Objective To investigate the family care needs in patients with primary hepatocellular carcinoma after transcatheter hepatic arterial chemoembolization (TACE).Methods One hundred and twenty patients of hepatocellular carcinoma with non first time of TACE were selected from August 2013 to June 2014 by convenience sampling method.Using a self designed questionnaire form,investigated patients' families care after discharge needs,including psychological demand,nursing requirements and other requirements.The independent factors affecting families psychological needs were analyzed by multi-factor analysis of variance.Results Patients' families care needs in dimensions,psychological demand was the highest score for (3.12±0.81) scores.Care requirements and other requirements were (3.04±0.56),(2.18±0.36) scores,respectively.Each demand entries,understand pain processing requirements was the highest score for (3.78 ±1.02) scores,hospice and funeral support was the lowest for (1.27±0.38) scores.Education degree,and patient relationship were independent factors affecting patients' families psychological demand,P＜0.05.Conclusions TACE reduces quality of life of patients,the role of families in the care of patients is also important.Care needs of families after TACE,the psychological demand should be satisfied first,required at the same time attention to relevant nursing requirements and other requirements;while education from families and relationships with patients are independent factors affecting the psychological needs of patients' family members.

Objective To evaluate the effect of health education on schistosomiasis in Kunshan City,so as to provide the ev?idence for making the consolidating strategy in the late stage of interruption of schistosomiasis transmission. Methods The resi?dents,middle school students and elementary school students were randomly sampled from one community,one middle school and one elementary school of each of two towns and they were investigated with interviews and questionnaires for the implementa?tion of health education on schistosomiasis prevention and control. Results A total of 452 middle school students(232 cases) and primary school students (220 cases) were surveyed. The awareness rate of total schistosomiasis knowledge was 98.21%among the students(the awareness rate of basic schistosomiasis knowledge was 98.42% and the awareness rate of preventive schistosomiasis knowledge was 98.01%). Among the 220 elementary school students,the awareness rate of total schistosomiasis knowledge was 97.21%(the awareness rate of basic schistosomiasis knowledge was 97.60%and the awareness rate of preventive schistosomiasis knowledge was 96.82%). Among the 232 middle school students,the awareness rate of total schistosomiasis knowledge was 99.17%(χ2 =34.661,compared with the rate of the elementary school students)[the awareness rate of basic schistosomiasis knowledge was 99.20%(χ2=13.045,compared with the rate of the elementary school students)and the aware?ness rate of preventive schistosomiasis knowledge was 99.14%(χ2 =21.796,compared with the rate of the elementary school students)]. There were significant differences between the elementary school students and middle school students in above?men?tioned awareness rates(all P<0.001). There were schistosomiasis health education materials or teaching plans in all the four schools. Among the 402 residents surveyed,the awareness rate of total schistosomiasis knowledge was 98.87%. Conclusion The effect of health education on schistosomiasis prevention and control is very well,and the total awareness rate of schistosomia?sis prevention and control knowledge among the population has reached the goal(more than 95%)of the medium?and long?term planning of schistosomiasis prevention and control in Kunshan City.

Objective To explore the causes and treatment of difficulty of urination after suprapubic prostatectomy. Methods 56 cases with difficulty of urination after suprapubic prostatectomy were reviewed.The causes of difficulty of urination were bladder outlet stricture,urethral stricture,relapse of BPH and other nonsurgical causes. Results 48 cases were surgically managed,and the success rate was 94%.The follow up time was 2～84 months with a mean of 38 months,all of them urinate normally except in 3 suprapubic cystostomy has been mandatory. Conclusions Correct presurgical diagnosis,adequate surgical treatment and postsurgical management are key points in preventing postsurgical difficulty of urination.

Objective To explore the management of T 1G 3 transitional cell carcinoma of urinary bladder. Methods 67 cases of T 1G 3 transitional cell carcinoma,average age of 63,were treated with TURBt.Followed by intravesical bacillus Calmette-Guerin instillation in 59 cases and mitomycin C instillation in other 8 cases. Results Within median 47 (range 12～78) months follow-up,28 cases had recurrence.20 cases had tumors progressed to muscle invasion(T 2 or higher).16 cases had received total cystectomy and 4 cases had long-distance metastasis. 9 cases died from the tumor. Conclusions Patients who have T 1G 3 transitional cell carcinoma initially should be treated by TURBt and intravesical BCG instillation and followed rigorously.When the tumor recurs and progresses into muscle invasion,total cystectomy is preferred.

OBJECTIVE: To evaluate the in vitro MTT assay for determining the drug sensitivity in fresh human tu- mors. METHODS: We tested the samples from 75 patients with gastric cancer, 55 patients with colorectal carcinoma, 55 patients with bladder cancer and 6 patients with carcinoma of tongue. RESULTS: On the average, the MTT sensitive drug in the above - mentioned first three kinds of solid tumors corresponded with the drug used by clinican experience. The response rate in MTT sensitive group was 73. 8% (48/65) while only 34.0% (17/50) in the control group and 0% (0/10) in MTT resistant group. CONCLUSION: MTT assay appears to be useful in exploring the possible anticancer effect of arsenic trioxide on tongue cancer. This test demonstrats that the MTT assay is clinically useful on an individual basis to optimize chemotherapy.

BACKGROUND:Total knee arthroplasty mainly used fixed platform, but more and more movable platform was selected year by year in the clinic. Rotary platform theoretical y can apparently improve the curative effects, but it has not been verified in the clinic.
OBJECTIVE:To summarize the design characteristics and clinical results of Gemini MKII (Link, Germany) rotary platform.
METHODS:From January 2004 to January 2009, 108 patients (119 knees) including 31 males and 77 females were treated with total knee arthroplasty by using Gemini MKII system. There were 97 unilateral cases and 11 bilateral cases at the first stage of replacement. Preoperative diagnosis:1 case of hemophilic arthritis, 1 case of venereal Charcot’s disease, 8 cases of post-traumatic arthritis, 11 cases of rheumatoid arthritis, and 87 cases of degenerative osteoarthritis. The prosthesis was fixed with bone cement. None received patel ar replacement. RESULTS AND CONCLUSION:None experienced infection, femoral condyle fracture or nerve and vascular damage. Except bilateral replacement, the average operation time of unilateral replacement was (56±13) minutes. Average postoperative hemoglobin decreased (25±5) g/L. After replacement, hemoglobin maintained over 100 g/L in 83 patients (86%), without blood transfusion. Knee joint score and function score recovered to (92.0±4.7) points and (90.3±6.8) points at half years after replacement, and became (94.4±3.2) points and (91.6±5.9) points during final fol ow-up, which was significantly improved as compared with that pre-replacement (P<0.01). No revision surgery should be done for lining prolapse or spin out, osteolysis and prosthesis loosening. These results indicated that the early and midterm results of Gemini MKII rotating-platform prosthesis were satisfactory, but the long-term results remain to be confirmed.

Objective:To study the clinical effect of herba Houttuyniae Injection by aerosol inhalation for respiratory tract infection. Methods: 107 patients with respiratory tract infection (the main symptoms were cough and expectoration) were treated with Herba Houttuyniae Injection by aerosol inhalation. The conditions of symptom improvement and bacteria in sputum before and after treatment were observed. The case control studies on parts of patients in acute attack and delayed peroids of chronic bronchitis were carried out. The treatment processes patients were given 25mL Herba Houttuyniae Injection and 25mL normal saline by aerosol inhalation for 30 minutes, twice a day. Results: The effective rates were 91.59% for cough symptom, 85.06% for expectoration symptom. 98.13% patients experienced comfort of pharyngeal and thoracic protions after treatment. Among 55 patients whose cough symptom wasn't improved after they were treated with anti inflammatory agent and antitussive the effective rate was 72.72% after treatment with Herba Houttuyniae Injection by aerosol inhalation. The 45 case control studies of acute attack and delayed periods of chronic brochitis showed no statistically difference between them. The clearance rate of bacteria in sputum after treatment was 80.39%. Conclusions: Herba Houttuyniae Injection by aerosol inhalation is effective for respiratory tract infection with the main symptoms of cough and expectoration.

Objective: To detect the relationship between variant of rsl255372 in transcription factor 7-Like 2(TCF7L2) gene and the type 2 diabetes mellitus (T2DM) in Tianjin Han populations. Methods: Three hundred and fifty-two T2DM patients and 176 healthy controls were randomly selected to extract the genome DNA. Denaturing high performance liquid chromatography was used to detect the PCR amplified fragment. The different peaks were chosen for sequencing. The differences of different genotype frequency between the groups were analysed. The Logistic regression was used to evaluate the risk factors of T2DM. Results: The G/T genotype frequencies of rsl2255372 were 18.5% and 2.8% in T2DM group and healthy control group respectively (P < 0.05). The G/T genotype, glycosylated hemoglobin, urea nitrogen and the systolic blood pressure were the independent risk factors of T2DM, and the high density lipoprotein was the protective factor. There was no significant difference of the G/T genotype frequency in T2DM patients with different complications. Conclusion: This study indicates that G/T genotype in the TCF7L2 gene significantly contributes to T2DM susceptibility in Tianjin Han populations. The G/T genotype is one of the risk factors of the T2DM.

Objective To investigate the influence of three kinds of different Ca~(2+) concentration of dialysate on serum calcium,calcium-phosphorus product,serum PTH and CRP in maintenance hemodialysis patients.Methods 45 patients of maintenance hemodialysis with ages ≥18 years were selected.They were divided randomly three groups,and be given three different kinds of Ca~(2+) concentration of dialysate to carry out hemodialysis with 3 months.Then the patients' variation of blood pressure and whether there were low calcium spasm or ostealgia were abserved during hemodialysis whole range.In addition,changes of their serum calcium、phosphorus、iPTH and CRP at pretherapy and three months after reatment were also detected.Results After three months of hemodialysis,serum iPTH in patients with low Ca~(2+) concentration dialysate increased obviously,and the serum CRP decreased significantly(P0.05),and serum CRP increase notably(P
