1.Advances on medical management of Lennox-Gastaut syndrome

Lennox-Gastaut syndrome ( LGS) is an globally recognized refractory epilepsy,also known as an age-dependent epileptic encephalopathy. It is characterized by multiple seizure types, mental retardation, and slow spike-wave complexes discharges or fast rhythmic bursts during sleep on electroencephalographic re-cordings. Many antiepileptic drugs were reported for the treatment of LGS,but it was difficult to compare their efficacy because of the small sample size and design limitations. Valproate,topiramate and lamotrigine are rec-ommended to be the first-line drugs by most guidelines or expert consensus,and the combination of valproate and lamotrigine or the combination of lamotrigine and topiramate should be preferred as early as possible for initial treatment of LGS in children. Recent progresses of medical treatment of LGS are reviewed and some optical re-gimes are proposed in this paper.
2.Application of guided bone regeneration technique in dental implantation

Guided bone regeneration technique (GBR) is regarded as one of the most important methods to increase the bone amount of alveolar ridge, which is often recommended to be applied in the dental implantation surgery. Barrier membrane is utilized in GBR technique to cover the bone defect and create a secluded space, which prevents the connective tissue from growing into the space and facilitates the growth priority of bone tissue. In recent years, along with the emerging of different kinds of barrier membranes and bone grafting materials, the scope of clinical application of this technology is continuously expanded. This article reviews the classification and property of the membranes and the application situation of guided bone regeneration technology in the field of dental implantation.
3.Efficacy of continuous glucose monitoring system combined with insulin pump in diabetic pafients aged 60 years and over

Objective To observe hypoglycemic efficacy of continuous glucose monitoring system (CGMS)combined with continuous insulin infusion(CSII)in the elderly diabetic patients aged 60 years and over.Methods A total 0f 100 elderly diabetic patients were randomly divided into two groups.50 were treated with CGMS or self-monitoring blood glucose(SMBG)and CSII.and whose insulin dose was adjusted based on results of monitoring,and the other 50 with SMBG and insulin pump as controls.whose insulin dose was adiusted based on the results of SMBG for three days.After two weeks.all the patients were monitored by CGMS for blood glucose.Results Mean blood level of glucose and mean amplitude of glycemic excursions(MAGE)were significantly lower in CGMS+CSII group than those in controls[(6.6±2.3)mmol/L vs.(7.5±2.1)mmol/L,and(3.9±0.9)mmol/L vs.(5.1±0.6)mmol/L,respectively,P<0.05 1.Average insulin dose used was significantly lower in CGMS+CSII group than that in controls [(0.64±0.21)U/kg vs.(0.82±0.41)U/kg,P<0.05],and duration of hypoglycemia was less in the former than that in the latter[(20±3)min vs.(40± 9)min,P<0.05 ].Conclnsions CGMS combined with CSII can decrease blood glucose.glucose excursions.average dose of insulin and duration of hypoglycemia in the elderly diabetic patients,as well as prevent occurrence of hypoglycemia.
4.Investigation on Contents of Antibiotics and Antiseptics Added in Cosmetics

Objective To study the contents of antibiotics and antiseptics in the cosmetics sold in markets. Methods The cosmetics sold in professional market and current market (in which cosmetics were retailed) were selected respectively in Guangzhou and the contents of four antibiotics (oxytetracycline, tetracycline, chlortetracycline and chloramphenicol) and five antiseptics (para-hydroxybenzcic acid, methyl para-hydroxybenzoate, ethyl para-hydroxybenzoate, propyl para-hydroxybenzoate and butyl para-hydroxybenzoate) were determined by high performance liquid chromatogram. Results In 111 cosmetics, 30 cosmetics were detected containing oxytetracycline, 62 cosmetics containing tetracycline, 7 cosmetics containing chlortetracycline and 1 cosmetic containing chloramphenicol. The detection rate was 78.4% and the detection rate of antibiotics among various kinds of cosmetics had no significant difference. The antiseptics used in cosmetics were mostly para-hydroxyl compound ester and there were four cosmetics detected containing antiseptics. Conclusion Abusing antibiotics in cosmetics market is very serious. The test standards and methods for antibiotics and antiseptics in the cosmetics should be developed as soon as possible. Cosmetic enterprises need to decrease microbe pollution in the process of production and prohibit adding antibiotics in cosmetics.
5.The general rule and the way for using the wedge filter

purpose:This paper is to provide the gerneral principle and method of using the wedge filter. Materials and Methods:A concept of the irradiation angle and a equation to calculate it were introduced. The radiation angle and the given dose for each field were derived from this equation.The wedge angles of each field were got.Results: By means of the examples of laryngocarcinoma and carcinoma of maxillary sinus,the isodose graphs from the TPS using the radiation angles of the equations comfirmed this method.Conclusion: The way introduced is brief and raliable. It may help to optimize the treatment plan.
6.Clinical Study on Kechuanling Effervescent Troche in Treating Cough in Children for Attacking on the Lung by Wind and Heat (Acute Bronchitis)

Conclusion There was no obvious difference in the effectiveness on treating cough in children for attacking on the lung by wind and heat (acute bronchitis) between the treated group and the control group. During the reliability observation, Kechuanling Effervescent Troche indicated good clinical reliability.
7.Multi-drug Resistance of Acinetobacter baumannii

OBJECTIVE To investigate the drug sensitivity situation of Acinetobacter baumannii(ABA) in 3 years to instruct clinical application of the antibiotics.METHODS The drug sensitivity test data of the 428 A.baumannii(ABA) strains from 2006 to 2008 were analyzed retrospectively.RESULTS It was showed from the drug sensitivity test in vitro that the most sensitive drug for A.baumannii(ABA) was imipenem,which resistance rate was 11.3-34%.The resistance rate to amikacin was 29.8-49.5% and to penicillins was is 57.7-91%,the resistance rate to of penicillins drugs was 54-74.5%.The resistance of ABA to ampicillin,cefazolin,cefpodoxime and Cefoxitin was over 91%,and showed the multi-drug resistance features.CONCLUSIONS According to the result of drug sensitive test,the most effective antibiotics are imipenem,meropenem and polymyxin B sulfate.
8.Metabolites of injected chlorogenic acid in rats.

Chlorogenic acid (5-CQA) is one of the major components in some Chinese herbal injections. However, the metabolism of 5-CQA in rats after intravenous injection has not been determined. An ultra-high performance liquid chromatography/quadrupole time-of-flight mass spectrometry (UPLC/Q-TOF MS) method was applied to identify the metabolites in bile, urine, feces and plasma after a single intravenous administration of 10 mg x kg(-1) 5-CQA to rats. Using MSE and mass defect filter techniques, a total of 35 metabolites were detected in bile, urine, feces and plasma. The predominant metabolites in bile were glutathione conjugates of O-methyl-5-CQA, accounting for approximately 80% of the metabolites excreted in bile. The major components in urine were parent drug, O-methyl-5-CQA, hydrolyzed metabolites and glucuronide conjugates. The major components in feces were O-methyl-5-CQA and its cysteine conjugates. The major component in plasma was the parent drug. The urinary and fecal excretion pathways were equally important to 5-CQA in rats. These results demonstrate that 5-CQA undergoes extensively metabolism in rats and are highly reactive to nucleophiles such as GSH. This finding indicates that attention should be paid on the injections containing 5-CQA, which may covalently bind to proteins, leading to allergenic drug reactions.
9.Consideration and Suggestion on Clinical Pharmacist Participation in the Treatment of One Dispute Caused by Severe Adverse Drug Reaction

Objective:To discuss the role of clinical pharmacists in avoiding, eliminating and alleviating the effect of medical dis-putes caused by adverse drug reaction ( ADR) and provide reflections and suggestions for the disputes. Methods:Clinical pharmacist participation in the treatment of one case of dispute caused by serious adverse drug reaction was described and analyzed. Results:Clini-cal pharmacists provided opinions and suggestions for the dispute, and promising effect was obtained. Conclusion: Clinical pharma-cists should have excellent medical knowledge and good communication skills, and medical personnel should closely monitor patients in order to avoid medical disputes caused by ADR.
10.Changes of Nasal resistance in obstructive sleep apneasyndrome

Objective To investigate the obstructive sleep apnea syndrome ( OSAHS) associated with nasal resistance.Methods 30 cases with obstructive sleep apneahypopnea syndrome ( OSAHS ) in the OSAHS group, 30 cases with snoring but without OSAHS in the snoring group and 30 cases with normal group based on polysomnog-raphy were detected the nasal resistance.Results The index of thsnoring index[(42.7 ±27.3),(19.4 ±18.5), AHI(38.7 ±30.2),(5.9 ±1.6)and Resl+r(1.63 ±1.11)Pa· s-1· mL-1,(0.72 ±0.31)Pa· s-1· mL-1] in OSAS group and snoring group were significantly higher than those of the normal group ( t=6.451,7.444,7.698,all P<0.05);The index of lowest oxygen saturation ( SaO2 ) ,Flowl+r in OSAS group and thsnoring group were lower than that in normal group(t=7.551,7.334,all P<0.05);There were significant differences among groups of nasal resistance (t=7.451,8.144,8.608,8.904,all P<0.05);There were positive correlation between nasal resistance and snore index (r=5.658,P<0.01);The index of nasal resistance correlated with AHI index (r=0.002,P>0.05),wake times (r=0.083,P>0.05) and lowest SaO2(r=0.105,P>0.05)were not significantly correlated;The cases with OSAS cephalometric.The results suggest majority snoring symptoms significantly more at supine posi-tion in OSAS.Conclusion Nasal resistance increased,which may be associated with the occurrence in patients with OSAHS.
