1.Observation on the clinical efficacy of neoadjuvant chemotherapy in the treatment of patients with advanced epithelial ovarian cancer

Objective To explore the clinical efficacy of neoadjuvant chemotherapy on advanced epithelial ovarian cancer.Methods From May 2006 to March 2010,60 patients with Ⅲ-Ⅳ stage advanced epithelial ovarian cancer were collected in Affiliated Cancer Hospital of Shanxi Medical University,which were divided into two groups based on different treatment methods.The observation group used the neoadjuvant chemotherapy,but the control group did not use it.The efficacy,operation time,blood loss,size of residual disease and the level of CA125 were observed to evaluate the safety and clinical efficacy of neoadjuvant chemotherapy.Results The total effective rate in the observation group was 60.0%.The satisfied cytoreductive rate was 70.0%,which was significantly higher than the control group (x2 =5.455,P <0.05).The mean operative time (127.21 ±27.62) min (t =10.113,P < 0.05),intraoperative mean blood loss (408.5 ± 112.62) ml (t =5.047,P < 0.05)and the level of CA125 (165.26 ±43.27) U/ml (t =14.100,P < 0.05) in the observation group were significantly lower than the control group [189.33 ± 45.81) min,(590.6 ± 162.41) ml,(339.13 ± 51.86) U/ml].Five year survival rates of the observation group and control group were 36.7% and 26.7% respectively,with significant difference (x2 =8.492,P =0.036).Conclusion Neoadjuvant chemotherapy can play a positive role in the treatment of the patients with advanced epithelial ovarian cancer.
2.Clinical Observation on Efficacy of Low -calorie Enteral Nutrition Therapy for Postoperative Recovery in Patients with Primary Hepatic Carcinoma.

Objective To study the clinical efficacy of low -calorie enteral nutrition support therapy for postoperative recovery in patients with primary hepatic carcinoma .Methods Retrospective analysis was performed in this study .120 patients, including partial liver or local liver resection , were divided into Hypocaloric parenteral nutrition group ( HPN) and Hypocaloric parenteral nutrition group ( HEN) by different nutritional treatment ways with each group of 60 case.Liver function ( TBIL, ALT, AST ) , nutrition indicators (ALB, PCHE, PA), immune function (CD4 +T cells, CD8 +T cells, CD4 +T cells/CD8 +T cell ratio), gastrointestinal tract function ( postoperative intestinal ventilation ) , the incidence of complications and nutritional support costs and hospitalization time difference were compared between two groups .Results Compared with the low -calorie parenteral nutrition group , TBIL, ALT, AST were significantly lower on low calorie enteral nutrition group (P<0.05); ALB, PCHE, PA, ALB, PCHE, PA, CD4 +T cells, CD8 +T cells, CD4 +T cells/CD8 +T cell ratio were significantly higher on low calorie enteral nutrition group ( P<0.05);low-calorie gastrointestinal function recovery, incidence of complications during hospitalization and nutritional support costs on enteral nutrition group were superior to low -calorie parenteral nutrition group .Moreover, the differences were statistical significance (P<0.05).Conclusion For postoperative pa-tients with primary hepatic carcinoma , both enteral and parenteral nutrition support therapies have significant effect , however ,enteral nu-trition therapy has more advantages , better clinical efficacy by comparing to parenteral nutrition therapy .Therefore, it is worthy of apply-ing in clinic .
3.Retinal vascular characteristics and the strategy to manage retinal vascular diseases

The retinal vessel changes are the primary and major features of retinal vascular diseases.The retinal vessel is part of systemic vessels with its own characteristics to sustain normal retinal function.These basic characteristics are important to the correct understanding and proper treatment of retinal vascular diseases.Always keep in mind that the retinal vessels is one part of the systemic vascular system,thus retinal vascular diseases may have systemic etiology,and systemic drug administration may have a profound effects to the whole body.However retinal vascular system also has its own structural and functional characteristics,thus retinal vascular diseases are also different from the systemic diseases.Finally the main function of retinal vascular network is to maintain the neuro-retinal function,thus we should balance the vision protection and treatments against abnormal retinal blood vessels.Over-treatments may damage the retinal vision.
4.Predictors of ventilator weaning in children

Early weaning from mechanical ventilation in children will lead to an extubation failure,however,prolonging mechanical ventilation will increase the risk of ventilator associated complication and the economical burden of disease.Nowadays there is no unified and objective ventilator weaning indexes in pediatrics,so it is significant to searching for appropriate and feasible ventilator weaning indexes.This article detailed the current progress on the condition and predictors of ventilator weaning in children,and comparing the predictability and accuracy of ventilator weaning indexes in children,also looking forward to the possibility of higher accuracy or scheme in ventilator weaning that used for clinical purposes.
5.The clinical significance of the detection of adenosine deaminase combined with procalcitonin in adult patients with active pulmonary tuberculosis

Objective To explore the clinical significance of the adenosine deaminase ( ADA) and the pro-calcitonin(PCT) in adult patients with active pulmonary tuberculosis .Methods 81 patients with active pulmonary tuberculosis were enrolled into A group ,43 patients with non-tuberculous lung infection were enrolled into B group and 27 patients with lung cancer were enrolled into C group .Serum ADA and PCT were detected .Tuberculosis patients were treated for 6 months of anti-tuberculosis treatment .Results There was a significant difference between group A and B or C group about ADA (t=12.983,10.630,all P<0.01),B and C groups showed no significant difference (t=0.144,P=0.885).Group A PCT compared with group B were significantly different (t=16.775,P<0.01), the A group and C group showed no significant difference (t=1.571,P=0.119),group B compared with group C were significantly different (t=11.235,P<0.01).Tuberculosis treatment remission in 67 cases,no relief in 14 ca-ses.The levels of serum ADA and PCT in remission were significantly lower than in patients with no remission group (t=3.087,3.404,all P<0.05).Conclusion The serum ADA can be used as diagnostic indicators of active tuber-culosis,ADA and PCT joint determination can determine the prognosis of active tuberculosis .
6.Colored Latex Injection for Use in Cadaveric Head:Anatomic Technical Note

Objective The purpose of this study is to determine the better conditions of filling blood vessels of the fresh freezing cadaveric head with latex.Methods Colored latexes were injected in vessels of adult fresh freezing cadaver heads.Results The vessels especially arteries were filled well and could be identified and dissected under microscope easily in specimens after injection.Conclusion The specimens choice,vessels irrigation,antiseptic treatment and filling pressure are all important factors in this process.A good filling increases elasticity of vessels,facilitates dissection of vessels.It is convenient in the study of the courser,distribution of branchings and variations of vessels.
7.Construction of recombinant adeno-associated virus vector rAAV2-PD-L1 and its biological efficiency in transfecting mouse vascular endothelial cells

Objective:To construct a recombinant adeno-associated virus vector rAAV2-PD-L1 and to investigate the biological efficiency of rAAV2-PD-L1-transfected vascular endothelial cells in co-stimulating secretion of cytokines by T cells.Methods: Mouse PD-L1 cDNA was amplified by RT-PCR from total RNA of mouse liver tissues and was cloned into shuttle vector pSNAV1;the products were then transferred into BHK21 cells by lipofectamine and rAAV2-PD-L1 was screened out.Mouse vascular endothelial cell line 2F-2B was infected with rAAV2-PD-L1 and were co-cultured with activated mouse T cells,and the IFN? content was identified by ELISA in the supernatant.Results: The sequences of PD-L1 cDNA and pSNAV-PD-L1 were confirmed to be correct.The recombinant rAAV2-PD-L1 was verified by PCR and SDS-PAGE analysis.The virus physical titer was 4?10~(12) virus genome/ml and the protein concentration was 0.355 mg/ml.There was a high expression of PD-L1 in mouse vascular endothelial cells infected with rAAV2-PD-L1.The content of IFN? in the culture supernatant was significantly decreased 48 hours after co-culture.Conclusion: The recombinant rAAV2-PD-L1 can infect vascular endothelial cells and inhibit secretion of IFN? by activated T cells through costimulation.
8.Synergistic induction of apoptosis of HEP-2 cell line by the combination of TRAIL and 5-FU,DDP

Objective:To investigate whether TRAIL can synergize 5-Fu and DDP to kill HEP-2 cell line.Methods:MTT was used to detecet the cytotoxicity of HEP-2 cell line intervented in 5-FU,DDP,TRAIL,5-FU+TRAIL and DDP+TRAIL.Fluorescence microscope and FCM were used to detecet the apoptosis rate of cell.Results:HEP-2 cell line was insensitive in TRAIL,but the apoptosis rate obviously was upgraded after intervented in 5-FU+TRAIL and DDP+TRAIL.Conclusion:TRAIL in combination with subtoxic dose of 5-FU and DDP can effectively kill HEP-2 cell line.The combination of TRAIL,5-FU and DDP induced apoptosis of HEP-2 cell line.
10.Clinical analysis of 43 children with paragonimus pericarditis

Objective:To study the clinical characteristics of children with paragonimus pericarditis.Methods:The diagnosis and treatment effect of 43 children with paragonimus pericarditis were retrospectively analysed.Results:There was diversity in the clicinal manifestations.Thirty-four cases were treated with praziquantel,pericardiocentesis,open pericardial drainage and so on.In 30 cases that were followed up,26 cases were cured and 4 cases developed pericardial constriction.Four cases with constrictive pericarditis were cured after pericardiectomy.Conclusion:The children with paragonimus pericarditis could be diagnosed early by careful asking of the case history of eating crabs,routine examination of the numbers of the acidophile in blood and pericardial effusion and paragonimus antigen intracutaneous test.It is possible to develop pericardial constriction after clinical cure in those cases that have had a long course of disease before treatment.
