1.Open reduction and Y plate internal fixation to treat intra-articular calcaneal fractures

Objective To observe and analyze the outcomes o f open reduction and Y plate internal fixation in treatment of intra-articular calcaneal fractures.Methods Open reduction and Y plate internal f ix-ation by the extended L-shaped lateral approach were performed in 68pati ents with 82cases of intra-articula r calcaneal fractures under X-ray.Results82cases were followed up for an avera ge of 26months,and the clinical results were evaluated wit h the Maryland Foot Score.58cases ac hieved excellent results,16good,6fair,with the rate of excellent and good re sults being 90.24%.Conclusion Open reduction and Y plate internal fixation can obtain good clinical re sults in treatment of intra-articul ar calcaneal fractures.[
2.Influential factors of auditory rehabilitation after cochlear implant

OBJECTIVE:How to explain and predict individual difference of artificial cochlea so as to evaluate rehabilitative effect of implant has been paid much attention by audiological scholars,clinical physicians,patients,and their relatives. This study was designed to analyze and evaluate rehabilitative status of patients accepting artificial cochlea implantation to investigate the relative influential factors of auditory rehabilitation. METHODS:A total of 28 patients were selected from Department of Otolaryngology Head and Neck,Affiliated Hospital of Qingdao Medical College from June 2002 to April 2008. They were accessed by age,time of rehabilitation training,usage of hearing aids,audiology and aural image before surgery,and NRT monitoring in cochlear implantation so as to observe auditory improvement. The clinical results were analyzed to detect the effect of various factors on auditory rehabilitation. RESULTS:Age,preoperative preparations and good audiology and aural image are important premises for a successful rehabilitation. Longer the time of using hearing aids and language rehabilitation are,and better results will get. NRT response during operation is also an important factor for good auditory rehabilitation. CONCLUSION:The influential factors of auditory rehabilitation after cochlear implant included the age,audition remains,usage of hearing aids,NRT response during operation,and the time after the cochlear implant operation.
3.Effects of propofol on beta-amyloid protein-induced injury to cultured rat cortical neurons

Objective To investigate the effects of propefol on β-amyloid protein(β-AP)-induced injury to cultured rat cortical neurons.Methods Eighteen days pregnant SD rats were anesthetized with ether.The fetal rats were obtained under sterile condition and decapitated. Cortices were then dissected under dissecting microscope.Cortical neurons were isolated according to the method described by Velly LJ et al and cultured for 7 days.There were 5×104 neurons in each well (200 μl).The experiment included 2 parts.In part T 15 wells of neurons were randomly divided into 5 groups(n=3 each ) : group I control(C);group II β-AP 25 μmol/L; group III and IV 2 propofol pretreatment groups (PP1,PP2) and greup V propofol treatment (PT).In group PPt propofol 50 μmol/L was added to the culture medium 24 h before the addition of β-AP 25 μmol/L and the neurons were incubated for another 24 h.In group PP2 propofol 50 μmol/L and μ-AP 25 μmol/L were added to the culture medium simultaneously and the neurons were then incubated for 24 h.In group PT propefol 50 μmol/L was added to the culture medium at 6 h after the addition of β-AP 25 μmol/L and the neurons were incubated for another 18 h.In part Ⅱ 18 wells of neurons were randomly divided into 6 groups(n=3 each):group I control (C) ; group IIβ-AP 25 μmol/L; group III intralipid; group IV,V,and VI 3 prepofol treatment groups (P1,P10,P50).In intralipid group equal volume of 10% intralipid was added to the culture medium at 6 h after β-AP 25 μmol/L and the neurons were then incubated for another 18 h.In group IV- VI propofol 1,10 and 50 μmol/L were added at 6 h alter β-AP 25 μmol/L respectively and the neurons were incubated for another 18 h.The amount of lactic dehydrogenase (LDH) released was measured.Neuronal viability was assessed by MTT assay.The neuronal apoptosis was detected using Hoechst33342 staining and TUNEL technique,and the.apoptosis rate was calculated.Results In part Ⅰ there was no significant difference in the amount of LDH released between group Ⅱ(β-AP) and the 2 propofol pretreatment groups(Ⅲ,Ⅳ).The amount of LDH released was significantly lower in group Ⅴ (propofol treatment) than in group β-AP(Ⅱ).In part Ⅱ the amount of LDH released was significantly lower,neuronal viability higher and the apoptosis rate was lower in group P50 than in group Ⅱ(β-AP).Conclusion Propofol 50 μmol/L given after β-AP can attenuate β-AP induced injury to cultured rat cortical neurons while prophylactic administration of propofol can't.
4.Prevention and treatment of artery complication after liver transplantation by HBO

Objective To observe the effect of HBO in the treatment of artery complications after liver transplantation.Method Five cases were treat by HBO(202.65 kpa,once every day,oxygen inhalation 60 min) and other therapies after operation.Result After HBO,the whole-body situations were obviously improved,liver functions were ameliorated,no new necrotic focuses were found,and the primary necrotic focuses were reduced or vanished.Conclusion HBO was an effective way to treat and prevent the artery complications after liver transplantation and protect the ischemic liver cells.
5.Nanog influences the proliferative ability of HepG2 cells through the modulation of the cell cycle related proteins

Objective To study the effect of pluripotency factor Nanog on the expression of the cell cy-cle related proteins,and then to explore its effect on the proliferative ability of HepG 2 cells.Methods TALENs gene editing tool was employed to induce mutation and downregulation expression of Nanog .T7 endonuclease 1 and genomic sequencing was used to analyze the mutation efficiency of Nanog .RT-PCR and western blot were used to determine the expression of mRNA and protein of Nanog ,respectively .Real-time cell based assay system was used to measure the proliferative ability of wild -type HepG2 cells and monoclonal HepG 2 cells with Nanog mutation.Results TALENs successfully induced mutation of Nanog gene .The targeting efficiency of mixed cells was analyzed by T7 endonuclease 1 approached 40%after two transfection with plasmid of Nanog -TALENs.Ad-ditionally,the Nanog mRNA expression level of monoclonal HepG 2 with Nanog mutation was downregulated by 3.4 times compared to the wild type HepG 2 cells,and the Nanog protein expression level was downregulated by 3.6 times.The cell cycle related proteins CyclinD1/D3,CyclinE1 and CDK2 expression were downregulated in monoclonal HepG2 with Nanog mutation in comparison to the wild type HepG 2 cells.Conclusion Nanog plays a role in influencing the proliferative ability of HepG 2 cells through modulating the expression of the cell cycle re-lated proteins CyclinD1/D3,CyclinE1 and CDK2.The downregulation expression of Nanog can inhibit the prolif-erative capacity of HepG 2 cells via the regulation of the cell cycle related proteins .
7.Investigation on Clinical Effect of Herba Houttuyniae Injection by Aerosol Inhalation for Respiratery Tract Infection on 107 Cases

Objective:To study the clinical effect of herba Houttuyniae Injection by aerosol inhalation for respiratory tract infection. Methods: 107 patients with respiratory tract infection (the main symptoms were cough and expectoration) were treated with Herba Houttuyniae Injection by aerosol inhalation. The conditions of symptom improvement and bacteria in sputum before and after treatment were observed. The case control studies on parts of patients in acute attack and delayed peroids of chronic bronchitis were carried out. The treatment processes patients were given 25mL Herba Houttuyniae Injection and 25mL normal saline by aerosol inhalation for 30 minutes, twice a day. Results: The effective rates were 91.59% for cough symptom, 85.06% for expectoration symptom. 98.13% patients experienced comfort of pharyngeal and thoracic protions after treatment. Among 55 patients whose cough symptom wasn't improved after they were treated with anti inflammatory agent and antitussive the effective rate was 72.72% after treatment with Herba Houttuyniae Injection by aerosol inhalation. The 45 case control studies of acute attack and delayed periods of chronic brochitis showed no statistically difference between them. The clearance rate of bacteria in sputum after treatment was 80.39%. Conclusions: Herba Houttuyniae Injection by aerosol inhalation is effective for respiratory tract infection with the main symptoms of cough and expectoration.
8.The clinical significance of acute insulin resistance during surgical sepsis

Objective To investigate the clinical significance of acute insulin resistance during surgical sepsis. Methods Forty surgical patients with sepsis in experimental group and twenty patients with similar APACHEⅡ but without sepsis in experimental control group were enrolled in our study. In the experimental group, the patients were categorized to sepsis group and septic shock and MODS(SM)group according to clinical manifestations, and were also grouped according to different causes of sepsis, i.e.severe acute pancreatitis(SAP)group, intestinal fistula group and other diseases group. Determinations of fasting blood glucose, fasting plasma insulin, clinical nutritional parameters, plasma TNF-α, APACHEⅡ, sepsis-related organ failure assessment(SOFA)and severe scores were simultaneously performed at designated time points. Insulin resistance index was calculated using homeostasis model assessment(HOMA)to assess insulin sensitivity of surgical patients with sepsis. Results (1)A significant elevation of lgHOMA-IR levels was found in 84% of tests in sepsis. Peak lgHOMA-IR values increased in all patients.(2)LgHOMA-IR was significantly higher in patients with sepsis than those without sepsis from the first 24 hours(1.22±0.23 vs 0.73±0.21, P<0.01)to the 7th day(0.46±0.32 vs 0.30±0.13, P<0.01).(3)There was no significant difference in lgHOMA-IR among SAP, intestinal fistula and other groups at different times(P>0.05).(4)There was significant difference in lgHOMA-IR between sepsis group and SM group during the whole septic episode(P<0.01).During the septic episode, there were significant correlation between lgHOMA-IR and APACHEⅡ(r=0.591, P<0.01), lgHOMA-IR and SOFA(r=0.496, P<0.01), lgHOMA-IR and SS(r=0.553, P<0.01), and lgHOMA-IR and TNF-α(r=0.658, P<0.01).(5)lgHOMA-IR was independently directly correlated with LDL, prealbumin, cholesterol and triglyceride. R2 of the equation was 0.188. Conclusion There usually is insulin resistance in surgical sepsis, regardless the primary diseases. Insulin resistance may simply be a marker reflecting an underlying physiological derangement, implying indicates higher mortality. Among several clinical nutrition parameters, LDL, prealbumin, cholesterol, and triglyceride showed closer relationship with insulin resistance in patients with sepsis.
9.Endovascular therapy for Stanford type B aorta dissection

Objective To evaluate the methods,applicability and results of cndovascular therapy for Stanford type B aortic dissection.Methods The clinical data of 36 cases of Stanford type B aortic dissection were analysed retrospectively.The stent-grafts were introduced via femoral arteriotomy.Stent-grafts were implanted to blockade tears of aortic dissection under the guidance of DSA.Results In all cases,the aortic dissection tears were blockaded,true lumens were opened,and organs function was restored.There were not complications such as internal hemorrhage due to trauma or stent displacement on CTA 3 and 12 months after endovascular therapywhile 2 cases developed new tears.The caliber of the 36 true lumens was restored to near normal.Twenty-two pseudocoeloms were obliterated,while the others were reduced is size and had varying degrees of thrombosis.Conclusions Endovascular thcrapy is safe and effectivc for the patients with Stanford type B aortic dissection.Its clinical application has a favorable outlook.
