1.Design and Application of Multidisciplinary Consultation System Based on the Clinical Data Center

The paper introduces the background of establishing the multidisciplinary consultation system,designs the architecture of the multidisciplinary consultation system including the links of consultation application,prompt,acceptance,recording,evaluation,etc.,and collects all diagnosis and treatment information of patients based on the clinical data center.The application of the multidisciplinary consultation system is able to optimize the process,improve the efficiency and quality,and achieve closed-loop management.
2.Characteristics and diagnosis of spinal hydatid disease

Objective To study the clinical characteristics of spinal hydatid disease and help to diagnozy and treat the disease. Methods 14 cases with spinal hydatid disease, admitted in our hospital between July 1994 and October 2003, were studied retrospectively. Besides routine examinations, plain X-ray films were performed in all 14 cases, CT in 8 cases and MRI in 13 cases, respectively. 8 cases sufferred serodiagnostic tests for human hydatidosis. All cases underwent operation and the histopathological examination. There were 5 males and 9 females with an average age of 25.8 years(range, 5-53 years) in this series. All 14 cases had the living history in endemic area and the average duration of symptom was 26.7 months(range, 4-120 months). The lesions most frequently involved the thoracic segments, accounting for 35.8%, and then 21.4% in the lumbar, and 28.6% in the lumbosacral segments, cervical segments were the least commonly involved. According to Dew'classification, spinal hydatid cysts located primarily at intradural extramedullary area in 1 case, extradural intraspinal in 2 cases, vertebral in 7 cases, paravertebral in 1 case and combination of various extradural tissues involved in other 3 cases. Results The clinical manifestations were localized back pain or radiating pain, mass and gradually progressive neurological dysfunction. Eight of 14 cases displayed abnormality in X-ray films which showed local, well-defined and irregular erosions with or without surrounding sclerosis or calcifications. CT scans presented multiloculated low density lesions. The MRI characteristics of the lesions were multiloculated cysts, signal intensities of the cyst content were similar to that of CSF. On T1W images the cyst wall appeared iso- or slightly more hypo-intense than the cyst content, T2W images demonstrated a low intensity rim and the hyper-intense cyst content resembling a bunch of grapes. Seven of 8 cases performed the 3-tests serodiagnosis were positive, and 3 cases with positive 3-tests serodiagnosis accepted 8-tests were positive. All the specimens were shown to be Echinococcus Granulosus. Conclusion MRI was the most sensitive diagnostic method and the best method to determinate the site and extent of the cystic lesions. CT and serological examination are useful for diagnosis. The key of avoiding misdiagnosis is to enhance understanding of the disease.
3.Lateral Lung Volume Reduction with Small Incision and Ligation for COPD:Report of 36 Cases

Objective To investigate the efficacy of lateral lung volume reduction with small incision and ligation for the treatment of serious chronic obstructive pulmonary disease(COPD).Methods From April 1994 to May 2006,36 patients suffering from serious COPD complicated with severe emphysema underwent lateral lung volume reduction in our hospital.Small incision and ligation were used during the operation.Results After the surgery,6 patients developed pulmonary air-leak,and were cured after 180-minute to 5-day treatments(mean,1.6 days).No perioperative death in this series.One year after the operation,the patient had significantly higher FEV1 and PaO2,and lower TLC and RV than those determined preoperation [(839.9?76.0)ml vs(589.0 ?80.5)ml,t=18.451;(72?10)mm Hg vs(62?8)mm Hg,t=4.686;(5167.6?639.9)ml vs(6725.8?976.6)ml,t=8.116;(3062.4?722.0)ml vs(4329.6?669.9)ml,t=7.720;all P=0.000].In addition,they had significantly longer 6MWT than that before the operation [(306.0?24.8)m vs(223.8?36.7)m,t=11.135,P=0.000].Conclusion For the patients with COPD who had indications for lateral lung volume deduction,resection of emphysemic tissues by the procedure using small incision and ligation can improve their symptoms and physiological conditions.
4.Diagnosis and treatment of gas gangrene in 8 transferred patients after China Wenchuan earthquake

Objective To analyze clinical manifestation,diagnosis and treatment of 8 patients with gas gangrene in Wenchuan earthquake.Methods Eight patients with gas gangrene in Wenchuan earthquake were selected for analysis on their clinical manifestations.We finally diagnosed gas gangrene based on patients'clinical manifestation and the cultured ciostridium in the focus of infection.All the patients were treated with antibiotics,repeated surgical debridement,hyperbaric oxygen and nutritional support.Results Most patients had no erepitation of the extremities but found gas in interstice of tissues identified by X-ray,B-ultrasound and CT scanning.Film preparation of efflusion of all patients showed clostridium and bacterial culture manifested mainly perfringens.Most patients were demonstrated to be with polyinfection.In addition,all patients were characterized by pitchy necrotic tissues,effusion,systemic decrease of red blood cells,increase of white blood cells and platelet.No death or amputation occurred under active combined therapy.Conciusions Earthquake induces mainly open trauma often combined with mixed infection,when gas gangrene is one of main special infections.The predominant combined therapies include utility of antibiotics and thorough surgical debridement,while hyperbaric oxygen and nutritional support are important and effective adjunctive therapies.
5.Minimally invasive screw and locking plate fixation for ankle fracture based on three-dimensional print model

BACKGROUND:Clinical physicians should pay attention on how to reduce ankle fracture healing wounds, reduce bleeding, improve operating technique, and improve repair effect, especialy for comminuted fracture. It is necessary to understand how to develop a detailed plan before repair, observe specific circumstances of the fracture from multiple angles, and establish three-dimensional images using two-dimensional X-ray films or CT scan. These raise a chalenge for each physician. OBJECTIVE:To explore the application effect of three-dimensional print, minimaly invasive percutaneous screw and locking plate fixation in the repair of ankle fractures. METHODS: A total of 26 cases of fracture of ankle joint were treated from January 2012 to June 2014. There were 9 cases of double ankle fracture, 12 cases of three ankle fractures, and 5 cases of three ankle fractures involved PILON fracture on the lower tibia. They received spiral CT scanning on the ankle joint, and stored in the format of di.com. After processing with minics software, data were converted into STL format, and then input into the three-dimensional printer. Thus, models of ankle joint were made at the ratio of 1:1. The repair scheme was made. After discharge, X-ray films were checked every month to observe the healing condition of fractures. According to Mazur ankle score standard, ankle joint function was assessed. RESULTS AND CONCLUSION: Al 26 cases were folowed up for 6-12 months. The fractures were healed. The healing time was 4-6 months, averagely 5.5 months. The healing was good, without vascular and nerve damage, or plate breakage. In accordance with the Mazur ankle score standard, there were excelent in 11 cases, good in 13 cases with an excelent and good rate of 92%. The operation time was (64±15) minutes, and intraoperative blood loss was (100±15) mL. The incision was healed in stage one. These findings confirm that the simulation training and operation in fracture models of three-dimensional print have smal injuries to patients with ankle fracture. The bleeding was less, the operation was simple, the healing time was short, and the functional recovery was rapid, especialy for comminuted fracture.
6.Analysis of factors influencing the disease-free survival after radical prostatectomy for prostate adenocarcinoma

Objective To analyze the factors that influence the disease-free survival time after radical prostatectomy for prostate adenocarcinoma and to study the expression and significance of Bcl-2,C-erbB-2,Ki-67 in prostate adenocarcinoma. Methods A total of 51 cases of prostate adenocarcinoma undergoing radical prostatectomy were reviewed.Cox proportional hazard model was used to investigate the impact and co-impacts of the factors,which included age, preoperative PSA level,Gleason scores of biopsies and operative specimens,EPE,MR,SVI,LN,PAT,PRT and having testes or not during follow-up.Immunohistochemical staining was used in biopsies and resected specimens to evaluate the positivity of Bcl-2,C-erbB-2,Ki-67 in 15 of the 51 cases. Results In stepwise Cox proportional hazards model, the prognostic factors for recurrence were age(P=0.011),maximal androgen blockade(P=0.017), positive margin of resection(P=0.000),postoperative Gleason score(P=0.002),having testes or not after operation(P=0.040).The expressions of 3 types of proteins showed negative correlation with the prognosis of prostate carcinoma. Conclusions Among clinicopathologic factors,positive resected margin is the most powerful prognostic factor.Patients will benefit from maximal androgen blockage before radical prostatectomy.Disease-free survival after radical prostatectomy is not influenced by not having pelvic lymphadenectomy after finding no enlarged lymph nodes intraoperatively.Not preserving testes after prostatectomy is a favorable factor.The prognosis of prostate carcinoma can be predicted with the expressions of Bcl-2,C-erbB-2 and Ki-67 in biopsies or resected specimens.
7.Effect ofTan-Re-QingInjection on Perioperative Period of TNF-α, IL-6, IL-8 and IL-10 in Lung Cancer Patients Complicated with COPD

This study was aimed to explore the possibility of reducing postoperative inflammatory response and lung injury degree by observing the effect ofTan-Re-Qing(TRQ) Injection on perioperative period of lung cancer patients complicated with chronic obstructive pulmonary disease (COPD). A total of 39 lung cancer cases complicated with COPD (phlegm-heat obstructing the lung) were randomly divided into the tested group of 18 cases and the control group of 21 cases. All patients underwent small incision surgery with video-assisted thoracoscopic surgery (VATS). The control group was treated with routine western medicine. TRQ injection was added in the tested group. On the preoperative 1D (T1), 1D after operation (T2), 3D after operation (T3), 7D after operation (T4), serum samples were collected for the determination of serum concentrations of TNF-α, IL-6, IL-8 and IL-10. At T1 and T2 time, there were no significant differences on levels of TNF-α, IL-6, IL-8 and IL-10 between two groups of patients (P > 0.05). At T3 time, levels of TNF-α, IL-6, IL-8 of the tested group were lower than that of the control group with statistical significance (P < 0.05). The level of IL-10 in the tested group was higher than that of the control group with statistical significance (P < 0.05). At T4 time, the levels of TNF-α, IL-6 and IL-8 of the tested group were lower than that of the control group with statistical significance (P < 0.05). The levels of TNF-α, IL-6 and IL-8 of both groups gradually declined at T3 and T4 time, compared with T2 time level with statistical significant (P < 0.05). It was concluded that TRQ injection reduced the releasing of inflammatory cytokines during perioperative period, increased levels of anti-inflammatory cytokine, thereby reducing the degree of inflammatory reaction and relieving lung injury for the protection of lung function of lung cancer patients complicated with COPD.
8.Effects of Different Dose Ambroxol Hydrochloride on Lung Protection of Patients in Perioperative Thoracoto-my

OBJECTIVE:To investigate the effect of different doses ambroxol hydrochloride on lung protection of patients in perioperative thoracotomy. METHODS:60 patients in perioperative thoracotomy were randomly divided into control group(30 cas-es)and observation group(30 cases). Control group received 30 mg Ambroxol hydrochloride injection from 1 week before surgery to 1 week after surgery,adding into 100 ml 0.9% Sodium chloride injection,intravenously,twice a day. Observation group re-ceived 300 mg Ambroxol hydrochloride injection from preoperative 1 week to postoperative 1 week,adding into 100 ml 0.9%Sodi-um chloride injection,intravenously,twice a day. All patients received the second-generation cephalosporin antimicrobial drugs for anti-infection after surgery. lung surfactant-associated protein A(SP-A),intercellular adhesion molecule(ICAM-1)levels preopera-tive 1 week and postoperative 1 week,and incidence of postoperative complications in 2 groups were observed. RESULTS:Preop-erative 1 week,there were no significant differences in the SP-A and ICAM-1 in 2 groups(P>0.05). Postoperative 1 week,SP-A and ICAM-1 in control group and ICAM-1 in observation group were significantly higher than preoperative 1 week,but SP-A and ICAM-1 in observation group were lower than control group,the differences were statistically significant(P<0.05),there were no significant differences in SP-A in preoperative 1 week and postoperative 1 week(P>0.05). The incidence of postoperative compli-cations in observation group was significantly lower than control group,the difference was statistically significant(P<0.05). CON-CLUSIONS:The effect of high-doses ambroxol hydrochloride on lung protection of patients in perioperative thoracotomy is signifi-cantly superior to small dose,which can reduce the incidence of lung complications.
9.Teeth segmentation from CBCT images using deformable triangle mesh model.

Considering the dental CBCT images' characteristics, the method of deformable surface of 3D triangle mesh model is proposed. The method uses a deformable model which is initialized from an icosahedron and evolves to fit the teeth's surface by the application of the locally adaptive external forces computed from the image data and internal forces coming from the model itself. The experimental results indicate that the proposed method is robust and accurate.

10.Prognosis and staging of non small ceil lung cancer that extends across the fissure into adjacent Iobe

Objective To determine the prognosis and staging non small cell lung cancer (NSCLC) that extends across the fissure into adjacent lobe after surgery.Methods 3752 patients with histopathologically confirmed non small cell lung cancer (NSCLC) received surgical reeessetion from January,1997 to April,2007.Among them,163 patients have a tumor invasion beyond fissure.After matching by pathologic TNM staging (7th),326 patients whose tumor defined in a single lobe were eligible for analysis.Results Histopatholngic staging of matched patients was I a:10 patiens(6.1% ),I b:79 patients (48.5%),Ⅱa:5 patients (3.1% ),111:44 patients (27.0%) and Ⅲa:25 patients( 15.3% ).5 years survival in patients with stage 1 tumors crossing the interlobar fissure was 51%,while in patients not cross the interlobar fissure was 63% ( P <0.05 ).There was no difference in survival for tumors stage Ⅱa and above with regard to importance of interlobar extension.The T2 tumor extending across a lung fissure had a reduction in survival compared with T2 tumor not cross the lung fissure and similar to the T3 tumor without the fissure invasion.Conclusion Our results suggest that TNM staging should be modified for tumor extends the fissure into adjacent lobe.
