2.Analysis of causes of pediatric bronchoscopy-induced complications and the nursing measures

Objective To analyze the causes of complications by pediatric fiberoptic bronchoscopy and find out the nursing measures. Method The clinical data of 60 children undergoing fiberoptic bronchoscopy were reviewed to analyze the causes of complications and concluded the nursing measures. Result Twelve in the sixty children contracted complications, including 4 cases of cyanotic , 4 cases of throat injury and 2 cases of laryngeal spasm . Conclusion Such measures as psychological nursing before operation to the children and their families for their cooperation, intraoperative oxygen suction, quick clinical examination, avoidance of frequent negative pressure suction and close observation of complications are of great importance for preventing complications induced by fiberoptic bronchoscopy.
3.Troubleshooting of bioinequivalence of compound valsartan tablets.

The study aims to evaluate the bioequivalence of valsartan hydrochlorothiazide tablets, and to investigate the potential cause of bioinequivalence. This was a single-center study with an open, randomized double-way crossover design. Test and reference preparations containing 160 mg of valsartan and 25 mg of hydrochlorothiazide were given to 36 healthy male volunteers. Plasma concentrations of valsartan and hydrochlorothiazide were determined simultaneously by LC-MS/MS. The pharmacokinetic parameters and relative bioavailability were calculated, while the bioequivalence between test and reference preparations were evaluated. The dissolution profiles of test and reference preparations in four different mediums were determined via dissolution test and HPLC. The similarity was investigated according to the similarity factors (f2). The F(o-t) and F(0-infinity) were (139.4 +/- 65.2)% and (137.5 +/- 61.2)% for valsartan of test preparations. It led to get the conclusion that test and reference preparations were not bioequivalent for valsartan. A significant difference was observed between test and reference tablets in the valsartan dissolution test of pH 1.2 hydrochloric acid solution. The key factor of the bioinequivalence might be that dissolution of valsartan in acid medium has marked difference between two preparations.
4.Research Analysis on Medication Compliances of Tuberculosis Patients

OBJECTIVE:To probe into the status quo of medication compliances and problems in the medication of patients with tuberculosis so as to direct their use of drugs.METHODS:Information on medication compliances of tuberculosis patients who experienced regular physical examinations was investigated by interview and questionnaires,and which were analyzed statistically.RESULTS:Of the562cases investigated,54%followed or almost followed medication directions;46%failed to follow normal drug therapy for different reasons.CONCLUSION:The pharmacists should do their utmost to expand the range of pharmaceutical care so as to improve patients'medication compliances.
5.Vibrio vulnificus: cultivation, identification and antimicrobial susceptibility

Objective To investigate the cultivation, biochemical features and drug susceptibilities of Vibrio vulnificus. Methods Three strains of Vibrio vulnificus were isolated from fatal patients in the Second Hospital of Jiaxing, Zhejiang province. Cultivation, identification and antibacterial susceptibility test were performed. Results Vibrio vulnificus grew on blood agar as dull-gray, opaque colonies with β-hemolysis. The organism presented positive in lactose, cellobiose fermentation and O/129 (10 μg) tests, but lack of inositol and rhamnus. The antibacterial susceptibility tests showed that Vibrio vulnificus strains were sensitive to ciprofloxacin, levofloxacin, imipenem, cefepime, ceftazidime, ceftriaxone, compound sulfamethoxazole and nitrofurantoin, however, resistant to gentamicin, tobramycin, aztreonam and cefazolin. Conclusions Vibrio vulnificus can be isolated from blood, bubbles fluid, and stool. Rapid identification, early diagnosis, and prompt empirical antibacterial therapy are important for reducing the mortality.
6.Construction of rabbit models of radiation-induced brain injury and selection of magnetic resonance parameters

BACKGROUND:Radiation-induced brain injury has recently become an increasing area of research, in particular in animal experimental studies. Domestic and international researches show that there have been no uniform scanning parameters used for examination of animal models of radiation-induced brain injury by magnetic resonance imaging. In this study, we performed magnetic resonance imaging in rabbits to determine related sequence parameters. OBJECTIVE:To establish the New Zealand rabbit models of radiation-induced brain injury, and obtain the brain magnetic resonance images of rabbits using LOOP7 coil, so as to provide experimental evidence for diagnosis of radiation-induced brain injury by magenetic resonance imaging. METHODS:Each of T2-weighted imaging, diffusion tensor imaging, magnetic resonance spectroscopy and magnetic susceptibility-weighted imaging were performed several times through the use of LOOP7 coil, to determine the optimal scanning parameters for each sequence. Rabbit models of radiation-induced brain injury were established and then their right hemispheres were irradiated using 6 MV X-rays at a single dose of 40, 80 and 120 Gy. The daily performance and dynamic magnetic resonance signs of rabbits were observed. The brain tissue was taken for pathological examination once abnormal magnetic resonance findings were observed or after 20 weeks of folow-up. RESULTS AND CONCLUSION:Only one rabbit model in the 40 Gy group had subdural hemorrhage. In the 80 Gy group, abnormal T2-weight imaging signals were observed in al rabbit models, which were pathologicaly confirmed as scattered degenerated neurons and infiltrated neutrophils. The abnormal signals that gradualy expanded over time were seen in rabbits from the 120 Gy group by magnetic resonance imaging and were pathologicaly confirmed as radiation-induced brain injury loci. The results confirm that establishing rat models of radiation-induced brain injury using radiation therapy system can better simulate the pathological process of radiation-induced brain injury; moreover, this model can be applied to receive routine magnetic resonance examination with LOOP7 coil.
7.Meta-analysis of influential factors of pre-hospital delay of patients with acute ischemic stroke

Objective To investigate the main risk factors influencing the pre-hospital delay of patients with acute ischemic stroke (AIS) for preventing methods.Methods A retrieval was done across the epidemiologic studies on pre-hospital delay of AIS patients during 2004 to 2015 using the key words of ischemic stroke,prehospital delay,treatment delay.According to the inclusion and exclusion criteria,the literature was collected and the literature quality was assessed.The data were extracted for meta-analysis by RevMan software.Results Sixteen epidemiologic studies were included.Totally,there were 2,966 cases of prehospital delay and 2,468 cases of non-delay.The pooled OR values and 95% CI by multivariate analysis were as follows:the degree of awareness of stroke was 0.5 (0.3,0.82),emergency medical service (EMS) was 0.49 (0.29,0.85),National Institute of Health Stroke scale (NHISS) score 0.58 (0.36,0.95),outpatient visit as first visit 4.28 (1.44,12.74),distance 0.76 (0.61,0.95),medical history of ischemic stroke 0.37 (0.18,0.79) (with significant differences in OR value,P<0.05).Conclusions The protective factors for pre-hospital delay of acute ischemic stroke patients include:strengthened awareness of stroke,effective EMS,high NHISS score,short distance,medical history of ischemic stroke.The main risk factor is that their outpatient visit is their first visit for medical treatment.Therefore,we need to enhance the health education to the patients and their families about awareness of stroke,improve the use of EMS and build complete and efficient green path for the stroke patient.
8.Pathogens and Their Resistance to Antibiotics from 721 Serous Effusion Specimens

OBJECTIVE To analyze the pathogenic results and drug resistance of bacteria isolated from serous effusion specimens from Jan 2006 to Feb 2008 so as to provide evidence for clinical pathogenic analysis and selection of antibiotic. METHODS The bacteria identification and the antimicrobial susceptibility were assayed by routine methods. RESULTS The positive rate of pathogens was 6.80% from 721 serous effusion specimens.There were 49 strains of pathogens which included 15 of Gram-positive bacteria and 28 of Gram-negative bacteria and 6 fungi strains.The most frequently isolated pathogens were Enterobacteriaceae(32.65%) followed by Enterococcus(18.37%),and Pseudomonas aeruginosa(12.25%).The most active compounds against Enterococcus were vanconmycin and linezolid(100.00%),nitrofurantoin and tetracycline(66.67%);the most active compounds against Staphylococcus were vancomycin,linezolid and nitrofurantoin(100.00%),tetracycline and rifampin(75.00%);The most active compounds against Gram-negative bacilli were piperacillin/tazobactam(67.86%),imipenem(64.29%),tobramycin(60.71%),gentamicin(53.57%) and levofloxacin(46.43%). CONCLUSIONS The prevailing pathogens in serous effusion are Gram-negative bacilli,especially Enterobacteriaceae.Data collected in present study provide a valuable information for prophylactic and empirical antibiotic use for serous infection.

10.Keshan disease condition in Gansu Province in 2011

Objective To observe the condition of Keshan disease (KSD) in Gansu Province in order to provide a scientific basis for prevention and control of the disease. Methods In 2011, according to “The national Keshan Disease Monitoring Program (trial edition)”, based on searching of KSD cases, 12 villages of 12 towns in 6 counties of Gansu Province, were selected as surveillance sites. All the residents in surveillance sites were clinically examined and given 12-lead ECG tracings; suspected cases were taken anterior chest X-rays in the distance of 2 meters, while staple food and life conditions of the residents were surveyed. Diagnosis of KSD was based on“Keshan Disease Diagnostic Criteria”(WS/T 210-2011). Results Among investigated 5 484 residents, the incidence of abnormal electrocardiogram was 15.41%. Of the 196 X-ray films, 61 cases had enlarged heart(in which 22 of mild, 21 of moderate and 18 of severe). Two hundred and forty-three cases of KSD were detected, the general detection rate was 4.43%, in which 31 cases of chronic, the detection rate was 0.56%; 212 cases of potential, the detection rate was 3.87%. In all cases, 47 cases were under the age of 30, including 46 cases of potential and 1 case of chronic. The major abnormal electrocardiogram change of KSD cases was ST-T changes[22.68%(71/313)], followed by complete right bundle branch block[16.29%(51/313)], low voltage[12.46%(39/313)], and left anterior fascicular block[6.71%(21/313)]. Per-capita annual income in surveillance site was 1 763 Yuan;and the major staple food was flour, accounted for 87%, and the staple food rice accounted for 5%. Conclusion The detection rate of KSD in Gansu Province is mainly to potential, and there is a cosiderable portion of patients under the age of 30;monitoring and investigation should also be strengthened in the younger age of KSD cases detected village.
