1.Association research between metabolism syndrome and fatty liver of 5782 health examination cohort in Chongqing

Objective:To study the incidence rates of metabolism syndrome(MS)and fattyliver of 5782 health examination cohort in Chongqing to further investigate the interrelation of the two diseases.Methods:Subjects were divided into different groups according to age and gender,the incidence rates of MS and fatty liver of 5782 health examination cohort were analyzed and the incidence rate variances of MS and fatty liver between different age and gender groups were compared.Logistic regression analysis was used to analyze the dependability between fatty liver and age,gender,BMI,GLU,TC,TG,HDL-C and LDL.The incidence odds ratio(OR)of obesity,high blood pressure(HBP),high blood glucose and blood lipids disorder between fatty liver group and non-fatty liver group were compared.Results:Among the 5 782 participants,the incidence rates of MS and fatty liver were 9.58% and 25.9% respectively.The incidence rate of MS increased as age increases,and the incidence rates of MS and fatty liver in male were obviously higher than those in female(P
2.Clinical significance on early diagnosis of brain injury in premature infants with multiple sequence joint inspection of magnetic resonance imaging

Objective To discuss clinical significance on early diagnosis of brain injury in premature infants with multiple sequence joint inspection of magnetic resonance imaging (MRI).Methods The brain MRI findings of 160 premature infants treated by Neonatal Intensive Care Unit were analyzed retrospectively.Results In 160 premature infants,brain injury occurred in 76 cases,the incidence of brain injury was 47.5%.Ischemic lesions were seen more in brain injury in premature infants,cerebral white matter injury was the most common,especially periventricular leukomalacia.Ischemic brain injury performed patchy or large sheet increased signal intensity on T1-weighted images(T1 WI),decreased signal intensity on T2-weighted images (T2WI) and obviously increased signal intensity on diffusion weighted imaging (DWI) in half egg circle center and around the lateral ventricle.Periventricular leukomalacia performed patchy decreased signal intensity on T1WI,increased signal intensity on T2WI and decreased signal intensity on DWI.Periventricular-intraventricular hemorrhage was seen more in hemorrhagic lesions.Hemorrhage stove was performed different signal because of different bleeding time.MRI performance in acute phase was iso-signal or slightly decreased signal intensity on T1WI,increased signal intensity on T2WI,increased signal intensity on T1WI,slightly decreased signal intensity on T2WI in early subacute,increased signal intensity on T1 WI and T2WI in late subacute and obviously decreased signal intensity on magnetic sensitive weighted imaging.The detection rate of ischemic lesions by DWI was higher than the conventional MRI,and DWI could show cerebral white matter damage of premature infants much earlier than the conventional MRI.The detection rate of hemorrhage stove by susceptibility weighted imagingc (SWI) was higher than the conventional MRI (x2 =23.78,P < 0.05),and SWI could show hemorrhagic lesions much earlier than conventional MRI (x2 =27.02,P < 0.05).Conclusions MRI,especially combined multiple sequence checking,could provide accurate imaging evidence for the early diagnosis of brain injury in premature infants.
3.Association of serum proinsulin with insulin resistance,and serum proamylin with islet beta cell function

Objective To study the serum levels of proinsulin(PI),insulin(INS),proamylin(PA) and amylin(AMY) in abnormal glucose metabolism individuals and to explore the relationship between PI and insulin resistance(IR) and between PA and islet beta cell function.Methods Totally 79 subjects who received Oral glucose tolerance test(OGTT) and insulin release test in our department from March to May 2008 were divided into 4 groups according to the results of OGTT,that is,normal group,impaired glucose regulation(IGR) group,T2DM 1[fasting blood-glucose(FBG)15 mmol/L) group.All subjects underwent examination of anthropometry and serum bio-chemical indicators,HOMA-IR and HOMA-B were calculated.Results No significant difference was found in the serum levels of PI or AMY among the 4 groups,but the level of PA,PI/INS and PA/AMY among the 4 groups did have significant differences(P
4.Clinical analysis of rotavirus enteritis in children in Yueqing city

Objeetive To explore the situation of rotavirus infection and extraintestinal organe damage in children in Yueqing city.Methods Two hundred and eighty-seven cases with acute rotavirus gastroenteritis in our hospital were analyzed for prospective study from October 2011 to January 2013 by stool tests.Results Rotavirus infection was found to be more common in autumn and winter.There were 223 cases (17.7%) got extraintestinal organe damage,175 cases (60.80%) got myocardial lesion,and 78 cases (27.18%) got respiratory infection.At the same time,there were 51 cases (17.77%) and 21 cases (7.31%) got liver function lesion and convulsion respectively.Among the metabolic acidosis(48 cases),39 cases were combined with myocardial lesion.While non metabolic acidosis were 239 cases (81.25%),and 136 cases were combined with myocardial lesion.Therefore,the myocardial lesion was significant correlation with metabolic acidosis (P < 0.01).Iron deficiency anemia was 123 cases and combined with 15 cases (12.19%) convulsion,while the convulsion prevalence rates of non iron deficiency anemia was 6 cases (3.65 %).There was a statistically significant difference between the convulsion prevalence rates of iron deficiency anemia and that of non iron deficiency anemia (P < 0.01).Conclusion Rotavirus diarrhea can lead to extraintestinal organe damage,and the clinical doctors should pay attention to them.
5.The Clone, Expression and Site-directed Mutagenesis of 3-Ketosteroid-Delta (1)-Dehydrogenase from Arthrobacter Simplex

Objective:In this study,a prokaryotic expression of the 3-ketosteroid-Delta (1)-dehydrogenase (KSDD) which came from Arthrobacter simplex was built.Moreover,in order to investigate the catalytic mechanism of KSDD and improve its stability,the structure of KSDD was predicted by computer and the critical sites were confirmed by site-directed mutations.Methods:The recombinant plasmid was constructed by eukaryotic expression vector pET-22b and the recombinant strain was constructed and expressed in Escherichia coli BL21 (DE3).High-performance liquid chromatography was used to determine the transformation rate of 4-AD to ADD.The KSDD structure and key sites were predicted by SWISS-MODEL.Site-directed mutations for the amino acid residues of key sites were constructed and activities of the mutations were detected.Results:The recombinant strain E.coli pET-22-ksdd was successfully constructed.It was induced to express the dehydrogenase by IPTG and the conversion rate of 4-AD to ADD was 27% at 21 ℃.The structure of 3-ketosteroid-Delta (1)-dehydrogenase and the four key sites was analyzed by SWISS-MODEL.Four mutants,Y120R,Y320L,Y488F and G492Y were constructed.Mutants Y120R and Y488F were inactivated,so they were proved to be the key active sites of KSDD.The conversion rate of mutant Y320L was consistent with that of wild type,but the stability at 37 ℃ was improved.The conversion rate of mutant G492Y was 1.2 times of the wild type and the stability has been improved at 37 ℃.Conclusions:At present,there are few studies about the structure and catalytic mechanism of dehydrogenase.The active sites of the enzyme were verified by this study,which laid the foundation for the further study of the properties of the enzyme KSDD.
6.Prevalence of nontuberculous mycobacteria isolated from pulmonary specimens in Wenzhou during 2014 to 2016

Objective To investigate the isolation rate, distribution and trend of nontuberculous mycobacteria (NTM) in Wenzhou during 2014 to 2016.Methods Sputum or alveolar lavage specimens of patients with suspected pulmonary tuberculosis were collected for mycobacteria culture from January 2014 to December 2016.Mycobacterium culture positive strains were further identified by gene chip, 16S rRNA and hsp65 gene sequencing.SPSS 19.0 software was used to analyze the data.Results After excluding repetitive strains, 3 295 mycobacteria strains (MTB) were isolated from respiratory specimens, included 3 032 mycobacterium tuberculosis complex strains, 238 NTM strains, 20 Gordon genera strains, 3 Nocardia genera strains and 2 Tsukamurella genera strains.The proportion of NTM among confirmed mycobacteria was 8.5% (86/1 006), 6.7% (72/1 079) and 6.8% (80/1 185) in 2014, 2015 and 2016, respectively (x2 =2.459,P > 0.05).The overall prevalence of NTM was 7.3 % (238/3 270).There were 15 species of NTM, and the most common NTM strain was Mycobacterium intracellulare (52.5%,125/238), followed by Mycobacterium abscessus (22.7%, 54/238) and Mycobacterium avium (10.1%, 24/238), other species were only accounted for 14.7% (35/238).The ranking of Mycobacterium avium went up rapidly from the fifth in 2014 to the second in 2016 (x2 =18.259, P < 0.01), while proportion of Mycobacterium abscess, dropped from 34.9% (30/86) in 2014 to 17.5% (14/80) in 2016 (x2 =7.335, P<0.01).Among patients from whom the NTM strains were isolated, 56.7% (135/238) were male and most of them were aged 45 years or above (79.8%, 190/238).Conclusions In the past three years, the trend of NTM isolation rate in Wenzhou is steady.The most prevalent NTM species is Mycobacterium intracellulare, followed by Mycobacterium abscessus and Mycobacterium avium.Mycobacterium avium shows a continuously upward trend, while the separation of Mycobacterium abscessus shows a downward trend.
7.The correlation between the timed up and go test and fall risks in elderly frail patients

Objective:To evaluate the correlation between the timed up and go(TUG)test and fall risks in elderly frail patients.Methods:From July to September 2019, elderly frail patients who were treated at the cardiovascular department of our hospital were enrolled.Basic clinical data and fall-related information of patients were collected.Patients were divided into the fall group and the non-fall group.Results on the body mass index(BMI), TUG, 4-meter maximum walking speed(4 m MWS)and Barthel index were compared between the two groups.The correlation between TUG and each indicator was examined.Multivariate Logistic regression analysis was used to analyze the correlation between the TUG and falls in elderly patients.Results:A total of 96 eligible patients were enrolled, including 35 in the fall group and 61 in the non-fall group.The average TUG time was longer in the fall group than in the non-fall group(16.45±6.44 s vs.10.17±2.91 s, t=-6.556, P<0.001). The correlation analysis results showed that the TUG was correlated with falls and 4 m MWS( r=0.582 and 0.875, both P<0.001). Multivariate Logistic regression analysis showed that the TUG( OR=1.201, 95% CI: 1.111-1.470, P=0.004)and 4 m MWS( OR=1.146, 95% CI: 1.063-1.244, P=0.015)were risk factors for falls. Conclusions:The TUG is correlated with fall risks in elderly frail patients and should be recommended as a routine test in clinical practice.
8.Triptolide induces apoptosis of human acute T lymphocytic leukemia Jurkat cells via inhibiting transcription of human endogenous retrovirus HERV-K Np9 gene.

OBJECTIVETo investigate the molecular?mechanisms by which triptolide induces apoptosis of human acute T lymphocytic leukemia Jurkat cells.
METHODSMTT assay was employed to detect the proliferation inhibition of Jurkat cells by triptolide, and the IC50 was calculated by OriginPro8. Flow cytometry was used to analyze apoptosis of Jurkat cells. Np9 mRNA levels were detected by RT-PCR and analyzed quantitatively by Kodak 1D 3.6 software. Correlation between the inhibition of Np9 transcription and the cell apoptosis was analyzed by SPSS 19.0.Western blotting was employed to determine Np9 downstream signaling molecules c-myc, β-catenin, ERK, AKT and Notch1 protein level in Jurkat cells after exposure to different concentrations of triptolide for 48 h.
RESULTSTriptolide treatment resulted in dose-dependent inhibition of Jurkat cells proliferation and its IC50 was 12.7 nmol/L. Triptolide induced apoptosis of Jurkat cells in dose- dependent manner. Furthermore, triptolide inhibited Np9 mRNA transcription level in Jurakt cells in a dose-dependent manner. There was a correlation between the triptolide-mediated the apoptosis and the inhibition of Np9 transcription of Jurkat cells (R(2)=0.907). Western blotting results displayed that triptolide inhibited transcription levels of Np9 mRNA with a concomitant decrease of its downstream signaling molecules c-myc, β-catenin, ERK, AKT and Notch1 at protein levels.
CONCLUSIONInhibition of HERV-K Np9 mRNA and its downstream signaling molecules c-myc, β-catenin, ERK, Akt and Notch1 protein might be one of important molecular?mechanisms by which triptolide induces apoptosis of human acute T lymphocytic leukemia Jurkat cells.

9.Triterpene compounds from Cirsium setosum.

To investigate chemical constituents contained in cytotoxic petroleum ether extractive fractions from ethanol extracts of Cirsium setosum. The constituents were separated and purified by a combination of various chromatographic methods including silica gel, Sephadex LH-20, and preparative HPLC. Structures of the isolates were elucidated by spectroscopic methods including 1D, 2D NMR and MS methods. The compound structures were also determined by reference to literature. Twelve compounds were separated from the petroleum ether fraction of ethanolic extract and elucidated as lupenyl acetate (1), lupeol (2), lupenone (3), beta-amyrin (4), psi-taraxasterol (5), psi-taraxasteryl acetate (6), taraxasteryl acetate (7), marsformoxide B (8), alpha-amyrenone (9), beta-amyrenone (10), taraxasterone (11) and psi-taraxasterone (12). Of them, compounds 3, 5, 7-12 were separated from this genus for the first time.

10.Molecular epidemiological analysis of the Acinetobacter baumannii isolates from blood and sputum specimens of patients with ventilator-associated pneumonia

Objective To investigate the molecular epidemiological characteristics of the Acinetobacter baumannii strains isolated from blood and sputum samples of patients with ventilator-associated pneumonia (VAP) in ICU.Methods The patients were analyzed in two groups:Group A,A.baumannii was isolated from both blood and sputum,and Group B,A.baumannii was isolated only from sputum.Clinical data of the patients were collected,including the results of antimicrobial susceptibility test.Pulsed-field gel electrophoresis (PFGE) and multilocus sequence typing (MLST) were performed for the strains.Results During the study period from June 2015 to December 2105,28 nonduplicate A.baumannii strains were collected from 14 patients in group A and 28 nonduplicate strains from 28 patients in group B.The 56 A.baumannii strains were multidrug-resistant (MDR).More than 80% of the strains were resistant to carbapenem,third-generation cephalosporins or aminoglycoside,but highly sensitive to tigecycline.No significant difference was found for the resistance rates between group A and group B.The nonduplicate A.baumannii isolates from blood and sputum samples of the same patient in group A were all homologous strains confirmed by PFGE.Six pulsotypes were identified from the 28 strains in Group A and 9 pulsotypes in Group B.Five pulsotypes were shared between the two groups.MLST analysis showed that there were 9 ST types (ST195,ST208,ST229,ST369,ST373,ST457,ST836 and two new phenotypes ST N2,ST N5) in Group A and 8 ST types (ST195,ST208,ST381 and 5 new phenotypes ST N1,ST N2,ST N3,ST N4,ST N5) strains in group B.There was no significant difference in the proportion of the main ST types between the two groups.eBURST analysis indicated that ST195,ST208,ST457,ST369,ST N1,ST N2,ST N51 belonged to CC92 prevalent strain.Conclusions The antimicrobial susceptibility profile and genotype of A.baumannii isolates from blood and sputum samples are similar.There was CC92 prevalent strain in the ward.There is no direct relation between the risk factors for bloodstream infection in VAP patients and the genotype of A.baumannii strain.It is particularly important to reinforce infection control for prevention and treatment ofA.baumannii bloodstream infections.
