1.Analysis of clinical curative effect of the therapy of goserelin combined with microwave on postoperative patients of endometriosis

Objective To observe the clinical efficacy of therapy of application of goserelin(Zoladex)com-bined with microwave on patients with endometriosis after laparoscopic surgery.Methods 188 patients with endome-triosis were randomly divided into two groups according to the randomized digital table method,with 94 cases in each group,as the observation group(Zoladex combined with microwave therapy group)and the control group(Zoladex treatment group).Among them there were 80 patients with infertility,45 cases in the observed group and 35 cases in the control group.After treatment,the clinical efficacy rate,recurrence rate and pregnancy rate of patients with infertil-ity were compared between the two groups.Results The efficiency of the observation group was significantly higher than that of the control group(80.12% vs.65.31%,χ2 =5.338,P <0.05),and the recurrence rate of the observa-tion group was lower than that of the control group(22.34% vs.38.56%,χ2 =8.325,P <0.05),the difference was statistical significance.The pregnancy rate of patients with infertility in the observation group were significantly higher than that of the control group,and the difference was statistically significant[64.38%(29 /45)vs.44.46%(16 /35)χ2 =4.531,P <0.05].Conclusion As for patients with endometriosis after laparoscopic surgery,the therapy of Zoladex drug treatment assisted with microwave treatment at the same time,could achieve good clinical curative effect and low recurrence rate,and could improve the pregnancy rate of patients with infertility.
2.The transfection of bone marrow stem cell with Tet-on regulating system and the pilot study of doxycycline depending expression with Tet-on regulating system

Objective To determine the feasibility of using Tet-on regulatory system to transfect primary rabbit bone marrow stem cells, and to control the activities of the target gene. Methods The Tet-on regulatory system were used to transfect bone marrow stem cells by lipofectamine with the increasing concentration of doxycycline (Dox), the activity of the fluorescence caused by the expression of the target gene (the luciferase gene) which transfected to bone marrow stem cells with Tet-on regulatory system were analyzed. Results Plasmids DNA after being digested by enzymes and agarose electrophoreses were analyzed. The DNA segments were cored with the plasmid map. The two DNA segments with the size of 6.02 kb and 1.38 kb could be detected after the plasmid of pTet-on, 5.2 kb DNA segment could be seen after the plasmid of pTRE-luciferase was digested by Hind Ⅲ. With the increasing concentration of Dox, the activity of the fluorescence caused by the expression of the target gene increased gradually. Although the uninfected bone marrow stem cells were induced, the relative-luciferase-unit(RLU) value was almost the same as that of the blanket control. It showed that uninfected bone marrow stem cells were not induced by Dox. The RLU value in infected bone marrow stem cells before induction was slightly high, but similar with the blanket control. This experiment shed light on that infected bone marrow stem cells showed good response to the induction by Dox. Conclusion With the help of Lipofectamine, the cultured bone marrow stem cells can be transfected by Tet-on regulatory system successfully. The expression of the target gene transfected to bone marrow stem cells with Tet-on regulatory system could be regulated by Dox.
3.Expressions of tumor related gene NAG6, NAG-7 and BRD7 in gastric cancers

Objective NAG6, NAG 7 and BRD7 genes were novel tumor related genes identified recently. This study was designed to examine the expression of these genes in gastric cancer and matched paracancerous tissues and to investigate their roles in the occurrence and development of gastric carcinoma. Methods The expression levels of NAG6, NAG 7 and BRD7 in 34 cases of human gastric cancer and matched paracancerous tissues were analyzed by RT PCR, Northern blot analysis and Dot hybridization. Results The expression of NAG6 was absent in 61.8% gastric cancer tissues(21/34). The expression of NAG6 was significantly down regulated in gastric cancer tissues compared with that in paracancerous tissues ( P 0.05 ). The expression rates of NAG 7 in gastric cancer and paracancerous tissues were 88.2% (30/34) and 82.3% (28/34) respectively. BRD7 was expressed in all gastric cancers and matched paracancerous tissues. The results of RT PCR, Northern blot analysis and Dot hybridization all showed that the expressions of NAG 7 and BRD7 did not display any difference between gastric cancer and matched paracancerous tissues. Meanwhile, Dot hybridization also confirmed that NAG6 was significantly down expressed in gastric cancer. Conclusions NAG6 gene was significantly down regulated in gastric cancer tissues. The down regulation of this gene may play a key role in occurrence and development of gastric cancer, however it may be not associated with lymph node or distance metastasis. The expression levels of NAG 7 and BRD7 were not altered in gastric cancer. It seems that NAG 7 and BRD7 gene may not contribute to the occurrence and development of gastric carcinoma.
4.Application of echo-guided percutaneous radiofrequency ablation by using a LeVeen needle electrode combination in the treatment of large liver tumor

Objective To evaluate the therapeutic effectiveness of echo-guided percutaneous radiofrequency ablation(RFA) by using a LeVeen needle electrode combination in the treatment of large liver tumor,and to discuss the relationship between the amount as well as the distribution pattern of the electrodes and the therapeutic effectiveness.Methods During the period of Feb.2006-Dec.2008,echo-guided RFA was performed in 113 patients with large and unresectable liver tumor,with a total of 118 lesions.According to the size of the tumor,the patients were divided into four groups.The tumor’s diameter of Group A(n = 64) was 4.0-5.0 cm,Group B(n = 28) was 5.1-6.0 cm,Group C(n = 11) was 6.1-7.0 cm and Group D(n = 10) was 7.1-9.3 cm.Based on the tumor’s diameter,the amount of the electrodes to be used and the sites to be ablated were determined.After the procedure,the follow-up checks with MRI or CT scanning were carried out to observe the necrotic extent and the local recurrence.Results Two months after the treatment,the complete necrosis rate of the tumor in Group A,B,C and D was 88.4%,78.6%,63.6% and 40.0%,respectively,with an overall necrosis rate of 79.7% in total 118 lesions.During a follow-up period of 3-36 months,the local recurrence rate in Group A,B,C and D was 5.5%,10.0%,28.6% and 50.0%,respectively.Severe complications,including intrahepatic infection(n = 2),puncture passage bleeding(n = 1),symptomatic pleural effusion(n = 4) and biloma(n = 2),occurred in 9 cases.No death related to RFA treatment occurred.Conclusion Echo-guided percutaneous radiofrequency ablation by means of multi-sites superimposition method with a LeVeen needle electrode combination is an safe and effective therapy for the hepatic tumors with the diameter over 4 cm.
5.Effects and mechanisms of 5-aza-2'-deoxycytidine on endometrial cancer cell

Objective To investigate the effects and mechanisms of 5-aza-2'-deoxycytidine (5-Aza-CdR) on endometrial cancer cell.Methods In vitro experiments of 5-Aza-CdR were done using human endometrial cancer cell line HEC-1B.Evaluation of cellular proliferation and apoptosis was ascertained respectively using trypan blue exclusion and flow cytometry.RT-PCR and methylation specific PCR(MSP) was done to detect the expression of RASSF1 A mRNA and methylation status of RASSF1 A promoter of HEC-1B cell line.Results (1) The status of cellular growth and apeptosis of HEC-1 B cell line:the growth inhibition effects of 5-Aza-CdR on HEC-1B cell line were both concentration-dependent (P < 0.01) and time-dependent(P <0.01),as well as the apoptosis rate of HBC-1-B cell line depended on the dose of 5-Aza-CdR obviously(P <0.01).(2)The expression of RASSF1A mRNA of HEC-1B cell line:RASSF1A mRNA was expressed in HEC-1B cell after 5-Aza-CdR treatment,but it was undetectable before the treatment.In the groups with different concentration of 5-Aza-CdR (0.05,0.1,1,5,10 nmol/ml),the expression of RASSF1A mRNA was respectively 0.074±0.004,0.105±0.004,0.167±0.006,0.334±0.005,0.484±0.007,which were remarkably different from the group without 5-Aza-CdR(the expression of RASSF1A mRNA was 0;P < 0.01).(3) The hypermethylation of RASSF1A promoter of HEC-1B cell line:the hypermethylation of RASSF1A promoter was detected in HEC-1B cell line.The status of hypermethylation was decreased after treatment with 5-Aza-CdR of 0.05,0.1,1,5 nmol/ml,meanwhile,both methylation bands and demethylation bands were observed by methylation specific PCR.After the treatment with 5-Aza-CdR of 10 nmol/ml the hypermethylation was absent absolutely.Conclusions (1) In HEC-1B cell line,5-Aza-CdR can inhibit cell proliferation and induce cell apopotosis.(2) 5-Aza-CdR can renew the expression of RASSF1A mRNA of HEC-1B cell line and reverse the hypermethylation of RASSF1A promoter.
6.Influencing factors of selecting vascular access device for tumor patients undergoing intravenous therapy

Objective To probe into the influencing factors of selecting the intravenous vascular access device for tumor patients undergoing intravenous therapy? Methods With convenience sampling,376 tumor patients in oncology department from 3 hospitals of class A hospitals in Guangzhou were selected for the survey including such 6 categories with 41 items as general data, patient’s willingness,punctured blood vessels,use of vascular access and clinical conditions? Results In 124 patients,peripheral venous catheter was used (33?0%)? Central venous catheter was used in 252 patients (67?0%),Multiple linear regression analysis showed that way for paying the expense,vascular conditions for venous punctuation,phased diagnosis,decision of the doctors,and prospected duration for detaining catheter significant influencing factors? Conclusions The state should raise their medical allowances for those with lower incomes? The administrative should enhance the training to doctors and nurses at grass levels,promote the communication between medical workers and patients and improve their recognition of transfusion devices so that they can make a right choice?
8.Analysis of the Changes of Burden Factor Loading for Chronic Disease Inpatients before and after the Im-plementation of Zero Price Policy by Establishing Structure Equation Model

OBJECTIVE:To construct the structure equation model for the burden of chronic disease inpatients in primary hos-pital before and after the implementation of zero price policy,and analyze the changes of burden factor loading. METHODS:6 pri-mary hospitals were randomly selected in Danyang,Jiangsu province. The data of hospitalization expenses for chronic diseases(hy-pertension,diabetes,bronchitis)were collected before and after the implementation of zero price policy. Using drug cost,nursing fees,inspection fees and treatment fees as independent variables,hospitalization burden as latent variable,SPSS and AMOS 24.0 software were adopted to establish the model. RESULTS:The burden factor loading of inpatients changed greatly before and after the implementation of zero price policy. χ2=24.586,χ2/df=1.446, RMSEA=0.019,GFI=0.995,AGFI=0.989,CFI=0.988,NFI=0.963 manifested good model fitting. Factor loading of drug cost increased greatly,indicating the burden of inpatients was reduced after the implementation of zero price policy. Factor loading of inspection fees and treatment fees increased significantly,the charac-terization effects of them to the burden of inpatients were enhanced. Factor loading of nursing fees was the lowest,and it had the weakest effects on the burden of inpatients,of which the government should enhance the regulation. CONCLUSIONS:Established model has good reliability and validity. It can reflect the change of burden factor loading of inpatients before and after the implemen-tation of zero price policy.
9.Analysis of cerebral hemorrhagic infarction and relative factors in acute ischemic stroke

ObjectiveTo analyze cerebral hemorrhagic infarction (HI) and relative factors with the database of Standard Stroke Registry (SSR).Methods1 487 consecutive patients with acute ischemic stroke admitted within 3 days after onset were analyzed with SSR.ResultsHI was observed in 11% of patients, of whom 51% were diagnosed as having cardiogenic embolism. In patients with supratentorial infarction of cardiac origin, 28% had HI (mild 67%, moderate 23, hematoma 10%). On multiple logistic regression analysis, independent factors related with HI were found to be age, prosthetic cardiac valve and NIHSS scores at admission. Patients with more severe HI were associated with a poorer outcome at discharge. Clinical outcome tended to be better in patients receiving thrombolytic agents than in those without receiving agents, while the incidence of HI was slightly more frequent in the former.Conclusion It is confirmed that the SSR database is useful for understanding and analyzing the status of stroke diagnosis and management throughout the nation, and revisions are needed in some formats.
10.Effect of peroxisome proliferator-activated receptor-γ ligand on acute necrotizing pancreatitis associated lung injury in rats

Objective To investigate the effect of Rosiglitazone on acute necrotizing pancreatitis (ANP) associated lung injury in rats and its potential mechanism. Methods Fifty-four male Wistar rats were randomly divided into sham operation group (SO group), ANP group and Rosiglitazone pretreatment group (Rosi group). ANP model was induced by retrograde infusion of 5% sodium taurocholate into the biliopancreatic duct. The rats of SO group, ANP group were injected with 10% dimethyl sulfoxide (DMSO) (0.2 ml/100 g) via femoral vein 30 minutes prior to the operation. In Resi group Rosiglitazone partes aequales (6 mg/kg), which was dissolved in 10% DMSO, was injected. Rats were killed at 3 h, 6 h and 12 h after induction. The serum amylase, lung myeloperoxidase (MPO), lung wet/dry (W/D) weight ratio, protein content of bronchoalveolar lavage fluids (BALF) were measured, and pancreas and lung pathologic changes were evaluated. RT-PCR was used for detecting the levels of TNF-α and ICAM-1 mRNA in lung tissue. Results In ANP group, the values a of serum amylase, MPO, W/D weight ratio, protein content of BALF, pancreas and lung pathologic score, lung TNF-α and ICAM-1mRNA expression increased corresponding to the duration of ANP course, and the corresponding values at 12 h were (5353.0±728.2) U/L, (1.12±0.14) U/g, 3.00±0.14, 0. 438±0.056, 11.17±0.93 and 8.17±0.75, respectively, which were significantly higher than those in the SO group (P<0. 05 or P<0.01). The corresponding values in the Rosi group were (2847.4±841) U/L, (0.84±0.06) U/g, 2.13±0.36, 0.283±0.078 g/L, 7.75±0.27, 4.33±0.82, 0.26±0.04 and 0.84 ±0.02, respectively, which were significantly higher than those in the SO group (P <0. 05 or P <0.01). Conclusions Rosiglitazone had protective effect on lung injuries in ANP rats through down-regnlating the expression of TNF-α and ICAM-1 mRNA.
