1.Ethical consideration in academic medical writing

With the progress of human civilization,the harmonious development of society is gradually realized by different researchers.Medicine,as a science directly serving humans,emphasizes more on humanism than other sciences.Medical research is not only characterized by scientificity and innovativeness,but also by ethicality.More and more biomedical journals are paying attention to issues of ethics.This review discussed the ethical issues in academic medical writing based on the Uniform Requirements for Manuscripts Submitted to Biomedical Journals published by International Committee of Medical Journal Editors (ICMJE) and the discussions by Committee on Publication Ethics (COPE).
2.Protective effect of PPAR-αagonist on PAN-induced podocyte injury

Aim To investigate the function of fenofi-brate on PAN ( puromycin aminonucleoside )-induced podocyte injury. Methods SD female rats of 18-week-old were randomly assigned into 3 groups ( n =6 ) . Mice in PAN group and fenofibrate treated group received a single intravenous injection of PAN ( 65 mg ·kg-1 ) , while those in control group received equal volume of saline. Mice in fenofibrate treated group re-ceived 40 mg · kg-1 · d-1 of fenofibrate ( intragastric administration ) on day 1 after PAN injection , while those in PAN group and control group received equal volume of vehicle. 24 hours urine samples from all group were collected on day 0(1 day before PAN injec-tion), day 6, day 10. The 24 hours urine protein was detected by Bradford assay. All the rats were sacrificed 10 days after the induction of podocyte injury, and glo-merulus sample were collected. The expression of podocyte injury marker and transcription level in apop-tosis, podocyte cytoskeleton protein, slit diaphragm protein were evaluated by Western blot and real-time PCR. Results Compared with the control group, 10 days after injection of PAN, 24 hours urine protein was obviously increased, and the expression and transcrip-tion level of podocyte injury marker desmin, apoptosis, podocyte cytoskeleton protein, slit diaphragm protein were upregulated greatly, however, those were signifi-cantly lower in fenofibrate treated group as compared with those in PAN group. Conclusions PPAR-α ago-nist fenofibrate can ameliorate PAN-induced glomerulus podocyte injury, and the mechanism involved may be associated with inhibition of the mitochondria apopto-sis, TGF-β/Smad pathway and p38 pathway.
3.Advance in Exercise for Bone Metabolism Pathways (review)

The process of exercise regulating bone metabolism is complicated, which involves a number of signaling pathways. A large number of studies in vitro have indicated that mechanical stress regulates bone metabolism by Wnt, bone morphogenetic protein (BMP), and osteoprotegerin (OPG)/receptor activator of NF-κB ligand (RANKL)/receptor activator of NF-κB (RANK) signaling pathways. Both the in-tensity and frequency of mechanical stress have varing impact on bone tissue and cells. Plenty of studies in vivo also have shown that exer-cise regulates bone metabolism by key factors in bone metabolism signaling pathways. This paper reviewed the effects of exercise on bone metabolism pathways and their mechanisms.
4.3D Hydrogel Compression Model and Effect of Cyclic Compression on Osteoblast Differentiation

Objective:3D hydrogel cell model was established,and cyclic compressive loading on MC3T3-E1 cell with different intensities,frequencies and durations was applied,in order to research the suitable solution about promoting the osteoblast differentiation with cyclic compression.Methods:Cyclic compressive loading on MC3T3-E1 cell was applied with different intensity,frequency and time.After compressive loading finished,the total RNA extraction from cell-gel constructs were performed and quantified ATF4,ALP,Runx2,Osteocalcin,RANKL and RANK mRNA.Results:RANKL and RANK mRNA expression significantly with different frequencies cyclic compressive loading (P < 0.05),and ALP mRNA (P < 0.05) and Runx2 mRNA (P < 0.01) expression significantly with different intensities and frequencies cyclic compressive loading (P < 0.05).Meanwhile,Runx2 mRNA expression with 4h significant higher than 12h (P < 0.05),and RANKL mRNA expression with 4h significant lower than 12h (P < 0.05).Conclusion:Determine the stress intensity and frequency,1% intensity,frequency of 0.5 Hz,4 h of cyclic compression intervention could promote the growth of osteoblasts-like cells in the 3D hydrogel model.
5.Pattern recognition of surface electromyography signal based on multi-scale fuzzy entropy.

Action surface electromyography (SEMG) signals can be acquired from human skin surface. Its pattern recognition plays a very important role in practical applications such as human prosthesis and human-computer interface systems. For the purpose of increasing the recognition accuracy, we proposed a new recognition method combining fuzzy entropy (FuzzyEn) with multi-scale analysis. Considering the nonlinear and non-stationary characteristics of the SEMG, a multi-scale fuzzy entropy (MSFuzzyEn) feature was introduced and applied to the pattern recognition of six type action SEMG signals of the forearm. Firstly, multi-scale decomposition was applied to original signal using wavelet decomposition. Then MSFuzzyEn of the decomposed signals were calculated and inputted to support vector machine (SVM) for classification as feature vectors. The mean recognition accuracy reached 97%, which was 3% greater than that when FuzzyEn of original signal is applied to the classification of SEMG signals. The results have proved that the MSFuzzyEn is effective and precise in the classification of action SEMG signals.

6.Feasibility to simulating the osteotomy of mandibular angle hypertrophy using computer three-dimensional reconstruction

Computer tomography scanning was performed in 16 patients with mandibular angle hypertrophy before and after surgery. The imaging data of patients were recorded in three-dimensional reconstruction software (Mimics 10.0), and found the most appropriate surgery plan through simulating different surgical procedures. The geometry character indexes of the face were compared between before and after surgery, and then the results were analyzed. We got intuitive, accurate three-dimensional reconstruction graphs, simulation of surgery procedures, and calculation of difference in the geometry character index of face in all cases. The bone width of mandibular angle shortened (7.69?.89) mm; the soft tissue width of mandibular angle shortened (5.13?.19) mm; the both gonial angle increased (5.68?.43)?and (5.91?.66)? respectively. Applied to actual surgery procedure,the operation time shortened and precision of operation was improved. Results demonstrated that Computer three-dimensional reconstruction and computer simulation techniques is beneficial to reveal the variational regularity of bone and soft tissue, which is a good aided method in stimulation of surgery and choice of surgery plan.
7.Preparation and characterization of an egg yolk antibody (IgY) against the recombinant human A rotavirus VP8

Objective To prepare an egg yolk antibody ( IgY) against the recombinant human A rotavirus VP8 and evaluate its biological characteristics .Methods The complete VP4 gene was used as the template to clone VP8 DNA fragment by PCR .The VP8 gene was then cloned into the vector pET 28a for ex-pression of VP8 protein in the prokaryotic system.The expression plasmid of pET28a-VP8,identified by DNA sequencing,was transformed into E.coli BL21 to induce the expression of protein by 0.5 mmol/L IPTG at 30℃.The purified and refolded recombinant VP 8 protein was used to immunize laying hens in combination with complete Freund′s adjuvant ( CFA ) .Anti-VP8 IgY antibodies were extracted from egg yolks by using water dilution-ultrafiltration assay and were further analyzed by SDS-PAGE,ELISA,Western blot and Dot blot assay,respectively.Results The recombinant VP8 protein was expressed in E.coli BL21 as an inclusion body and its molecular weight was about 35 ×10 3 .The VP8 protein was well refolded in a buffer containing Tris-HCl (pH8.0).The isolated anti-VP8 IgY antibodies showed a high titer of 1∶100 000 and a high stabil-ity under the condition of pH3-11 and temperature 25-65℃.Moreover,the anti-VP8 IgY antibodies specific-ally recognized the wild type human A rotavirus .Conclusion The prepared anti-VP8 IgY antibodies showed the advantages of high titer ,good specificity and stability .It might be used as the tool in further investigation for the prevention and treatment of diarrhea caused by human A rotavirus .
8.Experimental studies on toxic effects of Tiangou Jiangya capsule.

OBJECTIVETo study the acute and chronic toxicity of Tiangou Jiangya capsule.
METHODTiangou Jiangya capsule was intragastrically administered to mice twice a day. The appearance, behavior, mental status, respiratory changes of mice and the number of poisoned and dead mice in each group were noted daily for 14 consecutive days. The amount of weight and feed of survived mice were recorded every day. The mice were divided into four groups: the treatment groups (minimum, middle, maximum dose of Tiangou Jiangya capsule) and the control group. After continuously orally administrated for 6 months, the rats' behavior, weight gain, food consumption, indications for hematology, blood biochemistry, urine analysis, electrocardiogram, systematic autopsy and histopathology were observed. The above physiological indexes were inspected again 1 month after cease of administration.
RESULTThe oral acute toxicity study of Tiangou Jiangya capsule in mice revealed that the maximum dose is 534.86 g x kg(-1), which was 534.86 times the recommended human maximum dose in clinical practice. Compared with normal control group, no significant differences were observed in rats' behaviors, food-intake, electrocardiogram and relative examination indexes among the treatment groups. There was no difference of hematology, biochemistry test, urine and histopathology.
CONCLUSIONThe minimum dosage of Tiangou Jiangya capsule is relatively safe. It caused weight loss by administrated with the middle and maximum dose for 6 months, which should be paid attention in clinical studies.

9.Application of healthcare failure mode and effect analysis in preventing nursing interruption with negative outcome in operating room

Objective:To explore the effect of healthcare failure mode and effect analysis (HFMEA) in reducing the incidence of nursing interruption with negative outcome in operating room, so as to maximize the smooth progress of the surgical process.Methods:This was a quasi experimental study. The gastrointestinal surgery room of Shandong Provincial Hospital Affiliated to Shandong First Medical University was selected for the study. According to the surgical sequence, 38 surgeries performed in the gastrointestinal surgery suite from August 15-30, 2021 were set as the control group, and the conventional healthcare cooperation model process was implemented; 42 surgeries performed from September 15-30, 2021 were set as the intervention group, and the operating room under the HFMEA model was implemented negative outcome care disruption event management process.A video tracking method combined with a surgical care disruption event register was used to investigate the occurrence of negative outcome care disruption events in the operating room, comparing the number, duration, source of disruption events and the incidence of near miss events in the operating room between the control group and the intervention group.Results:In the control group, there were 38 observed surgeries, 190 negative outcome care interruptions, negative outcome interruptions of (5.26 ± 1.02) min duration, and no near misses; in the intervention group, there were 42 observed surgeries, 84 negative outcome care interruptions, negative outcome interruptions of (2.06 ± 0.08) min duration, and no near misses. There were statistically significant differences in the number, duration of negative outcome care interruptions between the intervention group and the control group ( χ2 = - 18.71, t = - 20.28; all P<0.01). There was statistically significant difference in the source of negative outcome care interruptions between the intervention group and the control group ( χ2 = - 12.71, P<0.01). Conclusions:HFMEA model can effectively reduce the number of negative nursing interruptions in the operating room, shorten the duration of interruptions, and minimize potential safety hazards caused by nursing interruptions, which is conducive to ensuring the safety of patients.
10.Functional dyspepsia treated with WangShiBaoChiWan: a randomized, double-blind, parallel-controlled, multicenter clinical study

Objective:To compare the efficacy and safety between WangShiBaoChiWan and mosapride in the treatment of functional dyspepsia (FD).Methods:From September 2019 to September 2020, patients with postprandial fullness and early satiation who met the Rome Ⅳ criteria for FD diagnosis were enrolled from 15 hospitals, including the First Affiliated Hospital of Naval Medical University (Shanghai Changhai Hospital), Beijing Traditional Chinese Medicine Hospital Affiliated to Capital Medical College. The subjects were randomly divided into WangShiBaoChiWan (experimental) group and mosapride (control) group in the ratio of 1∶1. The treatment regimens were WangShiBaoChiWan+ mosapride simulator, WangShiBaoChiWan simulator+ mosapride, respectively with a treatment period of 2 weeks. The primary efficacy outcome was the improvement rates of main symptoms before and after treatment, the secondary efficacy primary efficacy outcome was the total clinical effective rate and the change of the single symptom score. And the safety indicator included adverse events. Independent sample t-test, paired t-test and chi-square test were used for statistical analysis. Results:A total of 251 FD patients were enrolled in the full analysis set, including 124 in the experimental group and 127 in the control group; 241 FD patients were in the per-protocol analysis set, including 117 in the experimental group and 124 in the control group. The analysis of per-protocol analysis set showed that the improvement rates of the main symptoms of the experimental group and the control group were (66±29)% and (60±30)%, respectively, and the difference was not statistically significant ( P>0.05). The improvement rate of the main symptoms of the experimental group reached 117% of that of the control group, which exceeded the expected non-inferiority standard of 80%. The total clinical effective rates of the experimental group and the control group were 76.07% (89/117) and 75.81% (94/124), respectively, and the difference was not statistically significant ( P>0.05). The results of full analysis set showed that the incidence of adverse events of the experimental group and the control group was 1.62% (2/124) and 1.57% (2/127), respectively, and the difference was not statistically significant ( P>0.05). There were no serious adverse events in the two groups. Conclusion:The improvement rate of the main symptoms of WangShiBaoChiWan is not inferior to that of mosapride in the treatment of FD, and it has good safety.
