2.Study on the Determination of Aristolochic Acid by RP-HPLC

Objective To optimize the RP-HPLC procedure for the determination of aristolochic acid. Method Different extraction solvents and methods have been screened to get the most efficient way for sample preparation. The influence of different mobile systems on quantitation were compared to choose an approprate mobile system for determination. Besides,the minimum limits of different detecting waves were measured to give the method to detect trace amount of aristolochic acid. Result Refluxing with 70% methanol was better than other ways in sample preparation. Both methanol-1% acetic acid (70∶30) solution and 0.3% ammonium carbonate solution (pH=7.5)-acetonitrile (75∶25) with wavelength of 250 nm and 319 nm can be used for quantitation while 0.3% ammonium carbonate solution (pH=7.5)-acetonitrile (75∶25) with wavelength of 224 nm for trace detection. The minimum detectable amount was 0.02 ng. Conclusion The method established can be applied to determinate aristolochic acid and detect trace amout exactly.
3.Efficacy of Different Therapies for Immunosuppression after Liver Transplantation: A Meta-analysis

OBJECTIVE: To evaluate the safety and efficacy of FK506-based dual therapy and CsA-based triple therapy for immunosuppression after liver transplantation. METHODS: The literature about the randomized controlled trials (RCT) on FK506 and CsA for immunosuppression after liver transplantation was retrieved for analysis using RevMan 5.0 software. RESULTS: A total of 7 RCTs were included; 3 were considered as high quality of RCT, 4 as moderate quality. The results of Meta-analysis of FK506-based dual therapy indicated the patients’ survival rate (RR=1.06, 95%CI=0.77~1.45,P=0.72), grafts’ survival rate (RR=0.99, 95%CI=0.76~1.29,P=0.92), infection rate(RR=0.98, 95%CI=0.60~1.58,P=0.93)and incidence of neurotoxicity (RR=0.59, 95%CI=0.32~1.07,P=0.08)was not significantly different. The incidence of acute rejection in CsA group was lower than in FK506 group (RR=1.57, 95%CI=1.23~2.01,P=0.000 3). CONCLUSION: The curative efficacy of FK506 group is not different from that of CsA group in respect of rate of patient/graft, infection but increase the incidence of acute rejection. Because CsA can increase incidence of neurotoxicity, it is suggestion to treat patients of nervous systerm disease with FK506.

Objective: To observe the role of alanylglutamine(Ala-Gln)-supplemented hypocaloric nutrition support in severe brain injury.Methods: 60 cases of severe brain injury were randomly divided into experimental group and control group.The experimental group was treated with peripheral parenteral nutrition support of 105 kJ/(kg?d) and administration of alanylglutamine for 8 days.The control group was treated with peripheral parenteral nutrition support of 146 kJ/(kg?d) for 8 days.Blood glucose,serum albumin,transferin,hemoglobin,nitrogen balance,IgA,IgG,IgM,CD4(%) and CD8(%)were detected before and after nutrition support.Results: Compared with control group,IgA,IgG,CD4(%),CD4/CD8 in experimental group increased significantly(P

Objective To investigate the possible role of nerve growth factors (NGF) and its receptors in ovarian follicle development,and detect the difference between polycystic ovarian symdrome (PCOS) and normal control.Methods Immunohistochemisty was applied to detect the expression of NGF,p75NTR,TrkA in ovarian follicle granulosa cells.Results In 54 cases (including 9 cases of IVM-PCOS,16 cases of IVF-PCOS and 29 cases of IVF-Normal),49 cases showed NGF protein positive in granulose cells with significance difference between groups (P<0.05),and especially NGF was the strongest expressed factor in IVF-PCOS group but there was no remarkable difference between the other two groups in the expression of NGF.41 were TrkA protein positive,almost all IVF cases were TrkA protein positive while only 2 in IVM and there was no remarkable difference in expression of TrkA between IVF PCOS and IVF Normal(P>0.05).The cases with stronger expression of NGF or TrkA had higher estradiol level than that of lower expression group (P<0.05).51 cases had positive expression of p75 NTR protein but there was no significant difference between groups (P>0.05).Conclusions TrkA is expressed in mature ovarian follicle,and the interaction of NGF-TrkA may be involved in ovarian follicle development,and overexpression of NGF may be associated with PCOS development.
6.Relationship of VKORC1 and CYP2C9 gene polymorphism and warfarin in patients with non-valvular atrial fibrillation

Objective To evaluate the guidance value of VKORC1 and CYP2C9 gene polymorphism on clinical application of warfarin in patients with non-valvular atrial fibrillation (NVAF). Methods A total of 290 cases of patients with NVAF were identified and their baseline data , initial dose of warfarin and base INR measurement results were recorded, then the follow-up was conducted. The initial administration of warfarin to INR standard time for the first time, total amount of warfarin and the average daily amount were recorded. All participants′ venous blood was extracted for VKORC1 and CYP2C9 gene polymorphism test. Results VKORC1 AG/GG genotype patients had longer INR standard time and higer amount of warfarin than patients with AA (103.38 ± 65.29)g vs. (53.26 ± 24.02)g, P < 0.05. Patients with type CYP2C9 gene mutation had shortest INR standard time(9.10 ± 2.01)d vs. (13.07 ± 4.28)d, P < 0.05. and lowest administration amount of warfarin (28.80 ± 17.35)g vs. (55.45 ± 23.67)g, P < 0.05. Conclusion There exist significant differences of first adminstration amount of warfarin in patients with NVAF to INR standards. Warfarin dose for VKORC1 AA genotype patients is lower than that for GG/AG type; there is short INR standard time and less adminstration amount of warfarin for CYP2C9 genotypes AC/CC patients.
7.Comparative study on clinical features of cytomegalovirus infection after allogenic hematopoietic stem cell transplantation from HLA haploidentical related donors vs HLA-matched sibling donors

Objective To compare the clinical features of cytomegalovirus (CMV) infection and CMV disease after allogeneic hematopoietic stem-cell transplantation (HSCT) from HLA haploidentical related doors vs.HLA-matched sibling donors.Methods A total of 327 patients who received allogeneic HSCT from Jan.2011 to Dec.2011 were enrolled.There were 312 patients who had complete serological data before HSCT including 216 cases of HLA haploidentical related HSCT and 96 cases of HLA-matched sibling HSCT.Monitoring of CMV antigenemia was performed by using real-time quantitative (RQ) PCR after transplantation.Risk factors were compared by univariate and multivariate analysis.Results The cumulative incidence of CMV infection and CMV disease was (80.1 ± 2.7) % and (8.7 ± 2.0) % in HLA haploiddentical HSCT group,and (21.1 ± 4.9) % and 0 in HLA-matched sibling HSCT group respectively,and the difference was statistically significant between the two groups (P<0.01).Univariate analysis revealed that HLA haploidentical related HSCT,less than 20 years of age,high risk disease,CMV-IgG serum positivity in patients or donors,acute graft-versus-host disease (aGVHD),EB viremia,and hemorrhagic cystitis were the risk factors of CMV infection.HLA haploidentical related SCT and hemorrhagic cystitis were the risk factors for CMV disease.Multivariate analysis showed that patients less than 20 years of age had a significantly high incidence of CMV infection.Patients from HLA-matched sibling HSCT,low risk disease,aGVHD,hemorrhagic cystitis had a significantly low incidence of CMV infection.Conclusion Compared with patients receiving HLA-matched sibling HSCT,those who received HLA haploidentical related HSCT had significantly high incidence of CMV infection and CMV disease,which were correlated with incidence of hemorrhagic cystitis.

Objective: To study the effect of Astragalus-supplemented enteral nutrition on nutritional metabolism in type-2 diabetic patients with ischemic stroke . Methods: 64 cases of type-2 diabetic patients with ischemic stroke were randomly divided into treatment group and control group. The treatment group was treated with homogenized diet and Radix Astragalis(equivalent to 60g crude drug), and the control group was treated with homogenized diet. Two groups were treated with nutrition support of 105kJ/(kg?d) for 30 days. Estrogen Receptor (ERalpha) is a member of superfamily of ligand-activated transcription factors which play critical roles in many biological processes. To screen novel modulators of ERalpha for drug development and biological function research, we developed a mammalian one-hybrid-based high-throughput screening model for ERalpha modulator. We cloned the ERalpha LBD gene from the total mRNA of fat tissue by RT-PCR and fused it with the GAL4 DNA binding domain of pBIND-GAL4 plasmid to construct a chimara expression plasmid pBIND-GAL4-Eralpha(LBD). The L02 cells was cotransfected with pBIND-GAL4-ERalpha(LBD) and a GAL4-responsive luciferase reporter plasmid pGL3-GAL4, and following treatment with test compounds for 24 h, the activities of luciferase were detected to evaluate the transactivities of ERalpha modulators. After manner optimizations of transfection conditions, Estradiol, an agonist control, induced the expression of luciferase in a dose-dependent with EC50 of 0.17 micromol/L, the maximum folds of induction was about 28.1. Tamoxifen, an antagonist control, efficiently suppressed the estradiol-mediated luciferase induction with EC50 of 0.10 micromol/L. Using this screening model, we discovered four ERalpha agonists from 2000 natural and synthetic compounds.

The knee osteoarthritis(KOA)is a kind of degenerative disease, which is more common in the elderly.In recent years, according to the Knee Uneven Settlement Theory, a new treatment method for the knee osteoarthritis by using the proximal fibula osteotomy is popularized in many hospitals.In this article, we reported 1 case undergoing the total knee arthroplasty after the proximal fibula osteotomy for the treatment of knee osteoarthritis and reviewed the relevant literatures.
