1.Effect of Reduning Injection combined with spleen aminopeptide oral solution on children with hand-foot-and-mouth disease and its regulative effect on myocardial enzyme and inflammatory level

Objective To investigate the effect of Reduning Injection (RI) combined with spleen aminopeptide oral solution on children with hand-foot-and-mouth disease and its regulative effect on myocardial enzyme and inflammatory level.Methods Totally 184 cases treated in Children's Hospital of Kaifeng City from September,2014 to September,2016 were randomly divided into observation group and control group with each group of 92 cases.Control group was given conventional therapy,on the basis of which,observation group was given RI combined with spleen aminopeptide oral solution.Clinical effect and levels of inflammatory cytokine and myocardial enzyme were compared between the two groups.Results The total effective rate in observation group was 93.48%,which was significantly higher than that in control group (80.43%) (P < 0.05).After therapy,the levels ofhs-cTnT,CK-MB,CK,TNF-α,IL-6,CRP,CD3+,CD4+,CD8+,IgA,IgM,and IgG in observation group showed no statistical difference compared with healthy group.Index mentioned above of control group were significantly improved,however there were still statistical difference compared with healthy group (P < 0.05).Conclusion RI combined with spleen aminopeptide oral solution could effectively decrease the levels of inflammatory cytokine as well as myocardial enzyme,which deserves clinical expansion.
2.Continuous blood purification for patients with severe heart failure and renal failure

Objective To investigate the clinical effect of continuous blood purification therapy in patients with severe heart failure and renal failure.Methods The clinical data of 38 patients with severe heart failure and renal failure treated by continuous blood purification were analyzed retrospectively.Heart rate,mean blood pressure, APACHE II score,Boston score,blood biochemistry,blood gas analysis and cardiac function changes were compared before and after the treatment of continuous blood purification.Results After treatment 12h and 72h,patients breath-ing[(19.24 ±2.88)times/min],heart rate[(88.57 ±15.68)times/min],APACHE Ⅱ [(14.28 ±3.26)points] and the score of Boston[(6.27 ±1.25 )points]were significantly decreased compared with those before treatment [(35.68 ±5.97)time /min,(131.24 ±24.26)time /min,(26.34 ±5.96)points,(17.88 ±2.87)points],and the differences were statistically significant (t =3.13 3.45,2.12,4.11,3.67,5.68,3.44,609,all P <0.05).38 cases of severe heart failure with renal failure after continuous blood purification treatment,markedly effective in 16 cases, effective in 12 cases,ineffective in 10 cases,the total effective rate was 73.68%.Patients after continuous blood purification treatment,the SCr[(168.15 ±31.16)μmol/L],BUN [(13.13 ±3.44)mmol/L]were significantly decreased.pH[(7.41 ±0.13)],HCO3 [(25.57 ±5.11)mmol/L],PaO2 [(88.26 ±7.72)mmHg],SaO2 [(96.43 ± 3.14)%]and blood biochemistry and blood gas index decreased markedly,and the differences were statistically significant (t =4.55,3.21,2.11,3.45,673,4.21,all P <0.05).After the examination of echocardiography,stroke volume (SV)[(59.31 ±6.58)mL],cardiac output (CO)[(4.57 ±0.62)L/min]and left ventricular ejection fraction (LVEF)[(68.12 ±4.88)%]increased significantly compared with before treatment,and the differences were statistically significant (t =4.33,5.12,367,all P <0.05).Conclusion Continuous blood purification could effectively maintain the hemodynamic stability of patients,and is safe and effective in the treatment of severe heart failure with renal failure.
3.Value of heart-type fatty acid-binding protein in early diagnosis of acute myocardial infarction

Objective To explore the value of heart-type fatty acid-binding protein (H-FABP) in the early diagnosis of acute myocardial infarction (AMI). Methods Sixty confirmed AMI patients were observed, the data of H-FABP, cardiac troponin T (cTnT), creatine kinase MB (CK-MB) were detected in < 6 h and 6-12 h after the symptoms appeared, and the sensitivity of the three markers was calculated. The specificity was compared with 15 uncertain AMI patients and 45 healthy subjects. Results The early diagnosis sensitivity of H-FABP in < 6 h was 94% in AMI, which was higher than that of cTnT (50%) and CK-MB (56%) (P < 0.05 ). The diagnosis sensitivity of H-FABP, CTnT, CK-MB in 6-12 h was 100%, 92%, 92% respectively (P> 0.05 ). There was no significant difference in the specificity among the three markers (P > 0.05). Conclusions H-FABP has more sensitivity and specificity in the early diagnosis of AMI. It is applicable in the screening of patients who suffered chest pain and the diagnosis of early AMI.
5.The value of antinucleosome antibody in diagnosis of juvenile systemic lupus erythematosus and the association with disease activity

ObjectiveTo evaluate the frequency of antinucleosome antibody(AnuA) in juvenile systemic lupus erythematosus(JSLE),comparing it to that observed for anti-dsDNA,anti-Sm antibodies,and explore the correlation of these antibodies with clinical manifestations and disease activity.MethodsWe included 80 children with JSLE and 56 children with other rheumatic diseases into this study.Clinical records were reviewed.AnuA,antinuclear antibody (ANA) and anti-Sm antibody were detected by ELISA,ⅡF and Western-blot respectively.Anti-DNA were detected by ELISA and ⅡF.Disease activity was assessed by SLEDAI score.ResultsAnuA showed sensitivity of 76.25% and specificity of 98.21%.AnuA combine with anti-dsDNA antibody or anti-Sm antibody was detected,the sensitivities and specificities were 83.05%,86.44% and 96.43%,98.21%,respectively.It showed that the sensitivity was higher than any one of the three.The presence of AnuA was associated with red blood cell count,hemoglobin level,ESR,hematuria,low complement levels and anti-dsDNA antibody (r=-0.499,-0.503,0.388,0.227,0.303,0.531,P=0.000,0.000,0.000,0.042,0.006,0.000).The presence of AnuA was associated with the SLEDAI score(P=0.000). Conclusion AunA hasthe highest sensitivity and specificityamongthese autoabtibodies,particularly,when combined with anti-dsDNA antibody or anti-Sm antibody.The level of AnuA is associated with the disease activity of JSLE.AnuA is not only useful for the diagnosis of JSLE but also for evaluation of the disease activity.
6.The relationship of heart-type fatty acid binding protein and high-sensitivity C-reactive protein in patients with chronic heart failure

Objective To examine clinical significance and relativity of heart-type fatty acid binding protein (H-FABP) and high-sensitivity C-reactive protein (hs-CRP) in patients with chronic heart failure. Methods Ser-um concentrations of H-FABP and hs-CRP were measured in 60 patients with chronic heart failure and 30 control subjects. Left ventricular ejection fraction (LVEF) was examined by Doppler echocardio graphic in all subjects. Re-sults Serum concentrations of H-FABP and hs-CRP were higher in patients with chronic heart failure than in con-trol subjects[(6.11±1.49)μg/L vs (4.24±1.40)μg/L,and (12.77±3.65)mg/L vs(4.85±1.35) mg/L,t=5.746 and 7.543,P<0.01] but LVEF was lower in patients with chronic heart failure than in control subjects [(42.13±6.55) % vs (61.50±3.89) %,t=-14.902,P<0.01]. In CHF subgroups,H-FABP and hs-CRP lev-el increased with advancing NYHA class (F=26.288 and 351.784,P<0.01) but LVEF decreased (F=252.834,P<0.01). The serum H-FABP concentrations had a positive correlation with serum hs-CRP concentrations (r=0.801,P<0.01),and a negative correlation with LVEF (r=-0.718,P<0.01) ;serum hs-CRP concentrations had a negative correlation with LVEF(r=-0.881,P<0.01). Conclusion Serum H-FABP and hs-CRP levels are in-creased with the worsening of CHF. H-FABP and hs-CRP level are pnsitiviely related. The quantitative determination of serum concentrations of H-FABP and hs-CRP is valuable for risk stratification in patients with chronic heart fail-ure.
7.cDNA Cloning, Expression Analysis of Slc24a5 and Its Relationship With Melanin Deposition in Chicken

Melanin is a kind of biopolymer, composed of a complex quinine/indole-quinone derived mixture which is produced in melanocytes from tyrosine. More than 100 genes related to melanin deposition have been found in mouse. Slc24a5 (solute carrier family 24, member 5) is a novel gene first cloned in zebrafish. And it has been confirmed that the Slc24a5 is involved in controlling the melanin deposition in zebrafish. Here the full length of the chicken slc24a5 mRNA sequence, its expression profile and a discussion of its relationship to melanin deposition in chicken were reported. Chicken Slc254a5 has a CDS of 1 269bp, predicting a protein of 423 amino acids which is about 80 amino acids shorter than in human and mouse. It has 9 exons and 8 introns and spans more than 1lkb of genome sequence. The quantitative real-time PCR confirmed that the chicken Slc24a5 was highly expressed in the eye of White Leghorn and Beijing Fatty Chickens, and in the eye, skin and muscle of Silky. The relative expression in Silky eye is more than two times that of White Leghorn. The relative expression in Silky skin is about 70 times that of White Leghorn and about 15 times that of Beijing Fatty Chickens. The relative expression in Silky muscle is about 15 times that of White Leghorn and about 3 times that of Beijing Fatty Chickens. And the expression result is in accordance with the melanocyte and melanin deposition in these tissues.
8.Modulation of Baroreflex Activity by Exogenous and Endogenous NO in SD Rats

Objective To assess the modulation of baroreflex activity by exogenous and endogenous nitric oxide (NO) in SD rats. Method The normal baroreflex sensitivity of SD rat was evaluated by intravenous (I.v.) administration of Phenylephrine (PE, 30 μg·kg-1) to elevate blood pressure. Exogenous and endogenous nitric oxide (NO) were respectively produced by I.v. Infusion of sodium nitroprusside (SNP, 10.2 μg·kg-1) and L-arginine (L-Arg, 25 mg·kg-1). And NG-nitro-L-arginine methylester (L-NAME, 10 mg·kg-1) was infused to inhibit the generation of NO. Meanwhile, baroreflex sensitivity fluctuations caused by PE were estimated. Result Compared with the normal state, L-Arg significantly reduced the baroreflex sensitivity (Gmax) and the slope coefficient τ of the blood pressure-heart rate curve [Gmax=-1.469±0.120 beats/(min·mmHg) vs. -0.664±0.080 beats/(min·mmHg), P<0.01; τ=0.0860.005 vs. 0.0490.007, P<0.05]. SNP also led to attenuation of Gmax and τ [Gmax=-1.277±0.392 beats/(min·mmHg) vs. -0.943±0.297 beats/(min·mmHg); τ=0.074豹0.027 vs. 0.071±0.023] but the changes were not significant. In contrast to L-Arg and SNP, L-NAME induced raise of Gmax and τ [Gmax=-0.705±0.231 beats/(min·mmHg) vs. -1.065±0.200 beats/(min·mmHg); τ=0.0450.011 vs. 0.0590.026], however, they were statistically insignificant. Conclusion The results demonstrated that NO imposes effect on baroreflex activity, and the behaviors of exogenous and endogenous NO are different. Endogenous NO may be involved in the regulation of autonomic nervous system via central nervous system or carotid sinus, and inhibits the baroreflex sensitivity.
9.Repair of fingertip soft tissue defect with the dorsal branches island flaps of the proper palmar digital arteries: a report of 35 cases

Objective To discuss the treatment method and effect of the application of the dorsal branches island flaps of the digital arteries in clinic.Methods During January,2010 to February,2015,a total of 20 cases of palmar soft tissue defect of the 2nd-5th distal segments of single fingers were incorporated and repaired by the dorsal branches island flaps of the digital arteries in the base of the distal phalanx.The size of the selected flaps was 3.5 cm × 2.5 cm-2.0 cm × 2.0 cm.In addition,palmar soft tissue defect (n =15) of the 2nd-5th middle and distal segments of single fingers were repaired by the dorsal branches island flaps of the digital arteries at the 1/3 site of the proximal digital arteries,and the selected flaps size was 4.5 cm × 2.5 cm-3.0 cm × 2.0 cm.Full-thickness skin graft urethroplasty was used in the donor sites.Flap shape,texture,sensory recovery and texture of the skin region,function recovery of the wounded fingers were followed up at 1,3,and 6 months after operation.Results Thirty-five patients were followed up from 6 to 13 months (average of 7 months).All flaps survived completely in the incorporated 35 cases postoperatively.Flaps had satisfactory appearance,fine texture,and soft texture in the skin transplantation area.The postoperative improvement of sensory function was between S2 and S3,and the two-point discrimination was 10-15 mm.Assessment of the upper limb function using the standard issued by the Hand Surgery Society of Chinese Medical Association graded 23 cases as excellent,10 cases as good,and 2 cases as fair.Conclusion The dorsal branches island flaps of the digital arteries are characterized by easy dissection,reliable blood supply,and no major blood vessels needed to be sacrificed,which is an effective procedure for finger soft issue defect repairing.
10.Interactions between the recombinant human CREG protein and cathepsins and M6P/IGFIIR

BACKGROUND:It has been found that cel ular repressor of E1A-stimulated genes (CREG) is a lysosomal protein binding directly to the mannose-6-phosphate (M6P)/insulin-like growth factor II receptor (IGFIIR) and depends on the interaction with M6P receptors for efficient delivery to lysosomes OBJECTIVE:To study the interactions between the exogenous CREG protein and cathepsins and M6P/IGFIIR and to confirm the effect of CREG protein on expression and distribution of M6P/IGFIIR. METHODS:Double-stained immunofluorescence and coimmunoprecipitation were applied to observe the interactions between the exogenous CREG protein and cathepsin B, cathepsin L and M6P/IGFIIR. Using gain-of-function and loss-of-function approaches, the effect of CREG on expression and distribution of M6P/IGFIIR were studied by western blot assay and immunofluorescence staining. RESULTS AND CONCLUSION:Double-stained immunofluorescence and coimmunoprecipitation analyses confirmed the direct interactions between the exogenous CREG protein and cathepsin B, cathepsin L and M6P/IGFIIR. It was verified that CREG plays a critical role not in the expression but in the distribution of M6P/IGFIIR using gain-of-function and loss-of-function approaches. These findings provide evidence that exogenous CREG protein is located in lysosomes and has interactions with cathepsins and M6P/IGFIIR, also CREG plays a critical role in the distribution of M6P/IGFIIR.
