1.Lipid profiles of patients on continuous ambulatory peritoneal dialysis

Objectives To investigate the serum lipid profiles in patients with continuous ambulatory peritoneal dialysis(CAPD).Methods Serum lipid levels were cross-sectional and prospective investigated in patients with CAPD.Results Compared with those in normal controls,serum TG and apoB increased while HDL,HDL 2,apo A1/apoB and LCAT activity decreased significantly in CAPD patients.TC,LDL-ch and Lp(a) also increased in CAPD patients.Serum Lp(a),LDL-ch,apoB and apoE increased significantly following the CAPD therapy.Conclusions Hyperlipidemia is quite common in CAPD patients and serum lipid levels may change with the time on CAPD.
2.Correlation between Plasma Concentration and Dose of Tacrolimus in the Treatment of Membranous Ne-phropathy

Objective:To investigate the correlation between plasma concentration and effective dose of tacrolimus in the treatment of membranous nephropathy. Methods: Totally 41 patients with membranous nephropathy were given tacrolimus combined with hor-mone at low dose. The valley plasma concentration of tacrolimus was detected using a homogeneous enzyme multiplied immunoassay method. According to the changes of 24-hour urine protein,serum albumin and kidney function, the clinical efficacy was evaluated. The correlation between curative effect and plasma concentration and dosage of tacrolimus in the treatment of membranous nephropathy was analyzed. Results:The plasma concentration of tacrolimus was (7. 47 ± 2. 74) ng·ml-1 and the dosage of administration was (0.047 ±0.007)mg·kg-1·d-1 in complete remission group, that in partial remission group respectively was (5.72 ±1.19)ng· ml-1 and(0.049 ±0.008)mg·kg-1·d-1, and that in non-remission group was respectively (3.30 ±1.08 )ng·ml-1 and (0.052 ± 0. 01)mg·kg-1·d-1. Conclusion: The clinical efficacy of tacrolimus in the treatment of membranous nephropathy has close rela-tionship with the plasma concentration,and that is promising when the plasma concentration within the range of (5. 10-9. 32) ng· ml-1 . The plasma concentration of tacrolimus isn't increased with the dosage increase in some patients.
3.Relationship between First-episode Schizophrenia and Metabolic Syndrome: a Case-control Study

Objective To explore the relationship between first-episode schizophrenia and metabolic syndrome (MS). Methods 231 first-episode schizophrenia patients (schizophrenia group) and 63 healthy controls (healthy group) were selected to detect the height, weight, waistline, fasting plasma glucose (FPG), blood lipid and blood pressure. International Diabetes Federation (IDF) MS Diagnostic Criterion (2005) was used to diagnose MS. Results The FPG and Total Cholesterol (TC) of the schizophrenia group were significantly higher than the control group (P<0.001). The MS detection rate in the schizophrenia group was higher than in the control group (P=0.021). Conclusion First-episode schizophrenia patients could have glucose-lipid metabolism disorder during the early stage.

Object To investigate the chemical constituents from the whole plant of Notoseris gracilipes Shih Methods Compounds were isolated by various column chromatography and identified by spectral analysis Results Eight sesquiterpene lactones and four other compounds were isolated and identified as jacquilenin (Ⅰ), scorzoside (Ⅱ), ixerisoside D (Ⅲ), austricin (Ⅳ), notoserolide A (Ⅴ), crepidiaside A (Ⅵ), crepidiaside B (Ⅶ), and lactuside B (Ⅷ) The other four were 6, 7 dihydroxycoumarin (Ⅸ), cichoriin (Ⅹ), luteolin 7 glucoside (Ⅺ), and daucosterol ( ⅩⅡ ) Among them, compounds Ⅰ and Ⅱ showed antibacterial activity against Bacillu cereus AS 1 1688 Conclusion All compounds were isolated from this plant for the first time

Objective To assess the diagnostic predictive value of Wells score and modified Geneva score for acute pulmonary embolism by prospective case series and to explore a more suitable scoring system for Chinese population.Methods All the patients suspected of pulmonary embolism (PE) and received CT pulmonary angiography (CTPA) were enrolled consecutively in Fuxing Hospital,Capital Medical University,China,from June 2009 to August 2011.Before CTPA test or on condition that test results were unknown,clinical scoring was assessed prospectively by the Wells score and the modified Geneva score.The probability of PE in each patient was assessed and the patients were divided into low,moderate and high probability groups according to the clinical scores.The result of CTPA was used as the diagnostic gold standard for PE.Diagnostic accuracy in each group was analyzed.The predictive accuracy of both scores was compared by AUCROC curve.Results A total of 139 patients met our enrollment criteria and 117 eligible patients entered our study at last.PE was diagnosed in 47 patients by CTPA with an overall prevalence of 40.2%.Prevalence of PE in the low,moderate and high pretest probability groups assessed by the Wells score and by the simplified modified Geneva score were 7.1% (3/42),42.9% (21/49),88.5% (23/26)and 10.0% (3/30),48.1% (37/77),7/10,respectively.AUCROC curves for the Wells score and the simplified modified Geneva score were 0.872 ( 95% CI 0.810-0.933 ) and 0.734 ( 95% CI 0.643-0.825 )respectively,with a significant difference ( P =0.005 ).Conclusion The Wells score is more accurate for clinical predicting acute PE than the modified Geneva score.

Anatomy experimental teaching for nursing students in higher vocational college should reflect the actuality of nursing work,strengthen anatomical structure knowledge related with nursing procedures,help solve the difficuhies in nursing operation.We reformed on experimental item,experiment teaching content,design of experiment teaching procedure and making of experiment teaching specimens,etc.We also created situational teaching scene and combine anatomical structure with nursing ideas thus to make the students master the knowledge of anatomy,which can meet the clinical nursing operation related requirements.Therefore,basic medical education must make students use what they learn and closely correlated with its profession and clinical application.
8.Clinical analysis of 32 cases of pituitary hyperplasia secondary to primary hypothyroidism

Objective To improve the recognition of pituitary hyperplasia secondary to primary hypothyroidism by analyzing clinical and imaging data. Methods The clinical features, hormone data,imaging findings, and treatment were reviewed in 32 patients with pituitary hyperplasia secondary to primary hypothyroidism in Huashan Hospital from 1999 to 2008. Results Thirty-two patients, most juvenile,presented clinical and imaging features suggestive of functional primary pituitary adenoma. The dose of levothyroxine was increased to maintain the thyrotropin concentration at normal values. Following adequate thyroxine replacement, pituitary hyperplasia regressed on average within 6 months. Conclusions Pituitary hyperplasia secondary to primary hypothyroidism seems to be quite prevalent in children and adolescents.Complete regression will be achieved with thyroxine replacement therapy.
9.Resistance Genes of Antibacterial Agents in MRSA and MSSA

OBJECTIVE To investigate the condition of drug-resistant genes in MRSA and MSSA. METHODS The drug-resistant genes mecA,ermA/B/C,aac(6′)/aph(2″),aph(3′)-Ⅲ,ant(4′,4″) and tetM of MRSA and MSSA were detected by polymerase chain reaction(PCR). RESULTS The 5 kinds of drug-resistant genes,such as mecA,ermA/B/C,aac(6′)/aph(2″),(aph(3′)-Ⅲ) and tetM were positive in MRSA. CONCLUSIONS MRSA is a multi-resistant pathogen.
10.Pharmacokinetics and enterohepatic circulation of jervine, an antitumor steroidal alkaloid from Veratrum nigrum in rats

Jervine, a novel steroidal alkaloid from Veratrum nigrum L., exhibits both antitumor effect and potential toxicity. The aim of study was to characterize the pharmacokinetic behaviors and enterohepatic circu-lation of jervine in rats. A rapid and simple ultra-high performance liquid chromatography-tandem mass spectrometric method was developed and validated for quantification of jervine and alpinetin (internal standard) in rat plasma. After extraction from rat plasma by a simple protein-precipitation method, the analyte was separated on a C18 column (2.1 mm × 50 mm, 1.7μm) using water with 0.1%formic acid and acetonitrile as the mobile phase delivered at a flow rate of 0.4 mL/min. Jervine and alpinetin were determined in the positive mode with multiple reaction monitoring (MRM) of the ion transitions at m/z 426.3→108.8 and m/z 271.0→166.9, respectively. Molecular docking method was used to investigate the binding of jervine to p-glycoprotein and dehydroepiandrosterone sulfotransferase. The method was well validated within acceptance limits including specificity, matrix effect, recovery, precision, accuracy, and stability, and was successfully applied to the pharmacokinetic study of jervine after oral and intravenous administration to rats. Jervine presented a small volume of distribution, fast absorption, high oral bioavailability, and enterohepatic circulation. The enterohepatic circulation was first observed in veratrum alkaloids, and was further investigated by molecular docking studies, which was related to the binding of jervine to p-glycoprotein and dehydroepiandrosterone sulfotransferase. The pharmacokinetic properties and enterohepatic circulation of jervine in rats provided a significant basis for the drug-drug interaction and toxicity study in the future.
