostic factors for patients with esophageal squamous cell carcinoma with lung metastases (all P<0.05). Conclusions:Age and number of lung metastases are the independent prognostic factors for patients with esophageal squamous cell carcinoma with lung metastases. Surgery or radiotherapy-based regional therapy can enhance clinical prognosis.

Objective To investigate preventive measures and pathogenic related factors of periodontitis. Methods A total of 138 patients with periodontitis from August 2012 to March 2014 in our department were selected as the experi-mental group. Meanwhile 138 healthy cases matched with the patients within 2 years age difference in the same regions were selected as the control group. Questionnaire survey was used to inquire the basic information, educational status, economic income, acquisition of knowledge in periodontal disease, brushing habits, negative life events and so on. The differences in results between two groups were compared. Results Among 138 patients with periodontitis, there was 1087 disease parts on maxillaries, 872 parts on mandibles, 1243 parts on molars, and 876 parts on anterior teeth and bicuspids. According to Amaitage standard, 138 patients with periodontitis, the rate of mild to moderate periodontitis patients was 81.16% (112 cases), and the rate of severe periodontitis was 18.84% (26 cases). Acquisition of knowledge in periodontal disease, smoking, the frequency and sustained time of brushing per day, negative life events and mouth care between two groups showed statistical significance(P<0.05). There was no significant difference between two groups in age, gender and educational status(P>0.05). Non conditional Logistic regression results showed that: independent pathogenic risk factors of periodontitis included acquisition of knowledge in periodontal disease, the frequency and sus-tained time of brushing per day, negative life events and regular mouth care, and regular mouth care ranked first among these factors(the odds ratio=3.102, P<0.05). Conclusion Periodontitis is the result of interaction of multiple risk factors, therefore, comprehensive measures especially regular mouth care should be adopted to prevent the occurrence of peri-odontitis.

Objective To investigate the changes of sex hormone and androgen receptor levels and evaluate the relationship of the sex hormones and androgen receptor with coronary heart disease (CHD) in elderly men. Methods A cross-sectional study was performed in 539 elderly men, including 400 healthy people aged 62-92 years and 139 CHD patients aged 60-88 years. The plasma concentrations of total testosterone (TT), free testosterone (FT), dehydroepiandrosterone sulfate (DHEAS), sex hormone binding globulin (SHBG), estradiol (E2), luteinizing hormone (LH) and follicle-stimulating hormone (FSH) were measured. The androgen receptor (AR) level was tested by flow cytometry. Results The fluorescence intensity of DHEAS, TT, SHBG, FT and AR were significantly lower in CHD group than in healthy group (P<0.01);however, FSH and E2 in CHD group were higher(P(0.01). Age was negatively correlated with TT(r=-0.28,P<0.01) and FT (r=-0.17,P<0.05), and positively correlated with SHBG(r=0.14,P<0.05) and E2 (r=0.33, P<0.01). AR fluorescence intensity was negatively correlated with systolic blood pressure (r=-0.12,P<0.01). Logistic regression analysis indicated that TT (OR=1.065,9% CI: 1.012~1.121,P<0.05), SHBG(OR=0.994,95% CI:0.990~0.998,P<0.01) and AR (OR=0.971,95%CI:0.956~0.986, P<0.01)were significantly associated with CHD in elderly male patients. Conclusions The levels of DHEAS, TT, SHBG, FT and AR are lower in elderly men with CHD than in elderly healthy men;however, the FSH and E2 concentrations are higher. Low levels of TT, SHBG and AR may be the independent risk factors for CHD in elderly men.

Objective To evaluate and compare the consistency and repeatability of three-dimensional ultrasound (3D-US) virtual organ computer-aided analysis (VOCAL) technique and two-dimensional ultrasound (2D-US) in the measurement of thyroid volume. Methods The thyroid volume of 50 healthy female adults aged 18 to 68 years were measured by 2D-US and 3D-US.The 2D-US was used to measure the three diameter lines (L,H,W) of the largest thyroid section, and the thyroid volume was calculated by the classical ellipsoid formula V=0.523×L×H×W.The 3D-US VOCAL technique was used to acquire the thyroid three-dimensional data, and VOCAL software was used to measure the thyroid volume. The two methods of measurement were also compared and analyzed. Results The coefficient of variation in the intra-assay was 0.738% and 1.59% respectively for the 3D-US VOCAL technology and traditional 2D-US measurement of thyroid volume, and the 95% limit of agreement in the intra-assay was (-0.26 cm3,0.22 cm3) and (-0.32 cm3,0.46 cm3).In addition, the correlation coefficient in the inter-assay were 0.970xx and 0.942xx,and the 95% limit of agreement were (-0.36 cm3,0.37 cm3) and (-0.75 cm3,0.64 cm3).To compare the two methods of measuring thyroid volume, the 3D-US VOCAL had a lower variation coefficient in the intra-assay,a higher correlation coefficient in the inter-assay and a narrower 95% limit of agreement, which showed that the repeatability of the 3D-US VOCAL technique measuring was better than the traditional 2D-US in measuring thyroid volume. Conclusions The 3D-US VOCAL technique shows a higher repeatability in measuring thyroid than traditional 2D-US.As a safe, simple and feasible precise measurement method,it provides a reliable and effective new method for clinical measurement of irregular organ volume.
