1.Mental status and related factors in patients with maintenance hemodialysis in Huzhou City

Objective To investigate the psychological status and related factors in patients with mainte-nance hemodialysis in Huzhou city.Methods 376 cases of maintenance hemodialysis were selected as the research subjects in Huzhou First People's Hospital.The anxiety and depression status in maintenance hemodialysis patients were investigated by issuing questionnaires.To study the relationship between anxiety and depression,gender,age, social support,nutritional status,complications,and in maintenance hemodialysis patients.At the same time,the quality of life was evaluated by SF -36 scale.Results Issuing 376 questionnaires,a total of 373 points,the effective recovery rate was 99.20%.In 373 patients,there were 182 patients (48.79%)with no depression,mild depression in 78 cases (20.91%),moderate depression in 59 cases (15.82%),severe depression in 54 cases (14.48%). 51.21% patients had different degree of depression.Of the 373 patients,there were 123 patients (32.97%)with no anxiety,mild anxiety in 141 cases (37.80%),mild and moderate anxiety in 55 cases (14.75%),moderate anxiety in 33 cases (8.85%),severe anxiety in 21 cases (5.63%).A total of 67.03% patients had different degrees of anx-iety.Huzhou city maintenance hemodialysis,female patients had depression rate of 80.70%,anxiety rate of 65.50%, which were significantly higher than the rate of depression 55.45%,anxiety rate 39.11% in male patients,the differ-ences were statistically significant (P <0.01 -0.05).In maintenance hemodialysis patients,the anxiety and depres-sion status gradually increased with age.Severe malnutrition in maintenance hemodialysis patients,the rate of depres-sion,anxiety rates were 80.65%,90.32%,the rates of depression,anxiety were 44.49%,61.81% in normal and mild malnutrition patients,the differences were statistically significant (all P <0.01).SF -36 scale showed that most of the patients with maintenance hemodialysis patients had poor quality of life,and the function was limited. Conclusion Maintenance hemodialysis patients,the general existence of anxiety and depression in the mental state in Huzhou city.Moreover,the psychological state of the patients is influenced by sex,age,nutrition status,and other factors.In clinical,it is needed to provide effective psychological intervention for patients to improve the quality of life,according to the psychological status and related factors of maintenance hemodialysis patients.

Sarcomas collectively represent over 100 different subtypes of bone and soft tissue tumors.They are not sensitive to chemotherapy,which requires the development of tissue-specific or pathway-specific therapies.As our understanding of the molecular mechanisms driving sarcomas is rapidly advancing,the number of targeted therapies is also increasing.Recently identified novel druggable targets including the MDM2 amplifications in welldifferentiated and dedifferentiated liposarcomas,the fusion NAB2:STAT6 of solitary fibrous tumor,the SDH mutations in gastrointestinal stro mal tumors,the suppression of Mcl1 in synovial sarcomas,CDK4 in alveolar rhabdomyosarcoma.They will play an important role in the treatment of sarcoma.Here,the author do an overview of these factors.
3.The establishment of the mouse congenital infection model by MCMV and the observation of the nervous system infection

Objective To establish the murine congenital infection model by MCMV and observe the pathological changes and infection status of brain tissue.Methods After anesthesia,mice who were pregnant 11-13.5 days (E11-13.5 d) were intra-amniotic injected one uterus by one with virus (MCMV K181 suspension,1 μl,1×103 PFU).The control group of the same period was intra-anmiotic injected with culture medium DMEM (1 μl).Carefully reset the uteruses and close the abdomen.After 5 days of separated feeding,kill the pregnant mice,take the fetus out of the uterus,anesthetize and kill them.Make frozen sections of these fetal brains.Some sections were stained using conventional HE method,to observe the pathological changes under the light microscope.Detect MCMV early antigen in the brain tissue by immunohistochemistry staining and immunofluorescence assay.Results The survival rates of the infected group were 71.9%.Compared with the control group,intra-amniotic inoculation of MCMV does not affect the rate of fetal survival,fetal absorption,fetal death and the average weight of the heads,but decrease their average weight of the bodies.The pathological changes are found in the brain tissue of the mouse in the infection group.Through enzyme immunohistochemistry assay,there are many MCMV infected cells in brain-ventricular zone,brain subependymal zone,cerebral cortex and hippocampus area in the infection group.Similar findings were observed by immunofluorescence method.Conclusion By intra-amniotic injection of MCMV suspension,murine model of MCMV congenital infection can be successfully established.This model could be used to study the mechanisms of encephalodysplasia caused by congenital CMV infection in vivo.
4.Effect of Shenshuaining capsule on nutritional status and blood rheology in patients with chronic renal failure

Objective To explore the effect of Shenshuaining capsule on nutritional status and blood rheology in patients with chronic renal failure.Methods 150 cases of chronic renal failure were randomly selected and were randomly divided into observation group and control group, 75 cases in each group.The control group were given hemodialysis for treatment,and the observation group was treated with Shenshuaining capsule one week after dialysis.The clinical effect,nutritional status and biochemical indicators in the two groups were observed.Results The total effective rate in the observation group was 86.67%, which was higher than 72.00% in the control group,the difference was statistically significant(x2=4.920,P<0.05).After treatment,the nutrition indicators of HBG,ALB,IgG,IgA and IgM in the observation group were (135.7±16.4)g/L,(34.5±2.9)g/L,(10.8±0.7)g/L,(2.1±0.4)g/L and (1.7±0.3)g/L respectively, which were higher than those in the control group[(113.8±16.9)g/L,(28.4±2.5)g/L,(7.6±0.8)g/L,(1.3±0.3)g/L and (1.2±0.2)g/L], the differences were statistically significant(t=8.054,13.797,26.070,13.856,12.010,all P<0.01).In the observation group,the incidence rate of undernutrition was 25.33%, which was lower than 46.67% in the control group, the difference was statistically significant(x2=45.905,P<0.05).After treatment,the blood urea nitrogen and blood urea in the observation group were (349±64)μmol/L and (10.4±3.7)mmol/L respectively,which were lower than those in the control group[(462±51)mol/L and (14.2±4.5)mmol/L], the differences were statistically significant(t=11.958,5.649,all P<0.05).After treatment,the creatinine clearance rate and concentration of hemoglobin in the observation group were (25.1±6.1)mL/min and (92.1±21.1)g/L respectively, which were higher than those in the control group[(21.7±5.4)mL/min and (84.7±15.3)g/L], the differences were statistically significant(t=3.614,2.459,all P<0.01).After treatment, the high blood viscosity,low blood viscosity,plasma viscosity and hematocrit in the observation group were (3.59±0.94)m/Pas,(7.84±0.97)mPa/s,(1.31±0.15)mPa/s and (0.33±0.07) respectively, which were lower than those in the control group[(4.41±1.13)mPa/s,(8.72±1.05)mPa/s,(1.43±0.24)mPa/s and (0.42±0.05)], the differences were statistically significant(t=11.958,5.649,2.459,all P<0.01).Conclusion For patients with chronic renal failure,Shenshuaining capsule combined with hemodialysis can improve the nutritional status and blood rheology.
5.Chemical Disinfectant Application:Problems and Countermeasures

OBJECTIVE To investigate the problems and countermeasures in the chemical disinfectants application.METHODS To analyze the existing problems in the chemical disinfectants application from 8 hospitals of different care levels at the same city and provide corresponding measures.RESULTS There were many problems in the use of chemical disinfectants,therefore the key points were enhencing the leadership and management standards,staff training,of corresponding knowledge and the correct methods,and strengthening the guidance,inspection and supervision by functional departments.CONCLUSIONS Medical personnel must properly use the chemical disinfectants to ensure the medical safety and prevent hospital infection effectively.
6.Study on effect of repaglinide combined with Xiangsha Liujun pillon patients with diabetic nephropathy

Objective To explore the ziggy joint amomum six columns nai jun pill in patients with diabetic nephropathy urinary inhibition c and serum c-reactive protein levels.MethodsSelect 80 patients with diagnosis of diabetic nephropathy, randomly divided into 2 groups, 40 cases in each group.Craig column nai treatment, control group given on the basis of the observation group in the control group given six jun pill treatment, treatment of 12 weeks, before and after treatment were detected blood glucose (FBG), glycosylated hemoglobin (HbA1C), urinary inhibition c (CysC) and serum c-reactive protein (CRP) and 24 h urinary microalbumin urine trace albumin (24 hUAE) changes.ResultsCompared with before treatment, 24 hUAECysC, CRP and 2 group were lower (P< 0.05);Compared with control group, 24 huae CysC, CRP and the observation group were lower (P< 0.05), the clinical curative effect is higher (P< 0.05), and high safety.ConclusionZiggy joint amomum six columns nai jun pill treatment of the diabetic nephropathy is a effective way, and can significantly reduce serum CysC, CRP levels.
7.Effects of lipoxin receptor agonist BML-111 on IFN-βand IE86 mRNA expression of macrophages infected by human cytomegalovirus

Objectives To investigate the effects of lipoxin receptor agonist BML-111 on IFN-βand IE86 mRNA expression of macrophages infected by human cytomegalovirus (HCMV). Methods Macrophages were infected with HCMV (MOI=0.5), and the cultured cells were randomly divided into control group, HCMV group, HCMV+BML-111 group, and HCMV+MP group. The cells were collected at 0,1,2,4,8 and 12 h after infection, and the levels of IFN-βand IE86 mRNA were tested by real-time PCR. Results Compared with HCMV group, the levels of IFN-βmRNA in HCMV+BML-111 group increased significantly (P < 0.05), while the levels of IFN-βmRNA in HCMV+MP group decreased significantly (P < 0.05); Compared with HCMV group, there were no significant differences of the levels of IE86 mRNA in HCMV+BML-111 group (P>0.05), while the levels of IE86 mRNA in HCMV+MP group increased significantly (P < 0.05). Conclusion BML-111 exerts antiviral activity by promoting the expression of IFN-βmRNA at the early stage of HCMV infection.
8.Effect of lipoxin receptor agonist on human cytomegalovirus replication and proliferation

BACKGROUND:The anti-inflammation and protective effects of lipoxin have been verified in several immunity-related disease models. Preliminary studies of our research group have shown that, lipoxin receptor agonist BML-111 has negative regulation effects on the human cytomegalovirus (HCMV)-induced immunological injury. However, the effect of BML-111 on the HCMV replication remains unclear.
OBJECTIVE:To observe the influence of lipoxin receptor agonist BML-111 on HCMV replication and proliferation in THP-1 macrophages and human embryonic lung fibroblasts.
METHODS:THP-1 macrophages were infected by HCMV AD169 strain, and were divided into three groups:mock infection, HCMV infection, HCMV+BML-111. The final concentration of BML-111 was 100 nmol/L. cells in each group were col ected at 0, 1, 2, 4, 12, 36, 48 hours, the mRNA levels of IE86 and pp65 in the THP-1 macrophages were tested by RT-PCR method. Human embryonic lung fibroblasts were infected with HCMV (MOI=0.1), and were divided into two groups:HCMV infection and HCMV+BML-111. The patho-morphous changes of human embryonic lung fibroblasts were observed under light microscope, and the cellnumber was measured. The infective virus titer changes in human embryonic lung fibroblasts were examined by plaque assay.
RESULTS AND CONCLUSION:After the macrophages were infected by HCMV, compared with the mock infection group, the mRNA levels of IE86 and pp65 in the HCMV group and HCMV+BML-111 group were increased significantly;compared with the HCMV infection group, the mRNA levels of IE86 and pp65 in the HCMV+BML-111 group were increased significantly in the early stage (within 4 hours) after infection, but the pp65 mRNA levels were decreased significantly in the medium and late stages (24-72 hours) after infection. After human embryonic lung fibroblasts were infected by HCMV, the degree of the patho-morphous in the HCMV+BML-111 group reached 100%2 days earlier than the of HCMV infection group. The infective virus titer reached the peak 2 days earlier than the HCMV infection group, but no significant difference was found between the two groups. BML-111 accelerates the replication of HCMV in the early stage of infection, but inhibits the expression of pp65 gene in the late stage. BML-111 has no impact on the proliferation of the infective HCMV titer in vitro.
9.A study of arteries of foot by flow sensitive dephasing prepared balanced steady-state free precession MR angiography in diabetes

Objective To investigate balanced steady-state free precession with flow-sensitive dephasing magnetization preparation (FSD-bSSFP) in the assessment of arteries of foot in diabetic patients.Methods The lower-extremity peripheral arteries of 43 diabetic patients were evaluated by FSD-bSSFP no contrast MRA and contrast-enhanced MRA (CE-MRA)in. Two experienced observers assessed the image quality, degree of venous contaminated and visibility of pedal artery branches by FSD-bSSFP and CE-MRA respectively in consensus. The signal intensity( SI), signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) of the source images of both groups were measured and Wilcoxon and t tests were performed. Results The image score of FSD-bSSFP group was 2.7 ± 1.1 and CE-MRA was 2.6 ± 0.8, there was no statistical difference ( Z = 0. 134, P > 0. 05 ). The image score of demonstration of the pedal artery branches and degree of venous contamination on FSD-bSSFP were 3.2 ± 0. 9 and 1.8 ± 0. 4 respectively which were superior to that of CE-MRA (2.5 ± 0.9 and 2.1 ± 0.8 respectively). Significant statistical difference existed between the two groups in demonstration of pedal artery branches ( Z = 5.246, P < 0.05 ) and degree of venous contamination (Z =2.541 ,P <0.05). SNR of FSD-bSSFP was 148.6 ±26.7, CNR was 88.3 ± 19.0. SNR of CE-MRA was 148.5 ± 45.6, CNR was 121.0 ± 41.0. No statistical difference existed between SNR between two methods (t = 0.013, P > 0.05 ). But CNR of CE-MRA was superior to that of FSD-bSSFP and significant statistical difference existed between these two methods ( t = 5.113, P < 0.01 ). Conclusion FSD-bSSFP without contrast could be used in the evaluation of foot arteries in patients of renal dysfunction and diabetes.
10.Influence of matrix effect upon cyclosporine A test by fluorescence polarization immunoassay and enzyme-multiplied immunoassay technique methods

Objective To explore the matrix effect on cyclosporine A (CsA) test by fluorescence polarization immunoassay (FPIA) and enzyme-multiplied immunoassay technique (EMIT), explain the discrepancy of external quality control results between these two methods and find the corrective action.Methods One hundred whole blood samples with various concentrations were adopted and CsA levels were detected by FPIA and EMIT.The results were compared with each other.Moreover, the influence of residual metal ions upon immunoreactions was assessed by adding Cu2+ and Zn2+.The effect of non-whole blood matrix on extraction efficiency for quality control materials and CsA calibrator was evaluated by adding identical volume of Hb-rich reagents followed with re-extraction.Results There is good correlation between results measured with FPIA(X) and EMIT(Y) methods ( Y=0.926 8X -8.115,R2 =0.996 9).Neither FPIA nor EMIT was affected by residual metal ions ( P > 0.05 ). Non-whole blood matrix decreased the extraction efficiency of two methods, but it could be corrected by supplementation of the Hb-rich reagents (≥30 g/L).Conclusions Non-whole blood matrix may be the main reason for the inconsistent results measured by FPIA and EMIT methods.It could be corrected by using Hb-rich reagents.In addition,we should consider the influence of low lib on CsA test,espocially for organ transplant patients with lower Hb ( <30 g/L).
