1.The investigation for common hepatic cartery lymph node metastasis of esophageal squamous cell carcinoma

Objective:To investigation for common hepatic cartery lymph node metastasis of esophageal squamous cell carcinoma.Methods:From 2015 to 2017, 413 patients with esophageal squamous cell carcinoma who were dissected the common hepatic cartery lymp node were admitted in Cancer Hospital of Sichuan. The relationship between metastatic of rates of common hepatic cartery lymp node and the clinical data including sex, age, tumor site, diameter of tumor, invasion depth, differentiation degree, pathological stage, neurovascular invasion status were reviewed retrospectively.Results:A total of 10 649 lymph nodes were dissected.The lymph node metastatic rate was 45.27%(187/413), and the metastatic lymph node ratio was 7.96%(848/10 649). Lymph nodes adjacent to the middle esophagus, cervical esophagus, cardia of stomach had a higher metastatic rate, while the Lymph nodes adjacent to the hilar, supraclavicular, common hepatic cartery had a lower metastatic rate.A total of 775 lymph nodes were dissected in common hepatic cartery.The lymph node metastatic rate was 7.02%(29/413), and the metastatic lymph node ratio was 4.51%(35/775). To analysis the relationship between metastatic of rates of common hepatic cartery lymp node and the clinical data.We conclusion that the common hepatic artery lymph node metastatic rates of upper, middle and lower esophageal squamous cell carcinoma were 2.89%(2/69), 5.35%(12/224) and 12.50%(15/120), with significant difference.The common hepatic artery lymph node metastatic rates of patients with diameter of tumor under 3 cm, 3-5 cm and above 5 cm were 5.20%(9/173), 5.52%(9/163)and 14.28%(11/77), with significant difference.The common hepatic artery lymph node metastatic rates of sex, age, tumor site, diameter of tumor, invasion depth, differentiation degree, pathological stage, neurovascular invasion status, with no significant difference.Conclusion:The metastatic rates of common hepatic artery lymph node in esophageal squamous cell carcinoma is lower. For the upper thoracic esophageal cancer with tumor diameter under 5 cm, the dissection of common hepatic lymph node can be ommitted in surgery.
2.Evaluation of health education of medium-and long-term planning of schis-tosomiasis prevention and control in Kunshan City

Objective To evaluate the effect of health education on schistosomiasis in Kunshan City,so as to provide the ev?idence for making the consolidating strategy in the late stage of interruption of schistosomiasis transmission. Methods The resi?dents,middle school students and elementary school students were randomly sampled from one community,one middle school and one elementary school of each of two towns and they were investigated with interviews and questionnaires for the implementa?tion of health education on schistosomiasis prevention and control. Results A total of 452 middle school students(232 cases) and primary school students (220 cases) were surveyed. The awareness rate of total schistosomiasis knowledge was 98.21%among the students(the awareness rate of basic schistosomiasis knowledge was 98.42% and the awareness rate of preventive schistosomiasis knowledge was 98.01%). Among the 220 elementary school students,the awareness rate of total schistosomiasis knowledge was 97.21%(the awareness rate of basic schistosomiasis knowledge was 97.60%and the awareness rate of preventive schistosomiasis knowledge was 96.82%). Among the 232 middle school students,the awareness rate of total schistosomiasis knowledge was 99.17%(χ2 =34.661,compared with the rate of the elementary school students)[the awareness rate of basic schistosomiasis knowledge was 99.20%(χ2=13.045,compared with the rate of the elementary school students)and the aware?ness rate of preventive schistosomiasis knowledge was 99.14%(χ2 =21.796,compared with the rate of the elementary school students)]. There were significant differences between the elementary school students and middle school students in above?men?tioned awareness rates(all P<0.001). There were schistosomiasis health education materials or teaching plans in all the four schools. Among the 402 residents surveyed,the awareness rate of total schistosomiasis knowledge was 98.87%. Conclusion The effect of health education on schistosomiasis prevention and control is very well,and the total awareness rate of schistosomia?sis prevention and control knowledge among the population has reached the goal(more than 95%)of the medium?and long?term planning of schistosomiasis prevention and control in Kunshan City.
3.Main Pathogenic Bacteria Resistance and Application of Antibacterials of Urinary System Infection in T2DM

OBJECTIVE To investigate the variety of main pathogenic bacteria and their resistance in urinary system infection of type 2 diabetes mellitus(T2DM)in endocrinology department of our hospital and find out the best choice antibacterials of application.METHODS By retrospective analysis of clinical history,the results of urine bacterial culture,drug sensitivity reports of T2DM patients with urinary system infection were analyzed.The data were dealed with SPSS 11.0.RESULTS The drugs of experiential therapy were mainly quinolones.And drug resistant rates of Escherichia coli and Staphylococcus epidermidis to levofloxacin and ciprofloxacin were 18.2% and 63.6%,and 16.7% and 50.0% respectively.The treatment periods of 2 groups were(8.4?4.3)d and(10.4?5.4)d,respectively.CONCLUSIONS ?-Lactam/?-lactamase inhibitor and aminoglycosides are the first choice in experiential therapy.Thetreatment periods could be decreased according to bacterial culturingresults.
4.Application of Spiral CT in Atlantoaxial Lesion

Objective To evaluate the diagnostic value of three-dimensional CT(3DCT)in atlantoaxial lesions.Methods The findings of 3DCT were analyzed and compared with conventional CT and radiography in 124 patients with atlantoaxial disease.Results 124 cases of atlantoaxial lesion included fracture in 42,atlantoaxial rotatory dislocation in 73,hypoplasia of the dens in 4,and congenital atlantoaxial deformity in 5.Among the 124 cases,89 cases were diagnosed by X-ray,109 cases by conventional CT,121 cases by MIP,122 cases by SSD,124 cases by MPR and VR.Conclusion 3DCT can find the slight rotatory dislocation,so it is the most effective means for the diagnosis of atlantoaxial rotatory dislocation.3D and MPR of spiral CT are useful in the diagnosis of atantoaxial fracture.Combining conventional CT and X-ray film with 3DCT can improve the acuracy of the diagnosis.

7.Study on in vitro release consistency between self-prepared Torasemide Sustained-Release Tablet and original preparation

Objective To establish a method for determining the dissolution oftorasemide sustained-release tablet in vitro and study the methodology of the determination.The consistency of the in vitro release behavior between self-prepared torasemide sustained-release tablet and original preparation were evaluated by constructed method.Methods HPLC method was applied to detect the cumulative release percentage of self-prepared torasemide sustained-release tablet and original preparation in five kinds of release media (water,0.1 mol/L hydrochloric acid solution,pH 4.5 acetate buffer,pH 6.8 phosphate buffer,and 0.1 mol/L hydrochloric acid solution turn to pH 6.8 phosphate buffer).Similarity factor (f2) was used to evaluate the similarity of release curves.Results There was a good linear relationship between the quality concentration of torasemide and peak area in the range of 1.0-12.0 μg/mL (r =0.9995).Results of precision and stability tests were good,and the RSDs for probational liquid were all lower than 2.0%.The average recovery of accuracy test was 100.04%,and RSD was 0.54% (n =12).The homogeneity of within group of self-prepared preparation met the technical requirement,RSDs of each sampling points in six Dissolution Vessels were lower than 10.0%.The f2 factors of self-prepared torasemide sustained-release tablet and original preparation were 72,60,77,66,and 60 in five kinds of release media.Conclusion The method in the paper is suitable for the release test of torasemide,meanwhile,the self-prepared tablet shows consistent in vitro release behavior with that of the original preparation.
8.Diagnosis and treatment of bilateral bronchial foreign body in children.

OBJECTIVE:
To discuss the clinical characteristics of bilateral bronchial foreign body in children, increase the curative rate and reduce the complications.
METHOD:
The clinical data of 35 cases with bilateral bronchial foreign body in children were retrospectively analysed, including surgery, key surgery points, and postoperative combined therapy.
RESULT:
The foreign bodies of all cases were removed under intravenous general and tropical anesthesia without complications.
CONCLUSION
The bilateral bronchial foreign body is a serious case, the timely and effective treatment can lower the mortality rate and postoperative complications. The children lack of oxygen for a long time before and in operation should be give comprehensive therapy, for example: sedation and hyperbaric oxygen.

9.Effects of ureteral stent on renal pelvic pressure

Objective To explore the effects of ureteral stent on renal pelvic pressure and other urodynamic parameters. Methods Forty-one patients, 28 males and 13 females, with unilateral renal calculi and/or ureteral calculi were recruited in this study. The mean patient age was 47 years old (ranging from 20 to 72 years old). All cases were placed a 4.7 F ureteral stent and 16 F nephrostomy tube after minimal invasive pereutaneona nephrolithotomy (MPCNL). There was no hydronephrosis and residual crushed stone in the ureter after MPCNL in all cases. Renal pelvic pressure, intra-abdo minal pressure, detrusor pressure, bladder pressure changes during the filling and voiding phases with intravesical perfusion flow rate of 40 ml/min were recorded and analyzed. Results At the baseline, IPP0, IAP0, DP0 and BP0 were (33.1±17.0)cm H2O, (27.5±7.0)cm H2O, (3.3±2.9)cm H2O and (30. 9±7.2)cm H2O, respectively; At the maximum cystometric capacity during the filling phase, IPPvol, IAPvol Dpvol and Bpvol were (39.4±67. 3)cm H2O, (31.1±7.3)cm H2O, (10.7±6. 6) cm H2O and (41.6±10.3)cm H2O, respectively; At the maximum bladder pressure during the voiding phase, IPPmax, IAPmax Dpmax and Bpmax were (65.7±17.0)cm H2O, (33.7±9. 7)cm H2O, (41.9±7.8)cm H2O and (75.0±12. 8)cm H2O, respectively;There were statistical significance comparing between any of IPP0, IPPvol and IPPmax(P<0. 01). 27% (11/41)patients were with the pain in kidney area at voiding IPPmax (87.1±14.6) cm H2O, which was significantly higher than IPPmax (57.8±9.5)cm H2O of asyrnptomatic group (30 patients)(P<0. 01). In all cases, the renal pelvic pressure was higher than 40 cm H2O during the voiding phase. Conclusions Renal pelvic pressure increases during the filling phase after placing the ureteral stent, especially during the voiding phase. As renal function will be damaged by the high renal pelvic pressure, we should decrease the utilization of ureteral stent if possible. It is encouraged to remove the ureteral stent as early as possible.
10.Effects of the intervention with enriched environment and fluoxetine on the depression-like behavior and myelin basic protein of brain tissue in rats

Objective To investigate the effects of intervention with the fluoxetine and the enriched environment on chronic stress induced depression behavior of rats,and the changes of myelin basic protein in hippocampus and prefrontal regions.Methods 50 adult male SD rats were randomly divided into control group,fluoxetine group,model group,enriched environment (EE) group and EE plus fluoxetine group.Fluoxetine group,model group,EE group and EE plus fluoxetine group underwent chronic unpredictable stress stimulus in the first to third week,and fluoxetine group,EE group,EE plus fluoxetine group underwent the intervention with EE and (or) fluoxetine in the fourth to sixth week.The changes of behavior in rats were evaluated by sucrose water consumption,open field test and weight changes.The content of MBP in each subregion of hippocampus and prefrontal regions of rats was measured with immunocytochemical methods.Results At the third weekend,the assessed behaviors of stressed rats decreased significantly compared with control group (P<0.05);and at the sixth weekend,the behaviors of stressed rats restored after treated with EE and (or) fluoxetine.The content of MBP in the rat hippocampus CA1,DG area and prefrontal area of model group declined clearly compared with control group (mean density of model group orderly:0.199±0.024,0.204±0.021,0.225±0.028;control group orderly:0.279±0.034,0.288±0.043,0.308±0.053,P<0.05).The content of MBP in the rat of fluoxetine group,EE group and EE plus fluoxetine group increased obviously compared with model group (fluoxetine group orderly:0.259± 0.047,0.266± 0.052,0.284 ± 0.031;EE group orderly:0.257±0.038,0.258±0.042,0.286±0.037;EE plus fluoxetine group orderly:0.271± 0.046,0.279±0.040,0.289±0.041,P<0.05).Conclusion The depression-like behavior of rats induced by chronic unpredictable stress is associated with the change of the content of MBP in hippocampal CA1,DG area and prefrontal area;and the depression-like behavior and the content of MBP decrease are reversed after the intervention with fluoxetine and EE.
