1.Cost-effectiveness Analysis of Duloxetine, Escitalopram Oxalate and Mirtazapine in the Treatment of De-pression

Objective:To evaluate the cost-effectiveness of duloxetine, escitalopram oxalate and mirtazapine in the treatment of depression. Methods:Totally 120 cases of patients were randomly divided into three group. Group A was given duloxetine 40mg twice a day, group B was with escitalopram oxalate at the initial dose of 10mg per day, and up to 20mg per day in 2 weeks, and group C was treated with mirtazapine 30mg per day. The treatment course was eight weeks. At the 1st, 2nd, 3rd, 4th and 8th weekends after the treatment, Hamilton depression scale ( HAMD) was used to evaluate the total effective rate, and treatment emergent symptom scale ( TESS) was applied to assess the adverse drug reactions. Results:The total effective rate of group A, B and C was 87. 5%,90% and 92. 5% with the cost of 23 822. 22 yuan,33 866. 02 yuan and 19 586. 62 yuan, respectively. The cost-effectiveness ratio respectively was 27 225, 37 629 and 21 175. The incidence of adverse reactions respectively was 30%, 30% and 17. 5%. Conclusion:The cost-effectiveness of mirtazapine is the lowest in the treatment of depression, which can be considered as the best treatment regimen.
2.Risk factor analysis of intervention for the treatment of neurological complications in the elderly patients with ruptured intracranial aneurysm

Objective To evaluate the risk factors for interventional treatment of neurological complications in elderly patients with rupture intracranial aneurysms.Methods Form December 2004 to December 2014,520 consecutive old patients (≥ 60 years) with ruptured intracranial aneurysm treated at the Department of Neurosurgery,Changhai Hospital,the Second Military Medical University were enrolled retrospectively.The imaging and clinical follow-up results at day 30 after procedure were documented.According to whether the occurrence of interventional therapy-related neurological complications (including intraoperative aneurysm rupture,interventional therapy-related thromboembolic events,early postoperative aneurysm rebleeding,new postoperative subdural hemorrhage or intraparenchymal hemorrhage),they were divided into a complication group (n =68) and a non-complication group (n =452).Univariate statistical analysis and multivariate Logistic regression analysis were used to screen the independent risk factors for the occurrence of neurologic complications after interventional therapy.Results The incidence of neurological complications in 520 patients with aneurysm was 13.1% (n =68).Multivariate Logistic regression analysis showed that the history of hypertension (OR,2.207,95 % CI 1.149-4.240,P < 0.05),Hunt-Hess grade Ⅳ-Ⅴ (OR,4.287,95% CI 2.048-8.971,P < 0.01),Fisher grade Ⅲ-Ⅳ (OR,2.686,95% CI 1.483-4.865,P < 0.01),wide-neeked aneurysm (OR,2.884,95 % CI 1.511-5.505,P < 0.01),aneurysm bleb (OR,4.560,95 % CI 2.500-8.321,P < 0.01),and aneurysm < 3 mm (OR,5.748,95 % CI 2.122-15.570,P < 0.01) were the independent risk factors for treatment-related neurological complications in the interventional treatment of intracranial ruptured aneurysms in the elderly.Conclusion The history of hypertension,high Hunt-Hess grade,high Fisher grade,wide-neck aneurysm,aneurysm bleb,and micro-aneurysm are the independent risk factors for treatment-related neurological complications in interventional treatment of intracranial ruptured aneurysms in the elderly.
3.Comparison and evaluation between helical computer tomography and color-Doppler ultrasonography in primary hepatocellular carcinoma after transcatheter arterial chemoembolization

Objective To evaluate the usefulness and necessity of spiral computer tomography (SCT) and color-Doppler ultrasonography (CDU) in primary hepatocellular carcinoma (HCC) after transcatheter arterial chemoembolization (TACE). Methods Plain SCT scanning and contrast-enhanced SCT scanning or / and CDU were performed for 68 HCCs before and four weeks after TACE. Factors related to overall survival (OS) were observed as tumor size necrosis, relapse, lipiodol deposition, blood supply of the survived tumor. Images of SCT and CDU were compared. Clinical significance obtained through direction of diagnostic radiology was evaluated. Results 139 masses observed in the 68 cases were shranked or necrotized in a different degree after TACE. 8 new metastasized massed were observed. Complete lipiodol deposition was observed in 27 masses and no lipiodol deposition in 14 masses. As for estimating the types of lipiodol deposition, SCT was better than CDU with significant difference. SCT could indirectly estimate the blood supply of tumor after TACT through 3 dynamic enhanced phases, while CDU could directly visualize the hemodynamic changes of liver, internal and peripheral parts of the tumor. Conclusion TACE is an effective way to treating HCC. SCT scanning and / or CDU are necessary before and after TACE to estimate the imaging changes so as to provide indispensable information for the best therapeutic schedule in the future.
4.Effect of Dlk1 and Jagged1 gene on the proliferation and transdifferentiation of the primary type Ⅱ alveolar epithelial cells

Objective To study the effects of the Notch ligands Dlk1 and recombinant human nucleu factorκB (Jagged1) on the proliferation and transdifferentiation of the type Ⅱ alveolar epithelial cells when the Notch signaling pathway activated.Methods The primary type Ⅱ alveolar epithelial cells (AEC Ⅱ) cultured with recombinant protein Dlk1 and recombinant human nucleu factor-κB (rhNF-κB) (activator of Jagged1),respectively,and then cultured with DMEM (containing 120 mL/L FBS) as controls.Proliferation and differentiation conditions of the AEC Ⅱ were observed at 48 h,72 h,96 h time point by the light microscope and electron microscopes separately.Cell number was counted with hemacytometer; the proliferation rate was measured by methyl thiazolyl tetrazolium (MTT) ; Immunofluorescence double standard method was used to detect the AEC Ⅱ specific surfactant protein C (SP-C) and AEC Ⅰ specific protein aquaporin5 (AQPS) ;the expression of SP-C,AQPS,Dlk1,Jagged1,Notch1 and Hes1 mRNA were detected by real time-PCR.Results The cell population and proliferation:compared with control group,AEC Ⅱ proliferation was promoted in the Dlk1 group [cell numbers (× 109/L) 9.05 ± 0.45 vs 7.95 ± 0.65,11.68 ± 0.43 vs 8.68 ± 0.52,11.55 ± 0.17 vs 8.73 ± 0.48,all P < 0.05 ; MTT results (value A) 0.699 ± 0.050 vs 0.462 ± 0.080,0.912 ± 0.080 vs 0.535 ±0.040,0.726 ±0.050 vs 0.540 ±0.020,all P <0.05] and decelerated AEC Ⅱ transdifferentiation into AEC Ⅰ ; while AEC Ⅱ proliferation was inhibited in rhNF-κB group [cell numbers (× 109/L) 4.95 ± 0.33 vs 7.95 ± 0.65,4.73 ±0.71 vs 8.68 ± 0.52,4.04 ± 0.11 vs 8.73 ± 0.48,all P < 0.05; MTT results (value A) 0.398 ± 0.030 vs 0.462 ± 0.080,0.402 ± 0.070 vs 0.535 ± 0.040,0.380 ± 0.110 vs 0.540 ± 0.020,all P < 0.05] and accelerated AEC Ⅱ transdifferentiation into AEC Ⅰ.One-Way ANOVA showed that the difference among the 3 groups had statistical significance (cell numbers:F =486.73,P =0.02; cell proliferation:F =37.16,P =0.02).The mRNA expression:compared with control group,the expression of SP-C mRNA of Dlk1 group was significantly higher (P < 0.05) while the expression of AQP5 mRNA was remarkably lower and delayed (P < 0.05),the expression of Jagged1 mRNA was weak or little,Dlk1 and Notch1 mRNA were up-regulated (P < 0.05),and the Hes1 mRNA was reduced (P < 0.05) ; the expression of SP-C mRNA of rhNF-κB group was significantly reduced (P < 0.05),while the AQP5 mRNA expressed ahead of time and increased (P < 0.05),Jagged1,Hes1 and Notch1 mRNA were higher (P < 0.05),and the Dlk1 mRNA was weak.One-Way ANOVA showed that the difference in the expressions of SP-C,AQP5,D1k1,Jagged1,Hes1 and Notch1 mRNA among the 3 groups had staistical significance (F =96.80,P =0.01 ; F =82.55,P =0.01 ; F =269.80,P=0.00;F =312.34,P =0.00;F =169.17,P =0.01;F =19.85,P =0.02).Conclusions There are varied effects on proliferation and differentiation of the AEC Ⅱ when the Notch signaling is activated by different ligands:Dlk1 promoted proliferation and inhibited differentiation,while Jagged1 inhibited proliferation and promoted transdifferentiation.
5.Effect of all-trans retinoic acid on proliferation of fetal alveolar epithelial type Ⅱ cells and expression of pulmonary surfactant C and aquaporin 5

Objective To investigate the effect of all-trans retinoic acid (at-RA) on fetal alveolar epithelial type Ⅱ cells (fAEC Ⅱ s) proliferation and the expression of pulmonary surfactant C (SPC) as well as aquaporin 5 (AQP5).Methods fAEC Ⅱ s were isolated and purified from fetal lung of pregnant SD rats (19 days).After being cultured for 1 day,and the fAEC Ⅱ s were interfered by at-RA for 1,2 and 3 days.Cell proliferation,viability as well as growth state,expressions of SPC mRNA as well as AQP5 mRNA and expressions of protein SPC as well as AQP5 were respectively detected by using 4,5-dimethylthiazol-2-yl-2,5-diphenyltetrazolium bromide (MTT),inverted microscope,real-time fluorescence quantitative PCR (RT-PCR) and Western blot.Results (1) When fAEC Ⅱ s were treated with at-RA for 1 day,and the cell proliferation and viability did not change (P > 0.05),while the proliferation and viability were significantly improved in 2 days (P < 0.05),and the difference was the most obvious (P < 0.05) at 3 days.(2)Compared with the control group,the cell growth state was better,and the cell adherence was tighter and the refraction was higher in at-RA group.(3) Compared with the control group,at-RA up-regulated the expressions of AQP5 mRNA and AQP5 protein(t =-19.58,-10.44,-16.01,-46.25,-12.79,-27.96,all P < 0.05),and the percentages of control group were 281.07%,766.67%,1 163.33% and 792.65%,1 310.52%,1 561.56% in 1,2 and 3 days respectively.(4) Compared with control group,the expressions of SP-C mRNA and SPC protein were up regulated when cells were exposed to at-RA for 1 and 3 d,but while they were down-regulated at 2 days(protein:the percentages of control group were 615.480%,369.450% and 11.269%,respectively ; mRNA:728.33 %,400.83 %,66.57%,respectively)(t=-26.34,-25.26,13.80,-25.25,-31.71,9.12,all P<0.05).Conclusions at-RA can promote the proliferation and differentiation of fAEC Ⅱs,enhance the fAEC Ⅱ s viability,and improve the expression of SPC and AQP5.
6.Kinetic Migration Behaviors of Formaldehyde and Acetaldehyde from Paper Packaging Materials into Food Stimulant Tenax

The migration of two potential contaminants, formaldehyde and acetaldehyde, from two paper materials into food simulant Tenax was studied. A rapid and simple one-step extraction-derivatization method combined with ultra high performance liquid chromatography was established for the determination of formaldehyde and acetaldehyde in paper packaging materials and food stimulants. The linear correlation coefficients (R2) were greater than 0. 9999 in the range of 0. 14-35. 7 mg/m2 for both formaldehyde and 0. 20-49. 1 mg/m2 for acetaldehyde, with detection limits of 0. 03 mg/m2 for formaldehyde and 0. 04 mg/m2 for acetaldehyde. The recoveries were 90. 1%-108. 6% for paper sample and Tenax. The migration behaviors of formaldehyde and acetaldehyde at different temperatures and migration times were investigated. Both for formaldehyde and acetaldehyde, the amount of migration gradually increased with time first, then decreased, finally reached a steady value. The migration rates of formaldehyde and acetaldehyde were differently influenced by temperature. After reaching a steady value, the maximum migration rate for formaldehyde was at 30℃, and for acetaldehyde was at 70℃ and 50℃. The migration rates of formaldehyde and acetaldehyde greatly varied, and the migration rate of acetaldehyde was far more than the migration rate of formaldehyde when reaching a steady value.
7.Primary hyperparathyroldism in the mainland of China

Objective In China primary hyperparathyroidism is not a kind of common disease as in the wesyrn countries.This article reports the current status in the diagnosis and treatment of primary hyperparathyroidism in the mainland of China. Methods We collected 730 cages of primary hyperparathyroidism diagnosed and treated in 7 top hospitals for endocrine surgery from 1965 to 2005.Results In this study.652(89.3%)cases were clinically symptomatic while 78(10.7%)cases were asymptomatic:442 cases were positive on 99mTc-MIBI scanning.Bilateral explorations were undertaken in 377 patients and unilateral or uni-gland exploration through the conventional incision in 204 cases.Minimally invasive parathyroidectomy in 143 cases.Endoscopically assisted 2 cm incision was taken in 6 cases for unilateral gland exploration.Pathologically 632(86.6%)cases were identified as adenoma,58(8.3%)cases were of hyperplasia and 40(5.5%)cases were of carcinoma.There were no major postoperative complications.While 20 patients suffering from recurrence or persistent postoperative hyperparathyroidism,the others are of normal or depressed serum level of calcium. Conclusions Preoperative localization is very helpful: Unilateral exploration for parathyroid adenoma is feasible; minimally invasive parathyroidectomy throush minimal incision is a kind of improving procedure for the localized parathyroid adenoma.
8.Antitumor components screening of Stellera chamaejasme L. under the case of discrete distribution of active data.

This is to report the screening, extracting and validating antitumor components and compounds from Stellera chamaejasme L. under the case of discrete distribution of active data. In this work, different components from Stellera chamaejasme L. were collected by HPD macroporous resin and polyamide resin column, and their antitumor activity on A549 were tested by MTT assay. Activity results indicate that activity of components at 30-39 min is more potent than that of Stellera chamaejasme L. extract, and the activity of components at 33.97 min is equivalent to positive drug, cis-platinum at 100 microg x mL(-1), but with totally different mode of action. Under the case of discrete activity, the weight analysis is capable of screening active components and compounds from natural products.
9.Ultrasonographic diagnosis of pseudothrombophlebitis

Objective To investigate the ultrasound characteristics of pseudothrombophlebitis.Methods The ultrasound characteristics of thirty-two popliteal cyst cases with pseudothrombophlebitis which were confirmed by MRI or puncture were retrospectively analyzed.The former group was matched with a case control group of sixty-four patients with asymptomatic popliteal cysts,the ultrasound images were comparative analysis between the two groups.Results Compared to the control group,both the length and width of the case group were larger [(12.4 ± 4.7) ×(2.5±0.4)cm vs (5.3±2.9) × (1.2±0.4)cm,P < 0.001],and there were 18(43.8%) cases with the cysts extension into the calf.The case group were more prone to show cyst with poor ultrasound penetration or solid-cystic echo (78.1% vs 9.4%,P =0.000),which were diagnosed as popliteal cyst with hematoma or infection.Six cases of case group showed irregular anechoic area surrounding the inferior border of the cysts,which were diagnosed as ruptured popliteal cyst,no similar imaging detected in the control group(P =0.001).Both the two group showed septation in the cyst,but it did not differ significantly(P >0.05).Conclusions Ultrasonography is helpful to the diagnosis of pseudothrombophlebitis,the ultrasound characteristics include large cyst extension into the calf,cyst with poor ultrasound penetration or solid-cystic echo and irregular anechoic area surrounding the inferior border of the cysts.
10.Effect of early continuous enteral nutrition combined with interstinal mucosal protective agents on gut barrier in patients with severe acute pancreatitis:A multicenter prospective randomized controlled trial

Objective To assess the effect of continuous early enteral nutrition(EEN)combined with intestinal mucosal protective agents on gut barrier function in patients with severe acute pancreatitis.Methods A total of 79 patients with severe acute pancreatitis selected from four centers between May 2004 to June 2006 were enrolled and divided into EEN combined with intestinal mucosal protective agents group(combined group,n=39)and total parenteral nutrition(TPN)group(n=40).The patients were received either EEN or TPN when homeostasis were achieved within 72 hours after onset.The patients in combined group were administered pepti-2000 variant combined with glutamine,arginine and intestinal mucosal protective agents.The patients in TPN group were administered through a central vein.APACHE-Ⅱ score was recorded every week;The concentration of serum amylase,plasmic diamine oxi dase(DAO)and endotoxin were mesured on day 1,7,14 and 21 as well as urinary excretion of lactulose (L)and mannitol(M).Complications,lenth and charges of hospital stay were recorded.Results There was no death in both groups.The APACHE-Ⅱ score decreased on day 7,but lower in combined group (6.00±1.60)than that in TPN group(7.08±2.34)(P<0.05).On day 7,14 and 21,the concentrations of endotoxin in combined group was(39.30±15.82),(22.64±14.31),(14.81±10.93)Eu/L,respectively,urinary L/M ratio was 0.28±0.25,0.21±0.18 and 0.08±0.04,respectively,IFABP-c was 15.62±5.26),(5.46±1.18)and(3.26±0.94)pg/ml,respectively.All of these parameters were significantly lower than those in TPN group(P<0.05).The infectious rates including pancreatic,peritoneal and respiratory infection in TPN group were much higher than that in combined group(26.47% vs 3.44%,P<0.01).The composition of flora fecal remained unchange in combined group rather than TPN group.The mean hospital stay was shorter in combined group[(20.0±5.7)days]compared to TPN groups[(34.5±12.9)days].The charges were also significantly lower in combined group,with average cost of RMB 25,900±14,200,while it was 46,800±4,030 in TPN group.Conclusions EEN combined with intestinal mucosal protective agents can improve gut barrier function via reducing the gut permeability,improving the hypoperfusion,maintaining the integrity and gut fecal flora.It might reduce the course and charges of hospital stay.
