1.The analysis of glucose tolerance test results of pregnant woman in Nanjing Liuhe area

Objective To analyse oral glucose tolerance test (OGTT) screening results of pregnant women in Liuhe District of Nanjing and understand the incidence of gestational diabetes mellitus(GDM) in the region .Methods From July 2014 to March 2015 ,1 308 cases of healthy pregnant women who received 75 g OGTT in Nanjing Liuhe People′s Hospital for 24 to 28 weeks were enrolled in the study .When people were with fasting plasma glucose ≥5 .1 mmol/L ,OGTT was not necessary for a diagnosis of GDM to be made .According to the American Diabetes Association′s diagnostic criteria for gestational diabetes ,the number of gesta‐tional diabetes and the detection rate was calculated .According to the pregnant women′s age ,they were divided into three groups :18- <25 years old (group A) ,25- <29 years old (group B) and ≥29 years old (group C) ,the GDM detection rate were compared statistically among groups .Results The total detection rate of GDM in the region was 7 .49% (98/1 308) .The rate of group A ,B and C were 5 .24% (20/382) ,6 .97% (48/689) and 12 .66% (30/237) respectively .The detection rate of GDM among different age groups were statistically different(χ2 =6 .477 ,P<0 .05).There was no statistically significant difference of detection rate between group A and group B (χ2 =1 .238 ,P>0 .05) .There was statistically significant difference between group A and group C in GDM detection rate(χ2 =10 .853 ,P<0 .05) ,and also group B and group C was significantly different(χ2 =7 .406 ,P<0 .05) .Conclusion If pregnant women were older than 29 years old ,the incidence of GDM increased significantly .
2.HSV AMPLICON-MEDIATED NEUROTROPHIN-3 TRANSDUCTION PREVENTS MOUSE SPIRAL GANGLION NEURONS FROM DAMAGE

Objective To determine whether NT 3 overexpression could abrogate DDP toxicity in vitro . Methods We constructed a herpes simplex virus(HSV) amplicon vector to transduce a c myc tagged version of rat NT 3. In the HSVnt 3myc vector, the chimeric neurotrophin cDNA was placed under the transcriptional control of the CMVIE promoter. The resultant vector was packaged utilizing the recently developed helper virus free method. Results Transduction of NT 3myc in cultured mouse cochlear explants at a multiplicity of infection(MOI) of 0 25 resulted in production of NT 3 up to 3?g/L over a 48 hours period. The mouse cochlear explants were transduced with HSVnt 3myc or HSVmiap(control vector expressing the reporter gene, murine intestinal alkaline phosphatase) for 48 hours and the exposed to cisplatin for 48 hours.The cochlear explants transduced with HSVnt 3myc had a significantly greater number of SGN survival than the control group. NT 3 powerfully enhanced the process length and the density of SGNs.Conclusion These data demonstrate that the neurotrophic cDNA transduction via HSV amplicon helper free virus system can attenuate the ototoxic action of DDP on organotypic culture. The potency of NT 3 in protecting spiral ganglion neurons from degenerating suggests that neurotrophins might be useful for the prevention or treatment of hearing disorders. [
3.Influence of gestational diabetes mellitus on pregnancy and delivery outcomes

Objective To explore the influence of gestational diabetes mellitus(GDM)on pregnancy and delivery outcomes.Methods 278 pregnant women with GDM were chosen as experimental group who underwent their antenatal care and delivery in Liuhe People's Hospital in Nanjing,according to the 75g oral glucose tolerance test(OGTT)performed in pregnant women at 24-28 pregnancy weeks from January 2014 to October 2015.Meanwhile,the other 278 healthy women were chosen as control group.The pregnancy and delivery outcomes of the two groups were compared.Results The 24-28 weeks' 3 time-point OGTT blood glucose (FPG,1 h PG and 2 h PG) (mmol/L) values of experimental group were 5.08±0.56,9.22±1.71 and 7.62±1.48 respectively,which were significant higher than that of control group:4.45 ± 0.43,7.76 ± 1.35,6.34±4 0.96 (P<0.05).The incidences of pregnancy-induced hypertension syndrome,premature rupture of membranes,premature birth,fetal distress,postpartum hemorrhage,macrosomia and cesarean delivery of experimental group were significantly higher than that in control group (P < 0.05).Conclusion GDM has a relatively large effect on pregnancy outcomes,causing serious complications of maternal and infant.Therefore,glucose metabolism during pregnancy should be surprised closely and education should be conducted to improve maternal and neonatal outcomes.
4.Regulatory effect of glycyrrhizin on the expression of interferon (IFN)-γ and tumor necrosis factor (TNF)-β in peripheral blood mononuclear cells of patients with alopecia areata

Objective To observe the regulatory effect of glycyrrhizin on the expression of IFN-γ and TNF-β in peripheral blood mononuclear cells (PBMCs) of patients with alopecia areata.Methods PBMCs were obtained from 18 patients with mild alopecia areata,24 patients with severe alopecia areata and 20 normal human controls,and cultured with phytohemagglutinin (PHA) or the combination of PHA and glycyrrhizin for 24 hours.Then,reverse transcription (RT)-PCR was conducted to detect the mRNA expression of IFN-γand TNF-β in these cells.Results The mRNA expression levels of IFN-γand TNF-β in PBMCs were significantly higher in patients with severe alopecia areata than in those with mild alopecia areata and normal human controls (all P < 0.05),and higher in patients with mild alopecia areata than in normal human controls (both P < 0.05).A significant decrease was observed in the mRNA expressions of IFN-γ and TNF-β in the PBMCs from patients with alopecia areata after stimulation with the combination of PHA and glycyrrhizin (both P <0.05).Conclusion Glycyrrhizin can inhibit the expression of Th1-type cytokines and reverse Th1-type immune response.
5.Advance of treatment for newly diagnosed multiple myeloma

Multiple myeloma is a plasma cell malignancy.At diagnosis,most patients are older than 65 years,In newly diagnosed myeloma patients younger than 65 years,high-dose of melphalan followed by autologous stem cell transplantation(ASCT)is regarded as the standard treatment.In elderly patients,usually older than 65 years,oral melphalan and prednisone(MP)has been considered as the standard regimen recently.However,the discovery of thalidomide,lenalidomide,and the proteasome inhibitor bortezomib will increase the effect of clinical treatment.This review will summarize literatures and focus on the treatment of newly diagnosed myeloma.
6.Impact of calcium-sensing receptor on hyperparathyroidism

Calcium-sensing receptor(CaSR)is expressed in human parathyroid cells and renal tubular epithelial cells mainly.CaSR could control parathyroid hormone(PTH)secretion and the renal tubular's handing of the electrolytes and water.This receptor plays a central role in extracellular calcium homeostasis,and involves in the process of bone remodeling.The change of CaSR function,including anti-CaSR antibodies,CasR gene mutations or any part impairment in CaSR signaling pathway,not only results in the imbalaance of PTH and calcium homeostasis.but also relates to disorders of bone remodeling and renal tubular's mishandling in the electrolytes and water,which contribute to hyperparathyroidism.It makes possible clinical Use of cinacalcet in hyperparathyroidism in theory.
7.Histopathological observation after radial optic neurotomy in human eyes

Objective To explore the histopathological changes of the radial optic neurotomy (RON) in human eyes, and to establish the theoretical foundation for the effective RON. Methods Ten patients with unaffected eyeball or optic disc who had undergone ophthalmectomy (7 patients) or orbital exenteration (3 patients) because of intraocular or orbital tumor were gathered. A double-incision in pars plana was performed. One was inserted into illuminating fiber, and another was inserted into a standard microvitreoretinal (MVR) blade (unbent MVR blade in 4 patients and bent ones in 6; radial incision on nasal side of the optic disc in 4 patients and on both nasal and temporal side in 6). The histopathological examination was performed to observe the location and depth of the incision. Results Eleven incisions were found in 8 out of 10 patients, of which surgical spaces can be observed clearly. Three incisions were obliquely inserted into the optic nerves, 5 sieve plate lateral incisions had the surgical intervals connected with the subarachnoid spaces, and 3 incisions caused obvious damage of retinal ganglion cell axons due to the position close to the center. Nine incisions approached to the retrolaminal level, and the other 2 reached the laminal and prelaminal level respectively. Injured retina beside the optic disc in 1 incision was found. Central retinal vessels were not damaged. Conclusions An optimal incision may be in the edge of the lamina cribrosa, whose depth should be in the level of lamina cribrosa and retrolamina, with surgical intervals connected with the subarachnoid spaces.
8.Diagnosis and treatment of primary omental torsion: an analysis of 12 cases

Objective To analyze the clinical features, diagnosis and treatment of the primary omental torsion (POT).Method The clinical data of 12 patients with POT admitted to our hospital from May 2010 to May 2015 were retrospectively reviewed.The clinical manifestations, laboratory investigation, medical image, treatment and outcomes of patients were analyzed.Results Twelve cases were all males with median age of 50.The POT was diagnosed during surgical exploration and confirmed pathologically after operation.Right abdominal pain was the main complaint in 11 cases, and pain migration to right lower quadrant in 1 case.In physical examination, the local peritonitis signs were elicited, including right quadrant tenderness and rebound tenderness at same location.All cases had mal temperature except one presenting low fever of 37.5 ℃.White blood cell counts was 12×109/L in one case, and(4-10)109/L in other 11 cases.Abdominal solid mass was found in one case by ultrasonic scan, measured 7 cm×5 cm with unclear surrounding boundary.Mesenteric fat opacity and dropsy were shown in 10 cases on CT plain scan, while thickness and effusion of hepatic flexure of colon was found in one case;however, none of them were suggested as POT by these two image study before operation.All patients were gnosed as localized peritonitis or appendicitis clinically before emergent surgical exploration.The necrotic omentum was resected and all patients recovered smoothly.There were no complications in all cases during one-year follow-up.The CT images were reviewed after operation, which indicated that the mesentery of dropsy and the thickened hepatic flexure of colon should be the imaging signs of omental torsion;and also 9 of 12 cases had whirlpools sign.Conclusion The preoperative diagnosis of POT is difficulty, the typical whirlpools sign of abdominal mass on CT imaging is of highly value in diagnosis.The surgical removal of omental infarction is effective and the prognosis is good.
9.Long term efficacy of sodium hyaluronate on the treatment of moderate knee osteoarthritis

Objective To analyze the long term efficacy of sodium hyaluronate on the treatment of moderate knee osteoarthritis. Methods From January 2015 to December2016 year in our hospital , 84 patients with moderate knee osteoarthritis were participated in the study, and were randomly divided into two groups, 42 cases in each group. The control group were given oral glucosamine sulfate . Based on the treatment of the control group, the observation group were received sodium hyaluronate . The long-term effect in the two groups were compared. Results The excellent rate of knee joint function recovery was 80.95% in the observation group, and 54.76% in the control group, the difference in the two groups had statistically significant, P<0.05; Three months, nine months, and one years after treatment, the knee joint function Lysholm scores in the observation group were significantly higher than those in the control group; Three months, nine months, and one years after treatment, AIMS quality of life score in the observation group was significantly lower than those in the control group. Conclusion The long-term effect of sodium hyaluronate in the treatment of moderate knee osteoarthritis is obvious. It can help the knee joint function to be improved significantly, and promote the quality of life of patients significantly improved. It is worth to be widely applied.
10.The study of event related potential P_(300) in patients with anxiety disorder

Objective To investigate the characteristics of event related potential(ERP) P 300 in patients with anxiety disorder. Methods Thirty patients with anxiety disorder (AD group), diagnosed with CCMD 2R, and 30 normal people (NC group) were recruited. The odd ball auditory evoked model was used to test ERP P 300. Results All waveforms of P 300 were clear.
