1.Investigation on the Life Attitude of Medical Students in One Medical University in Wenzhou

Objective:To conduct an empirical study on the life attitude of medical students in one medical university in Wenzhou, and further strengthen the life education, to put forward some relevant suggestions for the prevention of recessive suicide and the cultivation of good life attitude of medical students. Methods: A stratified random sampling method was adopted to conduct the quantitative analysis to the survey results. Results: Most of medical students held positive attitudes to life, but some medical students still held negative attitudes to life (28. 6%), and had poor ability to master life common sense and skills (28. 6%). Medical students had weak ability to resist pressure, and academic stress was their major source of stress (27%) . Students easy to despise life, with the suicidal thought or behavior had a higher proportion (25. 8%). Different gender, grade, whether be single parent and economic level had significant significance for medical students to have suicidal thoughts or behaviors. Conclusion:By carrying out the belief education, survival education, setback education, love and care education and death education, it should cultivate the positive outlook on life of the medical students, so as to deliver qualified medical talents for the construction of "Healthy China".

Objective:To search and evaluate the literatures on the application of mechanical insufflation-exsufflation technique (MI-E) in patients with invasive mechanical ventilation in China and abroad, and to summarize the best evidence to provide evidence-based basis for clinical practice.Methods:The literatures related to the use of MI-E technique in invasive mechanical ventilation patients were searched from the establishment of the database to April 1, 2022 in BMJ Best Practice, UpToDate Clinical Advisor, Scottish Intercollegiate Guidelines Network (SIGN), Guidelines International Network (GIN), National Institute for Health and Care Excellence (NICE), National Guideline Clearinghouse (NGC), Registered Nurses' Association of Ontario (RNAO), medlive, Cochrane Library, Joana Briggs Instiute, Web of Science, PubMed, Embase, Cumulative Index to Nursing and Allied Health Literature (CINAHL), SinoMed, China National Knowledge Infrastructure (CNKI), Wanfang Database, China Science and Technology Journal Database, and Website of American Association for Respiratory Care, including guideline, expert consensus, clinical decision, evidence summary, systematic review and randomized controlled trial. The JBI Center for Evidence-Based Health Care Expert Consensus Evaluation Criteria (2016) was used to evaluate the quality of the included systematic reviews; the JBI Center for Evidence-Based Health Care Evaluation Criteria for Randomized Controlled Trials was used to evaluate the quality of the included randomized controlled trial. Two researchers independently evaluated the quality of literature, and extracted and summarized the evidence based on professional judgment.Results:A total of 7 literatures were enrolled, including 3 systematic reviews and 4 randomized controlled trials. After quality evaluation, 7 articles were all enrolled. Thirteen best evidences were formed from four aspects of indications, contraindications, parameter settings, and attention.Conclusions:The study summarizes the best evidence for the application of MI-E technique in invasive mechanical ventilation patients. It is recommended that medical staff undergo professional training, combined with their professional judgment as well as the patient's clinical specific conditions and willingness, and accurately apply MI-E technology to invasive mechanical ventilation patients.
4.Relationship between pancreatic fibrotic markers and glycated hemoglobin levels in patients with type 3c diabetes mellitus secondary to chronic pancreatitis

Objective:To investigate the relationship between pancreatic fibrotic marker transforming growth factor-β(TGF-β) and platelet derived growth factor-BB(PDGF-BB) and serum glycated hemoglobin (HbA1c) levels in patients with type 3c diabetes mellitus secondary to chronic pancreatitis(CP-T3cDM).Methods:The clinical data of 39 patients with CP-T3cDM admitted to the Department of Gastroenterology of the First Affiliated Hospital of Naval Medical University between February 2018 and August 2020 were collected, and the patients' age, gender, body mass index, duration of chronic pancreatitis and diabetes mellitus, smoking history, alcohol consumption history, serum HbA1c level at admission, degree of pancreatic atrophy, morphology of the main pancreatic duct, and treatment of diabetes mellitus were recorded. Serum TGF-β and PDGF-BB were detected by ELISA. Patients were divided into high and low level group according to the median TGF-β and PDGF-BB levels, respectively. Clinical characteristics of patients were compared between the TGF-β and PDGF-BB high and low level group. The correlation between TGF-β, PDGF-BB and HbA1c was analyzed by Spearman's correlation analysis.Results:A total of 39 CP-T3cDM patients were included; 35 were male and 4 were female. The age of first onset of chronic pancreatitis was (42±14) years old, and the duration of diabetes mellitus was 24(4, 36) months. The serum HbA1c level was (7.8±1.6)%, and the serum TGF-β and PDGF-BB levels were 20.5(10.5, 43.1) and 647.5(276.9, 1349.2)pg/ml, respectively. The serum HbA1c levels of patients in the high-level group of serum TGF-β and PDGF-BB were significantly higher than those in the corresponding low-level group [8.6%(7.4%, 9.9%) vs 6.7%(6.2%, 7.8%) and 8.6%(7.4%, 9.6%) vs 6.7%(6.1%, 7.8%), respectively] , and the difference was statistically different (both P value <0.01), while none of other indicators showed statistically significant differences between both groups. The correlation analysis showed that the levels of TGF-β and PDGF-BB were significantly positively correlated with HbA1c level ( r=0.45, 0.53, both P value <0.01). Conclusions:Increased pancreatic fibrosis in patients with CP-T3cDM was an important factor contributing to elevated blood glucose level. Patients with higher serum pancreatic fibrotic factors exhibited a significant increase in HbA1c level.

In 2009,the World Health Organization included snakebite on the list of neglected tropical diseases,acknowledging it as a common occupational hazard for farmers,plantation workers,and others,causing tens of thousands of deaths and chronic physical disabilities every year.This guideline aims to provide practical information to help clinical professionals evaluate and treat snakebite victims.These recommendations are based on clinical experience and clinical research evidence.This guideline focuses on the following topics:snake venom,clinical manifestations,auxiliary examination,diagnosis,treatments,and prevention.
6.Effect of early external diaphragm pacing on patients with mechanical ventilation

Objective:To investigate the effect of early external diaphragm pacing on the diaphragm function and prognosis of patients with mechanical ventilation.Methods:A total of 47 patients receiving invasive mechanical ventilation in the Emergency Intensive Care Unit of Hunan Provincial People's Hospital (The First Affiliated Hospital of Hunan Normal University) from October 2019 to July 2021 were selected and randomly divided into the treatment group ( n=23) and control group ( n=24). The patients in the control group received routine clinical treatment. On this basis, the treatment group received external diaphragm pacing treatment every day from the third day of mechanical ventilation until weaning, and was followed up to 30 days after discharge. The diaphragm thickness, diaphragm excursion, diaphragm thickening fraction, mechanical ventilation time, number of weaning failures, length of hospital stay and death toll were compared between the two groups. Results:Compared with the control group, the diaphragm thickness [(0.21±0.05) cm vs. (0.16±0.05) cm], diaphragm excursion [(1.38±0.37) cm vs. (1.11±0.48) cm], and diaphragm thickening fraction [26% (19%, 32%) vs. 18.5% (10.25%, 20%)] in the treatment group increased significantly (all P<0.05). The mechanical ventilation time was shorter in the treatment group [10 (7, 15) d vs. 13 (10.25, 19) d], and the difference was statistically significant ( P<0.05). There were no significant differences in the number of weaning failures (7 vs. 9), length of hospital stay [22 (15 , 30) d vs.. 24 (17.25, 34.25) d] and deaths (8 vs. 8) (all P>0.05). Conclusions:Early application of external diaphragm pacing can improve the diaphragm function of patients with mechanical ventilation, delay the decline in diaphragm function, increase diaphragm excursion and diaphragm thickening fraction, and shorten the mechanical ventilation time.
7.Analysis of influential factors for purchasing quantity changes in the procurement varieties of the first batch of drug centralized procurement

OBJECTIVE To investigate the factors influencing the changes in purchasing quantity in the procurement varieties of the first batch of volume-based drug centralized procurement (hereinafter referred to as centralized procurement). METHODS Using 25 procurement varieties of the “4+7” policy as research objects, the changes in purchasing quantity of procurement varieties were analyzed before and after the implementation of the “4+7” pilot, renewal and expansion policies. The influential factors were determined from the three levels of drugs, medical institutions and regions; and the multiple linear regression model was used to analyze the influential factors for the changes in the purchasing quantity of procurement varieties. RESULTS Before and after the implementation of the “4+7” pilot, renewal and expansion policies, the purchasing quantity increased by 52.1, -0.2, 85.8 ten thousand DDDs on average, compared with base period. During pilot, renewal and expansion period, DDDc decrease in procurement varieties was positively correlated with the increase in purchasing quantity (P<0.01). During the pilot and renewal period, the number of absolutely alternative varieties was positively correlated with the increase in purchasing quantity (P<0.1). During the pilot and expansion period, the number of alternative varieties to a certain extent was negatively correlated with the increase in purchasing quantity (P<0.05). During the renewal period, the increment of purchasing quantity in tertiary hospitals was smaller than that of primary medical institutions (P<0.05). CONCLUSIONS There is a relationship between the decline of DDDc and the changes in the purchasing quantity, that is, the more the drug price dropped, the more the purchasing quantity increased. The number of alternative varieties for centralized procurement will affect the changes in their purchasing quantity, but it is not always stable. With the implementation of the policy, the volume of primary medical institutions gradually exceeds that of tertiary institutions, indicating that the consumption of centralized purchased varieties is transferred to the primary medical institutions, and centralized procurement has promoted the implementation of the hierarchical diagnosis and treatment system.
8.Evaluation of the rationality and feasibility of the grouping method for centralized procurement of insulin drugs in Wuhan based on the results of questionnaire survey among endocrinology physicians

OBJECTIVE To investigate t he attitude of endocrinology physicians to clinical conversion and substitution of insulin drugs ,and to provide basis for improving the centralized procurement program of insulin. METHODS The proportion of convertible and substitutable insulin recognized by endocrinology physicians was investigated by questionnaire from 4 dimensions: intergenerational level ,bargaining group level ,common name level and brand/specification level. The subjects were endocrinology physicians in the third grade class A general hospitals in Nanchang. RESULTS A total of 89 questionnaires were successfully distributed,accounting for 80.2% of the on-the-job endocrinology physicians (111 in total )in the third grade class A general hospitals in Nanchang. Eighty-nine questionnaires were collected ,one of which was invalid ,and the effective rate was 98.9%. At the intergenerational level ,93.2% of endocrinology physicians preferred insulin analogues. At the bargaining group level ,the weighted average of the convertible ratio between group 3 and group 4 approved by physicians was 63.9%. At the levels of common name and brand/specification ,the weighted averages of convertible proportion of each group were more than 70%. CONCLUSIONS The method of insulin grouping in Wuhan is reasonable which can complete clinical conversion and substitution of insulin in the group safely. It is suggested to cancel long-acting human insulin group. The weighted average of the proportion of convertible and substitutable drugs in the group is high. It is suggested to increase the agreed purchase volume of insulin and conduct“volume price linked ”negotiations. When the surveyed physicians choose the initial treatment scheme of insulin ,they pay more attention to the factors such as efficacy and safety ,so the replacement of insulin should be based on the clinical efficacy and drug safety.
