1.Analysis of the reason that patients with chronic sinusitis having a negative CT manifestation

Objective:To explore the reason why the CT manifestation of the patients with chronic nasosinusitis is negative.Method:To analyze the clinical and CT manifestation of the patients with chronic sinusitis having a negative CT manifestation.Result:Pathological changes were found in all 8 patients during the course of endoscopic sinus surgery, such as pus storing in the sinus cavity, mucous swelling polypoidly, small polyps formation and so on. Conclusion:The reasons that the patients with chronic sinusitis having a negative CT manifestation are, ①CT scan can only give a static one-off image, ② partial volume effect,③ maybe result from the location of CT scan and the resolving power of tomograph,④ maybe attribute to the pathological classify of chronic sinusitis.
2.Effects of Xuefurong capsules on sexual function and nNOS expression in paraventricular nucleus in male castrated rats

Objective To evaluate the effects of Xuefurong capsules,the traditional Chinese medicine for invigorating the kidney,on sexual function and expression of neuronal nitric oxide synthase(nNOS) in paraventricular nucleus(PVN) in adult castrated SD rats.Methods The male castrated rat model was made and three days after operation the liquid of Xuefurong capsules(5 mg/kg) was given by intragastric administration once a day for 28 days.The sexual function was observed and the nNOS expression in PVN was detected by using ABC immunohistochemical method.The number,area and gray degree of nNOS were counted and analyzed with semi-quantitative method.Results ①Compared with that in the castrated group,the serum testosterone level increased obviously in Xuefurong group(P0.05).Conclusion Xuefurong capsules can increase the level of testosterone,enhance the sexual function of castrated rats and nNOS expression in PVN.
3.Tumor necrosis factor-alpha antagonist combined with tyrosine kinase C gene- modified neural stem cell transplantation for spinal cord injury

BACKGROUND:Whether controling of post-injury inflammatory response combined with neural stem cel (NSC) transplantation can improve the curative efficacy for spinal cord injury stil remains unclear. OBJECTIVE:To investigate the repair of spinal cord tissue, myelin regeneration, axon regeneration, motor function recovery and the possible mechanism after early application of tumor necrosis factor α antagonist (Etanercept) combined with tyrosine kinase C (TrkC) gene-modified NSC transplantation. METHODS:TrkC-overexpressed NSCs (TrkC-NSCs) were constructed by lentiviral transfection technique. The rat models of spinal cord transection injury were prepared, and then subjected to Etanercept combined with TrkC-NSCs transplantation. The number of neurons and neuroregeneration after injury were measured by Nissl's staining, immunofluorescence and western blot. The rat motor function was detected by Basso Beattie Bresnahan score and evoked potential. The myelin regeneration was detected by electron microscopy and toluidine blue staining. RESULTS AND CONCLUSION:Compared with the other groups, the Etanercept combined with TrkC-NSCs transplantation group had more survived anterior horn motor neurons at 28 days after injury, more myelin-encapsulated axons, higher Basso Beattie Bresnahan score, greater amplitude of the evoked potentials, and relatively shorter latency (alP < 0.05). These findings indicate that early application of tumor necrosis factor-α antagonist combined with TrkC-NSCs transplantation after spinal cord injury in rats can effectively promote myelin regeneration, axon regeneration, and further promote motor function recovery.
4.An aptamer-based biosensor for colorimetric detection of Enteropathogenic Escherichia coli

Objective To develop and evaluate an aptamer based biosensor (aptasensor) for rapid colorimetric detection of enteropathogenic Escherichia coli (EPEC). Method The aptasensor was fabricated by modifying the truncated LPS-binding aptamer on the surface of nanoscale polydiacetylene vesicles using peptide bonding between the carboxyl group of the vesicle and the amine group of the aptamer. Molecular recognition between EPEC and aptamer at the interface of the vesicle led to blue-red transition of polydiacetylene which was readily visible to the naked eyes and could be quantified by colorimetric responses (CR). Transmission electron microscopy (TEM) was used to confirm the specific interactions between EPEC and polydiacetylene vesicles. Result Truncated aptamer showed the similar LPS-binding activity. The aptasensor could detect the target bacteria in a range of 105-108 colony-forming units (CFU)/ml within less than 30 minutes and its specificity was 100% for detection of EPEC O111. The sensor reproducibiliry obtained at 106 CFU/ml was 6. 08% R. S. D. The results of TEM confirmed that the specific interactions between EPEC and polydiacetylene vesicles. Conclusion A new aptasensor was developed successfully for rapid colorimetric detection of EPEC.
5.A cross-sectional investigation on dementia caregiver status in veterans community in Beijing

Objective To investigate the dementia caregivers' awareness of dementia diagnosis for patients aged≥80 years in veterans community in Beijing.Methods Patients aged 80 years and over who were diagnosed as dementia were selected from Dec.2009 to Jul.2011 in the veterans community in Beijing.A face-to-face interview and questionnaire survey were conducted in dementia caregivers individually,including basic social demographic data (gender,age,educational background),awareness rate of dementia,treatment and drug use situation and their attitude to dementia patients.Results A total of 118 caregivers were investigated individually.The caregivers in this study were mainly the spouse of the patients,and most of them were elderly people.46.6% (55/118) of caregivers were the spouse of the patients,and 52.5% (62/118) of caregivers were 65 years old and over.The caregivers' awareness rate of dementia was 32.3% (38/118) when patients were diagnosed as dementia.There was no correlations of gender,age and educational background with the awareness rate of dementia diagnosis (all P>0.05).50% of dementia patients received the dementia treatment,and only 17.8% (21/118) of patients got the first line drug treatment for dementia (acetylcholinesterase inhibitors and / or memantine).98.3 % (116 / 118) of caregivers agreed that the dementia patients should be treated kindly.Conclusions The dementia caregivers are mainly patients' spouse in veterans community and most of them are the elderly.The awareness rate of dementia diagnosis is low among the caregivers.Most patients do not take the formal treatment.The social support for the dementia caregivers should be strengthened.
6. Repeated transcranial magnetic stimulation combined with Donepezil can improve the cognition of cognitively impaired stroke survivors

Objective:
To investigate the effect of repetitive transcranial magnetic stimulation (rTMS) combined with donepezil on the cognition of persons with post-stroke cognitive impairment (PSCI) and their ability to perform activities of daily living (ADL).
Methods:
A total of 106 PSCI patients were randomly divided into an observation group and a control group using a random number table. Those in the observation group received 10Hz rTMS (5 seconds on and 25 seconds off for 20 minutes daily) and donepezil daily, 5 days per week for 4 weeks, while those in the control group were provided with donepezil but only sham rTMS on the same schedule. Before and after 4 weeks of treatment, the Montreal cognitive assessment scale (MoCA), the Rivermead behavior memory test (RBMT) and the modified Barthel index (MBI) were used to evaluate the subjects′ cognitive functioning, memory capacity and ADL ability. The latency and amplitude of auditory event-related potential P300 were also assessed using a myoelectric evoked potential apparatus.
Results:
After the treatment, improvement was observed in all the measurements of both groups. After the treatment, the average MoCA, RBMT and MBI scores, as well as the latency and amplitude of P300 in the observation group were all significantly better than among the control group.
Conclusions
rTMS can supplement donepezil′s ability to improve the cognition and ADL ability of persons with PSCI. Such therapy is worthy of clinical promotion and application.

8.Mast cell and its relation to eosinophilic inflammation in CRSsNP.

OBJECTIVE:
To observe the pathologic characteristics, and investigate mast cell and its activation in chronic rhinosinusitis without nasal polyps (CRSsNP) and their relations with eosinophilic inflammation.
METHOD:
HE stain was used to observe tissue features and count total inflammatory cells, mononuclear cells, plasma cells and eosinophilic in lamina propria of CRSsNP and inferior turbinate. Toluidine blue stain and immunohistochemical stain for tryptase were used to detect mast cell and its activation respectively in CRSsNP and control, and their corelations with tissue eosinophilia were analysed.
RESULT:
CRSsNP has increased total inflammatory cells, mononuclear cells and plasma cells but comparable eosinophilic and lamina propria glands compared with inferior turbinate. Mast cells corelated with activated mast cells, but there was no difference between CRSsNP and control for both of them and there were no corelation between mast cell and its activation with tissue eosinophilia.
CONCLUSION
CRSsNP has more serious inflammation but no more mast cell and its activation and eosinophil compared with inferior turbinate, and there were no corelations between mast cell and its activation with eosinophil count which suggests that mast cell and eosinophilic inflammation mediated by it may not play an important role in the pathogenesis of CRSsNP.

9.Management of atrial fibrillation with integrated general-specialty model within medical consortium

In order to effectively manage the atrial fibrillation patients, West China Hospital has established an integrated general-specialty management model within medical consortium. This model takes the atrial fibrillation-stroke integrated management outpatient clinic as the platform, combines general practice and specialty to provide standardized care for atrial fibrillation patients. The model was characterized by primary diagnosis, two-way referral, up and down linkage, and differential management for acute and chronic conditions. This article, taking West China Hospital as an example, introduces the integrated team of cardiologists and general practitioners in the tertiary hospital with general practitioners in the community, and the preliminary accomplishment in the management of atrial fibrillation within the medical consortium. It would provide a reference for the long-range management of atrial fibrillation in other provinces and cities.
10.Impact of multidisciplinary diagnosis and treatment on postoperative 30-day mortality and complications in elderly patients with hip fracture

Objective:To investigate the impact of multidisciplinary diagnosis and treatment on postoperative 30-day mortality and complications in elderly patients with hip fracture.Methods:A retrospective analysis was conducted of the 260 elderly patients with hip fracture who had been treated by the mode of multidisciplinary diagnosis and treatment at Department of Orthopedics, Shenzhen Second People's Hospital from June 2018 to October 2019. The multidisciplinary group consisted of 66 males and 194 females with an age of 78.7 years ± 5.1 years, and 141 femoral neck fractures, 114 intertrochanteric fractures and 5 subtrochanteric fractures. They were compared with the 242 elderly patients with hip fracture (traditional group) who had been treated by the traditional mode from January 2017 to May 2018. The 2 groups were compared in terms of preoperative waiting time, 48-hour operation rate, 30-day mortality, and incidences of postoperative pneumonia and pressure ulcer.Results:There were no statistically significant differences in the preoperative general data or operative procedures between the 2 groups, showing comparability ( P>0.05). For the multidisciplinary group, preoperative waiting time was 41.9 h ± 36.5 h, significantly shorter than that for the traditional group (71.4 h ± 13.9 h), 48-hour operation rate 66.5% (173/260), significantly higher than that for the traditional group(8.7%, 21/242), incidence of postoperative pneumonia 3.1%(8/260), significantly lower than that for the traditional group(9.9%, 24/242), incidence of postoperative pressure ulcer (5.4%, 14/260), significantly lower than that for the traditional group(11.2%, 27/242), and 30-day mortality(2.3%, 6/260), significantly lower than that for the traditional group(5.8%, 14/242) (all P<0.05). Conclusions:Establishment of a mode of multidisciplinary diagnosis and treatment can significantly reduce the prolonged preoperative waiting time for elderly patients with hip fracture, thereby greatly reducing postoperative complications and postoperative 30-day mortality.
