1.Clinical Efficacy of α-Lipoic Acid versus Epalrestat in the Treatment of Diabetic Peripheral Neuropathy:A Systematic Review

OBJECTIVE:To systematically review the efficacy of α-lipoic acid versus epalrestat in the treatment of diabetic pe-ripheral neuropathy(DPN),and provide evidence-based reference for clinic. METHODS:Retrieved from CJFD,CBM,VIP,Wan-fang Database and PubMed,randomized controlled trials(RCT)about α-lipoic acid versus epalrestat in the treatment of DPN were collected,and the study was screened by inclusion and exclusion criteria. Meta-analysis was performed by using Rev Man 5.3 soft-ware after data extraction and quality evaluation by Cochrane software. RESULTS:Totally 6 RCTs were enrolled,involving 408 pa-tients. Results of Meta-analysis showed,compared with epalrestat,there were no significant differences in the total effective rate [RR=0.98,95%Cl(0.84,1.15),P=0.81],motor nerve conduction velocity [median nerve:MD=1.02,95%Cl(-1.10,3.14),P=0.34;common peroneal nerve:MD=0.23,95%Cl(-1.11,1.58),P=0.73] and sensory nerve conduction velocity [median nerve:MD=1.10,95%Cl(-0.39,2.59),P=0.15;common peroneal nerve:MD=0.95,95%Cl(-1.47,3.36),P=0.44] in α-lipoic acid group. CONCLUSIONS:The efficacy of α-lipoic acid is similar to epalrestat in the treatment of DPN,as well as the motor nerve conduction velocity and sensory nerve conduction velocity of median nerve or common peroneal nerve.
2.The Influence of the Loss of Basic Knowledge on Learning Clinical Knowledge

Objective:To explore the interns' degree of lost basic science and the influence on studying clinical knowledge.Method:Medical students attending the third(n=100) and the fifth(n=100) of medical studies selected randomly from the Guilin Medical School were given the same test composed of 20 pairs of questions and each pair contains one basic and one clinical question which were correlative.The scores of the two groups were compared.Result:Third year students scored significantly higher in basic than clinical questions(P0.05).Conclusion:There is a positive relationship between mastery of basic knowledge and the ability of dealing with clinical problems.Quite a few basic knowledge of medical students is lost when they begin clinical practice.
3.Cholinergic dysfunctions are correlated with memory deficits in rats exposed to chronic hypoxia-hypercapnia

Objective To investigate the relationship between changes of cholinergic system and memory ability impairment in rats long-term exposed to hypoxia-hypercapnia.Methods Forty-eight SD rats were randomly divided into control group,2 weeks hypoxia-hypercapnia group and 4 weeks hypoxiahypercapnia group.The chronic hypoxia-hypercapnia rat model was set up.The memory ability was assessed by eight-arm radial maze.Morphological changes were observed by the HE staining and Nissl staining.Acetylcholine(ACh) content,choline acetyl-transferase (ChAT) activity and acetyl-cholinesterase (AChE) activity in the hippocampus were detected by spectrophotometry,while expression of α7 neuronal nicotinic acetylcholine receptor (α7 nAChR) protein by Western blot.Results Memory ability,especially the working memory was impaired in rats exposed to chronic hypoxia-hypercapnia.And the memory ability decreased more markedly in four weeks group.Compared with those of normoxic rats,the levels of ACh and the activities of AChE and ChAT in the hippocampus of the two weeks group were significantly decreased (ACh:(58.9 ±2.7) vs (47.4 ±3.2) μg/mg (protein) ; AChE:(0.326 ±0.019) vs (0.247 ±0.020) U/mg (protein) ; ChAT:(127.1 ±8.6) vs (90.4 ±6.9) U/g (tissue wet weight),t =7.674,8.139,9.408,all P < 0.05).Compared with the two weeks group,those changes were more obvious in the four weeks group rats (ACh:(47.4 ±3.2) vs (32.5 ±3.2) μg/mg (protein); AChE:(0.247 ±0.020),(0.170±0.019) U/mg (protein); ChAT:(90.4 ±6.9),(55.6 ±6.0) U/g (tissue wet weight),t =9.279,8.036,10.781,all P < 0.05).Compared with the normoxic rats,the expressions of α7 nAChR protein were significantly decreased in two weeks group rats (t =4.481,P < 0.05).Moreover,the expressions of α7 nAChR protein were significantly decreased in four weeks group rats comparing with the two weeks group (t =4.965,P < 0.05).Conclusions An impairment of rat' s memory ability may be induced by hypoxia-hypercapnia,and the injury degree is correlated with the exposure time.Cholinergic system dysfunctions may contribute to the memory function defects in chronic hypoxia-hypercapnia rats.
4.Survey deep vein thrombosis and its risk factors in patients after stroke

Objective To study incidence of deep vein thrombosis (DVT) in the acute phase and follow-up period after stroke, and to investigate risk factors of DVT. Methods This was a prospective study at multi-centers. Ultrasonography was used for detecting DVT on both lower extremities in all patients at 10-14 days after the onset of stroke. All patients were followed up for 6 months after discharge. The incidence of DVT was examined in the acute phase and in the follow-up period of stroke. A variety of patient and treatment related factors were compared between stroke patients with DVT and without DVT to identify DVT risk factors. Results The incidence of DVT in the acute period of stroke was 4. 49%. Among DVT patients, 51.6% patients presented clinical DVT symptoms. By multiple factors logistic regression analysis,age ( ≥70 years, OR = 1.63, 95% CI 1.08-2. 84), bedridden( OR =4. 85, 95% CI 2.65-9. 68 ), wells score ≥ 2 ( OR = 3.96, 95% CI 1.86-7. 86 ), lower limbs NIHSS score ≥ 3 ( OR = 4. 56, 95% CI 2. 07-8. 85 ), high D-dimer ( OR = 3.45, 95% CI 2. 01-8. 52 ), low BI scores ( OR = 2. 98, 95% CI 1.52-6. 47 ), rehabilitation therapy ( OR = 1.82, 95% CI 1.22-3.43 ) and anticoagulant therapy ( OR =1.91,95% CI 1. 34-4. 92 ) were independent risk factors of DVT in the acute phase of stroke. Among them, the rehabilitation therapy and anticoagulant therapy were protective factors. The incidence of DVT in the follow-up periods was 1. 51%. Age ( ≥ 70 years, OR = 1.82, 95% CI 1.21-3.98 ), bedridden after discharge( OR = 5. 12, 95% CI 2. 82-11.32), lower limbs NIHSS score ≥3 ( OR = 4. 25, 95% CI 2. 11-7. 87), low BI score( OR = 2. 18, 95% CI 1.18-6.23 )at the time of discharge and DVT in acute period (OR =3.81,95%CI 1.87-7.48)were independent risk factors of DVT in the follow-up period of stroke.Conclusions Stroke patients, particularly old-aged stroke patients, are a high-risk group of developing DVT. 48.4% DVT patients had no clinical DVT symptoms but were diagnosed only by ultrasonography.There are multiple independent risk factors of DVT after stroke. It is necessary to monitor and prevent DVT in the stroke patients with the risk factors. The rehabilitation therapy and anticoagulant therapy may decrease incidence of DVT.
5.Effects of IL-24 delE5 on human leukemia cell line K562

Objective To investigate the antitumor activity of IL-24 delE5 in human leukemia cell line K562. Methods The expression of mda-7/IL-24 and its splice variant induced by TPA in leukemic cell lines, U937 and HL-60, was evaluated. The effects of IL-24 delE5 in K562 on cell proliferation, colony-forming ability, cell cycle, apoptosis, and tumor growth in vivo by using MTr assay, colony forming assay, flow cytometry, Annexin-V/PI and tumor xenograft models in nude mice were assessed. Meantime, the effects of IL-24 delE-5 and mda-7/IL-2A were compared. Results The expression of IL-24 dciE5 was detected in differentiated U937 and HL-60 cells. Transfection with IL-24 delE5 significantly reduced tumor cell viability, inhibited colony formation. Comparing with the control, G_0/G_1 stage add from (24.46±3.99) % to (42.69±3.04) %, caused cell cycle arrest in G_0/G_1 stage and significantly inhibited the growth of K562 transplantation tumor. No significant differences in the aforementioned antileukemia characteristics between IL-24 delE5 and mda-7/IL-24 was found. Conclusion Similar with mda-7/IL-24, IL-24 delE5 can efficiently inhibit the proliferation of K562 in vitro and in vivo, probably through induction of G_0/G_1 cell cycle arrest.
6.Blood compatibility of an axial-flow blood pump made in China:Verifying observation

BACKGROUND: Axial-flow blood pump is a main pump for ventricle assistance. Previous researches demonstrate that poor hemocompatibility of blood pump is an important factor for hemolysis and thrombus.OBJ ECTTVE: To design an axial-flow blood pump based on previous kinds of blood pumps through changing whole appearance and impeller shape of the pump by using Computer-Aided Design CAD) and Computational Fluid Dynamics (CFD), and manufacture it successfully.DESIGN: Rationality of theoretical design was verified by practical tests.SETTTNG: Beijing Anzhen Hospital of Capital University of Medical Sciences Department of Biomedical Engineering,Beijing Institute of Heart, lung and Blood Vessel Diseases; the Faurteenth Institute of China-Aerospace Science and Industry Corporation.MATERIALS: Body of blood pump and impeller were titanium alligation, and shaft bearing was ceramic. Test in vitro was accorded to artificial ventricular assist device which was provided by Department of Biomedical Engineering, Beijing Anzhen Hospital of Capital University of Medical Sciences. Experimental goats were provided by Experimental Animal Center, Beijing Anzhen Hospital of Capital University of Medical Sciences.METHODS: Since the beginning of 2005, a model of axial flow blood pump was designed in the 14th Institute of China-Aerospace Science and Industry Corporation base on decreasing shearing force and circulating dead bands. In the process, CAD and CFD were used to generate the geometrical data document of pump's structure, which included the figures of pump's body, shape and number of impeller's vanes, the structure and position of the guide vanes, and the size of impeller's screw-pitch. And then, NC machine tool was used for shaping. Finally, axial-flow blood pump was fixed on artificial ventricular assist device which was provided by Department of Biomedical Engineering, Beijing Anzhen Hospital of Capital University of Medical Sciences. The pump's hemodynamic output was 5 L/min and the average pressure was 13.3 kPa under the mixture of glycerin and water and fresh anticoagulation goat blood. The samples were collected at every one half hour during pumps being pumping for 4 hours. According to testing pressure output of blood pump, normalized index of hemolysis (NIH) was used to reflect content of free hemoglobin in plasma, observe thrombogenesis in pump and verify pump's hemodynamic output and vascular damaging degree.RESULTS: Shaped axial-flow blood pump included body, impeller, guide vanes, ceramic shaft bearing, export and import. The volume was 63 mL. Experimental results in vitro indicated that when the rotation speed of blood pump was 10 000 r/min, its pressure and flow output were 21.01 kPa and 6.0 L/min. The hemodynamic output might satisfy for left ventricular assistance. Surface temperature did not change obviously during successive rotation. The calculation indicated that most parts in blood pump showed a streaming flow. The mean NIH was (0.047±0.017) g/100 L, which was less than that of previous pumps; while, thrombogenesis was not observed in blood pump.CONCLUSTON: Axial-flow blood pump designed by using CAD and CFD can not only satisfy for the hemodynamics of a left ventricular assistant devices, but also the blood damage is milder than previous pumps. Therefore, axial-flow blood pump improves blood compatibility.
7.Neuroprotective effects of nGBE against glutamate excitotoxicity

AIM:To investigate protective effects of novel extract of Ginkgo biloba(nGBE) in different administration modes on glutamate-induced neuronal damage.METHODS: The values of Essential nGBE were obtained by supercritical CO_2 fluid extraction.Based on glutamate excitotoxicity of primary cultures from neonatal Wistar rats hippocampal,by use of Trypan blue dye staining,testing the lactate dehydrogenase leakage from cultured neurons and terminal deoxynucleotidyl transferase-mediated nick end labeling(TUNEL) method,we examined glutamate-induced necrosis and apoptosis.The protective effects of nGBE in different administration modes(pre-treatment and post-treatment) were adopted and compared with the NMDA receptor uncompetitive antagonist MK-801 in acute-treatment.RESULTS:Treatment with nGBE in two administration modes all could increase ratio of surviving neuron,decrease LDH efflux and reduce ratio of neuron apoptosis in different degree,and the benefit of pre-treatment was superior to post-treatment,but inferior to MK-801.CONCLUSION:The extracts of Ginkgo biloba used nowadays in cerebrovascular disease as post-treatment have no prominent effect as far as their mechanism of antiexcitotoxicity.However they may have more value if they were used for precautionary intervention in high-risk population.

9.Overexpression of heat shock protein 22 inhibits the growth of hematopoietic malignant tumor cells

Objective To investigate the effects of overexpression of heat shock protein 22(HSP22) in hematopoietic malignant tumor cell lines.Methods A lentiviral system was used to mediate transduction of HSP22 complementary DNA-containing expression vector or empty vector into K562 and Namalwa cells.The transduction effeciency was tested by fluorescence microscope scan and flow cytometry.Semi-quantitative RT-PCR and Western blot were used to identify the expression levels of HSP22 mRNA and protein.Growth curve analysis,cell cycle analysis,colony-forming assay,tumor growth in nude mice and apoptosis analysis were used to evaluate the role of HSP22 in K562 and Namalwa cells.Results Lentivector expression systemmediated delivery of HSP22 into K562 and Namalwa cells can inhibit colony forming of K562 and Namalwa cells,the average numbers of colonies per well were 108,72,125 and 80 for K562-V,K562-H,Namalwa-V and Namalwa-H respectively (P =0.000 16 and 0.000 37 for K562 and Namalwa respectively).HSP22 transduction can also inhibit proliferation of Namalwa cells in vitro (P =0.015,0.042 and 0.048 for day 5,6 and 7 respectively) and K562 cells in vivo (P =0.022 for day 21).No significant difference in cell cycle and apoptosis was found in K562 and Namalwa cells compared with controls (all P > 0.05).Conclusion Overexpression of HSP22 could inhibit the growth of hematopoietic malignant tumor cell lines K562 and Namalwa.
10.Study on Purification Technology of Total Alkaloid from the Flos of Aconitum kusnezoffii

OBJECTIVE:To optimize the purification technology of total alkaloid from the flos of Aconitum kusnezoffii. METH-ODS:The content of total alkaloid from the flos of A. kusnezoffii was determined by acid-base titration. The purification technology of total alkaloid from the flos of A. kusnezoffii was optimized by ion resin with resin type,mass concentration of loading liquid and exchange speed as factors,maximum adsorption quantity,desorption rate and mass fraction of total alkaloid as index,and verifica-tion test was conducted. RESULTS:The optimal purification technology was as follows as type 732 cation exchange resin,mass concentration of loading liquid 0.32 g/L,exchange speed of 7 column volume(BV)/h. In validation test,the content of total alka-loid was 86.88%(RSD=0.52%,n=3),and desorption rate was 92.81%(RSD=0.40%,n=3)averagely. The extraction trans-port rate of total alkaloid from 3 batches of the flos of A. kusnezoffii was 81.76% and purification transport rate was 89.47% in av-erage. CONCLUSIONS:The established method is stable and feasible,and shows high transport rate.
