1.Qualitative research on risk factors of lymphedema for patients with breast cancer

Objective To deeply understand the risk factors of lymphedema for patients with breast cancer after surgery. Methods The phenomenological method was applied in this study. Semi-structured interview was used to collected data from 9 female breast cancer patients with lymphedema after surgery in our hospital from June to September 2016 for generic analysis. Results The risk factors of lymphedema could be categorized into four themes:(1)choice of treatment is the primary cause:axillary lymph node dissection; radiotherapy; chemotherapy; (2)not paying enough attention to lymphedema:lacking the knowledge of lymphedema; imbalance of physical activities for the affected limb; lacking awareness of exercise and protection of the affected limb. Conclusions Axillary lymph node dissection after radical surgery for patients with breast cancer is the primary cause of lymphedema, and paying not enough attention is an important factor, especially lacking the consciousness of prevention, so the nurses should emphasize education about prevention of lymphedema after surgery for patients, to improve the consciousness of them to reduce the occurrence of lymphedema and its influence on their quality of life.
2.MRI Study of Strain Feature of Left Ventricular Long-axis Myocardium in Normal Young Adults

Purpose To measure the longitudinal and transverse strain and displacement parameters of each segment of long axis myocardium on cardiac MRI film images of normal young volunteers using feature tracking technique (FT), and to analyze the regularity of left ventricular long axis strain and establish peak and time to peak (TTP) reference range concerning the longitudinal and transverse strain and displacement. Materials and Methods Cardiac film images of 29 healthy volunteers aged 24 to 34 were collected using Philips Multiva 1.5T MRI scanner, including three typical long axis planes of the left ventricular long axis, left ventricular outflow tract, and four-chamber view. The MRI film images were analyzed drawing on myocardial feature tracking software TOMTEC, and the peak value and TTP of longitudinal and transverse strain and displacement of the 16 AHA segments excluding apex cordis were obtained thereby. The regularity of each parameter concerning the plane, layer, and segment was summarized and analyzed. Results The TTP of longitudinal strain, longitudinal displacement and transverse strain were consistent among each myocardial segment, which was a constant indicator in (41.24±12.51) % RR interphase. The peak value of longitudinal displacement was represented by apex cordis (2.56±0.89) mm < middle (6.09±2.03) mm < cardiac base (8.07±2.05) mm, and the peak value of transverse displacement was manifested as apex cordis (4.57±1.18) mm < middle (5.65±0.84) mm < cardiac base (8.48 ±1.18) mm, which was consistent with cardiac geometric deformation visually observed. The longitudinal strain showed consistency on 2CH-3CH-4CH plane, whose peak reference value was (-22.46±13.19)%; while the peak value of transverse strain displayed no consistency on different planes. Conclusion The longitudinal strain of the left ventricular on different planes has consistent peak value and TTP, suggesting good reference value.
3.MRI measurement of left ventricle myocardial strain on short axis in normal youth

Objective To learn the endocardial strain patterns of the left ventricles on the short-axis views in normal youth volunteers using feature tracking techniques on cine cardiac MR imaging,and to build up the reference ranges of strain peak and time to peak (TTP) for endocardial strain analysis.Methods Twenty-nine normal youth volunteers aged from 24-34 years old were enrolled in the study in march 2015.The Cine MR imaging were acquired using balance FFE sequence with 24 phases for each cardiac cycle.The standard short-axis imaging planes were basal,mid-cavity and apical levels and divided into 16 segments.The peaks and TTPs of myocardial displacement and its velocity,radial and circumferential strain and strain rate and its velocity in each segment were calculated using feature tracking techniques.The data with normal distribution were analyzed by ANOVA and Student-Newman-Keuls (SNK) method,and Kruskal-Wallis and Wilcoxon rank sum test were used for the data without normal distribution.Results The peaks and TTPs of radial strain and strain rate differed in 16-segmental levels and basal,middle and apical plane levels and had no concordant reference ranges.The circumferential strain and strain rate were homogeneous at basal and middle plane levels [(-27.09 ±9.51)%,(1.94 ±0.98)/s,respectively].Their TTPs were homogenous [0.38 (0.08)RR,0.54 (0.08)RR,respectively],and there were no significant differences in planar-segments or inter-segment level.Except for middle level,the peaks of radial displacement and velocity differed in basal and apical levels and had no concordant reference ranges.But radial velocity TTPs were homogeneous in all 16 segments [0.21 (0.04)RR].Except for the radial strain rate,TTPs showed no significant difference between basal level and middle level.The measurements at apical level revealed high variance.No regular pattern could be found on individual peak and TTP curves of velocity and strain rate.Conclusion The peaks and TTPs of radial strain are different in segmental and planar levels without a concordant reference range.The peaks and TFPs of circumferential strain and the radial velocity rTTPs are homogeneous in middle and apical plane level and have homogeneous reference range.
4.Construction and evaluation of clinical predictive model of stigma in patients with lymphoma

Objective:To establish and evaluate the clinical prediction model of stigma in patients with lymphoma so as to provide a basis for the assessment and response of stigma in patients with lymphoma.Methods:Convenience sampling method was used to select 130 patients with lymphoma who were hospitalized in Jiangsu Cancer Hospital from June to December 2019. We collected patients' clinical data, and assessed patients' quality of life and stigma. The R software was used to analyze and process the data, and screened characteristic factors were predicted by combining with the LASSO regression model. Multivariate Logistic regression analysis was used to analyze the influencing factors of stigma in patients with lymphoma. We constructed a nomogram model to evaluate the identification, calibration and clinical applicability of the predictive model through C index, calibration chart and decision curve analysis.Results:A total of 120 patients' valid data were gathered. Patients with lymphoma had a high level of stigma. The predictive factors of the predictive model included whether to be hospitalized for the first time, age, patient's medical insurance type, scores of the thirtieth and twenty-seventh question of the quality of life questionnaire. The model showed good predictive ability. The C index of training group, overall sample and validation group were 0.824, 0.776 and 0.684 respectively, and the C index performed well.Conclusions:The nomogram model developed in this research can help clinical nurses early identify lymphoma patients with a high level stigma, and give targeted response plans, which can greatly improve the quality of life and prognosis of patients, and is worthy of clinical application.
5.Correlation between perceived professional benefits and innovation behavior of oncology nurses

Objective To explore and understand the influence of perceived professional benefits on oncology nurses′innovative behaviors. Methods Totally 943oncology nurses were selected to join thestudy, Using the general information questionnaire, nurses′ perceived professional benefits scale and innovative behaviors scale for investigation. Results The average score of oncology nurses′ perceived Professional Benefits Scale was (114.08 ± 16.45) points, the average score of oncology nurses′ innovative behaviors was (31.00 ± 7.00) points. The score of perceived professional benefits of oncology nurses was positively correlated with score of innovative behavior of nurses (r=0.472, P<0.01). Nurses position, level and relatives and friends identity,their own growth and team belonging in perceived professional benefits were the influencing factors of innovative behavior of oncology nurses (t=2.022-4.486, P<0.05 or 0.01). Conclusion The perceived professional benefits of oncology nurses can play a positive role in promoting their innovative behaviors. Nursing managers should pay attention to improving perceived professional benefits of clinical nurses, promote their innovative behaviors and improve the quality of nursing services.
6.Development of the Nutrition Impact Symptom Scale for Colorectal Cancer Patients and its reliability and validity

Objective:To develop the Nutrition Impact Symptom Scale for Colorectal Cancer Patients.Methods:Guided by the theory of unpleasant symptoms, the initial draft of the scale was formed through literature research, expert consultation, and small sample pre-survey. From March to May 2023, convenience sampling was used to select 127 colorectal cancer patients who visited Jiangsu Cancer Hospital as the research subject for item analysis and reliability and validity testing of the scale.Results:The final scale consisted of five dimensions and a total of 17 items. The content validity index of the scale was from 0.83 to 1.00, with an average content validity index of 0.97. Exploratory factor analysis extracted five common factors, and the cumulative variance contribution rate was 61.622%. The total Cronbach's α coefficient of the scale was 0.708, and the coefficients for each dimension were 0.762, 0.642, 0.625, 0.510, and 0.644, respectively. The half reliability coefficient of the scale was 0.824.Conclusions:The development process of the Nutrition Impact Symptom Scale for Colorectal Cancer Patients is scientific, with good reliability and validity, and can be used to evaluate the nutrition impact symptom of colorectal cancer patients.
7.A qualitative study on self-management experience of adverse reactions of oral chemotherapy drugs at home in patients with gastrointestinal tumors

Objective:To explore the inner experience of self-management of adverse drug reactions in patients with oral chemotherapy at home, so as to provide a basis for medical staff to better implement home supervision of patients.Methods:This study was a qualitative study. The purposive sampling method was used to select 12 patients with gastrointestinal tumors who were admitted to Department of Oncology of a ClassⅢ Grade A hospital in Nanjing from April to May 2021 and were required to take oral chemotherapy drugs at home for semi-structured in-depth interviews. Colaizzi's 7-step analysis method was used to analyze and summarize the interview data.Results:A total of 3 themes and 7 sub-themes were extracted from the inner experience of self-management of adverse drug reactions in patients with oral chemotherapy at home. Theme one was the perception of self-management of adverse drug reactions in patients undergoing oral chemotherapy at home. This included anxiety and fear in the face of adverse reactions and ambivalence in seeking to communicate with medical staff. The topic two was the attitude and behavior of self-management of adverse drug reactions in patients with home-based oral chemotherapy drugs. This included insufficient attention to active review of adverse effects of myelosuppression and a lack of ability to identify adverse drug reactions. The topic three was the motivation and needs of self-management of adverse drug reactions in patients with home-based oral chemotherapy. The family's careful care provided strong support, the convenience of mobile communication relieved the patient's troubles and there was a need for information and knowledge of the adverse reactions of oral chemotherapy drugs.Conclusions:Medical staff need to strengthen drug knowledge education and psychological care for gastrointestinal tumor patients taking oral chemotherapy drugs at home and build a mobile information platform to improve their self-management capabilities.
8.Application of electrocardiograph positioning technology training based on PDCA cycle model in improving PICC tip positioning rate

Objective To investigate the effect of PDCA cycle management combing with ECG guidance in improving the rate of catheter tip in right place when is conducting the PICC catheterization. Methods A total of 36 intravenous treatment specialist nurses from Jiangsu Tumor Hospital were trained with PDCA cycle management method from 1st June to 31st September,2016. The training knowledge and skills included catheter tip positioning under intracardiac electrocardiogram. The knowledge and skills were analyzed and compared before and after training finally. Results The training not only could help to improve the score of knowledge and skill of PICC specialist nurses with knowledge before (88.28±3.10) and after (96.10±1.25) (P< 0.05) and skill before (83.55±5.11) and after (98.81±2.50) (P< 0.05); but also increased the rate of catheter tip in right place in the process of PICC catheterization (χ2=12.35,P <0.05). Meanwhile, the results showed that patient satisfaction with nurses could also be improved from 88.7% to 96.8% (χ2=10.79,P <0.05) and the operating time could be shorted [before (40.23±3.56)min,after (28.76±4.34)min](P< 0.05). Conclusions The use of PDCA circulatory method to train nurses can improve the ability to deal with puzzling questions of nurses in the process of PICC catheterization,and increase the rate of PICC catheter tip in right place.
9.Research progress of mobile health in exercise management of cancer patients

10.Meta-integration of medication experience and needs of oral chemotherapy drugs for cancer patients at home

Objective:To systematically evaluate the medication experience and needs of patients with oral anticancer drugs at home, providing reference for the formulation and implementation of home supervision measures for these patients.Methods:PubMed, Embase, Web of Science, The Cochrane Library, CINAHL, CNKI, Wanfang database, VIP database and Chinese biomedical literature database were searched by computer to collect qualitative studies on medication experience and needs of patients with oral anticancer drugs at home. The retrieval period was from the database construction to May 1, 2021. The quality of the literature was evaluated by the Australian JBI qualitative research quality evaluation criteria for evidence-based health care centres, and the results were integrated by the pooled integration method of Meta-integration.Results:A total of 11 studies were included, 52 results were extracted, 13 categories were summarized and 4 integrated results were obtained: heavy negative psychological experience; undergo physical trials; a desire for outside support; adjust to the disease.Conclusions:According to the four aspects of experience and needs of patients with oral anticancer drugs at home, medical staff should guide them to establish health belief, improve disease perception and coping ability, and take measures to meet patients′ needs for knowledge, communication and social support through multiple channels.
