1.Application of bone marrow mesenchymal stem cells in hematopoietic stem cell transplantation for sensitized and non-sensitized BALB/c mice

BACKGROUND:Hematopoietic stem cel transplantation has therapeutic effects on many diseases, but its application has some limitations, such as cel harvesting and age-limited number of cels.
OBJECTIVE: To investigate the application value of bone marrow mesenchymal stem cels in hematopoietic stem cel transplantation for sensitized and non-sensitized BALB/c mice.
METHODS: Bone marrow cels derived from BALB/c mice were isolated and culturedin vitro to harvest mesenchymal stem cels using adherent method. The cel surface markers were detected by flow cytometry. A murine model of sensitization was established by transfusion of alogeneic spleen cels. Mesenchymal stem cels labeled with green fluorescent dye were transplanted into non-sensitized and sensitized recipient mice, and the homing of mesenchymal stem cels in vivowas monitored at different time points post transplantation. Additionaly, under irradiation pretreatment, sensitized BALB/c mice under irradiation were subjected to combined transplantation of alogeneic bone marrow cels and syngeneic mesenchymal stem cels. Survival rate of BALB/c mice was monitored daily.
RESULTS AND CONCLUSION: At 48 hours after transplantation, mesenchymal stem cels in sensitized and non-sensitized recipients were homing to the spleen and bone marrow, respectively. In the experiment of hematopoietic stem cel transplantation, the sensitized recipients died at 12-15 days after combined transplantation, with a median of 14 days; however, the sensitized recipients only undergoing alogeneic bone marrow cel transplantation had a survival median of 13 days. These findings indicate that the transplanted mesenchymal stem cels in sensitized recipients are mainly homing to the spleen and bone marrow, but the combination transplantation cannot enhance the transplantation of alogeneic hematopoietic stem/progenitor cels in sensitized recipients.
2.Screening for epigenetically masked genes in Kashin-Beck disease by microarray

Objective We used the DNA methylation microarrays to investigate the differential methylation genes and loci sites in Kashin-Beck disease (KBD),to study the relationship between DNA methylation and KBD pathogenesis.Methods Totally 12 KBD adults and 12 healthy adults were selected and peripheral blood samples were collected and DNA was extracted.Illumina 450K bead-chip was applied to detect methylation status in KBD and healthy controls.Aberrant hyper-methylated sites were filtrated according to the P value after correction and methylation differences,together with GenomeStudio soft.Screened genes were validated using bisulfite sequencing polymerase chain reaction (BSP) technology.Results A total of 484 948 loci sites were analyzed and compared,93 differential methylated loci were found by comparing KBD and normal people,including 34 hypermethylated sites and 59 hypomethylated sites.There were 50 genes corresponding to the loci,43 genes not reported in literature.According to gene ontology analysis,the genes were involved in the immune response,antigen processing,phosphate and phosphoric acid metabolism and phosphorylation and the process of metal ions in combination.However,in the verification test using BSP method,there was no significant difference in methylation rate in human leukocyte antigen (HLA)-DRB1 between the case and the control group (48% vs 70%,x2 =3.688,P > 0.05).Conclusions The high and low differentially methylated sites in peripheral blood DNA of KBD patients are significantly different from those of the health control.HLA-DRB1 locus is not significantly different between the BSP verification test and methylation chip.
3.Effects of supplying iodine on thyroid autoimmunity in iodine deficiency subjects

Objective To study the effects of supplying iodine on thyroid autoimmunity in iodine deficiency subjects. Methods The levels and abnormal rates of immunoglobulins (IgA, IgG, IgM), complement C 3, and thyroid autoantibodies such as thyroid peroxidase antibody (TPOAb), thyroglobulin antibody (TGAb), thyrotropin receptor antibody (TRAb) and thyroid stimulating antibody(TSAb) were observed in inhabitants in iodine deficiency area before and after oral iodized oil capsules were administered. Results The levels of IgA, IgG and C 3 increased after iodine supplying for three months, the proportions of IgG and C 3 which exceeded upper limit of the normal range reached to 38..9% and 62.0% respectively. The levels of IgA and IgG began to decrease markedly six months after iodine supplying. The levels of TPOAb, TGAb and their positive rates after iodine supplying were all higher than those before administration, and reached the highest level between three to six months after iodine supplement. And 7 cases out of 79 or 64 subjects with 2 kinds of positive autoantibodies were observed 3 or 6 months after iodine supplying respectively. The cases were all adult women with the average age of 27.6 yr, and had no clinical symptoms. The levels of TRAb and TSAb after iodine supplying were much higher than that before iodine supplying. The rate of positive TRAb was increasedmarkedlyandreachedto24.4% twelve months later. Conclusion Iodine supplying for the subjects with iodine deficiency seems to induce immune reaction, increase the levels of autoantibodies, and might develop autoimmune thyroid diseases.
4.Common triggers of ischemic stroke

Ischemic stroke is often triggered under the effect of the basic lesions combined with a variety of incentives in daily life. Its generative mechanism is complicated, and there are more risk factors. Only according to the long-term existence of chronic risk factors, it cannot make appropriate explanation for the acute onset of this disease. At present, there are a few studies on the predisposing factors of stroke, and its related mechanism needs to be further discussed. This article aims to introduce the relationship between the common predisposing factors and ischemic stroke, discusses its possible mechanism, and provides reference for the clinical prevention work of ischemic stroke.
5.The clinical pathology analysis of papillary thyroid microcarcinoma with clinically node-negative sides of neck stage lymph node metastasis

Objective To investigate the clinical pathological factors of papillary thyroid microcarcinoma (PTMC) with clinically node-negative sides of neck stage central lymph node metastasis.Methods Analysis of 136 cases of cNo stage of PTMC of clinical and pathological data,and using the PCR method for the detection of BRAFV600E mutation in the paraffin coated tissue.Results One hundred and thirty-six cases with PTMC lymph node metastasis rate was 38.2% (52/136),BRAFV600E mutation rate was 44.9% (61/136).Single factor analysis showed that the lymph node metastasis was related with BRAFV600E mutation and capsular invasion (P< 0.05).The size of the tumor was close to significant level (P=0.057).Multiple regression analysis showed that BRAFV600E mutation and capsular invasion were independent factors for affecting the central lymph node metastasis (P < 0.05).Conclusion The routine central lymph node dissection should be performed in patients with BRAFV600E mutation and tumor invasion.
6.IODINE NUTRITION STATUS OF POPULATION IN THE AREAS WITH DIFFERENT IODINE CONCENTRATIONS OF DRINKING WATER

Objective:To study the master iodine nutrition status of population living in areas with different iodine in drinking water and the cut-off point of stopping iodized salt supply and the range of adequate iodine intake for the strategy of prevention from iodine deficiency or excess. Method:Thirteen townships points along the Yellow River downstream in Shandong were investigated and divided into six groups(A-F) based on water iodine concentrations of 0-50,50-100,100-150,150-300,300-800 and over 800 ?g/L. The indicators of water iodine,edible salt iodine,urinary iodine(UI) and thyroid goiter were observed. Results:From A,B,C,D,E and F group,the medians of water iodine concentration were 20.3,91.4,143.2,203.6,341.9 and 812.3 ?g/L,of edible salt iodine were 0,25.7,25.8,30.4,36.4 and 33.0 mg/kg,and of UI were 116.8,354.2,400.4,607.9,881.3 and 1 213.8 ?g/L respectively. The goiter rates were 10.8,8.6,15.0,14.2,14.9 and 25.0 % respectively . The sample proportion of UI below 100 ?g/L was 8.2%,of 100-300 ?g/L was 18.2% and of over 300 ?g/L was 73.6%. Frequency excursion of UI had a high trend towards water iodine concentration over 90?g/L in 5 groups. The proportion of UI over 300 ?g/L increased,consistent with higher water iodine level. Median UI of all groups with iodized salt decreased significantly,especially group F. There were significant correlations between UI and water iodine. Iodine nutrition status was markedly excessive in group B,C,D,E and F. Conclusions:Iodine nutrition status of most residents consuming drinking water with iodine concentration about 20 ?g/L should be adequately supplied with eligible iodized salt. Iodine intakes in groups B-F were excessive,and iodized salt supply should be stopped there. The cut-point of water iodine level where iodized salt stopped could possibly be 90 ?g/L,and the range of adequate iodine intake,in terms of drinking water,could be defined at range of over 20 to 90 ?g/L in iodine concentration.

Objective To explore the nursing strategies for preoperative ventilator-dependent patients undergoing discontinuing mechanical ventilation after lung transplantation.Methods The clinical data and nursing experiences of 10 preoperative ventilator-dependent patients undergoing discontinuing mechanical ventilation after lung transplantation from September 2002 to December 2008 were retrospectively analyzed.Results The time of discontinuing mechanical ventilation ranged from 7 to 30 days and the average time was 16.5 days.Conclusion The key points to ensure the success of discontinuing mechanical ventilation were careful observation of ventilation modes,preparations before discontinuing,airway management,nutritional support and psychological nursing.

10.A clinical analysis of reninoma-induced hypertensive crisis associated with reversible posterior encephalopathy syndrome

Objective Reninoma is a rare benign tumor of the renal juxtaglomerular cell apparatus that causes hypertension and hypokalemia via hypersecretion of renin,while it is extremely rare that reninoma induced hypertensive crisis with reversible posterior encephalopathy syndrome (RPES).To improve the clinical understanding for this disease,we conducted a case-analysis.Methods To analyze the clinical and pathological data of a case of reninoma-induced hypertensive crisis with reversible posterior encephalopathy syndrome,who was admitted to Peking University First Hospital in November,2007 and follow-up.Results This was a 16-year old female patient,onset with suddenly spasm with loss of consciousness,while blood pressure stepped up to 210/140 mm Hg( 1 mm Hg =0.133 kPa),and the head magnetic resonance imaging (MRI) revealed “multiple long-T2 signal”,and hypopotassaemia(2.8-3.2 mmol/L),urine protein positive,ultrasoundcardiogram revealed left ventricular hypertrophy,laboratory study revealed hyperreninism (38.23ng · ml-1 · h-1,normal range 0.07-1.15 ng· ml-1 · h-1 ) and hyperaldosteronism(660.9 ng/L,normal range 60-174 ng/L),abdominal CT-Scan revealed a mass at right kidney,blood pressure achieved safety range and the head MRI was rechecked and revealed “the abnormal long-T2 signal disappeared”.The clinical diagnosis was reninoma induced hypertensive crisis with RPES.The tumor was resected and the pathologic diagnosis was reninoma.The patient remained normotensive in the postoperative period without any medication.Conclusions Reninoma represents a rare but surgically curable cause of hypertension,thus the clinical suspicion of it is very important in young patients.If the diagnosis is confirmed,positive treatment must be done immediately to improve the prognosis.The most common cause of RPES is hypertension,and the diagnosis depends on the distinctive head MRI.There is always a good prognosis with the decline of blood pressure rapidly.
