Objective To explore the effect of comprehensive intervention on constipation caused by antipsychotic drugs.Methods Ninty patients with antipsychotic drug induced constipation who were admitted to our hospital from June 2012 to October 2013 were enrolled in this investigation and randomly assigned to the comprehensive intervention group(CIG,n=46) and the routine care group(RCG,n=44).The subjects of the RCG received routine diet and medication therapy; and those of CIG received comprehensive intervention of life style,psychological ability,cognition,nursing and traditional chinese medicine(TCM) for 3 months.Constipation symptoms scale and Patient Assessment of Constipation Quality of Life Questionnaire (PAC-QOL) were assessed before and after the intervention.Results CIG showed significant improvement in defecation difficulty,defecation force,defecation duration,frequency,abdominal distension,total symptom score,PAC-QOL,physical discomfort,psycho-social discomfort,anxiety and treatment satisfaction were(2.8±2.1),(2.2±1.6),(2.4±1.3),(2.1±1.7),(1.5±0.9),(14.0±8.1)respectively (t values were 2.629,3.818,2.328,2.593,2.578,2.913,7.499,2.570,5.012,2.249 and 12.259,respectively; all P＜0.05).Conclusion Comprehensive intervention may be an effective therapy against psychiatric drug induced constipation.

Objective To investigate the internal fixation for distal humeral fractures and its outcome .Methods A total of 74 patients with distal humeral fractures were performed surgical operation between Jan 2006 and Jun 2009 .According to AO/ASIF classification ,the factures were classified as type A (n=26) ,B(n=34)and C(n=14) .Internal fixation was applied to the patients , who did functional exercises after operation ,on the basis of classification .The healing of the fractures and the function the joints was examined via follow-up and X-ray films 1 ,3 and 6 months after operation .Results According to the Jupiter elbow score ,40 ca-ses were excellent ,22 cases were good ,12 cases were fair and 0 case was poor ,and the good rate was 83% .Conclusion Internal fix-ation with double plate ,is an effective therapeutic approach to distal humeral fractures in adults .Influencing factors of the efficacy include severity of the initial trauma ,operative scheme ,and post-operation functional recover .

Objective To analyze the clinical characteristics and resistance of group B hemolytic streptococcus septicemia in neonates,and to provide the evidence for clinical treatment.Methods 7 neonates with group B hemolytic streptococcus septicemia hospitalized from Jan.2001 to Aug.2011 were enrolled in this study.The clinical data and laboratory results of antimicrobial sensitivity were analyzed retrospectively.Results There were 4 cases of early-onset infection and 3 cases of late-onset infection,6 cases with leukopenia,6 cases with c-reactive protein elevated,4 cases with suppurative meningitis,3 cases with septic shock and 2 cases with disseminated intravascular coagulation.All strains were sensitive to penicillin, vancomycin and linezolid.Conclusion Attention should be paid to neonatal group B hemolytic streptococcus septicemia.Penicillin and vancomycin are first choice for this type of infection.

Objective To explore the risk factors and investigate the intracranial vascular characteristics in elderly patients with WSI. Methods According to imaging features of DWI,56 cases of WSI were divided into CWSI,IWSI and MWSI group. Distribution of intracranial vascular le?sions in different groups was compared. The degree of middle cerebral artery(MCA)and internal carotid artery(ICA)stenosis in different types of WSI was investigated. Results Among 56 cases,85.7%of them had a history of hypertension,53.6%had diabetes,62.5%had hyperlipoidemia, 51.8%had coronary heart disease(CHD),51.8%had smoking,and 73.2%of them had two or more risk factors. Totally 11 cases(19.6%)of them were cortical watershed infarction(CWSI),13(23.2%)of them were interior watershed infarction(IWSI),and 32(57.1%)of them were mixed cerebral watershed infarction(MWSI). Among all the patients,the incidence of MCA stenosis(37.5%)and ICA stenosis(53.6%)were signifi?cantly higher than the other intracranial vessels(χ2=37.188,P<0.001). The incidence of MCA stenosis in IWSI group was significant higher than CWSI and MWSI group(χ2=12.00,P<0.01). The incidence of ICA stenosis in MWSI group was significantly higher than CWSI group and IWSI group(χ2=11.10,P<0.01). Among all the patients,17 of them had severe stenosis or occlusion in MCA(30.4%),and 22 of them had severe ste?nosis or occlusion in ICA(39.3%). The incidence of severe stenosis or occlusion in MCA(58.8%)was significant higher in IWSI group(χ2=7.588,P<0.05)and those in ICA was significantly higher in MWSI group(χ2=7.091,P<0.05). Conclusion MWSI is more common in elderly patients with WSI. Most of the patients have more than one risk factors such as hypertension,diabetes,hyperlipoidemia,CHD and smoking. MCA and ICA lesions are more common than other intracranial vessels in elderly patients. IWSI is closely related with severe stenosis and occlusion of MCA,while MWSI is closely related with severe stenosis and occlusion of ICA.

Objective To discuss whether it is necessary to integrate metabolic indices into diagnosis of polycystic ovary syndrome(PCOS).Methods Taking ESHRE /ASRM diagnosis standard as gold standard,the case group composed of 51 women with PCOS and the control group composed of 47 women without PCOS were selected.By using classification tree C5.0,significant variables chosen by single factor analysis were used to establish a new diagnostic model which combined reproductive indices and metabolic indices.The validity and reliability of the new diagnostic model by using ROC curve analysis were evaluated.Finally,the consistence and difference between the new diagnostic model and the gold standard were analyzed.Results Single factor analysis chose 13 significant variables. ROC analysis showed an area under the curve of 0.930(P =0.000)and the optimal cut -off point was 0.190 with a sensitivity of 94.10%,a specificity of 91.50% and a consistency of 92.86%,which told us the new diagnostic model had superior validity and reliability.The two diagnostic methods had excellent consistence (Kappa =0.857,P =0.000)and there was no statistically significant difference (P =1.000).Conclusion Considering that whether metabolic indices are integrated into diagnosis of PCOS doesn′t change the diagnosis result,and it is unnecessary to integrate metabolic indices into diagnosis of PCOS.

Objective To evaluate the relationship between the ratio of the maximal value of the H-reflex to the maximal value of the M response (Hmax/Mmax), the ratio of the developmental slope of the H-reflex to the slope of the M response (Hslp/Mslp), and the ratio of the threshold of the H-reflex to the threshold of the M response (Hth/Mth) and upper-extremity spasticity in stroke patients. Methods Upper-extremity spasticity in 34 stroke patients was assessed by modified Ashworth Scale (MAS). H-reflex was recorded in flexor carpi radialis muscle on both sides. Hmax/Mmax, Hslp/Mslp and Hth/Mth in the spastic side were compared to the unaffected side. Results The mean of Hmax/Mmax (spastic side, 0.36±0.17; unaffected side, 0.14±0.07) and Hslp/Mslp (spastic side, 0.72±0.40; unaffected side, 0.26±0.22) showed significant difference between 2 sides (P<0.05). The mean of Hth/Mth (spastic side, 0.93±0.14; unaffected side, 0.98±0.12) showed no significant difference (P>0.05). Hmax/Mmax and Hslp/Mslp were correlated positively with score of MAS in the stroke patients (r=0.608, P<0.05; r=0.917, P<0.05), while there was no correlation between Hth/Mth and score of MAS (r=-0.128, P>0.05). Conclusion Hmax/Mmax and Hslp/Mslp could be sensitive electrophysiologic parameters to evaluate upper-extremity spasticity after stroke.

OBJECTIVE:To evaluate the effectiveness,safety and economy about Wenxin granules in the treatment of cardio-vascular diseases based on Meta-analysis. METHODS:Using“Wenxin granules”“Wenxinkeli”“Wenxin”as the search terms, searching PubMed,ProQuest,Springer,The Cochrane Library,CNKI,VIP and Wanfang data(Jan. 2000-Oct. 2015)through com-puter,relevant journals and conference papers by hand,randomized controlled trials about Wenxin granules in the treatment of car-diovascular diseases were screened and classified according to indications. Meta-analysis was performed by using RevMan 5.3 soft-ware,while cost-effectiveness analysis and minimum cost analysis were used for economic evaluation. RESULTS:Totally,38 liter-atures were involved,including 20 literatures of arhythmia,9 of premature ventricualr contraction,4 of coronary disease angina pectoris,3 of unstable angina pectoris,2 of pediatric viral myocarditis. Meta-analysis showed that compared with control drug, Wenxin granules showed better clinical efficacy for arrhythmia with less ADR;showed better clinical efficacy and ECG efficacy for premature ventricualr contraction with less ADR;showed better clinical efficacy for angina pectoris of coronary disease,unstable angina pectoris and pediatric viral myocarditis,with statistical significance (P<0.05). The incremental cost-effectiveness analysis showed that the additional cost of Wenxin granules in the treatment of above indications was worth it all,compared to control drug. Single factor sensitivity analysis supported the stability of results. CONCLUSIONS:Wenxin granules have a better effectiveness, safety and economy in the treatment of cardiovascular diseases,but still need high-quality evidence to support.

Objective To explore the interaction between polymorphisms of rs17222919 which located in the 5-1ipoxygenase-activating protein(ALOX5AP) gene promoter and environmental risk factors in ischemic stroke(IS).Methods We conducted a case-control study involving a total of 622 cases and 631 unrelated healthy controls which were selected from Henan Han populations,and the environment risk factors were recorded.Genotyping aimed at detecting both genetic and environmental factors in relation to IS was performed by TaqMan-polymerase chain reaction technology while interaction indexes (Υ) were calculated to determine interactions and their role models.Results The rs17222919 TG (189/622,30.4％),GG (18/622,2.9％)genotype frequencies and G (225/1244,18.1％)allele frequencies in IS subjects were significantly lower than those in controls (221/631,35.0％ ; 31/631,4.9％ ; 283/1262,22.4％ ; x2 =4.117,P =0.042 ; x2 =4.457,P =0.035 ; x2 =7.294,P =0.007).Negative interactions between TG + GG genotype and hypertension,diabetes or cigarette smoking in the occurrence of IS (Υ =0.943,0.922,0.830) were observed,whose role models were all super-multiplicative models.Conclusions According to our study,ischemic stroke is the result of the interaction of genetic and environmental factors and G allele of rs17222919 may have weakened the role of environmental factors for hypertension,diabetes and cigarette smoking in IS incidence.

Objective To evaluate the effect of therapeutic patient education on improving life quality of children with atopic dermatitis (AD). Methods A total of 109 children with AD were enrolled, including 53 patients in the intervention group and 56 patients in the control group. The intervention group was given therapeutic patient education in addition to routine treatment, while the control group was given routine treatment without therapeutic patient education. After three months two groups were compared with the disease severity and quality of life in children and their families. Results Compared with control group, the intervention group had significant improvements in severity of AD (P?=?0.003) and also significant improvements in quality of life (IDQOL and CDLQI) (P?=?0.004). The family life quality (DFI) of the two groups were both improved, but the difference was not signiifcant (P?=?0.492). Conclusions Therapeutic patient education can improve symptoms of atopic dermatitis, and the quality of life of children as well.

Objective To investigate the relationship between vaspin,Apelin,leptin and polycystic ovary syndrome.Methods We determined the serum vaspin,Apelin and leptin levels of 40 cases of PCOS patients and 30 cases of control group,and divided the PCOS group into three subgroups (normal weight,overweight and obesity) according to WHO criteria for obesity.Then we compared the serum vaspin,Apelin and leptin levels in PCOS and control group,and the differences in body weight index (body mass index,BMI) in women with polycystic ovary syndrome,and analyzed roles of adipokine vaspin,Apelin and leptin in the pathogenesis of PCOS.Results In PCOS patients adipokine vaspin,Apelin,leptin,insulin resistance index (Homeostasis model assessment for insulin index in resistence,HOMA-IR),fasting insulin (INS) and blood glucose,blood lipid were higher than those of the control group and increased with the increase of body mass index (BMI),and three kinds of adipokines and PCOS patients with HOMA-IR were positively correlated,there were statistically significant differences (P ＜ 0.05).Conclusion The results of vaspin,Apelin and leptin may be involved in the occurrence of PCOS.
