1.Ultrasonic manifestations of aggressive angiomyxoma

Objective To observe the ultrasonic manifestations of aggressive angiomyxoma(AAM).Methods Four patients with newly diagnosed AAM and 3 with recurrent AAM confirmed by pathology were retrospectively enrolled,and the ultrasonic manifestations were analyzed.Results Among 7 cases of AAM,the lesions located in subcutaneous regions of perineum in 3 cases,in both perineum and pelvic cavity in 2 cases,in pelvic cavity and in cervix each 1 case,with a median maximum diameter of 7.6 cm.The lesions mainly manifested as heterogeneous hypoechoic,scattered cord-like moderate/moderate-hyperechoic inside,regular or lobulated shape with finger-like protrusions,with clear boundary or unclear boundary with adjacent vagina/rectum.Layered structure could be observed with gray-scale ultrasound and CDFI.Conclusion AAM mainly affected pelvis and perineal region,often manifested as large lesion tended to infiltrate surrounding areas and form finger-like protrusions,most with clear boundaries and interior heterogeneous hypoechoic regions,also the characteristic stratified structures.
2.Safety of totally thoracoscopic mitral valve reoperation with hypothermia-induced ventricular fibrillation arrest in elderly patients

Objective To investigate the safety of totally thoracoscopic mitral valve reoperation with hypothermia-induced ventricular fibrillation arrest in elderly patients.Methods Clinical data of 108 elderly patients undergoing redo mitral valve surgery in our department from January 2008 to October 2024 were retrospectively collected,and according to surgical approach,they were di-vided into a totally thoracoscopic group(totally thoracoscopy with hypothermia-induced ventricu-lar fibrillation arrest,46 cases)and a median sternotomy group(cardiac arrest after median ster-notomy,62 cases).Their perioperative data were compared between the two groups.Results There were no statistical differences in baseline data between the two groups(P>0.05).When compared with the median sternotomy group,the totally endoscopic group had significantly longer cardiopulmonary bypass time and higher mitral valvuloplasty rate and postoperative LVEF value,but obviously reduced lowest intraoperative temperature,lower blood transfusion rate,less vol-ume of suspended red blood cells transfused,decreased troponin T level 1 d postoperatively,shor-ter duration of mechanical ventilation and lengths of ICU stay and postoperative hospital stay,and less postoperative drainage volume(P<0.05,P<0.01).But there were no statistical differences between the two groups in terms of postoperative stroke(6.52%vs 3.23%,P=0.420)or in-hospital death(6.52%vs 6.45%,P=0.988).Conclusions Totally thoracoscopy with hypothermi-a-induced ventricular fibrillation arrest is safe and reliable for the elderly patients who need to receive mitral valve reoperation.

Pancreatic cancer has a concealed onset and poor prognosis,and surgical resection is the main treatment method.Neoadjuvant therapy may improve the rate of negative resection margin and prolong overall survival of patients with pancreatic cancer.Functional and quantitative imaging can non-invasively reflect tumor microenvironment characteristics related to chemotherapy resistance,providing a new perspective for evaluating and predicting the efficacy of neoadjuvant therapy for pancreatic cancer.The research progresses in functional and quantitative imaging for evaluating and predicting the efficacy of neoadjuvant therapy of pancreatic cancer were reviewed in this article.

The new cultural construction of high-quality development of public hospitals proposes a patient-oriented ap-proach.People with disabilities,represented by the hearing-impaired community,faces many difficulties in the process of seeking medical care,and their needs have not been adequately addressed.This inadequacy adversely affects the development of harmoni-ous doctor-patient relationships and the patient medical experience.This study takes the deaf assistance outpatient clinic,first es-tablished by Shanghai East Hospital as an example to elaborate the practice of cultural brand construction under the leadership of Party building.Medical social workers,healthcare professionals,and volunteers work together to build a cultural brand through institutional guarantees,detailed care,and the establishment of a full process service system.This is expected to provide refer-ences for the construction of hospital cultural brands..
7.To explore the application of SP-based medical record writing training in prosthodontics practice

Objective To evaluate the practice effect of medical record writing training based on standardized patients and explore its application in prosthodontics practice.Methods Seventy-one undergraduate interns were randomly divided into two groups.At the first week of clinical practice,the test group(n=35)adopted the SP-based medical record writing training,and after the training,the students'evaluation of the teaching effect was investigated by questionnaire.And the control group(n=36)received traditional lectured medical record writing training.One week later,the same medical record writing exam was performed in the two groups.The scores of medical record writing of different teaching model were compared,and the evaluation of the teaching effect in the test group was carried out.Results The exam score of medical record writing of the test group(88.80±4.60)was significantly higher than that of the control group(84.92±5.51),and the difference was statistically significant(P=0.002).88.57%of the students in the test group were satisfied with the SP-based medical record writing training.The satisfaction score was 8.94.Conclusion Medical record writing training is a long-term clinical practice.SP-based medical record writing training is helpful to improve the medical record writing quality of medical students.
8.Effects of periodontal ligament stem cells-derived exosomes on biological characteristics of periodontal ligament stem cells in an inflammatory environment

BACKGROUND:In recent years,the application of exosomes of periodontal ligament stem cells in periodontal tissue regeneration engineering has been widely studied,but the effect of exosomes on periodontal ligament stem cells derived from inflammatory environment is still unclear.OBJECTIVE:To investigate the effects of exosomes secreted by periodontal ligament stem cells from healthy and inflammatory environments on the proliferation and differentiation of periodontal ligament stem cells from inflammatory environments.METHODS:Human periodontal ligament stem cells from healthy and inflammatory tissues were isolated and cultured by enzyme digestion method.Exosomes were extracted from two kinds of periodontal ligament stem cells using ultracentrifugation.Passage 3 periodontal ligament stem cells derived from inflammatory tissue were selected and cultured in three groups.Cells in the blank group were cultured routinely.The healthy exosome group was added with exosomes secreted by peripheral ligament stem cells derived from healthy tissue.The inflammatory exosome group was added with exosomes secreted by human periodontal ligament stem cells derived from inflammatory tissue.Cell proliferation and cloning were detected.The expression of alkaline phosphatase,the formation of mineralized nodules,and the expression of mRNA and protein of genes related to osteogenesis were detected under osteogenic differentiation.RESULTS AND CONCLUSION:(1) CCK-8 assay and clonal formation test showed that compared with the blank group,two kinds of exosomes could promote the proliferation and colony formation of periodontal ligament stem cells from inflammatory tissue (P<0.05),and the effect of the healthy exosome group was stronger than that of the inflammatory exosome group (P<0.05).(2) Alkaline phosphatase and alizarin red staining showed that compared with the blank group,the two kinds of exosomes could promote the expression of alkaline phosphatase and the formation of mineralized nodules in periodontal ligament stem cells from inflammatory tissue,and the promoting effect of the healthy exosome group was stronger than that of the inflammatory exosome group.RT-PCR and western blot assay showed that compared with the blank group,the two kinds of exosomes could promote the expression of alkaline phosphatase,RUNX2,and type Ⅰ collagen mRNA and protein in periodontal ligament stem cells from inflammatory tissue (P<0.05).The promoting effect of the healthy exosome group was stronger than that of the inflammatory exosome group (P<0.05).(3) The results showed that exosomes secreted by human periodontal ligament stem cells could promote the proliferation and osteogenic differentiation of periodontal ligament stem cells derived from inflammatory environments,and the promoting effect of exosomes secreted by human periodontal ligament stem cells derived from healthy tissues was better than that from human periodontal ligament stem cells derived from inflammatory tissues.
