The surgery of the breast cancer has dramatically changed,sentinel lymph node(SLN) is one of the aspects.SLN biopsy can precisely predict the status of axillary basin.If the SLN is negative,the patients can spare the unnecessary axillary lymph node dissection(ALND) and reduce the morbidity of the surgery.One of the most important issues with sentinel node biopsy is the intraoperative pathological evaluation of the sentinel nodes.Accurate intraoperative pathological examination of sentinel nodes would enable the selection of candidates for ALND during the initial operation,eliminating the need for second surgery.This article will review the two techniques(touch imprint cytology and frozen section) employed for intraoperative examination of SLN and compare these two methods.And evaluate the role of touch imprint cytology and existing problems in introperative diagnosis of the sentinel lymph nodes and raise the probably ways to improve the sensitivity of touch imprint cytology initially.

OBJECTIVE: To explore the therapeutic effect of the extracts from Cordyceps militaris on membraneous nephritis model rats.METHODS: Membraneous nephritis model rats were induced by using cationic bovine serum albumin.3 weeks later,urine protein quantity(24 h) was determined weekly;at the end of experiment,the hemorheology,serum creatinine(Scr),blood urea nitrogen(BUN) and serum lipid level were examined and the therapeutic efficacy was evaluated.RESULTS:C.militaris-treatment reduced urine protein quantity(24 h),blood viscosity,serum creatinine(Scr),blood urea nitrogen(BUN) and serum lipid level and delayed the progress of renal lesions.CONCLUSION: Extracts of C.militaris has the action of improving renal function.
4.Analysis of factors influencing mannitol to induce blood-brain-barrier opening

AIM: To study factors influencing mannitol to induce blood brain barrier (BBB) opening. METHODS: By constant flow pump, mannitol with different concentration (20% and 25%) was infused into left internal carotid artery of adult Wistar rats via their left external carotid artery at different speed and time. Firstly, after mannitol with a concentration of 25% was transfused, the optimal open time of BBB was investigated using Evans blue as an indicator by injection at different time (0, 2, 4, 6, and 8 min). Then influences of concentration of mannitol, speed of infusion, and change of time on mannitol to induce BBB opening were studied. Finally, death rate was counted according to sex and body weight in order to observe the influence of sex and body weight on tolerance of rats. RESULTS: The open degree of BBB was optimal after 2 min when 3.6 ml mannitol with a concentration of 25% was infused in 30 s at a speed of 0.12 ml?s -1 . However, mannitol with a concentration of 20 % did not almost have any effect on opening of BBB. Slowing of infusion speed or shortening of infusion time of mannitol reduced opening degree of BBB. Body weight affected rat tolerance to mannitol, and the tolerance of the rat was higher in those with higher weight. CONCLUSION: The factors influencing mannitol to induce BBB opening are as follows: concentration, infusion speed, and keeping time of mannitol. And the tolerance to mannitol is not concerned with the sex of the rats, but the body weight of the rats.
5.Clinical value of plasma level of tissue factor in patient with coronary heart disease

Objective To study the relationship between plasma level of tissue factor(TF)and severity of coronary heart disease(CHD).Methods Coronary angiography was performed in 237 patients with suspected CHD,with 149 diagnosed as CHD and 88 excluded.Seventy of stenosis of coronary arteries (CA)was evaluated by modified Gensini score on the basis of angiographic imaging.Blood glucose,serum levels of total cholesterol(TC),triglycerides(TG),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C),lipoprotein(a)[Lp(a)],uric acid,high sensitivity C-reactive protein(Hs-CRP),fibrinogen(FIB)and plasma level of TF were determined for all the subjects.Relationship between plasma level of TF and severity of pathological changes in CA was evaluated by linear correlation and multiple regression analyses for controlling other relevant factors.Results Plasma level of TF in patients with CHD[(138±39)ng/L]was significantly higher than that in controls[(61±19)ng/L](P<0.01).Plasma level of TF obviously correlated with severity of pathological changes in CA(r=0.710,P<0.01).Partial correlation analysis showed a positive correlation between plasma TF and severity of pathological changes in CA,independent of other potential influential factors,with a partial coefficient of correlation of 0.509(P<0.01).Multiple stepwise regression analysis indicated that plasma level of TF associated with severity of pathological changes in CA,as well as with diabetes,LDL-C,Lp(a),Hs-CRP and FIB,with coefficients of correlation of 0.172,0.180,0.147,0.308 and 0.175,respectively(P<0.05).Conclusion Elevated plasma level of TF may be one of important risk factors for CHD,which reflects severity of pathological changes in CA.
6.Relationship between serum cytokines and depression in patients with colorectal cancer

Objective To explore the correlation between serum cytokines and the depression degree in the patients with colorec-tal cancer.Methods 9 6 cases of colorectal cancer were selected and divided into 2 groups according to the patients health question-naire(PHQ-9):the depression group (52 cases)and the non-depression group (44 cases),and 40 healthy cases were recruited as the control.Serum interleukin-2 (IL-2),soluble interleukin-2 receptor (SIL-2R)and tumor necrosis factor-α(TNF-α)were detected by enzyme-linked immunosorbent assay(ELSIA).The detection results were compared among various groups.And the correlation be-tween the expression of various cytokines and the depression degree was analyzed by the Pearson′s correlation coefficient.Results The levels of IL-2,SIL-2R and TNF-αhad statistical differences among 3 groups (P<0.05);no statistical difference in the TNF-αexpression was noted between the depression and the non-depression groups (P>0.05),but which in these two groups was higher than that in healthy group (P<0.05);the IL-2 level in the depression group was significantly lower than that in the non-depression group(P<0.05);the SIL-2 R level in the depression group was obviously higher than that in the non-depression group (P<0.05), moreover,which in these two groups was higher than that in the healthy group (P<0.05).By correlation analysis,the depression degree in the patients with colorectal cancer was negatively correlated with the IL-2 level (r=-0.259,P<0.05)and positively correlated with the SIL-2R level(r=0.412,P<0.05).Conclusion The levels of IL-2 and SIL-2R have certain correlation with the depression degree in the patients with colorectal cancer,it is speculated that the above cytokines may play a mediating role in the pathogenesis process of depression in the colorectal cancer patients.
7.Correlation between Serum Testosterone and Type 2 Diabetes Mellitus in Elderly Male

: Objective To explore the correlation between serum testosterone and type 2 diabetes mellitus in elderly male.Methods 1012 elderly male were divided into 2 groups according to their histry of DM.Blood glucose(FBG,PBG),cholesterol(CHO),triglyceride(TG),blood pressure(DBP,SBP),body mass index(BMI),serum insulin(FINS,PINS),serum testosterone(T)and estradiol(E2)were measured.Results Eldly male with type 2 diabetes showed higher FBG,PBG,TG,FINS,PINS and E2 level than those without DM(P<0.05),and with lower HDL-C and T level(P<0.05).The analysis of multiple stepwise regression showed that T level was correlated with the level of HDL-C,FINS,PINS and TG.Conclusion TG,HDL-C,FINS and PINS level correlate with T level in eldly male with type 2 diabetes mellitus.
8.Effect of niaoduqing in combination with telmisartan on the diabetic nephropathy in rats

Objective To investigate the effect of Niaoduqing combined with Telmisartan on the diabetic nephropathy in rats. Methods Forty female Wistar rats were randomly divided into the normal control group ( NC ) , the diabetic nephropathy in the control group ( DM ) , the uremic clearance group ( ND ) , and the combination group (LH). Rats in the DM, ND, LH groups were used to establish the diabetic nephropathy model. Rats in each group received the corresponding drugs. Twelve weeks later, the interactions among angiotensin receptor and adiponectin receptor were detected by co-immunoprecipitation assay. Western blot was performed to detect the expression of MCP-1, TGF-βto reveal the injury of the renal tubular interstitial. Results (1)Exogenous co-immunoprecipitation assay indicated that AdipoR1 and AT1, AT2 can form dimerization. Levels of AdipoR1-AT1, AdipoR2-AT2 dimerization were incresaed in the DM group compared with those in the NC group, but were decreased in the ND group and the LH group. (2)MCP-1 and TGF-βprotein expression were increased in the DM group compared with the normal control group (P < 0.05), but were decreased in the ND group and the LH group (P<0.05, respectively). Conclusion Niaoduqing combined with Telmisartan can significantly inhibit the expression of MCP-1 and TGF-βto relieve the renal tubular interstitial injury in rats with diabetic nephropathy. This protective mechanism may associated with the decrease of the dimerizations of AdipoRs and ATs.
9.Pathogen Distribution and Drug Resistance in Venous Catheterization Infection

OBJECTIVE To investigate antibiotic resistance and distribution of bacteria to provide reference for using antibiotics reasonably.METHODS Antibiotic susceptibility test was performed by K-B method,meticillin-resistant Staphylococcus(MRS)and ESBLs were identified according to NCCLS(2004)of the USA.RESULTS From in 118 strains of bacteria there were 75 Staphylococcus strains,and the isolated rate of meticillin-resistant S.aureus(MRSA)was 35.0%,and that of meticillin-resistant coagulase-negative S.aureus(MRCNS)was 40.0%.Among 41 Gram-negative bacilli,ESBLs isolating rates in Escherichia coli and Klebsiella pneumoniae were 18.2% and 30.0%,respectively.The resistant rate to clindamycin,erythromycin and ciprofloxacin in MRS was higher than in meticillin-sensitive Staphylococcus(MSS)significantly.Imipenem showed proper antibiotic activity to Gram-negative bacilli.CONCLUSIONS We should pay more attention to resistant strains to control the nosocomial infection outbreaking.
10.Pathogens and Their Resistance to Antibiotics from 878 Cerebrospinal Fluid Specimens

OBJECTIVE To analyze the pathogenic results and drug resistance of bacteria isolated from cerebrospinal fluid specimens from Jan 2006 to Nov 2007 so as to provide evidence for clinical pathogenic analysis and selection of antibiotic.METHODS The bacteria identification and the antimicrobial susceptibility were assayed by VITEK-AMS system.RESULTS The positive rate of pathogens was 11.62% from 878 cerebrospinal fluid specimens.There were 102 strains of pathogens which included 19 Gram-positives,79 Gram-negatives and 4 fungi.The most frequently isolated pathogen was Pseudomonas(41.18%)followed by Staphylococcus haemolyticus(9.80%),Escherichia coli(8.82%),Acinetobacter spp(5.88%),Staphylococcus epidermidis(4.90%)and Shewanella putrefaciens(4.90%).The most active compounds against Gram-positive cocci were vancomycin(100.00%),linezolid(94.74%),nitrofurantoin(84.21%),tetracycline(78.95%),and rifampin(73.68%);The most active compounds against Gram-negative bacilli were piperacillin/tazobactam(75.95%),cefepime(58.23%),imipenem(54.43%),ciprofloxacin(53.16%) and levofloxacin(48.10%).CONCLUSIONS The prevailing pathogens in cerebrospinal fluid are Gram-negative bacilli,especially Pseudomonas and E.coli.Data collected in present study provide valuable information for prophylactic and empirical antibiotic use in intracranial infection.
