1.Duration of active phase affects delivery mode and pregnancy outcomes

Objective To investigate the effects of duration of active phase on delivery mode and pregnancy outcome.Methods Data of 417 women with abnormal active phase identified from 1744 term-birth,singleton,cephalic presentation primiparas who had tried vaginal delivery in Peking University First Hospital from January 1,2009 to December 31,2009 were retrospectively studied.Effects of different durations of protracted active phase on pregnancy outcomes were compared between women with vaginal delivery or cesarean section by Chi square test.Results The incidence of protracted active phase was 23.9% (417/1744); and the incidence of prolonged active phase was 2.9% (50/1744).There were 205 cases of cesarean section and 212 cases of vaginal deliveries.The incidence of neonates hospitalization in Department of Pediatrics in vaginal delivery group with active phase ≥6 h was higher than that of those with active phase <6 h [20.6% (26/126) vs 8.1% (7/86),x2 =6.07,P<0.05].The incidence of intrauterine infection in cesarean section group with active phase <6 h were higher than that of those with active phase ≥6 h [22.7% (30/132) vs 6.8%(5/73),x2 =8.37,P<0.01].In vaginal group with protracted active phase before 6 cm of cervical dilation,the incidences of postpartum hemorrhage (4.8%,9/186),maternal complications (3.8%,7/186) and neonates hospitalization (15.6%,29/186) were similar with those after 6 cm of cervical dilation [6.2% (1/16),x2=0.12; 0.0% (0/16),x2=0.01 and 12.5% (2/16),x2=0.00; all P>0.05] respectively.In cesarean delivery group with cervical dilation ≥6 cm,the incidence of neonates hospitalization was higher than that of those with cervical dilation <6 cm [60.0% (6/10) vs 19.9% (34/171),x2 =8.83,P<0.05].There were no difference in maternal age,gestational age,body mass index at delivery,volume of postpartum hemorrhage and neonatal birth weight between women with cesarean section whose protracted active phase ≥4 h or <4 h when cervical dilation at 3 cm (P> 0.05 respectively).The incidence of neonates hospitalization was low in women whose indication of cesarean section was protracted active phase (11.2%,9/80),while it was high when protracted active phase complicated with intrauterine infection (42.1%,16/38).Conclusions Protracted active phase with cervical dilation less than 6 cm might not need active management if neither the mother nor the fetus is compromised.The infants born vaginally should be closely monitored if the active phase is over 6 h.
2.Correction of penile curvature by allogeneic acellular dermal matrix

ObjectiveTo evaluate the effect of lengthening the short-side albuginea of the bending penis by the allogeneic acellular dermal matrix ( Allo-ADM ) for the treatment of penile curvature.MethodsFrom Jun 2007 to Jun 2010,18 patients with penile curvature due to malformation of the albuginea cavernous body were treated.The age of the patients ranged from 15 to 26 years (mean,20 years).Twelve patients were married.The curvature degree ranged from 30° to 80° (mean,55°).There were 17cases of single curvature and 1 case of complex curvature.The grafts ( Allo-ADM ) of different sizes were sutured to the albuginea at the curvatus side of the penis according to the extent of penile curvature through a circumcision incision.The extent of penile curvature and complications were evaluated postoperatively.ResultsPenile curvature was corrected in all 18 patients after the operation.No infection,hematoma and abnormal erection occurred postoperatively. No erectile dysfunction and penile re-curvature was observed during the follow-up period of 3 to 24 months.ConclusionLengthening the short-side albuginea of the bending penis by Allo-ADM could be a safe and effective way to correct penile curvature.
3.The protective effect of TanshinoneⅡA on rat models of uterine ischemia/reperfusion injury

BACKGROUND:The phenomenon of temporary ischemia of the tissues and organs caused by injury is common in the clinic. However, there are few studies on the treatment of ischemia/reperfusion injury. It is significant to investigate the effect of TanshinoneⅡA on improving uterine ischemia/reperfusion injury in a rat model. OBJECTIVE:To observe the effect of TanshinoneⅡA intervention on improving uterine ischemia/reperfusion injury in rats. METHODS:The rat models of uterine ischemia/reperfusion injury were established by suture method. The rats that were intraperitonealy injected with Sodium Tanshinone IIA Silate injection (20 mg/kg) at 5, 3, 1 days and 30 minutes before the operation were included in the experimental group. Model and sham operation groups were established for comparison. RESULTS AND CONCLUSION:Spectrophotometry and immunohistochemical staining results showed that at 1 and 3 hours of ischemia/reperfusion, the malondialdehyde concentration and Bax protein expression in the injuried rat uterine tissue were significantly decreased (P < 0.05), superoxide dismutase (SOD) activity and Bcl-2 protein expression were significantly increased (P < 0.05) in the experimental group than in the model group. The results confirm that TanshinoneⅡA has a protective effect on uterine ischemia/reperfusion injury in rats.
4.Superomedial pedicle reduction mammaplasty based on Würinger''s horizontal septum

Objective To discuss the surgical techniques and efficacy of a new method of breast treatment of hypermastia. Methods From January 2009 to February 2011, a series of 15 patients underwent a new technique for reduction mammaplasty. This technique involved a superomedial pedicle nipple-areolar complex (NAC). Results In our series, the resection weight per breast was (522. 9±218.3) g and the new nipple-to-sternal-notch distance was (21.7± 1.74) cm postoperatively. Mean nipple elevation was 8. 5cm (range, 4-16 cm). There were no hematoma and partial or complete NAC necroses occurred in our series. Minimal wound dehiscence occurred in one case ( 6.7 % ). Thirteen patients were followed up for 6 to 18 months. NAC sensibilities were comparable before and after operation, and good breast shapes were produced in all cases. Conclusion The superomedial pedicle technique provides good results with respect to NAC viability and sensibility, breast contour and last superomedial fullness.
5.Antimicrobial Sensitivity of Chryseobacterium meningosepticum:Analysis of 36 Clinical Isolates

OBJECTIVE To approach to the point of clinical diagnosis and treatment in patients with Chryseobacterium meningosepticum(CM) infection,and test the high sensitivey antibiotic in order to provide evidence for clinical rational drug utilization.METHODS We had retrospectively analyzed the 36 clinical infections with CM during 2003.A microdilution broth method was used to detect the C.meningosepticum MIC and metallo-?-(lactamases-)production.RESULTS C.meningosepticum infections related to the critical underlying diseases,old age,stay in hospital,nosocomial infections and broad spectrum antibiotics use.Metallo-?-lactamases-producing accounted for 100% in all CM isolates.CM were susceptible or partly susceptible to VAN,CTX/CA,PIP/SU,CFP/SU,PIP,CIA,PIP which were 100%,100%,91.7%,88.9%,86.1%,72.2%,and 44.4%,respectively.The(susceptibility) to other 21 kinds of antimicrobials were in 8.3%.CONCLUSIONS CM is multi-drug resistant.The main risk factors of CM infection are old age,long-term in hospital,critical underlying disease,and decreased immunological function.VAN,CTX/CA,PIP/SU, CFP/SU,PIP,CIA,and PIP are the effective antibiotics(against) CM.
6.Experimental study on the proliferation and invasion activity of the c-maf gene inhibited multiple myeloma RPMI8226 cells

Objective:To investigate the effect of c-maf gene on the MM cells' proliferation and invasion activity. Methods:Lipo-fectin Reagent was used to transfect c-maf siRNA into multiple myeloma cell of RPMI8226. The mRNA expression level of c-maf was detected by RT-PCR.Cell growth curve was measured by MTT assay. Transwell chamber test was used to measure MM cells' in vitro in-vasion activity. The cell cycle distribution were assessed by flow cytomentry. The protein expression levels of survivin,MMP-2, MMP-9, ARK5 and cyclin B1 were detected by Western blot. We also detected the activity of Caspase-3/7. Results:The c-maf siRNA was effectively transfected into cells and the mRNA expression of the c-maf gene was inhibited.MTT test and Transwell chamber test showed that the proliferation and in vitro invasion activity of transfected cells were significantly lower than those of other two groups (P<0.05). Cell cycle of c-maf siRNA transfected group cells was arrested in G2/M phase. The expression levels of survivin,MMP-2, MMP-9,ARK5,cyclin B1 and the activity of Caspase-3/7 between c-maf siRNA transfected group and the other two groups were sta-tistically different (P<0.05). Conclusion:c-maf gene by c-maf siRNA can remarkably inhibit proliferation and invasion of multiple my-eloma cell lines of RPMI8226. C-maf gene may be used as the target for multiple myeloma gene therapy.
7.The association between body fat mass and diabetes mellitus, hypertension and dyslipidemia in an elderly population in China

Objective To identify the threshold of percentage of body fat (BF%) associated with diabetes mellitus,hypertension and dyslipidemia in an elderly Chinese population.Methods An individual was considered as a subject if they exhibited any one of the three cardiovascular disease risk factors:diabetes mellitus,hypertension or dyslipidemia.A total of 1 140 subjects were recruited from the Physical Examination Center of Tianjin Hospital,China (male 511,female 629,aged ≥60 years).Questionnaire interview,physical examination,body composition measurement,blood biochemistry measurement,muscle strength measurement and other measurements were performed for each subject.Receiver operating curves (ROC) were created to determine the optimal percentage of body fat cutoff points,with the presence of diabetes mellitus,hypertension or dyslipidemia as outcome variable.Results The percentage of subjects with at least one of the following conditions (diabetes mellitus,hypertension or dyslipidemia) was 62.8% for men and 73.9% for women.Increased percentage of body fat was associated with an increased occurrence of diabetes mellitus,hypertension or dyslipidemia in the elderly,particularly among women.The ROC curve shows that the threshold of fat percentage in men and women was 28.2% and 36.6%,respectively.Conclusion The risks of diabetes,hypertension and dyslipidemia is closely related to elevated body fat mass in the elderly.The percentage of body fat mass is an important indicator of obesity,threshold of BF % can provide reference for clinical obesity intervention in elderly patients.It also can provide reference for the treatment and prevention of cardiovascular disease.
8.Establishment of liver transplantation model with improved two-cuff in rat

Objective To build rat orthotopic liver transplantation model and use this model as an experimental animal model for further research about liver transplantation.Methods Based on the classic two-cuff technique,a variety of skills were integrated,such as abdominal aorta perfusion first before ligation,suprahepatic inferior vena cava single continuous suture,preoperative and postoperative rehydration,hepatic portal vein and infrahepatic vena cava blocked by mosquito's haemostatic forceps.At last,the postoperative survival rates of 2 days and 2 weeks were recorded.Results Donors' operation time was (35±2) min,cuffing time was (7.0±1.5) min,receptors' operation time was (80±15) min,anhepatic phase was (21±3) min.The 2-day survival rate was 92 % (46/50) and 2-week survival rate was 86 % (43/50).Conclusion To some extent,the improved methods can not only reduce the difficulty of orthotopic liver transplantation operation,but also to create a uniform,stable and reliable orthotopic liver transplantation model.
9.The comparative study of deficit/non-deficit schizophrenia serum proteins

Objective To explore differently expressed proteins in serum samples obtained from Deficit Schizophrenia as well as Non‐Deficit Schizophrenia patients with proteomic techniques .Methods Make the comparative study of serum proteins between DS and NDS patients with two‐dimensional electrophoresis .The screened proteins were identified by mass spectrometer .Results Dur‐ing 18 differently expressed protein points screened in this study ,15 proteins were identified by mass spectrometer .Among of them , 12 proteins are up‐regulated while 3 proteins are down‐regulated in DS patients group .Conclusion It′s the first time to make the comparative study of serum protein profiles between DS and NDS patients by proteomic analysis .We expect that this study will contribute to better understanding of the pathophysiology of DS and explore the potential disease‐associated biomarker ,improve the future study for clinical medication of schizophrenia .
10.Effect of liver detoxification soup combined with compound glycyrrhizin on hepatitis related factors in patients with chronic hepatitis B

Objective To investigate the effect of liver detoxification soup combined with compound glycyrrhizin on hepatitis-related factors in patients with chronic hepatitis B.MethodsFrom April 2014 to August 2015, 86 patients with chronic hepatitis B were enrolled in this study.Random number table was divided into observation group and control group (43 cases).The control group was given routine glycyrrhizin and other conventional (ALT), serum total bilirubin (TBil), serum albumin transferase (ALT) and serum total bilirubin (TBil) were measured before and after treatment.(HA), laminin (LN), type Ⅳ collagen (Ⅳ-C) and type Ⅲ procollagen (PCⅢ) were used to analyze the effect of hepatic fibrosis, And observe adverse reactions.ResultsThe effective rate of the observation group was 93.02% higher than that of the control group (76.74%, P<0.05).After treatment, the patients were treated with chest discomfort, emotional depression, epigastric fullness, fatigue, ALT (35.28±1.35) U/L, HA (165.25±1.44) mg/L, and the treatment group (P<0.05).After treatment, the levels of TBil (30.34±1.59)μmol/L, Alb (48.32±1.33)g/L, and the level of serum was significantly higher than that of the control group (P<0.01).The incidence of adverse reactions was 11.63% and 6.98% in the two groups.ConclusionTraditional Chinese medicine hepatectomy decoction combined with compound glycyrrhizin can effectively treat chronic hepatitis B and improve the level of serum hepatitis-related factors, and the adverse reaction is light, it is worthy of clinical application.
